The COVID-19 pandemic has hit an unprepared world hard. The public health, social, and economic toll has been unprecedented in recent decades. As of mid-May 2021, more than 168 million people have been infected and 3.5 million have died of the disease. Based on estimated excess mortality for 2020, the World Health Organization (WHO) recently indicated that the death toll is likely higher. While this global crisis has unraveled social life across countries, the equally devastating decades-old pandemic of tobacco use has continued unabated to claim lives and make people sick as it is causally linked to diseases of nearly all organs of the body. Around 25% of all heart disease deaths and 80% of lung cancer deaths are caused by smoking, and many others are caused by exposure to secondhand smoke, for example. According to the Centre for Disease Control -CDC, in the USA,  and the World Health Organisation, more than 8 million people a year die from tobacco related diseases. According to the same sources Covid has killed 4.2 million since the beginning of the pandemic. These numbers need to be looked at and we need to be fair about this discussion. Smoking kills more than Covid yet no one is doing anything to stop it.  I think it's very important that everybody does the best they can to stop the transmission of the virus, and to help our healthcare services. We all accept this. We have been ordered to stay home, to wear masks and to vaccinate all in order to protect our hospitals and the weak, and its understandable, yet no one is talking about banning smoking during these challenging times. Why? The WHO clearly states that there is a 50% increase in mortality in smokers after contracting COVID-19. Why is it still okay to smoke during this pandemic? Smoking is not something that should be so overly protected. If civil liberties are being taken away to improve public safety and public health, then smoking is no longer a civil liberty that doesn't impact all of us. The same rational goes  for heavy drinking ( 3 million deaths a year according to WHO). Smoking directly contributes to the mortality rates and the comorbidity rates of COVID.  For decades smokers have overburdened our hospitals, created massive healthcare issues and no one talks about this. We all acknowledge it, yet nothing is done. Should we not take this opportunity to have this discussion?People get nervous if you don't wear a mask outdoors, but have no problem seeing parents smoke next to their infants. We need to have a serious discussion around smoking and the challenges that smokers present to our healthcare systems, to themselves and to this battle we are all fighting on a global scale. As a surgeon that operates patients every day for over 20 years, Dr Miguel Stanley has recently begun refusing treatment to moderate and heavy smokers, and works closely with them to help them find ways to quit and dramatically improve their systemic health in order to have a more healthier life and a longer lifespan. Should governments not be thinking the same?

When Sir Richard Peto began work with the late Richard Doll fifty years ago, the UK had the worst death rates from smoking in the world. Smoking was the cause of more than half of all premature deaths of British men. The fact that this country now boasts the biggest decrease in tobacco-linked mortality is in no doubt partly due to Doll and Peto's thirty year collaboration. Professor of Medical Statistics and Epidemiology at the University of Oxford and until last year co-director of the Clinical Trial Service Unit with Professor Sir Rory Collins, Richard Peto pioneered "big data", setting up enormous randomised clinical trials and then, in a novel approach, combining results in what became known as meta-analyses, amassing unequivocal evidence about how early death could be avoided. He showed how asprin could prevent heart attacks and how the oestrogen-blocking drug tamoxifen really did affect survival rates for breast cancer patients. Results on paper saves lives in the real world, he says, and he's famous for catchphrases like: "death in old age is inevitable, but death before old age is not" and "you can avoid more deaths by a moderate reduction of a big cause, than by a big reduction in a small cause" as well as "take the big numbers seriously". One of the world's leading epidemiologists, Richard Peto's landmark study with Alan Lopez at the World Health Organisation predicted that a billion people would die from diseases associated with tobacco this century, compared to a hundred million killed by tobacco in the 20th century. The chilling message galvanised governments around the world to adopt anti-smoking policies. And Professor Peto's studies about smoking cessation ("smoking kills, stopping works") provided the public health evidence needed to encourage smokers that, however long they had smoked for, it was always worth quitting. We can't ignore it - words that are found in no less a place than the 10 commandments: "I bring punishment on those who hate me and on their descendants down to the third and fourth generation." (Exodus 20:5). Thomas Jefferson referred to these words as "contrary to every principle of moral judgment." Suppose there is a woman who loves God with all her heart, mind and soul. She loves her neighbor as she loves herself. She gives of her time and resources for the poor. She selflessly cares for others and with no desire to receive credit or praise. Yet, her great-grandfather was a cruel man who hated both God and his neighber. Would God punish this woman for the sins of her great-grandfather? The subject of punishment is one of the most important subjects of all because of the implications about God's character. Does God impose an extrinsic punishment for sin? While I was in Las Vegas recently I observed hundreds of people lined along "the strip" who were passing out cards that had the picture of a naked woman on one side, on the other a phone number that allowed you to have her delivered to your hotel room. What I found interesting though was a smaller group of individuals passing out cards that said, "Repent and turn from your sins. Otherwise, it's Hell to pay!" Christians typically describe sinful behaviors as something that should be avoided primarily because God says so and because God will one day punish you for it. In other words, "sin is bad because God doesn't like it." While it's clear that God doesn't like sin, isn't this the case because sin inherently destroys his children? Imagine that this were the reputation of doctors towards smokers: "What's really bad about smoking is that it angers doctors. Doctors, in their wrath, will cause painful cancer in those who smoke." If that were believed, wouldn't the reputation of doctors be so tarnished they would be the last person a smoker would ever want to see? If this was the settled belief in the world about physicians, it would be an absolute necessity to have a campaign that reaches people with the message, "Smoking Kills, Not Doctors!" And perhaps with a subtitle, "Doctors are there to heal and to help you to stop smoking." Sadly, that is what has happened to God's reputation in the world. A 2006 study conducted by Baylor University showed that only 23% of individuals believed that God was benevolent or loving. 32% viewed God as authoritarian, 16% as critical, 24% as distant. Someday there should be a global campaign for God. A part of that message might include slogans such as, "Sin Kills"; "God is the Healer, not the Punisher."
