Josh Umbehr is the CEO of Atlas MD and a family physician in direct primary care. With over ten years of clinical education experience, he has many useful pearls on how to approach primary care education. He also covers a lot of business negotiating and business of medicine education material in their clinical rotations. Healthcare revolves around money and physicians should be well educated in many of the business aspects that they may be involved in. In primary care, there are standard inpatient costs and times associated with them. However, with a non-insurance-based practice, physicians and students may have more time for patient discussion and forming stronger connections. There are often richer experiences available to students by running many of their own labs and procedures in comparison to insurance-based primary care clinics. The model allows for more flexibility, according to practitioners. Strong preceptors are talented at phrasing their questions in the right. They take the time to educate their medical students and avoid “read my mind” questions. They help guide the learner past the textbook and how to find answers on their own. Clinical preceptors must also avoid using their learners as work-horses in order to maintain their busy schedules. There are many potential learning experiences that can occur each day and knowing which ones will benefit the student the most comes from experience. Although medical students must learn proper filling out and filing paperwork, this is not always a great learning opportunity. Josh also states that an added benefit to learners and patients in the direct care model is that of time for decision making. There is much more time to decide if a patient should be referred out or if this is something that can be handled in-house and potentially lead to substantial savings. The other end of this is that it can be anxiety-provoking at times when patients are in a gray area. However, the decision options can be discussed more thoroughly in a DPC setting than in many insurance-based settings. Dr. Umbehr recommends that students focus on a few key principles when they are on clinical rotations in primary care. Professionalism and leading by example is not only the ethical thing to do but also builds trust and helps to lead patients to be more honest with healthcare professionals. Keeping an open mind can greatly benefit learners. This can allow them to find answers that may not be top of the list or solutions outside the box. This can also allow them to listen and not simply “wait for their turn to talk.” Developing technology skills and communications also greatly impact the efficiency of our clinical training and should be a top consideration.

  As direct primary care practices have grown over the last few years, there is now an expanding commercial marketplace for products and consultants to help DPC practices find success. It is a sign that people outside of medicine are beginning to recognize that DPC is a force to be reckoned with. As such, all sorts of people are entering a market to provide services and help while most don't understand what it takes to be successful in running a direct primary care clinic.   My guest today is Dr. Josh Umbehr, someone who definitely knows how to make money as a DPC doctor and provide his patients with value. Part of his value is obtained through leveraging dispensing medications at a fraction of the cost of local or chain pharmacies. He has also created a large niche in the DPC world by creating the Atlas MD electronic record system designed especially for DPC practices. Unlike more electronic medical record systems in existence, it is designed to function as an aid for physicians and not collecting demographic or billing information.   We also discussed at some length the current battle to allow patients to use their HSAs to pay for direct primary care. What many expected would be an easy alteration in the IRS code has become a fight for some of the life blood of DPC practices. Currently, what is allowed under HSA rules is pretty lax except for membership plans for health. The probable intent was to prevent people from using their HSA for gym memberships. However, DPC got swept up in that too.   Unfortunately, the fix has been worse than the disease and now the legislation to allow for DPCs will set price ceilings on what doctors can charge. These price fixes could potentially stunt the growth of the DPC movement right as it begins to pick up steam. And to make matters worse, the American Academy of Family Practice has weighed in on the fight over HB 3708 in opposition to their own colleagues in the DPC movement.   Dr. Josh Umbehr is a partner at the direct primary care parctice Atlas MD and operates the product line sold through Atlas. The Wichita Business Journal started writing about Josh Umbehr and his Atlas MD medical practice even before it was opened. Seven years later Umbehr's practice is growing, his idea is spreading and he's getting national and international media attention. HIs idea was to create a physician practice in which patients pay a monthly fee and have no copay. They have access to their doctor through text and email on top of making phone calls. They can send pictures of their injuries or maladies to their doctors or even video. And physicians make house calls. That idea has had some appeal throughout the country for patients, policy makers and physicians, especially as we try to figure out our health care system. And he and his co-founder have been written up by national magazines, newspapers and TV shows. He's lost count of how many times Sean Hannity has invited him to appear on his Fox News program.
