Nicole talks about her own personal health history, first with the chronic pain condition Trigeminal neuralgia, then the lasting negative impacts of pain medication long after she was of it that severely impacted her quality of life. Dr. Bret Scher joins host Doug Reynolds on the LowCarbUSA® podcast to discuss his role as the director of Metabolic Mind.  A board-certified cardiologist, lipidologist, and therapeutic ketogenic diet expert, Bret has over 20 years of experience as a preventive cardiologist and served as the medical director for Diet Doctor for over three years. Metabolic Mind was founded by Roblox CEO David Baszucki and his wife, author Jan Ellison Baszucki, after their son Matthew recovered from a severe bipolar disorder using ketogenic metabolic therapy.  “They were having a really hard time getting Matthew treated in a way that not only controlled the symptoms, but gave him his life back,” Bret recounts. “There was a point where he was in and out of inpatient treatment facilities and he actually checked himself out and hitchhiked from Oregon down to California. He was homeless, giving away all his possessions, and living behind a dumpster.  These are the stories that often end tragically, but the Baszucki's were fortunate enough to connect with Dr. Chris Palmer, who was able to get Matt started on a ketogenic diet and that's what really turned things around.”  Matthew has been able to slowly get weaned off his medications, and is now living a normal life.  He has completed college and is currently employed full-time in the tech industry. As he continues to gradually reduce his medication dosage, he shows no signs of the debilitating symptoms—mania, depression, or anxiety—that once afflicted him.  You can read Jan Baszucki's account of the family journey here. Bret discusses the growing momentum in the field of metabolic psychiatry. He shares that research is underway, with five different trials being funded by Metabolic Mind, and how the psychiatric community has overwhelmingly welcomed these advances.  “Our brain needs energy, just like the rest of our body needs energy. And just like our body can be insulin resistant, and not be able to use glucose for energy, the same can happen in our brains,” he explains.  Despite the advancements, Bret highlights that there are still physicians with misconceptions about ketogenic diets, often fueled by mainstream media.  “This concern of keto diet equals fat equals high LDL and heart attack just needs to disappear, because that is purely false,” he states, citing data from Dr. Shivani Sethi and Dr. Ian Campbell that debunks this notion. Bret discusses the complex science behind how ketosis can benefit the brain. He points out that ketones can serve as alternative fuel sources to significantly improve brain function, reduce neuroinflammation, and balance neurotransmitters.  He emphasizes the critical role of education, pointing out that the knowledge of metabolic therapies is often missing from medical schools and residencies, creating a barrier for wider adoption. He hopes to disseminate this information to both clinicians and the public. Later in the interview Bret draws an analogy between the growing acceptance of the keto diet for type 2 diabetes and the growing interest in metabolic psychiatry.  “That's how we start to build this movement to really just help more people, right. That's the ultimate goal. We want more people being treated in this fashion to help them get their lives back,” he concludes, calling for collective efforts to make a lasting impact. Doug and Bret conclude the interview with a discussion about plans to organize a special focus day for Metabolic Health in San Diego on metabolic therapies for mental illnesses at a future Symposium for Metabolic Health.

His father passed away from a stroke, and his brother passed away from a heart attack. Growing up, Dr. Mansor was overweight. During his undergrad, Dr. Mansor learned about physiology and metabolism. Plus, during college, Dr. Mansor started to exercise and actually enjoyed exercise for the first time in his life. With all this knowledge, Dr. Mansor realized he could be healthy without worrying about the diseases his family members suffered from.  [10:35] Why Are 88% of American Adults Metabolically Inflexible?  Your nerves have a higher risk of getting damaged when there is a constant increase in elevated glucose in the blood. Your body knows something is wrong with increased glucose, which triggers chronic inflammation. Metabolism is all about balance and moderation.  Every enzyme has a specific temperature range at which it's optimal.  When you activate inflammation constantly, there are bound to be adverse side effects.  [15:35] Life Is All About Challenges and Growth - Use Trial and Error To Find Your Sweet Spot  Life is all about giving yourself a challenge, adapting, learning, and growing. Next time you face the same challenge, you can tackle it in a much more efficient way.  It can be difficult to find the sweet spot when you are challenging yourself. Dr. Mansor uses a trial and error approach when it comes to knowing his sweet spot while exercising.  Your body will adapt. People want instant change; however, you need to give your body time to change.  [22:40] You Need To Train Your Body To Respond To Certain Conditions Effectively and Optimally  Metabolic inflexibility is your body's inability to switch from burning glucose to burning fat. When there is dysfunction, there is bound to be a deficiency, and there is bound to be wasted energy. Everyone thinks cortisol is the bad guy; however, you need cortisol to function.  The goal is not to shut down whatever is related to chronic disease. Instead, you want to train your body to respond to specific conditions effectively and optimally.  [30:20] Exogenous Ketones Are a Source of Energy and Can Help Lower Your Hunger Hormones Ketones are fuel for you to create energy.  There are still calories with exogenous ketones.  You won't lose weight if you have too many calories and are not exercising.  Ghrelin is the hunger hormone. Ketones can help lower that hunger hormone. [36:05] Exogenous Ketones Are Useful For Getting Back Into Ketosis  If you are following a ketogenic diet, you may not need exogenous ketones. However, do you have a hard time getting back into ketosis? If you fall out of ketosis, exogenous ketones can be a helpful way for you to get back into ketosis. Exogenous ketones are also a great tool to use before exercising.  [43:20] The Brain Doesn't Metabolize Glucose As Efficiently After A Traumatic Brain Injury  Dr. Mansor researched how ketones can be a therapeutic intervention to treat traumatic brain injury. Within 48 hours, studies show hypermetabolism of glucose after trauma.  Some researchers think the body wants glucose for energy to repair.  Ketones can help give the brain another option to produce energy.  For some reason, the brain doesn't metabolize glucose as efficiently after a traumatic brain injury.  [55:55] Ketone-IQ Is A Useful Biohack To Improve Your Sleep and Recover Faster  A dose of ketones after a workout and a dose of ketones before sleep can be helpful for recovery.  Plus, ketones before sleep can improve sleep quality.  Ketone-IQ has been proven to support energy, focus, and endurance. In this episode of my Six Weeks to Fitness podcast, I interview Sophia Paliov to discuss how she used Intermittent Fast and a low-carb diet (Keto), to lose 40 pounds in 4 months. Sophia has been in the public relations industry for over 27 years. She is the owner of SPPR-LA, a lifestyle, hospitality, beauty & fashion pr firm out of Los Angeles, California.  Growing up and most of her adult life Sophia has never had an issue with weight. In fact, people thought as a child and teen at times she was too thin.  Sophia grew up always walking to school mostly and would take five days a week ballet and tap dance classes during her childhood years and into her early teens and remained at a good low weight (120-124) all the way through her late early thirties.   Around the age of 33, Sophia started to gain weight due to a lot of stress (cortisol) due to personal family stressors (father diagnosed with stage four prostate cancer) as well as eating way more processed foods and also dealing with some fibroid issues which has to be balanced out with stronger birth control.  In 2010 at the age of 38 Sophia's father died and by that point, she found herself about 15 pounds heavier even though she had begun to eat less processed foods and cut out a lot of her sugar. But the stress along with hormonal issues due to her fibroids still slowed down her weight.  Soon afterward, Sophia's mother was diagnosed with Alzheimer's and for the next 8 years till her death in 2018, Sophia's weight fluctuated.   In 2015-2017 she began a serious weight loss journey and workout regimen by working out to a lot of cardio and doing low carb/gluten-free lifestyle.  She got down to her goal weight of 127 and held it there till a workout injury in April of 2017 halted her progress.  At that point, her mother's illness had severely progressed and by early 2018 Sophia's hormonal imbalance caused her fibroids to produce super heavy bleedings which kept her from doing heavy workouts.   By June 2020, Sophia had ballooned up to 165 pounds and was miserable.  With surgery scheduled for that same month to deal with the fibroids, Sophia was finally able to get her hormonal balance in check; slowly.  But, as the months went by the drive and determination to get back to her comfortable weight just was not there.  She knew she had to make changes again and to get motivated to start moving her body but kept pushing it off.  One day a week or two of working out just was not cutting it at almost 50.  In April of 2021, Sophia saw a picture of herself in leggings and a tight top and just was floored at how bad she had gotten.  It was that instant that triggered her to get motivated again and to know her WHY.  She knew that she had to lose weight and that going low carb/keto would again be part of her life (which she has always done on and off), but she also knew outside of work she was missing something - she had to implement her WHY.  She did not want to end up like her mom with Alzheimer's and even prior to that walking with a walker, she also feared cancer, so doing what she loves to do, Sophia stayed up at late hours and researched longevity, weight loss and found Intermittent Fasting. Ordering the book "The Complete Guide to Fasting" by Dr. Jason Fung, Sophia read it in two days and then just dove into more research on fasting, deepening into keto and the importance of protein, weight training, cardio and of course the magic of fasting.   By October of 2021 just in time for her 50th birthday Sophia had lost her weight and reached her goal weight of 129, and to this day she fluctuates between 125-129 with a steady amount of muscle gain.  Her philosophy is not about just losing weight, but how you go about it and she always asks others to know their WHY. "It's not a diet, it's a lifestyle."

If you're eating minimal carbs and you feel great, then you're obviously functioning on fat and ketones. If your calories are very low, you may want to leave fat higher and decrease protein because fat is great for energy. Don't leave protein low indefinitely. [21:50] Should You Throw Away Your Scale? How To Track Your Health   Robert says, “don't throw away your scale!” He likes to look at his weight as a data point. Weight doesn't show the whole picture, but it does show one piece of data. Instead, your composition should be what you measure. Also, take pictures and measurements. To get through muscle soreness, you should train through it. If you're a seasoned lifter, you may benefit from a week where you decrease the intensity. When the general is not around, foot soldiers go crazy. This is exactly what happens inside our bodies.  [06:10] What You Need To Know About Mitochondria In The Human Body We've been trained to think of the mitochondria as an energy furnace. People think that the mitochondria just put out energy.  There are 40 quadrillion mitochondria in our body. The average cell has about 500 mitochondria. The average liver cell has 2,000, kidney cells have 4,000, heart cells have 10,000, and a mature egg in a woman's body has 600,000 mitochondria.  The cells that are primarily relied upon for survival have the most mitochondria.  Morley likes to say, “ignore the enemies; ignite the energy.”  Morley has learned that if you don't have copper in your diet and don't have retinol to make copper bioavailable, you're not getting excellent mitochondria function. If mitochondria cannot produce energy, they cannot recycle iron.  [21:30] Ferritin In The Body and The Reason You Need Copper To Stay Healthy  There are four different forms of ferritin in the body.  There are two principal flavors: heavy chain and light chain. Whenever you see the phrase heavy chain, it means copper is involved.  The light chain is expressed most extensively in the liver. The heavy chain is expressed chiefly in the heart.  There is also serum ferritin. Many people believe that ferritin in the blood accurately reflects what's happening inside cells. However, when you get serum ferritin high, you toxify the liver.   When you suppress copper in an animal's diet, iron will rip up in the liver. [32:20] We Need To Make 2.5 Million Red Blood Cells Every Single Second  70% of the iron in the body is in hemoglobin. There's a recycling system for iron called the reticular endothelial system.  Every second of every day, we need to make 2.5 million red blood cells. In 24 hours, we make 2 trillion red blood cells. It only takes 24 milligrams of iron to support that level of production.  [39:50] The Reason You Need More Cooper on A Ketogenic Diet  if you ferment sugar without oxygen, you get two units of energy.  If you oxidize the sugar, you're going to get around 34. If you oxidize fat, you'll get 140 units of energy.  90% of the energy comes out of the mitochondria. It does not take a lot of copper to run our body; copper creates energy.  If you're going to have a keto diet, you better have a healthy supply of copper. The metabolism of fats is copper-dependent.  You cannot digest copper in your diet if you don't have fat in your diet.  [48:10] Ascorbic Acid Is Destroying The Bioavailability of Copper  Ascorbic acid is not the whole food vitamin C complex. Ascorbic acid is a prooxidant, whereas vitamin C molecule complex is an antioxidant.  Ascorbic acid blocks the absorption of copper.  You need to know that ascorbic acid, vitamin D, and zinc destroy the bioavailability of copper.  When you destroy the bioavailability of copper, iron rises in the tissue.  [57:40] How You Can Get More Copper Into Your Body Through Diet  We need to lower the iron footprint in our bodies and increase the copper footprint. You need a mineral base in your water and in your foods.  Also, eat more real food that is organically grown. Plus, more fat will be essential.  Get more cooper from bee pollen, real vitamin C, and grass-fed beef liver.  There is much more beta carotene in the grass than in oranges and carrots.  Also, to deal with stress, you need to have more magnesium in your diet. Ditching the Complicated Diets Practicing Self-Love and Discovering Your Superpower Controlling Your Appetite and Cravings During Fasting Raise your hand if you've craved something sweet while fasting…we've all been there. In this episode, Ben and I discussed a few reasons behind these cravings and how to overcome them. Ben suggests that the cravings could be due to not being fat-adapted yet. Our brains may panic when glucose drops in a fasted state because the ketones are not present. It sends signals to our body to get some carbs that fulfill that desire. I recommend using fasting to pulse into a ketogenic state and then pulse out. What this means is that each time you go into your ketogenic state, it's healing something. If you get into that space, and suddenly your cravings go up, hang in there as long as you can and then dip back into food. And each time you go back, it gets easier and easier, just like going to the gym. It's important to note that every fast has its benefits, so if you did reach for that snack, there's no such thing as a failed fast. When you put yourself in a healing state, you can't fail. Understanding the type of craving you're having, whether it's a sugar craving, an adrenal issue, or a thyroid issue – you can support your body by adding more good fat and protein, eating more calories, or doing more 24-hour fasts.  Protein, Muscle, and Weight Loss If you're looking to build muscle and lose weight, you might want to pay attention to how much protein you're consuming. It's recommended to consume between one to one and a half grams of protein per pound of body weight. But if you fall short of meeting this requirement, using essential amino acids can be beneficial in filling these gaps. As we age, particularly during menopause, our bodies start to break down muscle to get amino acids if the adrenal glands are overworked. To combat this, protein cycling may be the answer – this involves consuming 30 grams of protein several times a day, combined with a lot of amino acids, to maintain muscle mass. Consuming more than 90 grams of protein risks turning it into glucose, which could impede weight loss. It's important to note that this applies to one meal, so consuming over 90 grams in a single setting is challenging for most people.  Can Keto Be Simple? Ditching the Complicated Diets  If you're tired of trying all the complicated diets and grabbing every new supplement that comes along, here's a solution: simplicity. In this episode, Ben and I discussed that often the natural healthcare system and biohacking world can be sometimes overcomplicated to the point where it's as confusing as the conventional healthcare system. For instance, if you want to get into ketosis, the best way is to start fasting. You don't have to count macros or purchase pre-packaged keto food. You can start simply by compressing your eating window and leaving long times for fasting to see if you can get into ketosis. Instead of counting macros, you can switch to nature's carbs. To simplify your approach, start with small changes that you can easily incorporate into your routine, like a daily walk or simple breathing exercises. It's important to not get bogged down in the details and to remember the ultimate goal of improving your health.  Practicing Self-Love and Discovering Your Superpower This year, I've promised myself to practice more self-love when I can and encourage you to find the time to give back and nurture yourself too. Ben shared in this episode that the daily self-love practices that help him feel grateful for his health and well-being are keeping a gratitude journal on his nightstand to taking breaks throughout the day to relax and read. To implement these ideas into your own life, you can start by incorporating simple self-love practices into your daily routine. This can include writing down a list of things you're grateful for each day, taking breaks to relax and do something you enjoy, or simply reminding yourself of the things you love about yourself. Gratitude has a profound way of helping shift your mindset and improve your overall well-being. This can involve writing down things you're grateful for, expressing gratitude to those around you, or simply taking a moment to appreciate the good things in your life.
