Guest Liz Reardon shares Liz Reardon has been a National Trainer for Mental Health First Aid since 2008, and is one of the founding members of the National Trainer program. She has over 30 years of experience in health care, from direct service to administration and public policy development, and has worked with a wide variety of consumers, providers and policymakers as a facilitator and trainer.As Managed Care Director for the Office of Vermont Health Access, she and her team pioneered the development of Medical Homes for people with special health care needs, including youth in Foster Care and adults with complex mental conditions. She was appointed by Governor Howard Dean to the Legislative Commission on Psychological Trauma and served on the task force that implemented the Commission's recommendations for Trauma-Informed care in Vermont's Agency of Human Services. Prior to her work in State government, Liz was the Administrative Duty Officer for Operations and Emergency Services and Assistant Director for Psychiatry and Prison Health at Bellevue Hospital in New York City.Liz studied Studio Art at Brown University and the Rhode Island School of Design and received a BA from McGill University in History and Philosophy of Science. She holds a Master of Public Health in Health Policy and Management from Columbia University. Troy Trygstad, PharmD, MBA, PhD Troy Trygstad is a community pharmacist, a health economist, and a pharmacy visionary who continues to encourage pharmacy industry leaders into new and innovative ways of thinking. Troy is the Executive Director of CPESN USA, a clinically integrated network of community-based pharmacy networks that endeavors to improve the quality and effectiveness of enhanced services provided by participating pharmacies and align them with the workflows and payment reform incentives of physicians, their support staff and other healthcare providers. Listen NowThe term "medical home" was first introduced in the 1960s by the pediatric profession.  Still to date this model of care emphasizes team-based comprehensive, continuous and coordinated care.   The care model is intended to improve primary care generally via improved patient communication, care quality, safety and outcomes.  In several ways the ACA encouraged the adoption of the, now termed, Primary Care Medical Home/PCMH for both the Medicaid and Medicare programs and among private health insurers.   (In some ways the PCMH is seen as a precursor for providers interested in becoming an Affordable Care Organization (ACO), i.e., taking on reimbursement risk.)  Over the past few years PCMHs have become widely adopted.  Over forty state Medicaid programs are experimenting with the model along with 90 commercial health plans and three federal initiatives. During this 23 minute discussion Dr. Nielsen discusses the PCPCC's purpose and goals, more specifically what is the PCMH model of care, the varying ways PCMH's are reimbursed, what does the research to date show regarding PCMH effectiveness and challenges in adopting this new model of care.Dr. Marci Nielsen currently serves as CEO of the Patient Centered Primary Care Collaborative (PCPCC), an organization dedicated to advancing an effective and efficient health system built on a foundation of primary care.  Prior to the PCPCC, Dr. Nielsen served as Vice Chancellor for Public Affairs and Associate Professor at the University of Kansas School of Medicine's Department of Health Policy and Management.  Dr. Nielsen was appointed by then-Governor Kathleen Sebelius as first Executive Director and Board Chair of the Kansas Health Policy Authority (KHPA).  She worked as a Legislative Assistant to Senator Bob Kerrey and later served as the health lobbyist for the AFL-CIO.  Dr. Nielsen is a board member of the American Board of Family Medicine and also a committee member for the Institute of Medicine's Leading Health Indicators for Healthy People 2020 and Living Well with Chronic Illness: A Call for Public Health Action.  Early in her career she served as a Peace Corps volunteer in Thailand and served for six years in the US Army Reserves.  Dr. Nielsen earned her MPH at The George Washington University and her Ph.D. from the Johns Hopkins School of Public Health.The PCPCC's 1/14 PCMH impact on cost and quality report can be found at: http://www.pcpcc.org/newsletter/annual-report-pcmhs-impact-cost-quality-2012-2013. This month we will address emerging health delivery models, with a discussion of the role of Advanced Practice Nurses and Health Care Reform, specifically with a discussion of the role of nurses in ACO’s and Medical Homes with Mary Jean Schumann. Ms. Schumann  is the Executive Director of the Nursing Alliance for Quality Care (NAQC). The Nursing Alliance for Quality Care (NAQC) is a bold partnership among the nation’s leading nursing organizations, consumers, and other key stakeholders to advance the highest quality, safety and value of consumer-centered health care for all individuals, their families, and their communities. NAQC believes that only with stronger, more unified nursing policy ‘voices’ will we achieve dramatic and sustainable achievement in quality and safety for the American public. Before joining the NAQC, Ms. Schumann served in various positions at the American Nurses Association, first as the Director, Nursing Practice and Policy, and later as Chief Programs Officer.  Currently, Ms. Schumann is on staff at George Washington University.  She taught pediatrics at the University of Wisconsin and Eastern Michigan University. She is a nationally certified pediatric nurse practitioner who maintained a clinical practice until 2006 as a hospice nurse in addition to her regular duties. Ms. Schumann co-authored a 2008 book with Dr. Margretta Styles, Kathi White and Carol Bickford, called Specialization and Credentialing in Nursing Revisited.

Clearly those chosing to sit on the sidelines notwithstanding, there is a land grab going on in plain sight. I note that as of early June alone, Dr. Charles Saunders reported a backlog of 180 health systems had approached Aetna's ACO Solutions Group to partner or sponsor some version of an ACO or medical home in one or more of their respective markets.
