Miho Fukui, MD, JACC: Case Reports Associate Editor, is joined by author Ree Lu, MD, discussing this study from Cho et al presented at ACC.25 and published in JACC: Case Reports. Intravenous leiomyomatosis (IVL) is a rare benign uterine growth that extends into the venous system. This case describes a 48-year-old female who presented with 1 month of chest pain and dyspnea on exertion. She was found to have a right atrial mass that transiently crossed into the right ventricle with associated moderate tricuspid regurgitation. Computed tomography revealed a uterine mass with contiguous intravascular extension through the inferior vena cava terminating in the right atrium. This case of symptomatic tricuspid valve obstruction due to IVL illustrates the importance of maintaining preload. Chest and abdominal imaging may be necessary to identify the source of the mass and ensure appropriate surgical planning.

Here are the truths about IVIG. I'm Annette Leonard of https://www.annetteleonard.com find me on Instagram https://www.instagram.com/theannetteleonard IVIG What is it? A blood product that's sold as a pharmaceutical product. IGg is a major component in our immune system. Intravenous immunoglobulin is used to treat a wide range of illnesses and conditions to bolster the immune system. Who is it for? For those of us with autoimmune diseases it is thought to overwhelm our immune systems and therefore, shut down our over-active immune systems. How is it administered? Are premeds required? How long does a dose last? How many donors are required for a dose of IVIG? What does IVIG therapy cost? Have you tried IVIG? For what condition? Has it worked? Did you have side effects? If a substance is a part of nature then it is very unlikely to become a part of mainstream medicine. If it can't be patented, if it can't be turned into a highly profitable product, then it's not going to see the light of day. Such is the fate of ozone! Remarkably healing, incredibly safe, capable of killing bacteria, fungi, viruses and mold, regenerative to joints and stimulating to cellular energy, there's much to love about ozone. Medical grade ozone gas is an important part of antiaging medicine. We use it to rejuvenate joints. Intravenous ozone can stimulate the immune system and support tissue oxygenation. Gently applied Rectal ozone can support patients fighting Crohn's, ulcerative colitis, diverticulitis and other chronic inflammatory bowel conditions. Ozone helps the body to heal itself and can spare you the trauma of surgery, or the toxicity of drugs.

"We've got to get with the program and start exposing some of these hidden stresses and get people screened for them. They're not being screened right now at all." - Douglas Mulhall Douglas Mulhall is an author and expert in the field of longevity and stress management. He is the author of the bestselling book "Discovering the Nature of Longevity: Restoring the Heart and Body by Targeting Hidden Stress," which has been recommended by the American Institute of Stress. With a background in developing certifications and standards for healthy products, Mulhall has a deep understanding of the impact of hidden stresses, such as heavy metals, on our health. He is passionate about educating people on the importance of addressing these hidden stresses and providing practical solutions for improving overall well-being. Episode Summary: In this episode, host Jana Short interviews Douglas Mulhall, author of "Discovering the Nature of Longevity," about the impact of hidden stresses on our health and well-being. Mulhall shares his insights on the role of heavy metals in causing heart disease and other health issues, and discusses the importance of addressing these hidden stresses. He highlights the effectiveness of intravenous chelation therapy in removing heavy metals from the body and restoring circulation. Mulhall also emphasizes the need for increased awareness and regulation of heavy metals in food and water, particularly for children who are more vulnerable to their effects. The episode provides valuable information and practical tips for reducing hidden stresses and improving overall health. Key Takeaways: Heavy metals, such as lead, cadmium, and arsenic, are a hidden stress that can have a significant impact on our health, including heart disease, behavioral issues, and neurological disorders. Intravenous chelation therapy is an effective treatment for removing heavy metals from the body and restoring circulation, as demonstrated by clinical trials. Children are particularly vulnerable to the effects of heavy metals, and it is crucial to take steps to reduce their exposure and protect their health. The FDA does not currently require manufacturers to test for heavy metals in food, highlighting the need for increased regulation and transparency. Total stress load, a new concept in medicine, takes into account the combined effects of various hidden stresses on our health and can help guide treatment and prevention strategies. Contributor: Ricky Dhaliwal, MD Educational Pearls: What are DKA and HHS? DKA (Diabetic Ketoacidosis) and HHS (Hyperosmolar Hyperglycemic State) are both acute hyperglycemic states. DKA More common in type 1 diabetes. Triggered by decreased circulating insulin. The body needs energy but cannot use glucose because it can't get it into the cells. This leads to increased metabolism of free fatty acids and the increased production of ketones. The buildup of ketones causes acidosis. The kidneys attempt to compensate for the acidosis by increasing diuresis. These patients present as dry and altered, with sweet-smelling breath and Kussmaul (fast and deep) respirations. HSS More common in type 2 diabetes. In this condition there is still enough circulating insulin to avoid the breakdown of fats for energy but not enough insulin to prevent hyperglycemia. Serum glucose levels are very high – around 600 to 1200 mg/dl. Also presents similarly to DKA with the patient being dry and altered. Important labs to monitor Serum glucose Potassium Phosphorus Magnesium Anion gap (Na - Cl - HCO3) Renal function (Creatinine and BUN) ABG/VBG for pH Urinalysis and urine ketones by dipstick Treatment Identify the cause, i.e. Has the patient stopped taking their insulin? Aggressive hydration with isotonic fluids. Normal Saline (NS) vs Lactated Ringers (LR)? LR might resolve the DKA/HHS faster with less risk of hypernatremia. Should you bolus with insulin? No, just start a drip. 0.1-0.14 units per kg of insulin. Make sure you have your potassium back before starting insulin as the insulin can shift the potassium into the cells and lead to dangerous hypokalemia. Should you treat hyponatremia? Make sure to correct for hyperglycemia before treating. This artificially depresses the sodium. Should you give bicarb? Replace if the pH < 6.9. Otherwise, it won't do anything to help. Don't intubate, if the patient is breathing fast it is because they are compensating for their acidosis. References Andrade-Castellanos, C. A., Colunga-Lozano, L.
