Middeldorp, MPH, PhD, of the University Medical Centre Groningen & University of Adelaide, is joined by Andrea Robinson, MSN, ACNP, from Riverside Methodist Hospital, OhioHealth, and Jeroen ML Hendriks, PhD, RN, of Maastricht University Medical Centre, in this episode. The AF-EduCare trial investigated whether targeted patient education (delivered in-person or online) could reduce unplanned cardiovascular events in patients with atrial fibrillation (AF). The study found that while education improved patient knowledge, medication adherence, and risk factor awareness, it did not significantly reduce cardiovascular hospitalizations, emergency visits, or mortality compared to standard care. However, subgroup analyses suggested that younger patients, those without heart failure, and asymptomatic AF patients might benefit more from in-person education. Ohio's mental healthcare needs are as acute as they've ever been, but Ohioans' struggle with mental health has also achieved a new and higher level of visibility, thanks to the persistent dedication of mental health advocates. Mental health struggles are touching every part of Ohio's population: children, adults, employers, and employees, and those of any age wrestling with addiction or thoughts of suicide. Ohio's leaders in the field have also made clear that mental health issues aren't merely healthcare issues, they're workforce development issues: the state's economy can't thrive if the mental health needs of its workers and their families aren't addressed and supported. Tellingly, a state official reported hearing most often from Ohio's mothers and sheriffs: two groups both dealing daily with the effects of fellow Ohioans struggling with mental health needs. There is clearly hope: national studies show that 3 of 4 people who experience addiction eventually recover, and up to 33% of people living with serious mental illness and 67% of adults with mild to moderate mental illness experience partial to full recovery over time. And Ohio is talking more about mental health than ever before, with the stigma of mental illness slowly evaporating, thanks to a changing public willingness to talk openly about mental illness. Some 70% of companies have also introduced or enhanced their well-being benefits for their employees. Optimal Health Series Human beings are wired for social connections, but when those connections are few, our health pays the price. The U.S. is suffering from a loneliness epidemic, and the toll it takes on our health is both enormous and measurable. In 2023, the U.S. Surgeon General declared loneliness and isolation a “public health emergency,” saying loneliness can increase the risk for premature death to levels comparable to smoking 15 cigarettes a day. The Surgeon General's startling advisory found that poor social connections can raise the risk of heart disease by 29%, increase the risk of stroke by 32%, and increase the risk of developing dementia in older adults by 50%. There is hope: leaders across Ohio are working to combat loneliness, to re-establish connections lost during the pandemic, and create new resources to reduce social isolation. Don Downs and Cara Bookman join Liberty Monks to discuss how corruption at the highest level ended the life of Brenda Downs. When Brenda Downs' doctors had no more medical interventions to save her from Covid-19, her family asked for doctors to administer ivermectin. When they refused, her family filed an affidavit seeking emergency and injunctive relief (Donald Downs v Ohio Health). A deal was struck with Ohio Health that if the Downs family agreed to a confidentiality agreement, Brenda would receive the medicine. Ohio Health was so worried about anyone finding out about this that they put a $1,000,000 penalty on each of the three members of the family. The family agreed under duress to save their wife and mother, but the ivermectin never came. In fact, the Downs family was ambushed and intimidated by doctors at the hospital, lying to them that ivermectin would harm Brenda. The following day, after still not having received the medicine, the family was informed that Brenda no longer had Covid-19. The family attorney, working behind the family's back, informed the judge of Brenda's new diagnosis and the judge pushed the agreement aside and never revisited it. When the family went to court to have the agreement set aside, Ohio Health sued them claiming their request to clear up the matter was frivolous. The family is facing a trial on this matter next year. Ohio Health killed their loved one and now outrageously claims it is the one that has been damaged. Imagine an institution that is so worried that one of its patients would be healed by ivermectin that they would go to extreme measures to ensure the patient dies without ever having taken the life-saving drug. That same institution also had the county judge in their back pocket, in addition to the Downs family's own attorney who worked in concert to protect the interests of Ohio Health. How many others throughout the country have perhaps had similar deals with hospitals? Silence in exchange for ivermectin! These institutions are responsible for millions of deaths worldwide and brave people, like Don and Cara, are trying to show how it was done. This isn't just a story of another patient on a ventilator who received Remdesivir – which both occurred. It's a story of corruption to the highest degree. The previous two episodes of Healthcare is Hard explored the “new normal” from the provider perspective with heads of strategy at an innovative regional health system, OhioHealth, and one of the nation's largest, CommonSpirit. For the third episode in this series, Keith Figlioli welcomes Humana's Chief Strategy and Corporate Development Officer, Dr. Vishal Agrawal, to shift the discussion towards payers and how they're adjusting to the post-COVID world.Today, Humana is one of the nation's largest health insurers that also provides payer-agnostic primary care, home health and pharmacy services through its CenterWell brand. However, delivering health care services is not new to Humana. The company's roots date back to the 1960s where it began as a nursing home business, and by the 1990s, it grew to become the nation's largest hospital company. While its care delivery assets eventually merged into HCA Healthcare as the company focused on insurance, this deep history provides a unique advantage as the industry continues its shift towards value.In this episode of Healthcare Is Hard, Dr. Agrawal shared some of the strategic priorities for Humana as he helps the organization navigate the new normal in healthcare. These include:Breaking the facility-centric model. The pandemic forced healthcare organizations to think differently about where and how they operate, accelerating the adoption digital and in-home solutions. For Humana, Dr. Agrawal describes how this was a focusing mechanism to help the company think about what it does best, and how it could enhance and integrate those pieces to deliver a more omni-channel approach to healthcare that people desire. This included doubling-down on the primary care, home health and pharmacy services Humana delivers through CenterWell and creating an integrated delivery system with a core focus of keeping people healthy. With this strategy, and a business especially well-suited for the Medicare Advantage payment model, Humana also announced in early 2023 that it would exit the commercial insurance market to focus on government-funded programs.Chronic care management. In a health system that was designed around the fee for service model and does a wonderful job treating conditions for sick care, Dr. Agrawal believes one of the biggest missing pieces is a similar focus on chronic care management. He's excited about the opportunity for building an integrated delivery network (IDN) that brings together high touch areas that can manage chronic health conditions more proactively, and is optimistic about the ability of the Medicare Advantage model to help push the industry in that direction.Removing waste from the system. Dr. Agrawal points out the extreme waste in the healthcare system – which by some estimates surpasses $1 trillion annually – and how eliminating that waste should be a key focus for everyone. He talks about opportunities for reducing avoidable admissions and ER visits by keeping people healthier and innovating in areas like nephrology, cardiology or oncology where much of the disease and health burden exists.Consumer choice & personalization. Healthcare is intrinsically personal and as a result, requires many different options for people to choose what's right for them. Dr. Agrawal describes how Humana is tailoring plans to meet these needs and uses the example of the Humana Honors Plan that it built in partnership with USAA around the capabilities of the Veterans Health Administration. He believes we'll see more customization in areas like this as healthcare becomes a more consumer-driven market.

Welcome to your weekly news update, we have 3 stories for you this week, New bill proposes banning Chinese drones for the Federal Government, DJI Matrice 3D leaks, and Zipline's new partnerships with Healthcare in Ohio. Make your voice heard: https://droneadvocacyalliance.com/?vvsrc=%2Fcampaigns%2F105982%2Frespond First up this week is a “new” bill that would prohibit the Federal Government from purchasing Chinese Drones, again… To be clear, this is a reintroduction of a past, failed bill. Florida Senator Rick Scott introduced the American Safety Drone Act in the Senate and now Representative Gallagher from Wisconsin is now doing the same in the House. This new bill is set to be announced this week, and would prohibit the federal government from using any tax dollars to purchase UAV equipment from China. The bill would require a federal report detailing the number of foreign commercial drones and “Covered” unmanned aircraft procured by federal departments. At the non-federal level, the bill would also bar local and state governments from purchasing Chinese drones using federal grants. Both Scott, in the past, and Gallagher now are trying to attach this to the National Defense Authorization Act, which is the only likely way for it to advance since it's highly unlikely to move on its own. Once again we see a country of origin ban rather than a set of comprehensive standards that any drone used for government purposes must conform to. The best way to fight this is to use the DAA's link in the description for instructions on reaching out to your representatives and oppose the National Defense Authorization Act. Next up this week are some DJI leaks, for the Matrice 3D. New pictures of the drone and new pictures of the rumored DJI Dock Mini. As we covered previously, the Matrice 3D appears to be a simplified DJI Mavic 3 Thermal with fixed arms, top-mounted SDK port and mounting, and built-in RTK. New leaks about the Mini Dock appears to shows an angled channel that the Matrice 3D would fit into in contrast to the flat landing pad of the existing DJI dock. The M3D also appears to be slightly larger than the Mavic 3 with much thicker arms and larger propellers. Pretty cool innovations coming from DJI, we'll see if this is released any time soon! Lastly this week is an announcement from Zipline, who has partnered with a number of healthcare providers in Ohio and Michigan. Zipline announced in September that they'd gotten FAA permission to operate BVLOS without visual observers, and now they appear to be ready to use that permission. Partnerships include the Cleveland Clinic, Michigan Medicine, Intermountain Healthcare, and Ohio Health. With these new partnerships, Zipline aims to deliver prescriptions directly to a home's front door by 2025. The Zipline P2 system cruises at 70 mph, an altitude of 300 feet and uses a small “droid” to drop packages in places where the main aircraft can't get to. Pretty cool tech! That's it! Dr. Meredith Smith received her doctorate in Physical Therapy from Elon University and her Bachelor's degree from the College of Charleston in Exercise Science. She completed a neurologic residency program with OhioHealth and Ohio University. Meredith is a Board Certified Neurologic and Geriatric Clinical Specialist. She works in the outpatient setting treating individuals with neurologic and/or vestibular diagnoses. Meredith is also an adjunct professor with Bowling Green State University's DPT program in the neurologic and aging adult classes. He points to health system megamergers and the difficulties those organizations have had driving ROI, and discusses how the market is rewarding systems with greater focus.Lumpy operating margins. While volumes are recovering in many places around the country, margins are still inconsistent for most systems and the rebound is not enough to avoid making tough decisions. With margins that are less than what's required to fund growth and capital for many health systems, Michael foresees the need for major decisions around cost savings and efficiency that will be required for survival.Spinning up new business lines quickly. The healthcare industry is not historically known for quickly or successfully establishing new business lines where it lacks experience. Michael believes this will have to change in the new normal and says one path to success is through partnerships. He discusses examples of OhioHealth's partnerships with ChenMed, Privia and Devoted Health and how they helped deploy new capabilities far more quickly than doing it alone.Inside innovation ecosystems. Trying to confirm if you have multiple sclerosis can be a frustrating process. Dealing with a new diagnosis a MS can be overwhelming. Julian Gamboa and Dr. Jakai Nolan shed light on a positive path forward living with multiple sclerosis. Julian Gamboa shares his first symptoms of multiple sclerosis including spinning sensation and double vision. He had substantial obstacles and setbacks in being diagnosed including a prolonged hospitalization. After being in a very low place, Julian rallied with the support of others including his family. He conveys how he eventually obtained superb MS care and utilizing his social media guru skills to help others being diagnosed with MS. Dr. Jakai Nolan opens up about her own unique insights on being diagnosed with multiple sclerosis since she is both a neurologist specializing in multiple sclerosis and a person living with MS. As she shares her own personal story, she reviews key symptoms suggestive of MS and the importance of advocating for prompt evaluation to prevent a delay in diagnosis and treatment. Dr. Nolan covers treatment selection, mental health concerns and lifestyle modifications for someone recently diagnosed.
