Optimal Health Series Is Ohio headed for a crisis in health coverage, especially for mental health and addiction services? More than a decade ago, Ohio's Medicaid expansion was a game changer, extending coverage to over 700,000 Ohioans and providing a lifeline to thousands of Ohioans in need of mental health and addiction treatment services. Expansion worked: uninsured rates dropped, medical debt decreased, health improved, and emergency room visits went down. In just four years, 630,000 Ohioans received treatment for mental illness or substance use disorder, a victory in the fight against opioid addiction. In 2024, forty percent of adults covered by expansion had a primary mental health and/or substance use disorder diagnosis. These gains may now be in jeopardy: Governor DeWine's new proposed biennial budget includes a so-called “kill-switch” provision that would automatically end mental health and addiction services provided through the expansion if federal Medicaid funding is reduced. With key leaders and policy experts, we'll unpack the impact Medicaid has had on health outcomes in Ohio and the consequences facing patients in recovery if the state's landmark care expansion is abandoned. Carolyn Harding, host, with Linda Jakes and Rachelle Martin, advocates for Issue 24, the Franklin County ADAMH levy to fund mental health services, and drug and alcohol treatment. With the trauma and uncertainty of COVID-19, education and work upheavals and the ongoing protests for justice, most of us are in need of some help, emotionally. Lets talk about what this levy, if passed will do for central ohio folks. Linda Jakes has been a staff member of Concord Counseling Services for 33 years and is now the Executive Director. The programs Linda manages at Concord incorporate support and community integration to increase community living days, highlighted in John Kasich's' book, Courage Is Contagious. She received her Masters degree in Social Work at Ohio State University, has taught at Columbus State Community College, in the Department of Mental Health and Substance Addictions. She is sought out to speak about severe and persistent mental illness from a service delivery perspective. She has been recognized in her field and Honored by her life-long service in mental health. Rachelle Martin has over 30 years of public service in the health care field, including coordination of health care systems nationally and internationally. Currently she's the Executive Director of the National Alliance on Mental Illness of Franklin County (NAMI) . She was formally the Director of Training and Prevention Network Services Manager for the ADAMH Board of Franklin County and the co-founding partner of the Suicide Prevention Advisory Committee and the Suicide Prevention Foundation and where she chaired the Advisory Committee for over four years. My chat today with Rachelle Martin and Olaf Sorensen is anything but dull! These two are the ultimate self-experimenters, particularly when it comes to diet (and, in Olaf’s case, finding the perfect exercise regime but that will be the subject of another podcast).Both Rachelle and Olaf started experimenting with a low carb diet several years ago, Rachelle, to correct some health issues and Olaf, to find the optimal way to manage his energy for long distance running e.g. ultra marathons. We heard about Rachelle's ketogenic diet in Episode #7 of Amanda’s Wellbeing Podcast (link below). Olaf, as you will hear, found the low carb way of eating agreed with his physiology but not content to stop there, he took it further and experimented with the carnivore diet and says, he felt so good, he never went back. Today, Olaf eats meat, organ meat, fat and eggs. We chat about the importance of the source of protein consumed and Olaf’s business with his parents, Grassland Nutrition, where they produced freeze dried organ meats in both powdered and capsule form. Links below.For many people, this style of eating may sound extreme and (quite rightly) ring alarm bells - where is the dietary fibre and vitamin C and what about antioxidants? Rachelle chats to us from a nutritionist's point of view about where those nutrients are obtained in a carnivore style diet. During life there is a constant death and renewal of body cells. Failure of apoptosis is implicated in the development of cancers.Insulin resistance (IR): IR refers to an impaired response of the body to insulin, resulting in elevated levels of glucose in the blood (a key component of type 2 diabetes).Lean mass hyper responder: A person who sees a significant rise (usually 50% or more) in their LDL cholesterol after adopting a low carb diet.Lipid profile: A lipid profile test looks at the various levels of cholesterol and other fats in your blood.The lipid profile includes total cholesterol, HDL-cholesterol (often called good cholesterol), LDL-cholesterol (often called bad cholesterol), and triglycerides. Lipids and cholesterol are fat-like substances in the blood and some are necessary for good health. However, if you have a high level of LDL-cholesterol and triglycerides, then you have an increased risk of heart disease. Researchers think that the presence of small dense LDL could be one of the reasons that some people have heart attacks. Today’s podcast guest is Rachelle Martin, a nutritionist who specialises in low carbohydrate (carb) and ketogenic (Keto) diets. Rachelle and I studied our Masters in Human Nutrition at Deakin University together and she alludes to our many interesting discussions and debates during that time. I thoroughly enjoyed this episode with Rachelle; her attention to detail and depth of knowledge is apparent in the interview. Rachelle explains the difference between the Paleo and Keto diets, both of which are low carb diets but are significantly different in terms of the ratios of fats, proteins and carbs consumed. Given the current popularity of the diets with celebrities and the like, it can be tempting to lump them into the fad category. However, in relation to the Keto diet, Rachelle explains its scientifically documented anti-inflammatory effects and the numerous benefits that flow from that. In her nutritional practice Rachelle has seen weight loss, the reversal of Type 2 Diabetes, improvements in symptoms of patients with Alzheimer’s disease, polycystic ovarian syndrome and insulin resistance.
