Ziel dieser Untersuchung war es, Daten zum Internetnutzungsverhalten, zu den Nutzungsstrukturen und zu den Nutzungsinhalten von psychiatrischen Patienten zu erheben. Von Juli 2000 bis März 2001 standen 688 Patienten in der psychiatrischen Klinik der Ludwig-Maximilians-Universität München potentiell zur Verfügung. Nicht teilnehmen konnten Patienten mit einer Diagnose aus dem organischen Störungsbereich (F00-09) bzw. Patienten auf der Station für Demenzerkrankungen sowie Patienten, deren Schwere der Erkrankung eine Befragung nicht zuließ. Insgesamt haben 396 (58 Prozent) Patienten im Alter von 14 bis 69 Jahren an der Umfrage teilgenommen. Die Antworten zum Nutzungsverhalten ergaben, dass bereits 169 Patienten (43 Prozent) mit dem Internet Erfahrungen gemacht haben. Es haben deutlich mehr Männer (60 Prozent) als Frauen (40 Prozent) das Internet benutzt. Das Internet wurde durchschnittlich seit 22 Monaten mit einer Intensität von 6,7 Stunden pro Woche gebraucht. Insgesamt nutzten die Patienten wahrscheinlich geringfügig weniger das Internet als die Bevölkerung in Deutschland. In den Fragen zu den Nutzerstrukturen sollte untersucht werden, ob Patienten mit bestimmten psychiatrischen Erkrankungen das Internet häufiger, länger und intensiver genutzt haben als andere Patienten. Am weitaus häufigsten (57,9 Prozent) haben Patienten mit einer Substanzabhängigkeit nach F11-19 das Internet bereits benutzt. Deutlich am wenigsten (ca. 33,6 Prozent) haben Patienten mit depressiven Störungen nach F32 und F33 mit dem Internet Erfahrungen gemacht. Für andere psychiatrische Erkrankungen konnten keine signifikanten Abweichungen festgestellt werden. Hinsichtlich der Nutzungsintensität konnten keine signifikanten Unterschiede zwischen den Gruppen mit den verschiedenen Diagnosen nachgewiesen werden. Eine Testung der Gruppen mit einer Diagnose F31 und F32-33 in Bezug auf die Gesamtnutzungsdauer war knapp nicht signifikant. Hier zeigte sich der Trend, dass diese Patienten das Internet auch erst seit kürzerem benutzen als andere. Die Untersuchung der Nutzungsinhalte ergab, dass das Internet vor allem in seinen informativen Funktionen (z.B. Aufrufen von Webseiten) und weniger in seinen kommunikativen Funktionen (E-Mail, Chat) wahrgenommen wird. Dieses Ergebnis wurde bestätigt von einer geschlossenen Frage, in der die Patienten am häufigsten die Informationsbeschaffung am bzw. für den Arbeitsplatz wählten, dann die Nutzung in Form von E-Mail und darauf folgend das allgemeine Aufrufen von Internetseiten. Im Besonderen galt es herauszufinden, ob Patienten das Internet bereits in medizinischer und psychiatrischer Hinsicht benutzt haben. 41 Prozent der Patienten mit Interneterfahrung hatten bereits medizinische Seiten besucht. 28 Prozent der Patienten, die das Internet benutzt haben, hatten auch psychiatrische Internetseiten besucht. Der Grund dafür war vor allem der Wunsch nach mehr Information, um sich über Medikamente aufzuklären und um sich vor einem Arztbesuch zu informieren. Ob die Patienten von den psychiatrischen Internetseiten im Sinne einer Krankheitsbewältigung profitiert haben, beantworteten immerhin 23 Prozent mit „ja“ und 32 Prozent mit „vielleicht“. Der größte Teil der Patienten (45 Prozent) hatte jedoch den Eindruck, dass die gefundenen psychiatrischen Informationen im Internet nicht hilfreich waren. Es ist anscheinend notwendig, mehr bzw. leichter auffindbare Seiten mit qualitativ hochwertigen Informationen zur Verfügung zu stellen. Die Mehrzahl der Patienten (60,5 Prozent) wünschte sich, dass das bestehende psychiatrische Angebot im Internet erweitert wird. Besonders interessieren die Patienten Informationen über psychiatrische Erkrankungen im Internet, an zweiter Stelle Möglichkeiten der Therapie bestimmter Erkrankungen. Auch Informationen über Medikamente und deren Nebenwirkungen sind für die Patienten wichtig sowie Kontaktmöglichkeiten und Adressen zu therapeutischen Einrichtungen. Überraschenderweise wäre es für die deutliche Mehrheit der Patienten mit Interneterfahrung (59 Prozent) leichter, mit einem Psychiater über das Internet in Kontakt zu treten.
