Today, we're discussing Care Management in Managed Medicaid with Tammy Yount, CHESS Application System Analyst. We'll explore why it's essential for organizations to tailor their care management programs to fit their own unique needs, so they can holistically focus on the patient while optimizing value in healthcare.Tammy Yount, welcome to the Move to Value podcast.Thank you Thomas, for inviting me and I'm happy to be here.So, Tammy, let's talk about care management in managed Medicaid. How does a care management program save money and healthcare?I would say that saving money is one goal of a care management program. However, I would offer that the goals of a care management program should align with the triple aim that's born out of the 2001 Institution of Medicine report Crossing the Quality Chasm. So that report underscored 3 aims, if you will, one primary aim and two secondary aims. So the primary aim is to improve the health of populations with the secondary aims of improving the patient experience of care and at the same time reducing the per capita cost. So these are lofty aims given our current healthcare landscape and the payment models that we exist in. Not all organizations are the same. You have some large organizations that have a plethora of resources and smaller organizations with very limited resources. So each organization has its own unique structure and individual challenges and there's no one-size-fits-all care management program. So I would say there are many ways to build a care management program that will allow you to achieve the triple aim and organization needs to find the blueprint that works best for them. So when an organization's doing the right things, measuring the right things and focusing on improving the right things, the cost savings should follow. And I believe it was W Edward Demmings that said it best, you know, manage the cause, not the result. That's not to say that the organization doesn't need to have a clear understanding of the underlying processes, cost drivers, the population characteristics. He also said if you can't describe what you're doing as a process, you don't know what you're doing. And my favorite quote for him is in God we trust, and all other things bring data.Tammy, tell me, how would a practice create a care management program?So it's a bit of a chicken and egg conundrum when you're trying to create your care management program, you need many things in the least of which is data. I would say you need to start with the data, but few organizations have the data to inform their program design. Most organizations design their care management program backwards, meaning they design the program around the resources they have versus identifying the resources that they need based on the characteristics of the populations they're managing. So I would say the first thing you need to do is collect data and evaluate the data. So from the data that you've captured, then you would begin to develop your road map for how you're going to operationalize your care management program. And these would be very specific to each organization because each organization serves different patient populations, has different resources and different needs.What are the keys to a successful care management program?It's going to depend on who you ask. So I'm a data person, so in my world, all things starting in with data. But if you were to ask the payer, the nurse, the CEO, the CFO, the CIO, and most importantly, the patients, their families or caregivers, you're going to get a different response and varying perspectives. For a care management program to be successful, it's going to need to combine all of these perspectives. And critical to any successful program is having a mission, vision and values. And you'll need to operationalize your plan with those elements. And so...

Our first interview finds a real-life, “Last Mile” solution that will bring historically under-served populations to the health care services they need and deserve. Our second interview reveals how improved efficiencies in digitalization actually make it easier to add even more data – and more data crunching – into the mix. PHIT4DC stands for the Public Health Informatics and Technology for the District of Columbia Workforce Diversification Program. PHIT4DC brings together public health programs at two HBCUs—historically Black Colleges and Universities—namely, the University of the District of Columbia and Howard University. Together, these esteemed institutions train professionals from historically under-served neighborhoods in 21st-century IT knowledge and skills so they can return to their neighborhoods and give back to their communities.PHIT4DC trains PCMH professionals in order to send them to work in and support their hometown communities and neighborhoods. It's a powerful solution for bridging the gap in health equity provision. And it's probably possible to set up this model in any U.S. city. But, as you can imagine, it's no small feat. Dr. Mary Awuonda currently serves as an Associate Professor and Director of the Center of Excellence at the Howard University College of Pharmacy. In her directorship role, she helps the College advance its health care workforce diversification mission and student academic success initiatives. She is published in the areas of minority health, health disparities, health outcomes research and workforce diversification. Hannah George is a health care consultant with years of service across the health care industry. She's been a college professor/mentor for nursing students and director of nursing for multiple home health agencies in the District of Columbia. She's worked on multiple health care research protocols and served as senior clinical lead on multiple projects and initiatives. Hannah is certified in Project Management, is a Certified Professional in Healthcare Quality (CPHQ) and a Certified Professional in Patient Safety (CPPS).Digitalization ensures the safe and efficient transfer and parsing of health care data between providers. Patients benefit, clinics benefit, clinicians can spend more time with patients. And improvements in data transfer and parsing reveal more population data than ever before, which uncovers gaps in health equity. And with that population data, researchers can start to reverse deficiencies and ensure better health care for all.Josh Hetler is Chief Operations Officer at DataLink and an expert in the potential revelations of supplemental data. Josh has over a decade of experience developing software products for advancing value-based health care. With coordination of care at the frontlines of every patient visit, she works to ignite the path towards healthcare excellence. Her other passion is leadership. The Matthew Walker Comprehensive Health Center has been serving North Nashville's uninsured and underinsured citizens since it opened in 1968. It's an indispensable health resource for the community, from family medicine and dentistry, to behavioral health and educational programs.  The center is named for Dr. Matthew Walker, an influential Black surgeon. Originally from Louisiana, he came to Nashville for medical school. Although he was an expert surgeon, he saw that basic preventative health measures like brushing teeth and eating healthy food could make a tangible impact on people's health. He was passionate about community health and educating the next generation of doctors. This this episode, we talk to Dr. Walker's family about who he was and hear about how the health center carries on his legacy. But first, Gov. As an Army Nurse, Kate served in Viet Nam (27th Surgical Hospital in Chu Lai) where she was awarded the Bronze Star for meritorious service. Kate has worked with orthopedic patients in several capacities including Head Nurse of Orthopedics in Newark, NJ., followed by Biomet working in various capacities.  Kate joined The Compliance Team (TCT) in early 2012 to direct TCT's rural health clinic accreditation program and has fallen in love with Rural. As VP of Clinical Services, she has spearheaded the TCT Rural Health Clinic Accreditation program combining her clinical expertise, business acumen, and passion for delivery of the best care possible to every patient.  She presently serves on the Board of the National Association of Rural Health Clinics. Kate also works with clinics in TCT's PCMH program and is ensuring that a PCMH accreditation is being increasingly rewarded by payers.

When you hear about value-based care, do you get tired of hearing about concepts without tangible best practices?  Do you ever wish you could just acquire insights from a leader who navigated a successful value journey?  If you want to learn from one of the best in the “value game”, look no further than Dr. Bill Wulf, the CEO of Central Ohio Primary Care (COPC). Dr. Wulf is a respected leader in the value movement and leads the largest physician-owned primary care group in the United States.  During his leadership tenure, COPC has grown to over 480 physicians and 83 locations in central Ohio. The growth of the practice has empowered a successful value journey, with COPC caring for 75,000 senior patients in full-risk arrangements with Medicare Advantage and ACO REACH in partnership with Agilon Health (and the current move to full-risk in commercial plans with employers in partnership with Vera Whole Health). Dr. Wulf describes a value journey that has been over two decades in the making.  It started with a merger in the late 90's to create a fully-integrated primary care practice platform. And then in 2010, a Patient-Centered Medical Home (PCMH) transformation led to unprecedented success in full-risk Medicare Advantage.  COPC has built upon their MA success to now partner with large employers in full-risk programs, and they are also one of the new participants in the ACO REACH program. In this interview, Dr. Wulf goes into great depth on the care delivery innovations that were made possible by prospective payment and capital investment. He discusses hospitalist and ER care coordination programs, home-based care delivery, after-hours primary care access, telehealth, onsite clinics at employer locations, and the importance of data-driven insights from a unified EHR. The NCQA PCMH Content Expert Certification program certifies individuals who demonstrate comprehensive knowledge of NCQA's PCMH program. These professionals know the how and the why of supporting NCQA's endeavors to improve quality through measurement. The 2022 NCQA CCE Award winners represent the best of the best in helping bring hundreds, if not thousands of patients to value-based health care.This year's winners are Kelsey Keith, Elizabeth Sledge, and Shannon Nielson.Kelsey Keith is the PCMH Program Manager for the Northeast Georgia Physician's Group. Kelsey earned her CCE from NCQA in July of 2014. In her current role, she manages all NCQA programs. Gilbert W. Foertsch Gilbert Walter Foertsch was born on July 24, 1933 to parents Oscar and Edna (Polster) Foertsch. He was reborn by God's grace in Christ Jesus in the waters of Holy Baptism on August 10, 1933 at St. John's Ev. Lutheran Church, Evanston, Indiana by Rev. F. Breuer. His baptismal faith was nurtured through God's Word taught in his home and at church. On March 30, 1947, at St. John's, he publically confessed his baptismal faith and was confirmed by Rev. Otto Mueller. For Gib's confirmation verse Pastor Mueller chose Revelation 2:10 “Be thou faithful unto death, and I will give thee a crown of life.” Gib rejoiced to receive the life-giving body and blood of Jesus. On April 12, 1958 the Lord united Mona Faye Huebschman and Gib in the blessed estate of marriage. They were married at Emmanuel Lutheran Church by Rev. John E. Schroeder. The Lord blessed them with 63 years of married life together. God also blessed their marriage with the gift of children: Jeff & David. God blessed Gib's life with many special people as he served God in his vocations at home, church, work, and in the community. He served the Lord laboring for 42 years for Can Clay. He had also served the Lord as a soldier in the U.S. Army. Gib was an active member of Emmanuel Lutheran Church were he served faithfully on several boards and committees, serving as a deacon or trustee for 32 years. On August 21, 2021, God called Gib home to heaven. He fell asleep in Jesus' loving arms at PCMH. On Saturday, August, 2021 the Lord delivered Gib from this valley of sorrow and took him Home to Himself in Heaven. Gib fell asleep in Jesus' nail scarred hands at Perry County Memorial Hospital, Tell City, at the age of 88. Left to look forward to the grand family reunion that awaits all Christians on the Last Day in the resurrection of their bodies and the life of the world to come are his wife, Faye, sons, Jeff Foertsch of Tell City, IN and David (Mary) Foertsch of Jasper, IN; grandchildren, Ryen, Dustie, Kyle, Kaci; great grandchildren, Clara, Alexander, Ruby, Sawyer, Julia and Conner. He was preceded Home to Heaven through death by his parents and brother Oscar. Precious in the sight of the Lord is the death of His saints.

Dr. Martha Rodriguez, founder of MMR Healthcare discusses interoperability and her firsthand experience of using data to improve both practice outcomes and patient experience. She talks about her use of eClinicalWorks who helps providers improve interoperability and have greater access to data, giving them access to live data, giving them access to Carequality and CommonWell Health Alliance networks, and securely exchanging over 50 million records per month with other practices and hospitals. As a well-established primary care physician in Boynton Beach, Florida, Dr. Martha Rodriguez has sculpted a foundation of medicine that prioritizes coordination of care. As a child, Dr. Rodriguez knew this would be her life's work. She earned her medical degree from the Universidad Central del Este, Dominican Republic, and completed her residency at Hackensack Medical Center, New Jersey in 1993. She went on to establish her private practice in 1997. Currently, Dr. Rodriguez owns and operates her private practice at a state-of-the-art facility she has coined, MMR Healthcare. Her field of specialty is geriatric care and making patients' golden years shine. MMR Healthcare has been PCMH recognized since 2014. In 2012, she co-founded one of the highest performing Accountable Care Organizations in the country and served as its Chairman of the Board until the end of 2016. With coordination of care at the frontlines of every patient visit, she works to ignite the path towards healthcare excellence. Her other passion is leadership. She was the medical director of several skilled, assisted, and independent living facilities for many years. Including all the local facilities for Life Care, which included Harbour's Edge and the Abbeys. She is a graduate of the Palm Beach Medical Society Leadership Academy and currently enrolled in the American College of Physicians Leadership Academy. She has been on the Board of Directors of the Palm Beach Medical Society for several years as well as part of the Executive Committee. Dr. Rodriguez also chairs the Women's Council for the Medical Society. She is a teacher for NSU College of Pharmacy where she is heavily involved in pharmacy students' and residents' training as well as research which has led to multiple publications with the University. Kate is a US Army Veteran and Bronze Star Recipient, awarded in recognition of meritorious service in a combat zone while serving in Viet Nam. You may know her as Vice President of Clinical Services with The Compliance Team. Here is what Kate had to say about receiving the Bronze Star: “I was humbled beyond belief… I was overwhelmed because there were so many more deserving, particularly men, who served in the jungles in great peril… And so frankly, I was shocked… It reads meritorious service in a combat zone, and that it was. I spent many nights in a bunker…” ~Kate Hill, RN Kate Hill, RN, grew up outside of Philadelphia and is a graduate of Einstein Medical Center School of Nursing. As an Army Nurse, Kate served in Viet Nam where she was awarded the Bronze Star. Kate has worked with orthopedic patients in several capacities including nearly 3 decades with Biomet in various capacities. Kate joined The Compliance Team in early 2012 to direct TCT’s rural health clinic accreditation program and has fallen in love with Rural. As VP of Clinical Services, she has spearheaded the TCT Rural Health Clinic Accreditation program combining her clinical expertise, business acumen and passion for delivery of the best care possible to every patient.  She presently serves on the Board of the National Association of Rural Health Clinics. She feels fortunate to have been able to speak at numerous state and national meetings about RHC compliance countrywide which gave her the opportunity to learn firsthand the diverse regional issues clinics are facing. Kate also works with clinics in TCT’s PCMH program and is seeing that a PCMH accreditation is being increasingly rewarded by payers. Kate lives in suburban Philadelphia with her husband and near her three granddaughters.

Pioneer Physicians Network, an independent physician group in Northeast Ohio, has used PCMH recognition and participation in CPC+ to develop a more structured and disciplined approach to care. As a result, the network realized the value of opening after-hour clinics to serve patients who would otherwise continue to go to local emergency departments. The network is now improving ED utilization, controlling costs, and better understanding the urgent care needs of their patients.

In literally minute-by-minute, life and death situations, healthcare providers around the world are putting their own lives at risk as they answer the call to treat overwhelming numbers of Coronavirus patients. The situation is nowhere more challenging than in New York State, Long Island and here in Stony Brook, where medical professionals have been keeping pace with the surge day by day. On this episode of “Beyond the Expected," hosted by Interim President MIchael Bernstein, Stony Brook University Hospital attending physicians share their personal experiences of being infected by, and recovering from, COVID-19, in this environment, and Stony Brook University Hospital's Medical Director of Healthcare Epidemiology provides her expert opinions on how to stop the spread among patients and providers. Susan Donelan, MD, FSHEA, Medical Director, Healthcare Epidemiology Department; Hospital Epidemiologist; Assistant Professor, Infectious Diseases and Medical Director, Regional Training Center, MARO Region Susan Donelan has a strong clinical expertise in infectious diseases such as Lyme Disease and West Nile Virus, hospital-acquired infections, and MRSA. She has an interest in emerging infectious diseases, and her Ebola care plan is adaptable for patients who may have diseases like MERS-CoV and avian influenza. She has certified training in pandemic planning and preparedness, and trained for a week in Anniston, Alabama at the Center for Domestic Preparedness, with a certification in Healthcare Leadership and Decision Making. Dr. Donelan will share how all of this training and experience is being put to work in our current pandemic crisis to help prevent the occurrence, and spread, of COVID-19 among patients and the healthcare professionals treating them. Beyond Stony Brook, Dr. Noel is an appointee to the New York State Department of Health Regulatory Modernization Initiative Telehealth Advisory Committee. She is leading a program for the American Telemedicine Association 2020 Conference for Telehealth Accessibility for persons with disabilities. James Vosswinkel, MD, FACS, Chief, Trauma, Emergency Surgery and Surgical Critical Care; Medical Director, Trauma Center Surgical ICU, Stony Brook Medicine; and Lillian and Leonard Schneider Endowed Professor in Trauma Surgery James “Voss” Vosswinkel's specialties include surgical management of injured patients in all aspects of traumatology. That includes management of diseases of the gastrointestinal and endocrine systems; treatment of soft tissue disease; repair of hernias; appendectomy; cholecystectomy; laparoscopic surgery; and pre- and post-operative critical care of adult surgical patients. Before and during this Coronavirus pandemic, he has been leading efforts to increase our Intensive Care Unit (ICU) capacity; collaborate with medical professionals locally, across the country and globally to share experiences and best practices; and develop innovative ways to treat patients in trauma under unimaginably challenging circumstances.

Eryc Taylor, Artistic Director/Founder of Eryc Taylor Dance, has presented work worldwide since 1995. Originally from Los Angles, he received his BFA in Dance from the University of the Arts, Philadelphia, PA, and an MFA in Dance from New York University's Tisch School of the Arts. Eryc Taylor has studied abroad at both the London Contemporary School of Dance and the Centre de Danse du Marais in Paris. Taylor has been awarded several major scholarships, including residency at Jacob's Pillow, and spent two years with Merce Cunningham's Junior Ensemble (RUG). He is currently on the University of the Arts Alumni Council. Taylor has performed works by Merce Cunningham, Mark Dendy, Group Motion, Paul Taylor, Milton Meyers, Sean Curran and Gus Solomons Jr, and strongly believes in artistic collaboration. With Eryc Taylor Dance, he has worked closely with artists Chris Annas-Lee, Mark Beard, Gerald Busby, Simon Hayes (Swarm Intelligence), David Kagen, Sidney Grant, Scooter LaForge, Salomon Lerner, Laura Peterson, Daniel Tobias, and Stephan Michael Smith. In this health care podcast, seven thought leaders talk about the areas of promise they see in health care in 2020. Seven thought leaders include: Kimberly Noel, MD, from Stony Brook Medicine Eric Weaver, from Innovista Health Solutions Suzanne Delbanco, from Catalyst for Payment Reform Sue Schade, from StarBridge Advisors Naomi Fried, from Health Innovation Strategies Joe Grundy, from Grundy Consulting Adrian Rubstein, from Merck Just a couple of comments up front here. I don’t want to further my reputation for dropping major spoilers, however, so I’ll keep this short. Many of the thought leaders today talk about AI in various contexts. Are you rolling your eyes right now? If so, let me remind everyone about the Gartner Hype Cycle. The first step is wild-eyed enthusiasm. The next step in the hype cycle is anger, the old trough of disillusionment. I’d suggest that as far as AI is concerned, we are coming out of that trough and AI—be it artificial intelligence or augmented intelligence or machine learning or deep learning or whatever you choose to call it—it is being used, for reals, for various applications. Other corroborations among our thought leaders include the importance of exalting primary care, in the form of what some may call direct primary care and Zeev Neuwirth calls complex-condition care or condition-specific care—a relationship model, if you will. Another idea that comes up in various ways is the idea of breaking down silos and getting everyone with a stake in patient health to the table and focused on achieving better patient outcomes using all the technology and wherewithal available to us in 2020. By all the stakeholders, I mean going beyond the usual suspects of providers and insurance carriers—meaning employers. Also meaning Pharma, in the sense of Pharma taking the opportunity to collaborate more deeply toward outcomes their medications can potentially confer … IRL with RWE. Today’s episode features the following guests: Kimberly Noel, MD, MPH, is a board-certified, preventive medicine physician. She serves as the telehealth director and deputy chief medical information officer of Stony Brook Medicine, where she provides leadership to all telehealth activities of the health system. Dr. Noel is also the chief quality officer of the patient-centered medical home (PCMH) for the family medicine department, working on quality improvement and population health management for National Committee for Quality Assurance (NCQA) designation. She practices occupational medicine clinically and provides digital solutions for employee wellness programs. She is an appointee the New York State Department of Health Regulatory Modernization Initiative Telehealth Advisory Committee and has won many service and innovation awards for health care. In academia, her research areas are in machine learning, risk models, and remote patient monitoring. Dr. Noel has developed several educational curriculums, including a 40-hour telehealth curriculum for the School of Medicine, as well as interprofessional educational curriculums with the School of Health Technology and Management, Nursing, Dentistry, and Social Work. Dr. Noel is a graduate of Duke, George Washington, and Johns Hopkins Universities. She is a proud graduate of the Stony Brook Preventive Medicine program, whereby she is now working collaboratively with the residency program leadership on development of a telehealth preventive medicine service. Eric Weaver, DHA, MHA, is nationally recognized for his work in primary care transformation and value-based care. As a corporate vice president for Innovista Health Solutions, he oversees enterprise strategy and technology adoption for a fast-growing population health management services organization. Dr. Weaver has been recognized for his contribution to the health care industry by receiving the ACHE Robert S. Hudgens Award for Young Healthcare Executive of the Year and the Modern Healthcare “Up & Comers” Award in 2016. Prior to joining the Innovista leadership team in 2015, he was the president and CEO of Austin, Texas–based Integrated ACO—one of the more successful physician-led accountable care organizations in the country. Suzanne Delbanco, PhD, is the executive director of Catalyst for Payment Reform (CPR), an independent, nonprofit corporation working to catalyze employers, public purchasers, and others to implement strategies that produce higher-value health care and improve the functioning of the health care marketplace. In addition to her duties at CPR, Suzanne serves on the advisory board of The Source on Healthcare Price & Competition at the University of California–Hastings and the Blue Cross Blue Shield Institute. Previously, she was the founding CEO of The Leapfrog Group. Suzanne holds a PhD in public policy from the Goldman School of Public Policy and an MPH from the School of Public Health at the University of California–Berkeley. Sue Schade, MBA, is a nationally recognized health IT leader and Principal at StarBridge Advisors providing consulting, coaching, and interim management services. She recently served as the interim chief information officer (CIO) at Stony Brook Medicine in New York. She was a founding advisor at Next Wave Health Advisors and in 2016 served as the interim CIO at University Hospitals in Cleveland, Ohio. Sue previously served as CIO for the University of Michigan Hospitals and Health Centers and, prior to that, as CIO for Brigham and Women’s Hospital in Boston. Her previous experience includes leadership roles at Advocate Health Care in Chicago, Ernst & Young, and a software/outsourcing vendor. Naomi Fried, PhD, is an innovative and digital health thought leader and founder and CEO of the boutique advisory firm, Health Innovation Strategies, which focuses on innovation program design and digital health strategy. Naomi was the first vice president of innovation and external partnerships at Biogen, the first chief innovation officer at Boston Children’s Hospital, and vice president of innovation and advanced technology at Kaiser Permanente. She served on the board of directors of the American Telemedicine Association and the Governor of Massachusetts’ Innovation Council. Joe Grundy has firsthand experience with nearly every aspect of primary care transformation. He has led policy and product development for the American Academy of Family Physicians, led in-the-trenches transformation of a primary care group, and served as national faculty for Medicare’s Comprehensive Primary Care Plus transformation project. He cofounded Grundy Consulting to work with stakeholders across the industry in order to accelerate the rate of effective transformation in primary care. Adrian Rubstein is a medical adviser and innovation manager at Merck KGaA, where he works to bring cutting-edge technologies to improve patients’ lives. There are four pillars that contribute to readmissions: ensuring patients are equipped to self-manage and properly take their medications; follow-up (usually by PCPs); managing transitions of care and care coordination, which might be known as interoperability; plus avoiding medical errors. Dr. Kim Noel and I discuss each of these pillars and how telehealth and other digital tools can close gaps and help patients do what they need to do to stay out of the hospital. It's summertime! Parents, children and teens are enjoying the fun and sun. But we can't forget about the importance of water safety and drowning prevention. According to the Centers for Disease Control and Prevention (CDC), drowning is a leading cause of unintentional injury-related death in the US. Children ages 1-4 have the highest drowning rates and theres a disparity in the rate of drowning with African-American children disproportionately impacted. On this episode, I speak with Dr. W. Steen James, a pediatrician, member of the American Academy of Pediatrics and its Georgia Chapter. She has a solo practice, Pediatrics Village P.C. in Peachtree Georgia. She shares information to help families stay safe this summer and beyond: Learn CPR Learn to swim (parents and children) Supervise children when in or around water Bio:Dr. Steen James was born in Newnan, GA. Graduated from Freedmans School of Nursing in Washington DC. Received a B.S. degree from Howard University in child Development. Attended Morehouse School of Medicine and received an MD degree from Emory University. Completed a residency at Emory University and also served as Chief Resident. Board certified in general Pediatrics and is a member of the National and GA chapters of the American Academy of Pediatrics. Serves on AAP GA chapter board, represents district IX as well as serving on the chapters legislative and PCMH committees. Recipient of the National Health Scholarship and served as Pediatrician at West End Medical Center for four years. How can we use tech to make healthcare better and more accessible? Dr. Nwando Olayiwola, a family physician, faculty member at UCSF, and Chief Clinical Transformation Officer for RubiconMD, a leading provider of electronic consultations between primary care and specialty care providers, has spent many years thinking about that question. She joins us this week to tell us about her career in primary care, tech, and leadership. We also discuss her powerful essay, Racism in Medicine: Shifting the Power, and her research looking at various aspects of tech and care delivery. A little more about our guest: Dr. Nwando Olayiwola is a family physician and the inaugural Chief Clinical Transformation Officer for RubiconMD, a leading provider of electronic consultations between primary care and specialty care providers. She is also currently an Associate Clinical Professor in the Department of Family and Community Medicine at University of California, San Francisco. She served as the Director of the UCSF Center for Excellence in Primary Care until February 2017. In that role, she supported the Center in achieving strategic objectives around primary care transformation and systems redesign regionally, nationally and internationally. Prior to her work at UCSF, Dr. Olayiwola served as the Chief Medical Officer of the largest Federally Qualified Health Center system in Connecticut, Community Health Center, Inc. (CHCI), where she developed expertise in medical administration, translational and implementation research, professional development, systems based and quality improvement and practice transformation of twelve primary care practices into Patient-Centered Medical Homes. Her work led to CHCI being one of the first organizations in the United States to receive both the NCQA Level 3 PCMH and Joint Commission PCMH Recognitions. She has been a leader in harnessing technology to increase access to care for underserved and disenfranchised populations and is an expert in the areas of health systems reform, practice transformation, health information technology and primary care redesign. This week we’re having a conversation with Kate Hill, Vice President of Clinical Services with The Compliance Team. “Our motto is actually safety, honesty and caring.” Kate Hill, RN, is a graduate of Albert Einstein Medical Center School of Nursing in Philadelphia, PA. As an Army Nurse, Kate served in Viet Nam (27th Surgical Hospital in Chu Lai) where she was awarded the Bronze Star for meritorious service. Kate has worked with orthopedic patients in several capacities including Head Nurse of Orthopedics in Newark, NJ., followed by Biomet working in various capacities. Kate joined The Compliance Team (TCT) in early 2012 to direct TCT’s rural health clinic accreditation program and has fallen in love with Rural. As VP of Clinical Services, she has spearheaded the TCT Rural Health Clinic Accreditation program combining her clinical expertise, business acumen and passion for delivery of the best care possible to every patient.  She presently serves on the Board of the National Association of Rural Health Clinics. Kate also works with clinics in TCT’s PCMH program and is ensuring that a PCMH accreditation is being increasingly rewarded by payers. In a study published in JAMA in February 2014, the largest of its kind, researchers followed 32 primary care practices certified as PCMHs over a three-year period, and were unable to find any impact on overall health care costs or patients’ utilization of health care services, including emergency departments. To put it mildly, this was not welcomed news by the health care improvement community — especially those who are looking to PCMHs as one jewel in the crown of a redesigned primary care system that offers patients more integrated, coordinated, cost-effective care. Headlines such as “Medical Homes Haven’t Saved Money or Substantially Improved Care” and “Medical Homes May Not Be The Answer” suggested just the opposite. This is not the entire story. The state of Minnesota recently issued a report on its success with PCMHs. And, where focused work is underway elsewhere, the uptake of process improvements is encouraging. But the study in JAMA has functioned as a sort of wake-up call, which many experts, including champions of PCMHs, believe is timely and beneficial. On this WIHI, we pulled together some of these experts for an intensive look at the early results and critical next steps for the patient-centered medical home.

Listen NowThe term "medical home" was first introduced in the 1960s by the pediatric profession.  Still to date this model of care emphasizes team-based comprehensive, continuous and coordinated care.   The care model is intended to improve primary care generally via improved patient communication, care quality, safety and outcomes.  In several ways the ACA encouraged the adoption of the, now termed, Primary Care Medical Home/PCMH for both the Medicaid and Medicare programs and among private health insurers.   (In some ways the PCMH is seen as a precursor for providers interested in becoming an Affordable Care Organization (ACO), i.e., taking on reimbursement risk.)  Over the past few years PCMHs have become widely adopted.  Over forty state Medicaid programs are experimenting with the model along with 90 commercial health plans and three federal initiatives. During this 23 minute discussion Dr. Nielsen discusses the PCPCC's purpose and goals, more specifically what is the PCMH model of care, the varying ways PCMH's are reimbursed, what does the research to date show regarding PCMH effectiveness and challenges in adopting this new model of care.Dr. Marci Nielsen currently serves as CEO of the Patient Centered Primary Care Collaborative (PCPCC), an organization dedicated to advancing an effective and efficient health system built on a foundation of primary care.  Prior to the PCPCC, Dr. Nielsen served as Vice Chancellor for Public Affairs and Associate Professor at the University of Kansas School of Medicine's Department of Health Policy and Management.  Dr. Nielsen was appointed by then-Governor Kathleen Sebelius as first Executive Director and Board Chair of the Kansas Health Policy Authority (KHPA).  She worked as a Legislative Assistant to Senator Bob Kerrey and later served as the health lobbyist for the AFL-CIO.  Dr. Nielsen is a board member of the American Board of Family Medicine and also a committee member for the Institute of Medicine's Leading Health Indicators for Healthy People 2020 and Living Well with Chronic Illness: A Call for Public Health Action.  Early in her career she served as a Peace Corps volunteer in Thailand and served for six years in the US Army Reserves.  Dr. Nielsen earned her MPH at The George Washington University and her Ph.D. from the Johns Hopkins School of Public Health.The PCPCC's 1/14 PCMH impact on cost and quality report can be found at: http://www.pcpcc.org/newsletter/annual-report-pcmhs-impact-cost-quality-2012-2013. In its quest to transform 70 to 80 percent of its physician practices to a patient-centered medical home (PCMH) over the next three years, WellPoint has adopted a "meet the practices where they are" philosophy, reports Julie Schilz, director of care delivery transformation for WellPoint. Each practice is at a different place in the transformation effort and requires specialized supports, she adds. Smoothing the transformation rollout is the simultaneous participation of 500 WellPoint practices in CMS's Comprehensive Primary Care (CPC) program, whose goals dovetail with key PCMH principles --- as though WellPoint had another partner in its transformation initiative, Schilz notes. Just as important as practice support is transparency with health plan members, Schilz adds, especially when it comes to explaining the concept of the medical home neighborhood --- where care coordination is a collaboration between primary care and the specialist. Ms. Schilz shared the key features of WellPoint's transformation initiative, including results from its pilot program that have led to a system-wide rollout, during an October 24, 2013 webinar, "Aligning Value-Based Reimbursement with Physician Practice Transformation."

Despite the migration of some Pioneer ACOs to CMS's Medicare Shared Savings Program (MSSP), expect the surge in commercial accountable care organizations to continue in 2014, predicts Steven Valentine, president, The Camden Group. In this audio interview, Valentine suggests improvements to patient handoffs, an area in which ACOs have disappointed, in Valentine's view, as well as expectations for the other much-modeled care delivery platform, the patient-centered medical home (PCMH). In both the ACO and the PCMH, Valentine anticipates specialists will be critical parts of the solution, especially when it comes to emerging payment models, quality and performance. Steven Valentine and Catherine Sreckovich, managing director, healthcare, Navigant, provided a roadmap to the key issues, challenges and opportunities for healthcare providers and payors in 2014 during an October 30, 2013 webinar, "Healthcare Trends and Forecasts in 2014: A Strategic Planning Session."

HIN's fifth annual survey on the patient-centered medical home (PCMH) recorded the highest PCMH adoption levels to date, reports Melanie Matthews in this benchmarks podcast. A substantial number of medical homes expect to participate in an accountable care organization (ACO); Ms. Matthews also shares key metrics from the 2011 survey, including time required for medical home conversion and the PCMH effect on medication adherence and patient satisfaction. The survey also identified an impressive jump in the embedding of case managers in medical homes. A year into the Colorado multi-payor medical home pilot whose practices provide care to 30,000 patients, Julie Schilz, B.S.N., M.B.A., prescribes a single tool that can help transform practices, improve quality and deliver evidence-based care. It's NOT an EHR, says the manager of the Improving Performance in Practice and Patient-Centered Medical Home (PCMH) initiatives for the Colorado Clinical Guidelines Collaborative, who lists this tool's four key functionalities. Also in this interview, Schilz describes the influence of other reimbursement models on the Colorado pilot and identifies two opportunities for NCQA to enhance its PCMH recognition process. Aetna's practice of sending its providers periodic "care considerations" --- detailed clinical data that identify opportunities to improve care --- has been formalized in its patient-centered medical home (PCMH) pilot with Partners in Care (PIC), explains Dr. Don Liss, the regional medical director of Aetna's mid-Atlantic region. PIC providers' engagement with the care considerations is now a factor in the pay for performance aspect of the pilot. Dr. Liss shares some short-term indicators that demonstrate that the PCMH is working as well as the long-term view for medical home ROI, which can vary greatly for payors and providers.
