Dr Cilia Nazef, Infectious diseases Fellow at the University of South Florida Morsani College of Medicine, explores how Pneumocystis jirovecii, a fungal infection typically affecting immunocompromised patients, can also affect patients suffering from hypercortisolism. Dr. Nazef begins by examining different cases of Pneumocystis jirovecii Pneumonia (PJP) in patients with Cushings disease. Next, she further explains the immunologic basis behind how the hypercortisol state induces immunosuppression. Dr. Nazef closes by examining the basis for PJP prophylaxis in Cushings disease patients, and the shortcomings of the current literature in advocating prophylaxis options.

Growing up we had several dogs, cats, rabbits & ducks, and a couple of lambs for a time. My grandparents even had a pair of pet skunks and a woodchuck! I loved horseback riding at the local stable and visiting the calves at the dairy farm. As an adult, being a military family (now retired) we were more limited, but our dogs and cats have always been a part of our family. I have experience with the young and old ones, the well and not-so-well. In our house, we've dealt with allergies, auto-immune disorders, Addison's disease, Cushings, arthritis, hip dysplasia, cancer(s), kidney disease, UTIs, surgeries, physical therapy, alternative therapies, the list goes on. EPISODE 12: Injustice - “What are you gonna do, a podcast or something?”Greetings Super friends! Welcome to Superman & Lois & Pals. I'm Henry Bernstein and alongside me is my favorite super pal, Professor Sam Brody. Tons of Superman news, specifically Superman and Lois. The good news is Superman and Lois was renewed for Season 4. The bad news was that it was at a great cost. Kind of like the end of a superhero movie. Only 10 episodes and The Cushings, Irons' and Sam Lane were all cut from the regular cast. Sam, what are your thoughts? In larger Superman news, The Flash movie finally came out. I know a lot of things have been spoiled and I'm not in the business of spoiling things, but I will say there are a lot of Superman treats and easter eggs in it. Is it a good movie? I don't think so. But I did enjoy myself. The Batman stuff is really fun. It is a fun theater experience, but since it's doing so poorly it'll probably be out on Max soon. Let's talk about Superman & Lois Season 3, Episode 12S03E12 - InjusticeDirected by:Sudz SutherlandWritten By:Michael NarducciThe IMDB & Apple TV Recap is: Lois and Clark clash with Jordan over his carelessness around using his powers in public. And finally, after seventeen years behind bars, Lex Luthor is set to be released from prison.HENRY's NOTES: There's Clark taking away the computer again. Love that he's washing her hair. So sweet. Injustice is a reference to her losing a part of her bodyI love that they're teeing up Luthor ready to kill Lois and Clark protecting her. Cool Lex music. The idea of Lex in prison for 20 years is so interesting. Love that he gets a nice soft boiled egg for breakfast. Who's the pic? Does he have a daughter? Who's the mom?Who's Janet? Foswell gave the Luthor story to Simone. Who's Simone?Love Clark mouthing “no no” hahaWhoa Lex with hair in the flashback? OTIS! Drops wedding ring but has pic of kid. LOVE Lois' look with the hat. Looks like the Jenny Frison cover from Lois Lane #1.Haha Bazoombas makes another appearance. Why has Bazoombas become a thing? What did you think about all the Alpha Male vs Alpha Male FD thing? Is Kyle being nice to Jonathan because he knows Clark is Superman?Sam Lane - I guess in a world where Superman is the only hero then it IS a big deal that there's another super in Smallville. Yes, they had Kryptonians attack them but another Hero is different. Not sure why.Why is it Jordan's fault that Sarah's having a rough timeWhat do you think about Luthor shaving his head vs having lost his hair at a young age? To paraphrase a quote from Larry David, those who shave their head to cultivate a look are not part of the bald community. I think a Luthor who has lost his hair all the way at a young age or is losing his hair so shaves it is better than a full thick head of gorgeous hair and shaves it. Why did Luthor let his ass get kicked? Mind games? False sense of security?Did you notice that the prison standard issue had the prisoner names on it? Isn't the whole thing about prison is they dehumanize you by giving you just a number? So Otis is not Luthor's henchman but a rival. Cool twist. I like that the Warden is just blatant about being crooked. So was Superman and Lois' Luthor one that had hair? Interesting….Damn this Lex is cold. I kinda love it. Oooooh Tornado in Smallville! Classic Superman Stuff! My first thought was, “will there be an MOS reference to Pa Kent putting his hand up?”Superman vs a Tornado is always good. One of the better parts of Superman IV which was deleted. Can you freeze a tornado?! Do you think you can talk inside a tornado?Haha, who are these idiots standing in front of a window? They've obviously seen a tornado before! Kyle and Jonathan just standing there watching?!Oy Jordan, not good. Doing the whole “I dunno” teenage thing. What was that little power display with Clark? Was Clark trying to grab Jordan and he used his powers to break away?Sam giving the advice of “reign him in” - clearly doesn't understand how teenagers work. I do like how Sam explained the flight path thing. That makes sense.Dean Jr looks like he's 45.Does Jonathan also have a video game system in his room?Oh my god did Lex Luthor walk from Stryker's Island to Smallville?!Oooooh Luthor is SCARY“What I want is to never have to read your words again”“But I wasn't wrong about what you are”Yeah Clark, stand up to that bully!“Don't ever come back” - “Then you'd better retire”“Tell Superman he's next”Ha! Otis is now his henchmanI know head shaving scenes are cool, but well you know how I feel about this. Better shave that beard. Haha I knew his chair would definitely be a person Who's Dr. Hook, who's Orr?YES LEX AND BIZARRO! The way it should be.Going with the Horror version of Bizarro. Eating rats and mutilating Otis. Nice.

Kidney Diseases: Symptoms, Causes and Treatments Dr. Marty Greer, DVM joins host Laura Reeves for a deep dive on diseases that affect the kidneys in dogs. They cover symptoms of kidney problems, causes and treatments for various common kidney issues. “The symptoms that most people catch first are a change in water consumption, an increase in water consumption and increase in urination,” Greer said. “Now, that's not the only reason that dogs can need to drink more and urinate more, and what goes in must come out. So those usually go hand in hand. “The most common things are changes in water consumption, changes in urination. Now other  things that frequently cause that are going to be diabetes; which happens in dogs and cats, Cushings disease; which happens in dogs, which is an adrenal gland dysfunction, and other things like pyometra, high calcium that can be related to different forms of cancer. So, there can be other things that we're looking for. But we're going to start looking at kidneys, diabetes and Cushing's disease in the dog most commonly. “We're going to get blood work and urinalysis as our basic starting point. But that's not the only place we're going to go. We're going to start with those two things. Because if the dog is still able to concentrate their urine, well, then that tells us something different than if the dog's urine was really dilute and the BUN and creatinine start to go up. “Once that happens, that means that only one fourth, only 25%, of the dog's kidneys are still working correctly, unless it's a secondary cause from dehydration like vomiting, diarrhea, other causes of dehydration, so it's super important. You go in and if your vet says we should do lab work, you shake your head up and down and you say yes, yes, yes we should. Please do not argue with them. Do not fight them on it because you can very quickly tell from a urinalysis and a blood panel. LR: Can a bladder infection go to the kidneys? “Number one, it can. It's not common, but it can.” LR: OK, so what's going to cause a kidney infection? Where's our causation? “It's usually hematogenous, meaning it starts off in the bloodstream, so can start as a pyometra. It can start as any way that bacteria gets into the bloodstream, but usually the kidneys are protected by the fact that the urine is concentrated so that helps to kill bacteria and remember urine is flowing from the kidneys down the ureters and into the bladder so that constant flushing should keep bacteria from being able to ascend up into the ureter and up into the kidney. So can they still happen? Yes, they can. They're not at all common, but they happen. And they're tricky to diagnose because sometimes it doesn't look obvious. So that's where that blood and the urine sample is really important because it is life-saving to a dog or a cat to have that diagnosed and be able to resolve that. Additional causal factors, Greer noted, can include tick born or infectious diseases such as Lyme Disease and Leptospirosis. For additional details on causes and treatments, listen in to the entire episode, or check the YouTube pod, and click to subscribe, @PureDogTalk.

Is Adrenal Fatigue Real?

Empowered Nutrition

Play Episode Listen Later Jun 21, 2023 28:49

Adrenal fatigue is not a real medical diagnosis. However, the phenomenon is alluding to an imbalance in terms of the levels of your adrenal hormones, related to the signaling between your brain, adrenal glands, thyroid, and gut. Adrenal hormones include cortisol, aldosterone, adrenaline, and noradrenaline. There are actual medical diseases that involve very abnormal levels of adrenal hormones such as Cushings disease – high cortisol.  However, the term ‘adrenal fatigue' refers more to a departure from optimal levels that doesn't reach an extreme. If you suspect adrenal fatigue or an imbalance of adrenal hormones and ask your conventional provider to check you, they will likely test your cortisol levels and tell you that you're normal.  That's becaue they're using the reference ranges for the traditional, overt adrenal disease. However, Empowered Nutrition, we acknowledge that many people have imbalances of their adrenal hormones that impact their health, yet do not meet the traditional cut-offs for overt disease.   Here are some of the things we see with adrenal imbalance: Irregular blood sugar, especially at night and upon waking (CAR) Weight gain, especially central, that is resistant to most diet attempts Anxiety or depression Low energy, especially in the afternoon Low libido Irregular menstrual cycle Symptomatic menstrual cycles: PMS, PMDD, cramping Slow digestion/constipation Feeling cold often Low circulation in extremities Sleep challenges: night waking, hard to get to sleep, early waking Remember, the adrenals are the foundation that then impact the sex hormones and thyroid so you will see symptoms related to any of the three. Some of the most common scenarios where we see adrenal dysregulation are: Busy/tired moms – missing sleep due to kids, just an overall high burden, trying to balance work with parenting and self care Excess fasting Excess high intensity exercise, especially fasted Excess dieting – calorie or carb restriction  Other high stressors: carers, high-stress job, divorce, chronic illness Past trauma: early life trauma that is emotional, sexual, or physical: this can manifest in adrenal dysregulation due to chronic sympathetic tone, event if current life is normal.   If you suspect adrenal imbalance, there are a few ways to test: Indirect: wear a CGM – there are certain patterns that match this.  Specifically, it will be high readings overnight/during sleep and when waking/still fasted. Get a cortisol curve: a one-point test doesn't work.  We use the Adrenocortext from Genova.  It measures cortisol across the day, including the CAR, plus DHEA – an important precursor to cortisol. Combine adrenals with sex hormones: we use DUTCH testing that shows the metabolites on both sides of the coin, from precursors to active hormones to the metabolites.   Whether you test or not, there are few tried and true steps that you can take to support your adrenal health.  These are the things that we find to be most helpful for our clients: Diet: ensure regular meals across a 12-hour period with some slow carb at each meal, coupled with protein and/or fat.  Avoid refined carbs, processed food, and excess caffeine.  Minimize alcohol.  Don't overly-restrict energy – only a minor deficit or none.  Get help if you need it. Sleep: prioritize this.  No blue lights for 3 hours before bed.  Get a tracker if you need to, so that you can quantify it.  The Oura ring will tell you if your sleep is sufficient to keep your HRV, resting heart rate, and body temp optimized. Exercise: avoid fasted, high-intensity exercise, especially caffeinated.  Instead of Crossfit-esque workouts, transition to traditional weight lifting, and cardio that shifts between steady state and occasional FED intervals/high intensity. Mental health: at a minimum, start a daily practice of meditation or biofeedback such as HeartMath.  Get a therapist – almost everyone can benefit.  If you have past trauma especially, look into brain re-training approaches such as the Gupta program or DNRS. Hopefully this helps and feel free to reach out if you want help with assessing and optimizing your adrenal health!   Links from this episode:  DNRS: https://retrainingthebrain.com/ Gupta program: https://www.guptaprogram.com/  Oura ring: https://ouraring.com/  HeartMath: https://www.heartmath.com/  Interested in our Lean for Life Membership? Who feels like Cushing's is a nice disease to diagnose and manage? Like, how often do you feel like your hyperadrenocorticism patients are really doing REALLY well? My guess is: not that often. This episode will change that. Dr Sue Foster is a registered specialist in Feline Medicine and Senior Lecturer in Small Animal Medicine at Murdoch University in Western Australia. She's also a medical consultant for Vetnostics and ASAP Laboratory, where a large part of her role is interpreting cortisol test results and supporting veterinarians in their clinical decision-making. In this conversation, Dr Sue challenges the belief that Cushing's is to some degree a 'lifestyle' disease that doesn't always need to be treated, we discuss the subtle ways that it can present, and of course we take a deep dive into those slippery cortisol tests, which should feel a lot less slippery after this episode. How do you show up for yourself? Hard work and dedication are definitional of what it means to be a pet sitter. The services you offer should be connected to the passions you have in life. Emily Duhancik, owner of Pet Sitting & Care by Emily, joins the show to discuss what it takes to offer farm care to very particular clients. Her motivation comes from a lifelong love of animals, and a desire to be an excellent role model for her daughter. Emily shares how she designs an amazing client experience, where to get more experience with farm animals, and how she honors her feelings and emotions through the services she offers. Main topics: Passion to profession Animal communication Farm care pricing Client experience Main takeaway: At some point, we are all a client of our own services, so design the experience well and make it a fantastic experience. About our guest: I've been fortunate enough to work with and provide care for animals the last 25+ years. I pet sat for neighbors and friends in high school and college part time. After I graduated with my Bachelor's in Human Development and Family Studies from Colorado State University, I worked in differing environments including outdoor recreation, teaching preschool, retail, human resources, and running operations for a million-dollar company. None of these filled my bucket like animals did but I continued jumping from career to career for fear of ruining my passion with them if I made it a career. Pet sitting and pet care is not a side job for me. I take this very seriously and treat every critter as if they're my own. I work with and have experience with small and large animals, special needs animals (cancer, illness, paralysis, Cushings/diabetes, timid, deaf, etc). I am bonded and insured, certified in animal first aid and cpr, have double certifications in equine massage, am a Certified Fear Free Professional and am a member of Pet Sitter Associates (PSA).   Links: https://www.facebook.com/PetSittingAndCareByEmily https://petsittingandcarebyemily.com Buy PSC Merch Give us a call! So much to cover on this one! We celebrate Professor Sam's birthday coinciding with Superman day, disliking comic book annuals,  Lana giving Jordan a verbal thrashing, Ally owning Lucy's mind, Clark's new job prospects, Jordan's flight school, Alanis Morsette and Kessel run being the same joke, Batman 66 reference, Nat and John Henry's unnecessary CW conflict, our wish list for finale/cliffhanger and to close it out a long tangent about the state of TV watching.  EPISODE  13 “I Used To Call It The Kessel Run.”Greetings Super friends! Welcome to Superman & Lois & Pals. I'm Henry Bernstein and alongside me is my favorite super pal, Professor Sam Brody. Happy Birthday Sam! Did you know that your birthday is one day before the official “Superman Day?” Officially, June 12 is considered Superman Day. Let's talk about Superman and Lois Season 2 Episode 13S02E13 - “All Is Lost” Directed by:Elaine MongeonWritten By:Kristi KorzeciTunes recap: Clark and Lois disagree on the best way to figure out if Ally Allston went to the Inverse World. Meanwhile, John Henry makes a surprising discovery and Lois sets out to track Lucy down.HENRY'S NOTES:Lois/ChrissyIs Clark wanting to work at the Smallville Gazette just a backup story or is it for real? It would make sense that Clark wants to start reporting again but he's never mentioned it.  Lol to Chrissy being the driver. Chrissy looking at Clark's resume and not wanting to hire him? Why? Wouldn't you want 2 Lois lanes?Lane FamilyLane family going down together. I like it. FINALLY we see Lucy in all of her traumatic delusions. Lois bringing back all the things Lucy did to them including fucking DRUGGING HER FATHER. The way Lois sounds when she's trying to convince Lucy that she loves her is so lectury and factual. Of course Lucy feels the way she does. “I am so sorry I ever made you feel like a burden because the truth is you were my life raft. I wouldn't have survived my childhood without you and I still need you. You have to mean it, you have to show it. Please give me a chance to prove how much I love you.” Dangerous game Lana is playing, sharing with Kyle that she has “secrets.” Good thing Kyle is a bit of a nice dumb guy and didn't bother following up. Sam Lane is great, and Lucy is right when she says she doesn't deserve him. Cushing FamilyIs “who's alanis morsette” and “what's the kessel run?” the same joke?Relax Lana, it's not a big deal that Kyle took Sarah to open mic, in fact it's really sweet. I gotta say I was super into Lana lecturing Jordan. “Not even Superman can keep everyone safe.” Get outta here kid. And I also like that she felt bad about it. Really good acting here Emmanuelle. Clark and the boysBoys, relax. The Cushings are annoying. You've got a cooler friend/sister in Nat and fucking powers like Superman. Jon feeling guilty about his mess up and his Bizarro's murderous spree. Good talk between Clark and Jon. “As a parent you care so much about your kids and I want to protect you but I want you to learn to make your own mistakes and that's really hard but I will always be here when you do.” When Superman says, “it's gonna be fine” he means it.  “I still love her, dad.” Please. But great timing to take him flying. The flying training was great because traditionally, Clark just flew and that was it. Even if it was shaky at first or he just leapt at first he then seemed to master it. Like in Man of Steel. I like the idea of Clark spending years at the Fortress just practicing flying. Btw, they could've picked up Jonathan, a winter coat and given him a job instead of just ditching him. Ally AlstonThe cold open “5 years ago” was interesting. It still doesn't give us that much background in her motivations. I like that they actually reference the “LL.” It's also huge that they make mention of being in Lois' shadow. That's gotta be tough. It's cool that they low-key backdoored their way into making Ally The Parasite. Lois literally called her Parasite. I'm not mad about it, even though we made fun of it last week. I actually kind of like it. On Bizarro world Ally has completely taken over. Ally's really good at subtly putting a wedge between Lucy and Lois. It's cool that Ally's doubled voice is her backwards voice behind her regular voice. When Ally demands that she summon Superman, Lois retorts that she doesn't have a red Superman phone to contact him, likely an homage to the red "hotline" Commissioner Gordon used to get in touch with Batman in Batman (1966).Is it consistent or inconsistent with Lucy's character that just when she seems to be coming around she goes full super-villain. I guess the parasite thing is slightly different because she doesn't need to touch people. Scary seeing Superman turn into Bizarro. How does her Parasite powers let her stop a hammer from hitting her? Nat and John Henry IronsNat is smart enough to make a fucking secret Iron Man suit but didn't think. John's overreaction to Nat's suit is both realistic but in the context of the show is kind of dumb. She's clearly an asset and I'm glad that John Henry came to his senses eventually. “I build things, not destroy them.” The conflict was very CW episodey. But it's very cool that Nat's tech is superior. “Agro dad.” I'm so sick of them over-using that term. Do teenagers really talk like that? Very nice resolution by John Henry and not taking things personally. Nice turn of events that Nat gets to save Superman. I think we called that earlier on. Why didn't they try throwing Superman into the Sun?What's next? What if the Earths merging isn't bad? What if it like brings the best out in everyone? I would love to see a cliffhanger where Superman loses all of his powers and Jordan, the Ironses and Jonathan suit up to take over. Next season can pick things up 3 months later with Clark having a summer as a regular guy.

Pikante is a 24 year old horse who had been getting sick with insulin resistance for many months causing him to be more lethargic, deposit fat all over his body and seem to head towards Cushings disease, thyroid disease and down a difficult metabolic pathway full of endocrine problems. But Kate, Pikante's person, put her thinking and researching cap on and changed his carbohydrate fat and protein ratios. He is now eating rice bran, teff and alfalfa pellets, taking Platnum performance and Insulinwise (supplements) and getting over 200 minutes of exercise per week. Kate's concentration was on keeping his nonstructural carbohydrates between 10-12% of his diet. If possible he also gets his hay soaked for less than an hour to remove the sugars, but retain the calcium. Please comment and share if you have thoughts! Pikante's insulin went from 209 to less than 9, meaning the problem became under control, not with pharmaceutical medications, but with management of his diet. --- Support this podcast: https://anchor.fm/donna943/support

Henry's notes:Opening: Love Clark and Lois in bed. They're my cozy mom and dad. Great shot of Superman flying out of the house over the farm. Very cool call back to Superman flying around the world in Superman The Movie. Love that he's flying in a small circle to pick up speed. What was happening to him as he was going through? Was he breaking apart? Was he transforming into Bizarro?Jordan: Training montage! Just needed “you're the best around” or “eye of the tiger.” Disturbing but also comforting seeing the world struggling without Superman. Lois/Sam/Jordan: Sorry Lois, I love you but I'm with Jordan. Chill, he has to help. It's what Clark would do. Great voice of reason from Sam Lane. I do like that Lois is getting close to finding the dealers. Glad Jonathan finally came to his senses. It was getting silly. Really enjoy seeing Lois do her reporter getting into life threatening predicaments thing. Jordan's heat vision/freeze breath/knockout punch was a total video game move. The little smile, very reminiscent of Superman Returns, bullet eyeball scene. Sorry Jordan, you want to be a superhero and keep your secret, you can't have a girlfriend. Read a comic book.Irons Family: Steel helping out is great. Love that the news report called him “new Man of Steel?” Seems like lots of planes have engine failure in Superman/Supergirl's universes. Also, again, where is Supergirl?I like that Natalie can fix a super-suit and interdimensional space ship but can't make waffles.Natalie is mad at her dad flirting with Lana. Chill out kid. I can't deal with her. Remember how last season I could barely stand the Cushings. This is how I feel about Natalie. I want to like her so much but instead of showing how awesome she is in the shop with her tools and tech it's just blah blah drama with her. It's annoying. I like John's explanation. It's totally reasonable. He's entitled to move on. Having said that it was pretty upsetting seeing alternate Lois with baby Natalie and John Henry losing it.  Lana's election: We laughed about election day coming but now that we know Superman has been gone for 1 month it makes sense. They don't have projections on the local news? 57-43 what?Funny to me that Coach Toxic Masculinity is supporting Lana. Jeez, really hitting us over the head with the whole “Divorced dad sad apartment” cliche.End: Whaaaat Superboy is Jonathan in Bizarro world because it's the opposite of Jordan!

This week's conversation is all about Equine Cushings or PPID as it is now known and Liz Somerville is once again joined by Jenny Croft MRCVS. If you are an owner of a golden oldie this episode will be particularly relevant as PPID affects so many of our older horses and ponies. Thankfully it is easy to treat and manage meaning we can prevent many of the complications that we see in undiagnosed cases of PPID. The conversation follows a simple format of Liz, as a horse owner herself, asking Jenny all about PPID, what it is, the symptoms we should be looking out for, how we diagnose it and importantly how we treat it……… We hope you enjoy it!
