Fisiopatología en pocas palabras: Trombo grande en circulación pulmonar → aumento de poscarga del ventrículo derecho → falla del VD → colapso hemodinámico. Por qué es tan letal: Deterioro muy rápido, ventana terapéutica corta. Frecuentemente se presenta como paro fuera del hospital. Conectar con ACLS: La EP masiva está dentro de las "T" (tromboembolismo) en las causas reversibles del paro. Las guías ACLS contemplan el uso de trombolíticos cuando se sospecha fuertemente EP como causa del paro. ¿Cómo se ve clínicamente un paciente con EP de alto riesgo? Disnea súbita, dolor torácico, síncope, hipotensión, antecedentes de riesgo trombótico. Resumen del estudio de Harjola et al. Objetivo principal del estudio Explorar supervivencia y complicaciones hemorrágicas del uso de trombolíticos prehospitalarios para embolia pulmonar de alto riesgo. Diseño Datos de EMS del área metropolitana de Helsinki + hospital universitario. Periodo aproximado: 2007–2019. Inclusión: Pacientes con EP de alto riesgo sospechada clínicamente. Tratados con fibrinolisis intravenosa prehospitalaria. Diagnóstico de EP confirmado posteriormente por imagen o autopsia. Grupo comparador: Pacientes con EP de alto riesgo que no recibieron fibrinólisis prehospitalaria. Resultados clave Total de pacientes con EP de alto riesgo: 60. Grupo con trombolíticos prehospitalarios para embolia pulmonar: n = 23. 44% mujeres. Edad media: alrededor de 57 años. 74% se presentaron en paro cardiaco. 26% en shock obstructivo. Mortalidad: Mortalidad prehospitalaria aproximada: 35%. Mortalidad intrahospitalaria: alrededor de 27% de los que llegaron vivos. Mortalidad total combinada: cerca de 52%. Todas las muertes en este grupo fueron en pacientes que llegaron en paro cardiaco. Complicaciones: 2 pacientes con sangrado mayor. Ningún sangrado fatal. Comentario para desarrollar: Es un estudio observacional, con n pequeño, no podemos concluir causalidad, pero sí hay "señales" interesantes de posible beneficio. ¿Qué nos dice realmente este estudio? Mensajes principales La EP de alto riesgo fuera del hospital tiene una mortalidad muy alta aun con intervenciones agresivas. En este contexto crítico, los trombolíticos prehospitalarios para embolia pulmonar: Parecen relativamente seguros (pocas hemorragias mayores, ninguna fatal). Podrían ofrecer un beneficio en supervivencia, especialmente en pacientes seleccionados. Limitaciones para mencionar Serie de casos; no es ensayo aleatorizado. Número pequeño de pacientes trombolizados. Posible sesgo de selección: Pacientes más jóvenes y potencialmente con menos comorbilidades recibieron trombólisis. No responde preguntas como: Detalle exacto del protocolo. Diferencias entre equipos. Tiempos exactos desde el colapso hasta la trombólisis. Falta de entrenamiento específico: No todos se sienten cómodos con indicaciones, contraindicaciones, dosis. Cómo ayuda ACLS aquí ACLS bien aprendido: Te obliga a pensar en H y T, no solo en adrenalina y ciclos. Te muestra dónde se colocan los trombolíticos prehospitalarios para embolia pulmonar dentro del algoritmo. Te entrena para liderar un equipo y tomar decisiones bajo presión. Después de varios ciclos recupera pulso. Llega vivo al hospital, se confirma EP por imagen y sobrevive. Puntos a resaltar Valor de: reconocer el patrón clínico, tener protocolos, estar entrenado en ACLS. Conectar con la serie de Helsinki: "Son justamente este tipo de pacientes los que aparecen en la serie: altísimo riesgo, pero con posibilidad real de supervivencia si somos agresivos." Desarrollar protocolos claros Criterios de inclusión y exclusión. Algoritmo que integre ACLS y trombólisis. Entrenamiento formal No basta con escribir el protocolo; hay que entrenarlo en simulación. Cursos ACLS con escenarios específicos de EP. Simulaciones y revisión de casos Simulacros periódicos con roles definidos. Morbimortalidad / debriefing de casos reales o simulados. J.P. is also a pro team athlete and spokesperson for Origin Maine and Jocko Fuel, an American clothing and supplement company. J.P. has a signature Energy Drink flavor "Sour Apple Sniper" with Jocko Fuel. Jeremiah spent nearly a decade in the SEAL Teams with three combat deployments. Sent to the violent terrorist stronghold of Ar Ramadi, Iraq in 2006 with SEAL Team Three's Task Unit Bruiser, J.P. served as point man, machine gunner, and lead sniper for Delta Platoon opposite the American Sniper, Chris Kyle, who was in Charlie Platoon. For his leadership and courage under fire, JP was awarded a Silver Star, 2 Bronze Stars with Valor and the Army Commendation Medal with Valor helping Task Unit Bruiser to become the most highly decorated special operations unit of the Iraq War. He worked closely with SEAL Officers Jocko Willink, his Task Unit Commander, and Leif Babin, and was the driving force on many of the daring combat operations Jocko and Leif wrote about in Extreme Ownership. Upon his return, J.P. again worked directly for Jocko as a training instructor at Naval Special Warfare Group One Training Detachment, where he orchestrated realistic and challenging training scenarios for Special Operations Urban Combat training and Close Quarters Combat training to better prepare SEAL units for the real-world battlefield. He also served as a Combatives Instructor, Marksmanship Instructor and earned his Master Trainer Specialist qualification while helping Jocko rebuild and enhance these training programs into the highly effective platforms they are today. J.P. brings exceptional experience and frontline leadership perspective from the winning mindset and culture of Task Unit Bruiser.

His first 10 years were at Central Division, where he gained experience in patrol, SWAT, FTO, crime suppression, undercover narcotics, and acting sergeant roles. He's been part of the Air Support Unit for over 13 years, progressing from tactical flight officer (TFO) to pilot, then to TFO trainer, flight instructor, and now chief pilot. He also served as a Combatives Instructor, Marksmanship Instructor and earned his Master Trainer Specialist qualification while helping Jocko rebuild and enhance these training programs into the highly effective platforms they are today. J.P. brings exceptional experience and frontline leadership perspective from the winning mindset and culture of Task Unit Bruiser.

Mike Durant is a retired U.S. Army Chief Warrant Officer 4 and Master Black Hawk pilot with the 160th Special Operations Aviation Regiment (Night Stalkers). Inspired by his father, a First Sergeant in the Army National Guard, and a family friend's helicopter flight, Durant enlisted in 1979. After studying Spanish at the Defense Language Institute and serving as a voice intercept operator in Panama, he graduated from flight school at Fort Rucker, Alabama, becoming a Warrant Officer in 1983. He flew over 150 medical evacuation missions in South Korea with the 377th Medical Evacuation Company and later served as an instructor pilot with the 101st Aviation Battalion. Joining the elite 160th SOAR in 1988, Durant flew in Operations Prime Chance, Just Cause, Desert Storm, and Restore Hope. During the 1993 Battle of Mogadishu (Operation Gothic Serpent), his MH-60 Black Hawk was shot down, leaving him severely injured and held captive by Somali militia for 11 days. Despite doctors' doubts, he recovered, ran the 1995 Marine Corps Marathon, and returned to duty, retiring in 2001. Mike's awards include: Distinguished Service Medal, Distinguished Flying Cross (second award), Bronze Star w/ Valor device, Purple Heart, Meritorious Service Medal, Air Medal w/ Valor device (third award), Army Commendation Medal (fourth award), Joint Service Achievement Medal, Army Achievement Medal, Prisoner of War Medal, Army Good Conduct Medal, National Defense Service Medal, Armed Forces Expeditionary Medal with Bronze Arrowhead Device (second award), Southwest Asia Service Medal w/ Bronze Service Star, Army Service Ribbon, Overseas Service Ribbon (2nd Award), United Nations Medal, United Nations Medal-Operations in Somalia, Kuwait Liberation Medal-Government of Saudi Arabia, Kuwait Liberation Medal-Government of Kuwait, Master Aviator Badge, and Air Assault Badge. In 2008, Durant founded Pinnacle Solutions in Huntsville, Alabama, a defense contracting firm specializing in military training simulators and veteran employment. He co-authored In the Company of Heroes, focusing on survival and leadership. He also led veterans' efforts for George W. Bush's 2004 and John McCain's 2008 presidential campaigns. (6:22) • I was about 95lbs and 5' tall at 15, and that recruiter took one look at me, and was like: “You are not what the SEALS are looking for.” (7:40) • There are way more operational opportunities for enlisted SEALS than there are for officers. (20:44) • Seaman to Admiral takes individuals who don't have a degree. They send them to school. It's an accelerated program. You're part of the ROTC program. You commission out of that and you come back as a SEAL officer. (24:02) • We were all bleeding so bad, and it created this mist in the helicopter of blood. The entire inside of the helicopter was coated in blood. (43:06) • My recovery time took 4 years and 40 surgeries to put me back together. (43:38) • Be thankful for the hardships you have in life and for hard things that happen because I think they set you up for success in other things. (43:48) • Me failing as a leader was probably the hardest thing I've ever had to come back from. (43:56) • Lead yourself, Lead others, Lead always. (44:39) • [Marcus] When he was in the hospital, he wrote on a bright orange piece of paper and tacked it on the door. And he wrote this letter saying “Don't come in here feeling sorry for me.
