Though not your typical "ace flyer", his organizational abilities and expert handling of crises - placed him at critical junctures in our 20th century military history. You'll hear how he was a key participant in many historical events, spanning pre-WW2 through the Cold War, and finishing his career as the 2nd commander of NORAD. His fascinating life will be presented by Dr. Brian Laslie, currently the Command Historian at the United State Air Force Academy. You will also hear about Kuter's wife, Ethel Lyddon Kuter, who was both a true partner and accomplished in her own right. Jacaranda FM — Gender-based violence is devastating South Africa. And while perpetrators roam the streets without conviction, their victims are burdened with the experience long after it’s over. Leilani Kuter was raped and left for dead when she was 18 years old. Twenty-seven years later, she’s still coming to terms with it. “Humans are not made to carry this pain with us,” Kuter says. She recently completed a 729-kilometre walk over 27 days to empower herself and others. “You cannot run from trauma, but you can run to prevent it,” Kuter says. On her journey, she wore a yellow shirt – the colour her attacker was wearing at the time of the assault. It symbolised Kuter’s strength and her decision to take back her power. What was once a trigger no longer hurts her, and Kuter aims to help others feel that liberation. “I want to be a voice for survivors and make a difference,” she says. Kuter has since raised over R200 000 to purchase comfort packs for people when they report an assault, as well as sponsor counselling and rehabilitation sessions. “As survivors, we need to unite,” she says. “We are not our trauma.” We all have the power within us to put our foot down, shed the pain that remains from our past, and move forward. Show notes By Hitomi Hosoya, MD, PhD, resident in the department of internal medicine, Pennsylvania Hospital of the University of Pennsylvania Health System, Philadelphia Causes of idiopathic thrombocytopenic purpura : Secondary causes include chronic lymphocytic leukemia, lymphoma, and autoimmune conditions and account for 30%-50% of ITP. Idiopathic ITP is associated with pregnancy, post-viral infection, and vaccination and accounts for 50%-75% of ITP. Pathophysiology: Antibodies attach to platelets, which are cleared by the liver or spleen (increased consumption). T cells or B cells attack bone marrow (reduced production). When to treat: Major bleeding. Platelet count less than 20 x 109/L. Initial treatment: Corticosteroids (dexamethasone or prednisone). These drugs should not be used for more than 6 weeks. Intravenous immunoglobulin is appropriate with major bleeding or when a patient requires immediate surgery. Subsequent treatment: Platelet count less than 20 x 109/L during steroid taper. Rituximab (50%-70% response rate). Thrombopoietin receptor agonist (90% response rate); eltrombopag (oral) or romiplostim (subcutaneous).
