A career change can often come with uncertainty for the future, but for Christina Morgan, Clinical Practice Transformation Advisor at Carevive, it gave her a voice to advocate for cancer patients helping them receive the highest quality of care. Oncology and Technology host Tyler Kern sat with Morgan to discuss her transition from the field of nursing to clinical practice. Morgan's journey began after she graduated from nursing school. “I decided to go right into oncology, and I worked as an oncology nurse at a medical center for a few years. We handled everything from patients who were initially diagnosed, to handling symptom management and even end of life care,” she said. “It really was a spectrum working with the cancer oncology population.” Wanting to educate herself further, Morgan's path led her to the University of Central Florida (UCF) where she worked as a nurse practitioner in neuro-oncology helping lymphoma patients, and people with brain tumors and other neurologic disorders. “I realized I really wanted to provide care in a way that was a little bit more advanced than what I was doing. I want to be able to diagnose and treat people, to work with physicians, and to do things on a higher level, which going to UCF allowed me to do,” Morgan said. Now as a clinical practice transformation advisor at Carevive, her background allows her to have a clinical perspective. “Being in the medical community, not only over the decade but also being with the pandemic over the last two years, has shown me how much cancer patients need the right care and the right people advocating on their behalf,” she said. Cancer never hits anyone at the right time buthaving the resources and information to make decisions is key, Morgan explained. “I think there is a correlation that I can bring, not only to Carevive but to patients because I understand what they're going through and I think,at the end of the day,that's all we ever want to do is advocate for patients that are going through difficult and challenging situations,” she said. “But working with a companythat is making the life of a cancer patient so much easier, means a lot. It takes work and we do have to make the effort and at Carevive I know we're doing that.”

In this health care podcast, I speak with Steve Blumberg, VP of practice transformation for GuideWell Health, a subsidiary of GuideWell. How’s this for an interesting career trajectory? Steve spent the last decade working on population health and value-based care delivery ... on the provider side. Recently, he transferred over to the payer side, working for GuideWell Health, which is the health services arm of GuideWell, which is part of a family of companies including Florida Blue. So, a payer, in other words. I wanted to find out a bunch of things from Steve, but the main one is this: How do—if they, in fact, do—payviders improve care for patients? Or what does it take for an organizational structure to drive Triple Aim results? Going into this conversation, here is what I was thinking about: Payviders have access to longitudinal data (potentially) that siloed entities will certainly not. They also have a goal to keep care affordable in a really real way, especially if the patient/member/client is on the ACA (Affordable Care Act) marketplace and shopping for premiums. My big concern with payviders, though, is whether they’re an “HMO in drag,” as they say. On the other hand, payers and providers, in the most cynical sense, have wildly divergent goals. Search #medtwitter any day of the week—you will find a galaxy of tweets wherein doctors complain about payers—to just get a tiny sense of those wildly divergent goals. Do separate payers working with separate providers offer a kind of check and balance? A historical knock on this hypothesis is the inarguably crappy outcomes for chronic conditions that US patients have the privilege of paying comparatively ridiculous sums for. I couldn’t tell you whether those crappy outcomes are a result of the separateness of payers and providers or some other factor, but so it is. Here’s the short version of one of Steve’s main points: It’s not about control. It’s about connection. It’s about being able to connect with patients over their continuum of care. It’s also about how consumers and employers are increasingly trading out choice and broad networks for an assurance of quality. You can learn more at guidewell.com. Steven Blumberg serves as vice president, practice transformation, for GuideWell Health. In this role, he is responsible for developing and implementing strategies for the further establishment of a high-quality, economically effective clinical system across Florida. He also provides guidance on value-based care and population health models. Prior to joining GuideWell in June 2019, Blumberg served as vice president for value-based care at Baptist Health South Florida, where he led the strategy and implementation for Baptist’s population health and value-based care efforts. Prior to that, he was senior vice president and executive director of AtlantiCare Health Solutions, the New Jersey division of the Geisinger Health System, where he was responsible for population health, the organization’s provider physician group, and home care and hospice continuum services. Earlier in his career, he was chief planning and business development officer at UHealth–The University of Miami Health System. Blumberg also held leadership roles at UF Health–Shands Healthcare and Baptist Health Jacksonville. Blumberg has been active in community and professional organizations, including serving on the boards of the Ronald McDonald House, Community Hospice, and the Northeast Florida Health Planning Council. He has also served nationally on the Premier Population Health Steering Group and on the National Institute of Standards and Technology’s Baldrige Board of Examiners. Blumberg holds a bachelor’s degree in business administration and marketing from the University of Florida and a Master of Business Administration from Florida State University. Siloam Health faced the hurdle of improving efficiency and effectiveness of the patient flow process. Claire Reagan, Nurse Manager at Siloam Health in Nashville, TN, joined Dr. Thomas Spain, primary care physician and Director of Practice Transformation at the Midsouth Practice Transformation Network, and Kirkland Ahern-Jones, Director of Operations for the Midsouth PTN, to discuss the effort the practice made to implement team huddles and the difference huddles made in the practice’s workflow and experience as part of their TCPI work. By initiating a clinical team huddle, Siloam Health moved to the next level of team based care. Taking the time to establish the huddle habit and to encourage providers to collaborate about patient care, Siloam Health improved both the team and the patients’ experiences and further developed relationships between patients and the entire team. Team huddles led to a 20% improvement in colon cancer screening over baseline, moving the practice performance above the national MIPS benchmark in just 18 months. The practice improved dramatically on mammogram screenings, showed a 15% over baseline in administration of Pneumovax to elderly and diabetic populations, and better incorporated behavioral health in patient care. With the implementation of daily huddles, Siloam Health is able to work as a more organized team to serve each patient’s unique needs. The Best Practices podcast explores the exemplary work done by practices in the MidSouth Practice Transformation Network over the last three years. Throughout the episodes, the podcast explores quality improvement efforts, but what are these continuous process improvement methods? Kirkland Ahern-Jones, Director of Operations for the Midsouth PTN, interviews her cohost, Dr. Thomas Spain, primary care physician and Director of Practice Transformation at the Midsouth Practice Transformation Network, to explore the continuous process improvement methods and why they are needed. The efforts of the MidSouth PTN were underpinned by the knowledge that to thrive in a value-based payment environment, practices needed to learn how to continually improve and how to use data to drive that improvement. There are a number of continuous process improvement methods, but the two most common in healthcare are Lean Process Improvement and the IHI Model for Improvement. Each method includes preparation practices need to make before they undertake improvement and before they begin making changes. Continuous process improvement methods are essential when striving to enact change, because these methods provide a feedback cycle that enables practices to learn from the changes being put in place and to allow what they have learned to influence the changes made in the future. The IHI Model for Improvement is simple and universal. The Institute for Healthcare Improvement provides a wealth of resources online, and their Model for Improvement begins with two parts. The first part is a trio of questions that aid a practice in setting aims, establishing measures, and selecting changes. A practice’s aim should be quantifiable; it should be specific, measurable, attainable, relevant, and time-frame bound. How a practice will measure to know a change is an improvement is a step that is often left out, but it is pivotal to the use of data to drive improvement. The second part of the first step of the IHI Model for Improvement is the Plan-Do-Study-Act (PDSA) method for testing change. For those interested in quality improvement, we invite you to explore the continuous process improvement methods. Key Lessons: Culture is the characteristics, knowledge, and beliefs of a group of people including their shared language, religious/spiritual beliefs, habits, and values.   Culture impact beliefs about diseases, medications, and healthcare. Many patients are reluctant to tell healthcare providers about their culturally-related health behaviors for fear of being judged or may believe such information is irrelevant. Behaviors and beliefs, regardless of source, can impact health outcomes and can augment, detract, or have no impact on the recommended treatment plan. Openly discussing beliefs and behaviors is critical.  Supporting patient decisions based on their beliefs builds trust. Communicating in the patient's preferred language is mandated by law. Use trained interpreters. Have written materials available in the patient's preferred language. Cultural awareness and moving toward cultural proficiency makes good business sense as you are better able to understand the needs of your patients.

During the converation, Quiring explained how her practice modified the depression and distress screening tools for the different populations it treated. For instance, it changed the language based on what was more acceptable to Native Americans receiving treatment. She also explained the various programs the practice offers to patients, such as yoga, an art workshop, and meal preparation assistance, as well as several support groups.

In its quest to transform 70 to 80 percent of its physician practices to a patient-centered medical home (PCMH) over the next three years, WellPoint has adopted a "meet the practices where they are" philosophy, reports Julie Schilz, director of care delivery transformation for WellPoint. Each practice is at a different place in the transformation effort and requires specialized supports, she adds. Smoothing the transformation rollout is the simultaneous participation of 500 WellPoint practices in CMS's Comprehensive Primary Care (CPC) program, whose goals dovetail with key PCMH principles --- as though WellPoint had another partner in its transformation initiative, Schilz notes. Just as important as practice support is transparency with health plan members, Schilz adds, especially when it comes to explaining the concept of the medical home neighborhood --- where care coordination is a collaboration between primary care and the specialist. Ms. Schilz shared the key features of WellPoint's transformation initiative, including results from its pilot program that have led to a system-wide rollout, during an October 24, 2013 webinar, "Aligning Value-Based Reimbursement with Physician Practice Transformation."

The Emory Clinic has had the benefit of electronic medical record capabilities for close to 20 years. With the implementation of our current EMR in 2005, certain tools have been adopted quickly, while others have been slower to be incorporated in the day to day work flow of the average practice. As Meaningful Use of Electronic Health Records became an integral part of the US Healthcare Reform initiatives, leadership of The Emory Clinic began to understand that constructive incorporation of the required program elements into daily practice workflow would require a very intentional redesign of that workflow. Given the size of the practice (close to 2000 providers) and its geographic dispersion, coupled with the integration with Emory Healthcare’s multiple hospitals, the team knew that this redesign would take tremendous coordination, planning and execution of process improvement methodology. The faculty practice operations team joined forces with a local consulting team that utilized a very disciplined process improvement method to redesign, standardize and implement a practice wide workflow that created consistent experiences for patients, allows providers to qualify for Meaningful Use achievement, and set the stage for future workflow improvements. The leadership team from Emory will share the background, decision making, strategic planning and execution methods used during this project. Redesigning a practice into one that is more efficient and patient-centered begins with a readiness assessment tool and goal-setting with key organization members, says Dr. John Michos, medical director of the Virginia Health Quality Center. The size of the practice can affect the approach to transformation. Smaller practices may find it easier to implement change, while larger practices may need to launch innovations on a small scale, then foster the spread of that success to other departments. Getting health IT and its associated efficiencies into a physician practice will help transform it into a medical home, but so will mapping communications processes for patient interactions such as phone calls, discharges and other situations. Dr. Michos and Dr. Lonnie E.
