Getting a cancer diagnosis today can mean something very different than it meant a few decades ago.  Cancer is still deadly. But thanks to advances in detection and treatment, cancer for some people has turned into a manageable condition. A report from the American Cancer Society out this week shows that for the first time that the five-year survival rate for all cancers has reached 70 percent. Tumors are being found at earlier stages, when treatment can be more effective. Surgery and radiation have gotten more precise.  Researchers have refined their understanding of different types of cancers and developed new drugs that zero in on the unique biology of specific tumors.Maybe most importantly, researchers are figuring out how to use the body's own immune system to fight cancer in ways that doctors couldn't have imagined 20 years ago.  MPR News host Angela Davis talks with her guests about how cancer treatments are improving.Guests: Dr. Emil Lou is a medical oncologist and professor at the University of Minnesota Medical School who sees patients at M Health Fairview Masonic Cancer Clinic. He specializes in gastrointestinal cancers, such as colorectal and pancreas cancers, and is also involved in cancer research.  Dr. Rachel L McCaffrey is a breast surgical oncologist at Allina Health who specializes in treating breast cancer.

In cancer research, the “seed and soil” hypothesis posits that the tumor is like a seed of misbehaving cells taking root in the body. Whether it grows—and where it grows—depends on the conditions, or soil. Since this hypothesis was proposed more than 100 years ago, most research and treatments have focused on the seed, or tumor. For nearly 50 years, Rakesh Jain has been studying the soil. But in a seed-focused field, his work was seen as wasteful and radical. Now, that very same research has led to seven FDA-approved treatments for diseases including lung and liver cancer, and earned him a National Medal of Science in 2016. Host Flora Lichtman talks with Jain about how his fringe idea led to lifesaving cancer treatments. Guest: Dr. Rakesh K. With testicular cancer, the urologist is going to be the one you meet, get the initial procedures done, and do the post-op follow-up."I'm very fortunate to have an excellent radiation oncologist that I work with, and very fortunate to have a couple of very, very good medical oncologists that I work with."Definitely anytime you have any kind of cancer, including testicular cancer, it is always a team approach." He shares his insights on treating testicular patients at Unity Urology, his practice in East Tennessee. In a conversation with CancerNetwork®, Geraldine O'Sullivan Coyne, MD, MRCPI, PhD, spoke about her new appointment as the principal investigator and director of Clinical Research at the Northwell Health unit of the Southern Texas Accelerated Research Therapeutics (START) Center for Cancer Research. Based on a strategic partnership formed between Northwell Health and START in May 2024, she will oversee early-phase clinical trial development at New York's first START research site at the R.J. Zuckerberg Cancer Center in New Hyde Park. Coyne began by sharing her excitement to put all her efforts into making the unit a “good opportunity” to consider patients who reside in community settings for enrollment in new clinical trials. Although large academic centers may offer possibilities for enrolling in trials, Coyne highlighted how many patients may need to travel or leave their social networks behind as they undergo treatment, thus illustrating a need for expanded options in the community setting. “Having the privilege to be able to bring trial opportunities into the community—into the heart of the community—is what we are aiming to do,” Coyne stated about her new role. “We do believe that this will [allow] patients to access novel and powerful therapies that are now increasingly part of a personalized treatment paradigm for patients with advanced malignancies.” Emphasizing the pursuit of early-phase clinical investigations, Coyne described how the new unit may leverage START's expertise to advance novel molecules across all stages of the drug development pipeline up to regulatory approval. She noted that being able to safely create and refine new therapeutic approaches in the community setting lies at the heart of a successful collaboration between Northwell Health and START. “A lot of the excitement surrounding the START unit and Northwell Health collaboration with them is exploring these novel molecules that will hopefully continue to build on the pursuit in oncology drug development, which has been finding better and more targeted therapies for patients in their treatment journey,” Coyne said. Human Papillomavirus (HPV) is a small DNA virus that infects skin and wet surfaces of the body. The virus is typically spread through vaginal, anal, or oral sex. There are over 100 different types of HPV. The most common types of HPV are found on the skin and appear as warts. It is one of the most common types of sexually transmitted infections worldwide. HPV is responsible for more than 90% of anal and cervical cancers. The high-risk types of HPV form in the back of the throat, also known as Oropharyngeal Cancer. Vivian F. Dr. Lauren Shih: Hello, and welcome to our 2025 ASCO annual meeting series where we cover some of the top JCO papers published simultaneously with their abstract presentations at this year's meeting. I'm your host, Dr. Lauren Shih, JCO editorial fellow, and I'm joined by JCO Associate Editor Dr. Stephanie Wheeler to discuss the Journal of Clinical Oncology article and abstract presentation "Use of Low-Value Cancer Treatments in Medicare Advantage Versus Traditional Medicare." Let's start with the relevance of the article. Dr. Wheeler, can you explain this to our listeners? Dr. Stephanie Wheeler: Thank you so much. Let's get right into it. So this article is really about understanding different types of Medicare plans and what we should expect to see in terms of their use of low-value treatments for cancer patients. So, as Medicare really is focused on trying to limit the use of low-value cancer treatments, we really need to better understand the drivers of variability. So we know that many cancer patients have multiple treatment options available to them. We also know that the vast majority of older adults beyond age 65 are insured by Medicare, and about half of them are on Medicare Advantage plans, which are serviced by private insurance. And private insurance companies in this case are receiving capitated payments for Medicare beneficiaries to manage their service utilization and reduce costs. So, with respect to Medicare Advantage versus the traditional fee-for-service Medicare, it's not really been known to what extent low-value treatments are differentially used by these types of plans for cancer patients. And so that was really the focus of this article. What the authors found is that across six different types of treatments, in general, the folks who were enrolled in Medicare Advantage plans had reduced use of low-value treatment. So that's a good sign for Medicare beneficiaries. And although the relative difference in that use was somewhat low, this translates to a significant number of Medicare enrollees across the country not receiving these low-value treatments. And of course, this translates to considerable savings at the society level. Dr. Lauren Shih: Are there any additional key results that we should review? Dr. Stephanie Wheeler: Yeah. So I'll tell you just a little bit more about the methods and also their findings. So they looked at six different low-value treatments, and this was in, again, 100% of national Medicare enrollees from 2015 through 2021. So the six low-value treatments that they examined were the use of G-CSFs among patients receiving low-risk chemotherapy and denosumab for those who had castration-sensitive prostate cancer. Then they also looked at four high-cost treatments, including using nab-paclitaxel instead of paclitaxel for patients with breast or lung cancer; second, adding bevacizumab to carboplatin plus paclitaxel for ovarian cancer; third, using brand-name drugs instead of generics when generics were available; and fourth, using biologics instead of biosimilars when biosimilars were available. And these are all, by the way, non-recommended treatments according to a variety of guidelines, including NCCN and ASCO's Choosing Wisely guidelines. So they used the Medicare claims data to examine use of these regimens. They also analyzed results by type of Medicare Advantage plan, whether people were enrolled in a health maintenance organization plan, or an HMO, or a preferred provider organization plan, or a PPO. They also looked at the largest Medicare Advantage insurers—including Aetna, Blue Cross Blue Shield, Cigna, Humana, and UnitedHealth—and limited their analyses to those that had complete encounter data. And what they found across the board is that the enrollees in Medicare Advantage plans generally had lower use of these low-value treatments. And the largest differences between Medicare Advantage and traditional Medicare plans were in the outcomes, including G-CSF use and using denosumab for castration-resistant prostate cancer, and then the combination of bevacizumab, carboplatin, and paclitaxel versus carboplatin and paclitaxel. And all of these had a change in use ranging from about 19% change to 24% change in use. This is significant as a field as we look at ways in which different plan organization can influence use of treatments, particularly given the excess cost of cancer care. This is something we really want to pay attention to. So I'd encourage folks to look more closely at the results by treatment type as well as the results by plan type to see a little bit more about what was going on across different plan types. Dr. Lauren Shih: Great. And are there any outstanding questions that need to be answered? Dr. Stephanie Wheeler: Yes, there always are, of course. I think the study has several strengths that are worth noting. First, they have 100% of Medicare enrollees, so there's national coverage there, which is, you know, quite outstanding. They also use an appropriate choice of analysis to help deal with some of the selection. So they use inverse probability of treatment weights, and they control for practice and county indicators to try to get some realistic adjustment for the selection that happens in terms of how patients are enrolled in different Medicare Advantage versus traditional fee-for-Medicare plans. These statistical approaches are a good idea, but they are limited by the observed variables that we can use for these kinds of adjustments. And so any unobserved—confounding or any unobserved factors that would influence selection in these plans aren't going to be captured well. So preferences, for example, that patients may have about different types of plans when they're insuring themselves and their families may not be captured. Second, the data that are used are only encounter data from those plans with complete records. That may mean that smaller Medicare Advantage insurers or those that don't have as comprehensive records are not included. So this may not be reflective of their practice patterns. And then third, of course, this only looked at six different low-value cancer treatments. It remains to be seen whether this kind of finding extends to other types of low-value cancer treatments, and that's an opportunity for future study. Finally, I would say that we don't exactly know why these patterns exist. It could be that Medicare Advantage plans have different approaches to prior authorization. They could have more in-house quality control and management to really understand, among their population for whom they're receiving Medicare Advantage payments, to really look at care quality and assess Choosing Wisely guidelines. We don't know exactly how that's playing out. And so we need additional data to really figure out what's working here and what are opportunities for future policy and payment innovations that can further reduce low-value care. Dr. Jennifer Wargo is an Associate Professor in the Department of Surgical Oncology at The University of Texas MD Anderson Cancer Center and a Stand Up To Cancer researcher. Jennifer is a physician scientist, and this means she splits her time between providing care to patients and doing research to find better ways of treating disease. Specifically, Jennifer performs surgeries and treats patients one day each week. She spends the rest of her week studying how to better treat patients with cancer and how cancer may ultimately be prevented. When she's not doing research or treating patients, Jennifer enjoys spending quality time with her family. Some of their favorite activities include going for walks, biking, hiking, and visiting the beach. Jennifer also likes to explore her creative side through art and photography, as well as to be active through running, biking, yoga, and surfing. She received her A.S. degree in nursing and B.S. degree in biology from Gwynedd-Mercy College. Afterwards, Jennifer attended the Medical College of Pennsylvania where she earned her M.D. Jennifer completed her Clinical Internship and Residency in General Surgery at Massachusetts General Hospital. Next, Jennifer was a Research Fellow in Surgical Oncology at the University of California, Los Angeles. She then accepted a Clinical Residency in General Surgery at Massachusetts General Hospital. From 2006-2008, Jennifer was a Clinical Fellow in Surgical Oncology at the National Cancer Institute of the National Institutes of Health. She then served on the faculty at Massachusetts General Hospital and Harvard University. In 2012, Jennifer received her MMSc. degree in Medical Science from Harvard University. Jennifer joined the faculty at The University of Texas MD Anderson Cancer Center in 2013. She is Board Certified by the American Board of Surgery, and she has received numerous awards and honors throughout her career. These have included the R. Lee Clark Prize and Best Boss Award from the MD Anderson Cancer Center, the Rising STARS and The Regents' Health Research Scholars Awards from the University of Texas System, the Outstanding Young Investigator and Outstanding Investigator Awards from the Society for Melanoma Research, as well as a Stand Up To Cancer Innovative Research Grant for her microbiome work. She has also received other awards for excellence in teaching, research, and patient care. In our interview, Jennifer shares more about her life and science.

BUFFALO, NY - March 25, 2025 – A new #review was #published in Oncotarget, Volume 16, on March 13, 2025, titled “Signaling pathway dysregulation in breast cancer." In this review article, Dinara Ryspayeva and colleagues from Brown University provide a detailed look at how breast cancer cells change the way they communicate and grow—helping tumors survive, spread, and resist treatment. The review highlights how certain gene mutations and disrupted signaling pathways influence therapy response across different types of breast cancer. It also outlines current treatment strategies and clinical trials, offering insights that could improve care for patients with aggressive or hard-to-treat cancers. Breast cancer is the most common cancer in women and a major cause of cancer-related deaths worldwide. While many patients respond to treatment at first, some cancers return or stop responding. The review explores how signaling disruptions inside tumor cells are often behind these setbacks. The authors discuss several major pathways involved in breast cancer, including PI3K/Akt/mTOR, RAS/RAF/MEK/ERK, HER2, Wnt/β-catenin, Notch, NF-κB, and the DNA damage response (DDR). These pathways help control cell growth, division, DNA repair, and survival. When altered by mutations or other changes, they can promote tumor progression and resistance to treatment. One of the most disrupted pathways is PI3K/Akt/mTOR. It plays a central role in cell growth, but in many breast cancers—especially hormone receptor-positive and HER2-positive types—it becomes overactive due to gene mutations, or the loss of a tumor-suppressing protein called PTEN. “Up to 25–40% of BC cases exhibit variations that hyperactivate the PI3K/Akt/mTOR pathway, underscoring its critical role in oncogenesis.” Another key pathway, RAS/RAF/MEK/ERK, can also promote tumor growth. Even without mutations, it may become active when primary pathways are blocked, particularly in HER2-positive and triple-negative breast cancers. The review also highlights several new and emerging treatments aimed at blocking down these signaling pathways. Some drugs are already approved, while others are in clinical trials. The authors suggest that combining different treatments may help stop multiple pathways at once, making it harder for cancer cells to adapt. Matching treatments to each tumor's unique genetic changes could also improve patient outcomes. This comprehensive review gives researchers and clinicians a clearer understanding of how breast cancer resists treatment and where future therapies should focus. A better understanding of these disrupted signaling systems could lead to more personalized and effective treatments for patients facing aggressive or recurring disease.
