Webberley welcome to our GenderGP podcast, where we will be discussing some of the issues affecting the trans and nonbinary community in the world today. Together with my co-host Marianne Oaks, a trans woman, herself, and our head of therapy.   Marianne Oakes: Good afternoon everybody, and welcome to the GenderGP podcast. Today, we're going to try and answer some questions that we frequently get asked by our patient groups and by new people coming to the service, and to help me do that, I've got the wonderful Katie. Instead of me trying to introduce her because she is a multi-talented person, I'm going to ask Katie to introduce herself. So welcome Katie, and would you like to just tell the listener a little bit about yourself? Katie Tiplady-Startin: Thank you, Marianne. I'm not entirely sure I'm going to live up to that introduction, but I'll do my best. So I'm Katie. I started working with Helen Webberley way back in the day. I was the first person that she brought into GenderGP. Some of you will have seen my name on messages and information on the website over the years. But now my role is Chief Operating Officer at GenderGP, and that actually encompasses all of the patient services. So the wellbeing teams, the medical teams, the, the pathway teams that most people would have had some interaction with if you're, if you're with the service. And my day-to-day role is basically to keep everything ticking along, making sure that it's still working well, and if we need to change anything that, you know, we do that quickly and well, so hopefully I can answer some of these frequently asked questions, but I think the key to say is that GenderGP is moving with what's needed. So some of the answers that you might hear from this episode might be different if you'd have looked at the website two, three years ago. Marianne Oakes: I just wanted to add as well, Katie, uh so the listener understands, when me and Helen get things wrong, Katie's the one that, the one that slaps our legs and keeps us all in check. In fact, if anybody gets anything wrong on the team, there is a lot to remember working at GenderGP and, and there's only one person who we can rely on to make sure we're all going in the same direction and that's Katie. Dr Helen Webberley: So, Ioannis, we've got lots of questions to ask you. We want to know it all, because I tell you a secret, I've never had top surgery. I don't think Marianne's ever had top surgery. And so we speak from a position of ignorance sometimes. You, I don't know whether you've had top surgery or not. I'm not going to ask you, but I think you might've done that surgery in your time. And I'd be really grateful if you could just give us some nuggets. What do people ask you? So I'd love to use this opportunity to really, I don't know, share the myths, share the truths, debunk the lies and teach people what they want to know. So over to you, Ioannis. Top surgery is a stop, is a part of someone's transition, but it's not necessarily a part for everyone. So top surgery includes chest surgery, in forms of mastectomies and chest wall for reconstruction from transmasculine individuals, or a BA, a breast augmentation for transfeminine. But it's not, it's not there for everyone. Okay. But it is a choice for, for some or some of those individuals. And you're right. I haven't had top surgery myself, but I have done a few. Okay. So, so I'm conducting these consultations regularly every week and some of them appear to be more meaningful than others. And when we were doing, Sophie, when we were doing that weekend of the fundraiser, I blundered several times in my pronouns, in my name. And then there was a member of the GenderGP team who joined, I blended with their pronouns and I'm just like, how can I do this publicly live on stream that's trying to help trans youth? And so I said, Sophie, please come and kind of teach me more about what it means to be nonbinary. And Marianne you're with me as well as always. And, you know, I've heard you say in the past and indeed to Sophie that you, you feel that your gender is more binary. Even though it's the opposite end of the one that everyone thought when you were born. So really, I'm really interested to learn today. And if you don't mind Sophie, just chuck some challenging questions towards you that we get asked all the time, but I don't know the answers to, because I don't, I don't understand it fully enough. So if that's okay, if that's okay, TheymerSophie: If it's okay, I'd just like to jump in and say that what you're referring to on the stream. I just want to open out with something that I think is really important. And it's something that Marianne said in our last discussion, which was that people are very offended by being told they've done something. And in many cases that's a lot more offended than anyone is by the thing that was done. The thing that you're describing there, you know, you used the wrong pronouns for a member of staff on the stream, and they said, oh, it's this one. And you said, oh, sorry. And we just carried on. And that's really the normal situation. I just want to, the strongest possible thing I can say, like to open out is that for anyone who's listening, who maybe you don't know trans people in your real life, like maybe you, maybe you just know one or a couple, you know it's, it's not a big deal. So I met Sophie just before Christmas 2020, when it came to my attention that there was a fundraising gaming stream, not really words that I completely understood at the time. But someone said, well actually my kids said, mom, you've got to have a look at this. There's someone on Twitch doing a live stream, raising funds for young people who were affected by the Tavistock ruling in late 2020. And I'm really happy to have Sophie here who's responsible for all of that. So as I normally do, Sophie, I'm going to hand over to you. I'll let you tell the world who you are and how come all this came about. And also, thank you. Theymer Sophie: Well, thanks. Thank you for everything you do. I've said this a million times on the stream already when we've chatted before, but you know, I played a game, you help people, but yeah. So I'm Sophie, pronouns they/them. I have a YouTube channel called Curio where I talk about media mostly from a queer, leftist perspective. But sometimes I do like quite purely political videos. I have one that's my most viewed video that's called the aesthetics of intellectualism and talks about people like Jordan Peterson. And I am working on one about the gender critical movement, which is called transphobia with British characteristics. And I I also, and this is as Helen was saying, sometimes stream games on the website Twitch, where Twitter and Twitch, as Theymer Sophie. And so after the Tavistock ruling I felt absolutely awful, a lot of pacing around the room saying F this country. And I saw people talking about what could be done. I saw Joe (unclear 2:32) initiative to try and overturn it, things like this. And I saw Casey Montgomery mentioned that GenderGP had set up a fund to help under sixteens. And I thought, well, this is a good opportunity. I know my audience, and I know they'll show out for helping out trans kids however they can. So I made a little trailer and I told them I would stream on Twitch, which if you're completely unfamiliar, if you're prestream Helen's condition, basically I play a game and a feed of it goes out to an audience, and I'm also there on camera chatting and, and, you know, gaming. And people can send in donations if I'm fundraising for something. I said in my announcement that I would keep going until we hit 10,000 pounds,

    How gaming became a lifeline for trans youth

      Fundraising for trans healthcare has found a new home thanks to the likes of @Hbombrguy who raised a staggering $340,000 for Mermaids in 2019 and @theymerSophie whose livestream on Twitch late in 2020 raised a phenomenal £50,000 towards the GenderGP Fund - an amount that was matched by GenderGP in in-kind donations. Inspired by these incredible fundraising feats, Twitch streamer Ed Sweet, is holding his own live stream to raise funds for trans youth over the weekend of February 20th and 21st. Excited to have you on. Tell us who you are, what you do and whatever are you doing on the GenderGP podcast? Neville Southall: Well, what I was, was a footballer for Everton for a long time, probably 20 odd years. And what I do now, I work in a special school in Ebbw Vale, which is, I know it's not too far from you. And what, why I've come on really is because I don't know nothing really. So I'm on here to learn a bit as well. And I think every time I go on something or I talk to people, then I learn a bit more. And as you know, good me going on Twitter and talking about transgender issues or anything like that. I don't think that's right. I think what you can do is support and help when you can. But the main thing is to get the right people to talk about the right things. So you can educate people because it's like anything else, any minority group needs to be understood, and it needs to have a fair platform to be able to do that. And the way the media sets up well, in most countries, it doesn't give everybody a fair view. So my platform is really to raise awareness of what people do and to give them a chance to actually say what they believe in. And I think everybody should have that chance. Dr Helen Webberley: A couple of weeks ago, you very kindly gave Marianne, our head of therapies, sole access to your Twitter account which obviously has massive following for your followers to ask all the questions. So, what's your interest in minority groups? Why have you decided to do this, to help people learn? What's your interest? Why you? Neville Southall: Well, mainly because I can. And I thought, as a society, we were brought up to help everybody. So when I first sort of went on Twitter and it was all because a lad was, you know, really homophobic, but he was obviously denying he was gay. So we had long chats and long chats. And eventually I came on Twitter and asked people what I should be saying, because coming from the football world, it was never talked about in nice friendly terms to be gay. It was completely the opposite. So I wasn't sure what I was supposed to say to him. So I was looking for advice, really. And then from that, it just sort of moved into the transgender.

Helen and Marianne, just the two of us today. A short and sweet podcast just talking about sex, which is everybody's favourite conversation at some stage. We're following on from the topic of sexual health and sexual pleasure that we've been doing. And talking about the idea that everybody has the right to have a fulfilling and happy sex life. So, we've been running a series of articles, and we also asked the community to come up with any questions and worries that they might have about the issue. And so, Marianne and I thought we would come and join together today and see whether we can flush out some of the answers to those questions. So, thanks for joining me, Marianne. I know it's early in the morning for both of us, so we've got some questions here. Should we just kind of run through them? This one was: so I started o my testosterone blockers and my sex drive has reduced. And I'd like to know if there's anything I can do to improve this important aspect of my transition. I feel ashamed to ask my GP or gender clinic because of the attitude towards sex and transgender women in society. This is something that we hear and see a lot. I certainly see it in the medical world and Marianne, and I guess you must see some of it in, in the therapy room. And there is that thing, isn't there, where women and men that have different hormone profiles and that certainly has an impact on sex drive and libido? And one of the things that people have to be aware of is that testosterone is a big sex drive hormone. And, you know, you see that during puberty when, when boys are going through puberty, and their testosterone levels are raging. And so is their need to procreate and further the generations. And we see the same don't we when women and men go through the menopause and that hormones drop-off? So cisgender men going through their late menopause when their testosterone levels fall off, we do see that their sexual function and their sex drive also reduces. So it's not unsurprising, is it? That when you, when a trans woman changes her hormones from testosterone to estrogen, she's going to face that same difficulty with a drop in libido and drive, which is sad, but it's part of being a woman. If you are transitioning from a male hormone profile to a female hormone profile and the medicine that you take completely blanks out all of your testosterone, it may well cause you problems in that area. And there's no harm in just taking a tiny weeny, little bit of testosterone hormone to help with that. So that's the kind of medical side,

I'm with Marianne as usual, and I'm also with a young man called Leo who's very kindly agreed to come on and talk to us and tell us all about himself and life, the universe, and everything else. But pre-warning, we're not going to talk about geography because Leo and I have both admitted that that is not our strong point. So anyway, Leo, lovely, lovely, lovely to meet you and lovely to have you here today. I'm just going to do a really awful grown-up thing and say, Leo, tell us all about yourself, tell us anything you want us to know. And just go for it. Leo: So, my name is Leo. I'm 16, I live in Bedford, I came out as trans when I was about 11. So I've been living as myself for quite a while now, about five years, which is exciting. And I'm coming up to a year on testosterone, which is very cool. Those are the basics. Dr Helen Webberley: Your life, your life milestones, aren't they? Leo: Yeah. Pretty much. Marianne Oakes: I'm loving the pondering because it just, to me, it's like you ask any 16-year-old, say about, tell you about themselves and nothing seems remarkable, does it? Leo: It's quite hard. I was just trying to think of what is most important to me right now. And those are the things, really. Marianne Oakes: What, if you don't mind, tell us a little bit about what kinds of things you like, what hobbies, if any? Leo: Well, I'm really into fitness, so I work out. Now it’s less so, now the gyms are closed, but again, a normal scene. So yeah, I work out quite a lot of biking, so my friends like to bike on a regular basis. Quite a lot of music, the acting, I like dancing, I like all the things, I should have mentioned that. Dr Helen Webberley: So, you say you came out at 11. What are you able to remember that? Tell us what that was like for you. What does that mean for you and your family and those around you? What, what happened? Leo: Yeah, I remember very well. At that time, it was near my 12th birthday, but it was a few months off. And I, I remember ever since I was young, I was what people would call a tomboy, I suppose. So as soon as I could, as soon as my parents started letting me pick my clothes, that'd be boys, things made for boys. Uh yeah, I was just tomboy. It was sort of an inside joke, my family. So really, really, really excited to welcome Arthur and his mum today. So, Arthur, thank you so much for joining us. I'm going to throw you right into the deep and say tell us all about you and your mum. Hi, mum. Arthur: Well, I do karate. (unclear 1:11). Anna: How old were you when you remembered being trans? And you said, I’ve always been. And that's about right. It was so long ago, cause how old were you when you identified? It was about seven. Arthur: Seven, six. Anna: Six or seven years old when Arthur drew a picture and tried, and he didn't have the word like. Obviously, he didn't have the words. And I didn’t know the terminology either or anything. And he drew pictures. So he was about seven. So we've been in services since then. Since we came with yourselves, the stress of this beast, it's been gone, hasn't it? So you've forgotten about being trans, you’re just Arthur, aren’t you? Arthur: Yeah. That makes sense. Marianne Oakes: Can I just—what was your journey to finding GenderGP, if that’s not too big a question. It sounds like it was with the NHS before, would that be correct? Anna: Yes, Mermaids. Arthur: I got my hormone blocker through the NHS. Anna: It was a bit of a battle to get it, though. Arthur: I wanted testosterone. I needed it, I believe. But I couldn't, because of how old do you have to be for the NHS? Older. Anna: Fifteen, sixteen. Arthur: Fifteen, yes. He was panic-stricken, weren’t you? And we saw the firm, but no, we found out about GenderGP when we met Helen actually at one of the Mermaids residentials. And Mermaids are fantastic in the early days. We haven’t been for a few years, but we just said I think we're ready to check back in with them because I think he’s ready to find out more information about all the stuff that is coming up in the next few years and questions for other teenagers. I’m not going to tell you anything more about her. I’m just going to pass you over as I like to do. Sophie, introduce yourself, tell us why you’re here and give the listeners and a little taste of what we’re going to talk about. Sophie Lasiuk: Of course. Well, first of all, I’d like to thank you for inviting me to your podcast today. So, I’m a fifty-two-year-old woman, as you said. We live in Toronto, Ontario. And I’ve been following your posts at doctor for several years now and reading up on some of the problems that people tend to have when transitioning in the UK. And it occurred to me as I went through my own transition over the last three or four years that the systems in place in our two countries are markedly different, and I wondered why that was, and I can’t figure that out. The system in place in Canada has been so welcoming and so user-friendly, for lack of a better word. And I just wanted to reach out to you and express how the differences have been helpful to me, and perhaps start a conversation on your side of the Atlantic about how things can be improved over there. Dr Helen Webberley: Well, I think that would be amazing. And I want to say thank you to you for reaching out to us in the first place and for making that first contact. You sent me some brief messages, which I always love receiving. And then you wrote a blog for our website just explaining how it was for you. So, if anyone hasn’t read that yet, then there's maybe some spoiler alerts in there. And I was astounded. I was like goodness gracious me, this is what we’ve been campaigning for the last kind of five years, and you’re there already. So just outline how—you said that you are in your fifties. How did it go from the first time when you actually wanted to go and ask a doctor about this. Sophie Lasiuk: So, my truth is that I have identified as female since my earliest memories. I’m in my fifties now. Still three or four years of age, I had this innate understanding of my gender, and I’ve carried that with me all my life. And society certainly has had some catching up to do and over the last ten years or so, certainly including my wife and discussions, we decided it was the right time for me to move forward with my gender transition. So I did approach my doctor in 2016, I believe it was, early 2017 to get assistance, medical assistance to move forward.

I'm Taryn De Vere. I live in Ireland, but I'm originally from Australia. And I have a child who identifies as trans, a young child who came out to me when she was at about five. And that kind of started me on the whole journey of learning all about—perhaps I didn't have friends who were trans before, and I did understand some of the issues relating to trans people, and it was actually my knowledge that came to the fore at that time because my daughter said to me in the car do all women have vulvas, and my instinctual response was yes, and then I said, “Oh hang on a sec. No, trans women. So no, trans women don't have vulvas. And she said, "Well, what's a trans woman?” and I started talking to her explaining and then she said, “That’s me. I'm a trans girl.” and I said, “Oh okay.” so that that was the starting point for it all you know that moment and then now we're a few years on you know in our story and so I just obviously, I would have been interested in trans rights before but not as passionately as when you have a child, you suddenly just get really thrown into it. And so I've become extremely involved in the trans rights movement and particularly with trans kids. Dr Helen Webberley: Yeah. So I haven't really obvious question rolling off my tongue which I'm guessing other people might have too. Did you know before that time in the car? Did you have any inkling that that was coming? Tell us all about you and over to you. Stephanie Hirst: I guess my story is quite classic because I think from a very early age, I knew that something wasn't right. And when you are at school, especially like nursery school and they put the girls and the boys into different groups, I would always go and stick with the girls, because that was my natural state of comfortable. And they would go and put me back with the boys. Now you sit over there with the boys. And this happened several times, they raised it to my mum. Back then, there was no google or anything like that. There was nothing. You know, my mum couldn't look this kind of stuff up. So, I think she just went with her gut instinct and just ignored it. And then as you grow up, you start to learn that this isn't, in inverted commas, I guess, acceptable. Because anyone who I guess looks like a boy and I guess well is a boy, then you are slightly effeminate, well, you tend to get bullied.

I’m going to let you come in and just tell us who you are, what you do, what your lovely and amazing team do. So, I’ll hand it over to you. Dr Raúl Bellinga: Alright, good morning to everyone. Thank you so much for your kind words. And let me introduce myself. I am Dr Bellinga, Raúl Bellinga, from Facial Team, Spain. We are placed in Marbella, in the south of Spain, the lovely Costa del Sol. And I am part of a surgical group and whose name is Facial Team. Facial Team is a surgical group which started in 2008, and we are just dedicated to facial feminisation surgery. So, we are pretty experienced about what we are doing now. At Facial Team, we are now six surgeons, and we are all dedicated to one specific feature in the face. In my case, for instance, I am the one who is dedicated one hundred per cent to rhinoplasty, to feminisation rhinoplasty, which is slightly different from cosmetic rhinoplasty. Dr Helen Webberley: Okay, wow, brilliant. So, the rhinoplasty being the surgery on the nose, the feminisation surgery on the nose. Just broadly, you've got six surgeons. Do you all do something different? Or how does it work if someone needs a little bit of attention on their nose as well as their forehead? How does it work to get that full kind of picture? I am here with Marianne as usual, and we have a lovely guest today as well. I'd like to welcome Alex, who I think is very exciting because her specialty is virtual facial feminisation. I love the fact that it's facial feminisation because this is going to help a lot of people, but also the fact that this virtual thing is very much what GenderGP is all about. So Alex, let me allow you to introduce yourself and tell us all that you do. Take the floor. Alexandra Hamer: Right. Do you know what I feel? It's really difficult because I have one of those jobs that doesn't have a proper name. People say what do you do? And depending on how much time I've got, the answer can be very long or very short. Anyway, I am Alexandra Hamer, and, so, essentially, I advise people on facial feminisation surgery. I am not a surgeon, I am not a doctor, so it's not that I advise them on the surgery, but I explain what makes their face masculine, and what techniques there are available that can feminise it. And I also simulate the changes in photoshop so that they can get a sense of how those different procedures will look in real life. By doing that, they get a sense of what's important and what isn't important. The thing is, if you go to a surgeon, they will often give you a very long shopping list of procedures, and a lot of those often will make very little difference but will cost you thousands and thousands of pounds. Today, I am really happy to welcome Mia Violet. And, as always, I’m going to let Mia introduce herself, tell you a little bit about her, and what she does, and why we are really excited to talk to her today. Hi, Mia. Mia Violet: Hello. Oh, gosh, I never know how to introduce myself. Dr. Helen Webberley: I’ve put you on the spot. Mia Violet: Yeah, it’s like, how do you—there’s no real way to say, “I’ve written a book” and not come across like you are just really arrogant, like, “Oh, I’ve written a book by the way. I’m an author.” I’ve got a book right here as well. Yeah, I am a trans woman, an author, I wrote Yes You Are Trans Enough, which is like part memoir, part primer on trans issues. I use Twitter a lot, more than I probably should, where I am @ohmiagod. I also use Instagram a bit, where I am @oh.mia.god, I talk about trans issues and self-love. Dr. Helen Webberley: So, Yes You Are Trans Enough is your book. Why did you feel the need to tell the world that you need to be trans enough to be anything? Mia Violet: Sure. Yeah, sure. I was at an event recently, and someone said, "What was the emotion behind why you wrote the book?" And I answered and said, honestly, rage. Like, anger. Because what happened, the genesis of the book is a really long story. I won’t bore everyone with it. But the short version is that I read an article by someone that quoted another trans woman, can’t remember who it was, and she was talking about another trans woman, and she said, “Oh, she’s not trans enough.” Like she’s not trans enough to have gotten this title or this award or whatever. And as a trans person that really, really annoyed me, because I am someone who’s dealt with, “Oh, am I trans enough?” feelings basically my entire life. So, I wrote an article, just off the top of my head, where I was, there is no such thing as trans enough. Yes, you are trans enough. You don’t have to be stereotyped. You don’t have to have gone through these milestones. I was thinking a lot about the stereotypes—like I always say, I don’t fit into that. I didn’t play with dolls as a child. I am like I always played with Batman as a child, you know, superheroes and action-y things. But obviously, I am still female. So, I wrote this article and just this kind of debunking things off the top of my head. I just threw it out on my blog,

I am very happy to be here with Marianne on our today’s edition of our podcast with Sara Phillips. And as usual, I’m going to pass over to Sara to let her introduce herself and tell us all about the work that she does and her ambitions for today, tomorrow, and for the future. So, Sara, welcome to our podcast. Sara Phillips: Thank you. It’s a pleasure to be with you this morning. As you said, I am Sara Phillips. I am the chair of Trans Equality Network Ireland. I’ve been in that position now for the past seven years. I have been out as an activist in trans rights for the past twenty-six years here in Ireland. I suppose most recently, I've been working quite a lot on gender recognition. I was one of the lead negotiators for our very progressive gender recognition legislation in 2015. And more recently I was on the government review group of that act in 2018, which provided some very progressive recommendations which have yet to be legislated for. I suppose as chair of our national organisation, I work across all areas of trans life, whether it's healthcare, whether it’s gender recognition, whether it's support. And there is still a huge amount of work to be done over here in Ireland. Currently, I am also a member of the steering committee of the International Trans Fund, which is an organisation providing funding to small trans organisations specifically in the global south but also in Asia, who work to try and support trans communities in different ways. Dr. Helen Webberley: Oh, wow. That is one introduction. I’m so excited to quiz you on some of this. So, just going back to the beginning, you say that you have been an activist for twenty-six years. How have things progressed, changed, developed, reverted in that twenty-six years? Sara Phillips: Well, it’s changed quite a lot. I think when I came out first was 1992. And I suppose trans people in Ireland were few. In fact, for most of us who were around at that point, we had tended to have looked to the UK quite a lot. We did have some groups starting up at that point. We had had a longstanding group which started in 1977 (unclear 2:45) Beaumont society ironically. More for trans people over here,

Hi Shane. Hi Marianne. Lovely to have you both here. Really excited to have you, Shay. I'm just going to hand over to you so you can tell everybody who you are, why you’re here, the work that you do, the inspiration that you give, the journey that made you arrive at the beginning of that inspiration. So over to you, and tell us all about you. Shay Robertson: Well, first of all, good morning, Helen and Marianne. Thank you for having me here. My name is Shay Robertson. I am an FtM female to male. I have been transitioning since 2011. What I've done over my journey is when I first started looking out, actually, I did struggle to find information for my journey. I found a lot of information male to female but not really my way around. I did struggle. It was kind of a bit like I was in the dark. I knew what I wanted to do but didn’t really know the process. So really from going to see the GP going through consultants, I was completely really unawares. So what I decided to do was during my journey I noted everything down so I could help others in the future. And then what I did was I managed to create all that and make a book about my journey, which on my part, I think is actually doing really well, and I'm getting so much positive feedback from it. You see a lot of things online. You see a lot of things in books where doctors can tell you things, surgeons can tell you things, but they can't actually tell you the feelings of what the actual person going through that journey is actually feeling. I put everything down into this book, so it doesn't just help the person transitioning; it can help partners and friends that can support as well. What I also did was document everything from the meeting from the first doctor to what they said to the surgeon, to the consultant. All the worries. It’s doing really well. I’m really proud of that. I also went on to create a Facebook page which I call “Transpirational,” and that’s been quite a big hit as well. It’s not just your everyday Facebook page.
