Contributor: Travis Barlock, MD Educational Pearls: Foul-smelling urine and cloudy urine are commonly misinterpreted as indicators of a UTI. However, these findings alone are not diagnostic. Criteria for UTI: Presence of localized urinary symptoms: Suprapubic pain Dysuria Hesitancy Urgency Urinalysis with WBC > 10 Urine culture with > 100,000 CFU/mL Colonization differs from infection - many patients harbor asymptomatic bacteria but do not have a true infection. Consequences of overtreatment One review showed 45% of patients treated with antibiotics for a presumed UTI actually had asymptomatic bacteriuria and were incorrectly treated. Unnecessary antibiotic treatment can have deleterious effects on the gut microbiome, increasing the risk of multidrug-resistant infections. Another problem with overdiagnosing UTI is missing the real diagnosis by explaining symptoms away as "just a UTI." Be mindful of the risk of overtesting versus not testing at all. Use of gabapentin with or without a prescription in substance use treatment settings: A national analysis of urine drug testing data, 2016–2023 Drug and Alcohol Dependence Gabapentin prescriptions have increased due to off-label use, including managing withdrawal/comorbidities in substance use disorder (SUD) treatment, despite gaps in evidence bases and corresponding increases in nonmedical use. This retrospective, serial cross-sectional study sought to identify trends in gabapentin use with and without a prescription in SUD treatment settings. Researchers found that gabapentin prescribing significantly increased in SUD treatment settings despite a lack of strong evidence bases for its utility. While rates of gabapentin use outside a prescription were nearly double that for prescribed use, this appears to be decreasing over time. But can you talk us through these and tell us a bit about why you decided to do this research?Speaker B00:01:07.940 - 00:03:41.440So, as you've just really nicely summarised, cervical screening is really important weapon against cervical cancer.So we know that it prevents cervical cancer and since the introduction of the NHS Cervical Screening program in the UK, we've seen deaths from cervical cancer drop by around 70%. So we know that it's very effective.But in the uk, the number of people attending is declining year on year and currently, currently only around 68% of those people who are eligible for cervical screening actually attend. There are a whole range of different reasons for non attendance.These include things to do with the speculum examination, so having to have an intimate examination to be examined. The anticipated embarrassment or fear of pain related to that procedure, I think are important barriers.But there are also barriers associated with access to reaching screening appointments, taking time off work, having childcare and so on and so forth. So we thought that there was some really important barriers there that could potentially be addressed by self sampling.Now, vaginal self sampling is actually been incorporated in many cervical screening programs around the world.Some cervical screening programs are using it just for people who are non attenders or underscreened by traditional screening routes and other countries are using it as a choice for everybody.Now, in the uk, we haven't yet taken up vaginal self sampling sampling, but it will be introduced this year in 2026, principally for under screened groups. And there is some work looking at whether or not it will be introduced as a choice for everyone in the future.But we know from research that's been done in the UK that only around 12 to 13% of people who are...

Chaque année, on y découvre des objets futuristes censés transformer notre quotidien : frigos connectés, montres qui analysent le sommeil, lave-linges autonomes… Mais cette année, un objet beaucoup plus intime a particulièrement retenu l'attention. Un objet sur lequel nous passons pourtant tous du temps chaque jour : les toilettes.L'intelligence artificielle s'invite désormais jusque dans la cuvette. Lors du salon, des toilettes coréennes ont même remporté un prix d'innovation. Leur promesse : analyser l'urine à chaque passage afin de suivre différents indicateurs de santé. Grâce à des capteurs intégrés et à un écran mural placé face à l'utilisateur, il devient possible d'obtenir un aperçu immédiat de son état de santé. Certaines pathologies pourraient ainsi être détectées de manière précoce, sans rendez-vous médical ni prise de sang.Mais la fonction la plus étonnante – et peut-être la plus sensible – concerne la surveillance indirecte des personnes vulnérables. Ces toilettes intelligentes peuvent envoyer une alerte à un proche si elles ne sont pas utilisées pendant une période anormalement longue. Un dispositif pensé notamment pour des personnes âgées vivant seules, afin de détecter rapidement un éventuel problème. Une forme de sécurité… à condition d'en accepter le principe. Car cette innovation a aussi un coût : environ 4 300 euros pour ce modèle nouvelle génération.Cette médicalisation des toilettes n'est pourtant pas une nouveauté. Le Japon, pionnier en la matière, développe depuis des années des WC capables d'analyser les selles. Les données sont ensuite centralisées dans une application qui fournit un aperçu de la santé intestinale. L'objectif est toujours le même : collecter des informations de manière continue, automatique et la plus objective possible.Aux États-Unis, des chercheurs vont encore plus loin. Depuis 2021, des dispositifs expérimentaux permettent de suivre des patients souffrant de troubles digestifs sans qu'ils aient à tenir un journal quotidien. Une caméra installée dans les canalisations capture des images qui sont ensuite analysées et transmises aux gastroentérologues. Du sang dans les selles, des inflammations ou des anomalies peuvent être détectés plus rapidement. Là encore, l'idée est de transformer les toilettes en outil de prévention médicale.Il existe même aujourd'hui des « abonnements pipi ». Le dispositif U-Scan Nutrio, par exemple, se fixe directement dans la cuvette. Il analyse l'urine à chaque utilisation et suit plusieurs marqueurs liés à la nutrition. Les résultats sont envoyés sur une application accompagnée de recommandations alimentaires personnalisées. Le module coûte une centaine d'euros, auxquels s'ajoute un abonnement pour les cartouches d'analyse. La prévention devient ainsi un service récurrent… et un nouveau marché.Les toilettes sont donc en train de devenir un espace ultra-connecté, capable de collecter des données parmi les plus intimes qui soient. Reste une question centrale : jusqu'où sommes-nous prêts à être analysés en continu ? Si ces technologies permettent de prévenir des maladies graves, l'intérêt médical est réel. Over the course of our lifetime, we around 34,000 litres of urine, according to Business Insider. Urine evacuates the nitrogen-containing waste produced by the organs and expelled into the blood, like urea and uric acid for example. It's also made up of water and mineral salts. It tends to be a clear shade of yellow, ranging from pale to dark at different times of the day. Nevertheless, our pee can come in as many as ten different shades, which can on occasion lead to unpleasant surprises when going to the bathroom. Some are due to what we eat and drink, while others are caused by health conditions like urinary infections, diabetes or kidney stones. Check out today's questions: Alicja: Hello Dr. Cabral, I hope you're doing well. My question is about dental sealants. My kids' dentist recommended them for my 8 yo daughter and I blindly believed it was a good advice. Later he recommended them for my younger daughter, but this time I looked up what are dental sealants and it turns out it is plastic. Since my older daughter has them already (on 4 of her molars) I'm struggling to make a decision which is less of two evils: leaving the sealants because the damage has already been done, or should we have them removed, risking that some plastic will be absorbed by her body during scraping the sealants of her teeth? Holly: Hi! I need help for my 20-year-old daughter. She has been dealing with an issue of having very terrible smelling urine, yet labs show no bladder or kidney infection. She says there's minor discomfort after emptying her bladder. Despite undergoing multiple physical exams and lab tests, everything has come back normal. Her urologist prescribed Solifenacin several months ago, but it hasn't provided any relief. She is staying hydrated, avoiding spicy foods, and has eliminated caffeine from her diet. This issue has persisted for a long time, and it's affecting her emotionally—she is becoming quite depressed over it. Her health history is that she is highly allergic to cow's milk and peanuts/tree nuts and has had eczema since she was a baby. Any advice is greatly appreciated! Kaitlin: I've been having tinnitus for the past 4 months. It started out in one ear, then both ears, and then transitioned to full head buzzing. It feels like my brain is attached to an electrical socket. I have also been experiencing extreme anxiety, insomnia, fatigue, and soreness(esp neck). I am a very healthy 31 year old-eat an extremely clean diet and very fit. I did notice my symptoms worsening after switching to methylated vitamins. It also intensifies with heavy lifts & exercise. I did the hair test and it showed elevation in mercury and aluminum and every mineral ratio was out of whack. I also did the OATS test and it showed some depletion of b vitamins as well as low levels of neurotransmitters. I'm not sure where to go from here. I'm scared supps are going to make it worse again. Riela: My husband has had rather severe problems with vertigo for these past years. It seems to have been triggered by having Covid, and also a period of high stress; but he also had a blow to the head some years ago, and I'm wondering if that may be a cause as well. He's been on Pristiq for some time, and doesn't have as many problems with vertigo now, but he would like to get off it. Anxiety or stress worsens his vertigo. I'd appreciate it if you could help us find a direction, healthwise, that we could go. Jen: Hello! I enjoy your podcast and it has helped me so much over the years. I have a question regarding contact lenses. It has been reported that most are full of PFAS. I stopped wearing my contacts after wearing them for over 30 years when I heard this. I hate wearing glasses and am wondering if wearing contacts is truly dangerous. I found one brand that claims to be PFAS free (Eiyan), but they don't cover my prescription. In addition to this, most contacts now have UV protection which I am also concerned blocks sunlight signals from entering my eyes, and thus disrupting my circadian rhythm.
