The major change early on was the transition from that of nurses mixing chemo to that of pharmacists. Regulatory agencies like NIOSH and OSHA defined chemotherapy as hazardous drugs, and professional organizations became involved, leading to the publication of the joint ASCO and ONS Standards of Safe Handling,” ONS member Scarlott Mueller, MPH, RN, FAAN, secretary of the American Cancer Society Cancer Action Network Board and member of the Oncology Nursing Foundation Capital Campaign Cabinet, told Darcy Burbage, DNP, RN, AOCN®, CBCN®, ONS member and chair of the ONS 50th Anniversary Committee during a conversation about the evolution of chemotherapy treatment. Highlights From This Episode Hillson: “I remember as a new grad, from back in '98, walking up to the oncology floor. We had patients with pink labels on the chart and that was the radiation oncology service. I hadn't heard of such a thing before. … I'd gone through nursing school and hospital orientation and unit orientation without ever hearing of these therapies. At the time, both the management and the union had no interest in specialist nurses, and the really weren't any books that were targeting the role. And it was very isolating and frightening. I was very glad to find ONS when I moved to the U.S. Right now, the Oncology Nursing Society Manual for Radiation Oncology, Nursing Practice, and Education, it's in its fifth edition and a sixth is underway. There's nothing else like it. Most books are very much geared towards other professions.” TS 5:34 Mueller: “We mixed our chemo in a very small medication room on the unit, under a horizontal laminar flow hood, which we later discovered should have been a vertical laminar flow hood. Initially, we did not use any personal protective equipment. I remember mixing drugs like bleomycin and getting a little spray that from the vial onto my face. And to this day, I still have a few facial blemishes from that.” TS 14:28 Anderson: “As the increasing number of these actionable mutations continue to grow, so will the number of oral anticancer medications that patients are going to be taking. And we are already seeing that there's multiple combination regimens and complex schedules that the patients have to take. So this role the oral oncolytic nurse and the nursing role, like you said, it cannot be owned by one individual or discipline. So it's not a pharmacist; the pharmacies aren't owning this. The nurses are not owning this. It takes a village.” TS 32:12 Shannon-Dorcy: Then as immunotherapy comes into the picture, we start to learn about [cytokine release syndrome]. All of a sudden, we had no concept that this was a deadly consequence. ONS was on the front lines, convening people across the country together so we could speak to the investigative work with science and find ways that we could intervene, how we can look for signs of it early on with handwriting testing.” TS 39:58

By providing nurses with the essential skills and knowledge to lead compassionate palliative care conversations, this initiative empowers staff to facilitate critical "goals of care" discussions with patients and families. Kelli has her ONS chemotherapy administration certification, is a certified Aromatherapy provider, and a Level 2 Reiki practitioner. Kelli has planned, presented, hosted, and facilitated many conferences from Women and Heart Disease to Nursing Research to Integrative Therapies but the most rewarding was her work with End of Life Nursing Education Consortium (ELNEC). Kelli implemented ELNEC across a five-hospital health system from 2013-2018.  This included securing grant money for the project and educating more than 900 staff in end-of-life patient care. Kelli was the Winner of International ELNEC Award for excellence in education, has as presented at local and national conferences, and is published in her field.  Kelli continues to be a strong advocate for palliative care and started a palliative care champion model for nurses in her health system to support identified knowledge and practice gaps. Kelli is a Nurse Residency Facilitator where she helps to support and mentor new graduate nurses in their transition to their first year as a professional nurse and lectures for the program on End-of-Life Nursing and Self Care. She teaches classes in oncology, telemetry nursing, and preceptor development for her health system. Christine is an adoptee born in January of 1972, at the tail end of the baby scoop era. She was adopted in March of that year through Catholic Family Charities. Her mother had bought a book that explained how a family adopts a child in a very simplistic way. In fact, until the age of seven, she thought that all children were adopted. It wasn't until a friend's mother got pregnant that she realized there was another way to have a baby. Her mother explained to her that she was special—that she was chosen.Growing up, the subject of her adoption was not spoken of in her household. She had been told at a young age that she was her parents' child and that they would not discuss the matter of her adoption. She rarely told anyone about it, not even her closest friends. Christine never searched for her birth family. She knew it would make her parents unhappy and was scared about what she might find out. As they got older, her children's curiosity about their actual ethnicity led her to Ancestry DNA. Two years later, after receiving the results, Christine received an email through Ancestry. It stated, “I believe that you are my niece. That would be so wonderful.”The next few months were a process of meeting her birth mother, Pat, two half-brothers, and her maternal birth family. She found out that Pat was 16 when she got pregnant. She had been sent to a home for unwed mothers and gave birth to Christine two weeks after her 17th birthday. Pat was supposed to relinquish her parental rights after Christine's birth but refused to do so. Christine was placed in foster care for two months until Pat realized it was a losing battle and signed the relinquishment papers. For years, Pat slept with Christine's picture under her pillow.Christine and Pat had the opportunity to meet and get to know one another. They were developing a relationship when Pat succumbed to cancer, only 18 months after their reunion. Christine remains in contact with her brothers, aunts, uncles, and cousins.Today, Christine lives in Connecticut with her partner, Rob. Between them, they are fortunate to share four boys and a girl. She also has completed a fellowship in pediatric palliative and hospice nursing through University of Illinois in Chicago and through the Coleman Foundation. This led her to also become a board-certified pediatric hospice and palliative nurse in 2009.  Meggan, along with an interdisciplinary committee, started the Pediatric Palliative and Supportive Care program at Advocate Children's Hospital - Oak Lawn in October of 2012.  Her background as a pediatric intensive care nurse is where her love and passion for working with children with critical and chronic illness grew.  She is an End-of-Life Nursing Education Consortium (ELNEC) trainer and received that training at St. Jude's Hospital in Memphis, TN.  Meggan is an active member of the Hospice and Palliative Nurses Association (HPNA) where she serves on the Advocacy and Legislative Committee. She participated as both an item writer and chair for the pediatric hospice and palliative nurse exam from 2010-2016. Meggan was also recently inducted as a Fellow of Palliative and Hospice Nursing (FPCN®) and is so thrilled to be recognized for her true passion and purpose to this field.   Becoming a widow and a solo parent in 2017 after her husband Andrew died from advanced colorectal cancer has only grown her passion for delivering and advocating for nothing short of the best delivery of palliative and hospice care. Meggan is blessed to be given a new chapter and now lives with her husband Nate, eight year old son Andy, bonus seven year old daughter Elizabeth, and two and a half year old rescue dog Zoey, in Plainfield.   Within palliative care, Meggan has a strong interest in the areas of patient-provider boundaries, patient and family advocacy, and the delivery of diverse, equitable, and inclusive care.   Meggan believes that caring for children with serious illnesses truly starts at the grassroots of relationships. She believes that the family is the center of the care being provided and it is a relationship built on partnership. Helping children live life to their fullest ability without burdensome symptoms is something that she strives to achieve each day.  Brett Snodgrass, DNP, FNP-C, ACHPN®, FAANP Dr. Brett Snodgrass has been a registered nurse for 28 years and a Family Nurse Practitioner for 18 years, practicing in multiple settings, including family practice, urgent care, emergency departments, administration, chronic pain and palliative medicine. She is currently the Operations Director for Palliative Medicine at Baptist Health Systems in Memphis, TN. She is board certified with the American Academy of Nurse Practitioners. Today on Yoga|Birth|Babies I have Caitlin Jeanmougin. Caitlin is a registered nurse with 21 years of experience caring for childbearing families. She has a master's degree in Nursing Education as well as a Doctorate in Nursing Practice. In this episode, we explore two innovative projects—Project UPHOLDS (Utilizing Palliative care for Heart failure Optimized using Lay navigators to Decrease Suffering) and Project ADAPT HF (ADdressing All Pain Through navigator-led palliative care optimized for Heart Failure)—which focus on utilizing community-based lay navigators to optimize palliative care for heart failure, especially among older Black adults. We delve into the importance of shared decision-making and how co-designed interventions are shaping pain management trials, with a special emphasis on inclusivity and addressing under-representation in research. Rachel shares her personal journey and career trajectory, highlighting the privilege of being able to advocate for these communities and the impact of culturally responsive care. Rachel Duncan Wells, PhD, MSN, RN, CNL Dr. Rachel Wells is an Assistant Professor and Core Mixed Methods Faculty in the School of Nursing at the University of Alabama at Birmingham. As a nurse and Clinical Nurse Leader with more than 10 years of clinical experience in cardiac critical care, palliative medicine, and rural health, her research bridges the fields of chronic illness care and early palliative care and is focused on refining palliative care access for under resourced older adults living with serious illness. Specifically, Dr. Wells has focused on the development and testing of highly efficient and effective models of early palliative care for those living with advanced heart failure. Her emerging program of research focuses on the examination of active palliative care intervention elements and dosing of palliative care to develop optimized interventions that uniquely address palliative care needs, a novel approach to addressing health disparities in under resourced palliative care populations. She has been involved with a number of federally-, foundationally-, and intramurally funded grants as a PI and Co-I that have involved intervention development and tailoring and clinical trials testing and implementation of models of early palliative care for those living with serious illness and their family caregivers. Dr. Wells is regularly engaged in the American Academy of Hospice and Palliative Medicine, American Heart Association, American College of Cardiology, and the Hospice and Palliative Nurses Association, where she has championed efforts to educate clinicians and others to identify, monitor, and intervene early for cardiovascular challenges in under resourced populations.    Brett Snodgrass, DNP, FNP-C, ACHPN®, FAANP Dr. Brett Snodgrass has been a registered nurse for 28 years and a Family Nurse Practitioner for 18 years, practicing in multiple settings, including family practice, urgent care, emergency departments, administration, chronic pain and palliative medicine. She is currently the Operations Director for Palliative Medicine at Baptist Health Systems in Memphis, TN. She is board certified with the American Academy of Nurse Practitioners. She is also a Fellow of the American Association of Nurse Practitioners and an Advanced Certified Hospice and Palliative Nurse. Highlights From This Episode “Some challenges with subcutaneous injections are with the administration, especially when we're thinking about large-volume drugs. … Some of these patients who have been through multiple therapies, they've been on a long journey, or just in general they may have small amounts of subcutaneous injection areas and tissues, so that could be problematic. … Also, some patients may want to go back to receiving IV medications if they experience severe pain at an injection site during administration, or maybe they had a site-related reaction. This is where the nurses play a huge, crucial role in the administration of these subcutaneous drugs.” TS 5:17 “When administering large-volume subcutaneous injections, good ergonomics is very important during the administration because this can help reduce the fatigue and discomfort not only for [nurses] but for the patients as well. If you're trying to hold the needle in place for 5–10 minutes, it's a lot of work. Your arms can start to shake, and that shaking can cause discomfort for the patient as well. The utilization of a winged infusion set for these large volumes allows more space between the patient and the nurse, which supports better ergonomics.” TS 11:20 “When they came to the market, there was an unfounded concern from patients and practitioners that these injections would not be as effective as their IV counterparts. This is totally incorrect. We know that these options have the same efficacy and may actually also help to reduce the incidence of any infusion-related reactions, as well as lower side-effect impacts on patients, so overall, a lot of improvement with these high-volume subcutaneous injections for the patient experience.” TS 21:37 “I'm just really looking forward to the future landscape of oncology practice and drug approvals and drug administration. It's so important that subcutaneous injections have really made a name for themselves in nursing practice today. We continue to see more subcutaneous formulations on the market that are available for patients, allowing them less time in infusion chairs and more flexibility and freedom outside of the healthcare setting.” TS 24:39

You can actually directly visualize the veins to assess not only the health of the vein, but some of the complications that could be there, like a thrombus in the vein or sclerosis or tortuous anatomy, arteries, nerve bundles. Those are things that you can now see with your machine.” TS 8:55 “I think that the most important part of [training] is having a really good didactic session where nurses come in and they learn reminders about the anatomy. Where are these veins? Where are the best veins to canulate when you're using ultrasound? And we like to avoid the veins above the antecube for regular long peripheral IVs that we insert with ultrasound because we want to preserve those veins up higher for our [peripherally inserted central catheter] lines and midline. So we want to teach to try to use the forearm. The cephalic vein in the forearm is a really excellent vein to choose.” TS 17:24 “[Patients] are usually kind of impressed with the machine and the technology, and I explain that ‘We're not able to get it without being able to see better, so I'm going to use my machine so that I can see better.' And almost every time after I'm done, the patient is like, ‘Wow, are you done?' … It's the initial little puncture that hurts the patient. But unlike when we do it blindly and maybe we don't get it right in the vein, and we're having to dig around and reposition ourselves and get into that vein, we're not doing that with ultrasound because you're going to go into the vein, and then you're starting to do the threading, and you're pulling your probe up as you go to get that catheter in the vein. The patient doesn't feel that part. So they often comment about how they barely felt it and they can't believe it's over.” TS 21:21 “This is kind of my measure of success when we're no longer kind of putting this on the patient. We're not saying, ‘You have difficult veins. Your veins roll. You're not drinking enough.' That's not okay anymore. We've got to take responsibility and use technology to do this more successfully.” TS 30:24

In this podcast episode we want to introduce you to our BCEN Friends, Sean Elwell and Adam Oliver. Sean Elwell is the Senior Director of Emergency Services, Critical Care Transport, and the Pediatric Trauma Center at Nemours Children's Hospital in Wilmington, Delaware, where he has dedicated over 21 years of service. Starting his career as an EMT, Sean transitioned to become a registered nurse and earned his Doctor of Nursing Practice from Wilmington University. He holds a master's degree in nursing leadership and health systems management from Drexel University, graduating Summa Cum Laude. A recognized leader in his field, Sean has published numerous articles and presented at regional and national conferences. He is a Past-President of the Society of Trauma Nurses and currently serves as Secretary/Treasurer on the Board of Certification for Emergency Nursing. In addition to his professional roles, Sean actively contributes to his community as the Mayor of Elsinboro Township, where he has served for 18 years, including 14 years as Mayor. He has held various leadership positions within local government and is currently on the Executive Board for the New Jersey League of Municipalities, set to become First Vice-President this November. Sean also dedicates time to volunteer with the Elsinboro Fire Company, where he serves as Chief. His extensive experience and commitment to both healthcare and public service make him a respected leader in his community. Adam Oliver is a dedicated healthcare professional with over two decades of experience in the emergency medical field. He resides in Gilliam, Louisiana, located in northern Caddo Parish, approximately 30 miles north of Shreveport. Currently Adam serves as the Director of Integrated Health for LifeNet, Inc. and manages three rotor-wing assets and one fixed-wing asset within the Air Division, while also overseeing the development of community paramedic and telehealth programs. As a passionate advocate for community service, Adam is the Mayor of Gilliam and has previously served as a town council member. His commitment to public safety is further exemplified by his ten-year service as a Board Commissioner for Caddo Fire District Seven, which operates in a large rural area. Recently appointed to the Hospital Service District board, he plays a vital role in supporting the North Caddo Medical Center, a critical access hospital that includes five rural health clinics, two pharmacies, and a dental clinic. Since a young age, Adam has been committed to serving others in their most vulnerable moments. His diverse experiences as a paramedic, emergency nurse, ICU nurse, and flight nurse have shaped his approach to healthcare and community engagement, reinforcing his belief in the importance of compassion and support in times of need. The first four years of her practice were spent bedside on a busy ortho/neuro unit where she found her passion for newly graduated RNs, interdisciplinary collaboration, and professional governance. Sam is an unwavering advocate for medical-surgical nursing as a specialty and enjoys helping nurses prepare for specialty certification.    Laura Johnson, MSN, RN, NPD-BC, CMSRN has been a nurse since 2008 with a background in Med/Surg and Oncology.  She is a native Texan currently working in the Dallas area.  She has held many positions throughout her career from bedside nurse to management/leadership to education.  Laura obtained her MSN in nursing education in 2018 and is currently pursuing her DNP.  She has worked both as a bedside educator and a nursing professional development practitioner for both new and experienced staff.  She enjoys working with the nurse residency program as a specialist in palliative care/end of life nursing and mentorship.  She is currently an NPD practitioner for oncology and bone marrow transplant units.             Neil H. Johnson, RN, BSN, CMSRN, epitomizes a profound familial commitment to the nursing profession, marking the third generation in his family to tread this esteemed path. Following the footsteps of his father, grandfather, grandmother, aunt, and cousin, all distinguished nurses, Neil transitioned to nursing as a second career after a brief tenure as an elementary school teacher. Currently on the verge of completing his MSN in nurse education, he aspires to seamlessly integrate his dual passions. Apart from his unwavering dedication to nursing, Neil actively seeks serenity in nature alongside his canine companions. In his professional capacity, he fulfills the role of a med-surg nurse at the Moses Cone Health System in North Carolina.   Eric Torres, ADN, RN, CMSRN is a California native that has always dreamed of seeing the World, and when that didn't work out, he set his sights on nursing.  Eric is beyond excited to be joining the AMSN podcast and having a chance to share his stories and experiences of being a bedside medical-surgical nurse.   Maritess M. Quinto, DNP, RN, NPD-BC, CMSRN is a clinical educator currently leading a team of educators who is passionately helping healthcare colleagues, especially newly graduate nurses. She was born and raised in the Philippines and immigrated to the United States with her family in Florida. Her family of seven (three girls and two boys with her husband who is also a Registered Nurse) loves to travel, especially to Disney World. She loves to share her experiences about parenting, travelling, and, of course, nursing!   Sydney Wall, RN, BSN, CMSRN has been a med surg nurse for 5 years. After graduating from the University of Rhode Island in 2019, Sydney commissioned into the Navy and began her nursing career working on a cardiac/telemetry unit in Bethesda, Maryland.  Currently she is stationed overseas, providing care for service members and their families.  During her free time, she enjoys martial arts and traveling.

Dr. Kristin Arden is Lead Clinician at Mindbloom (a ketamine therapy service) and is a board-certified Psychiatric Nurse Practitioner with over 15 years experience working in mental health. Her work has been instrumental in the development of Mindbloom's treatment methodologies.In addition to work at Mindbloom, Arden was a guest lecturer at NYU on topics such as psychedelic therapy and substance use disorders, and practices addiction medicine, from a harm reduction mindset, with individuals in the justice system. She graduated from NYU's Psych NP program with additional specialization in substance use disorders and has a Doctor of Nursing Practice from Rush University.Shari Botwin, LCSW, has been counseling survivors in recovery from all types of traumas in her Cherry Hill, New Jersey private practice for over twenty-eight years. She is a regular guest trauma expert on the Law and Crime Network. Additionally, she has served as an expert witness in high profile cases of sexual assault and childhood abuse.
