Rumay Alexander, EdD, RN, FAAN, a noted presenter, provocateur, consultant, professor of nursing, and the American Nurses Association's current Scholar-In-Residence. In the course of their conversation, Keith and Dr. Alexander converse on a variety of subjects, including being a Black nurse who embodies the dream that Dr. Martin Luther King, Jr. so passionately described in his famous speech. Dr. Alexander abhors what she refers to as "truth decay", and she is passionate about busting stereotypes. Detesting being put in a box, she has dedicated her life to kicking down the sides of the box and speaking and giving voice to the hurts and slights suffered by so many throughout history. Dr. Alexander feels blessed to have had a seat at so many tables, and advises others to get themselves into spaces where they can be seen and heard, and thus have more impact. G. Rumay Alexander, EdD, RN, FAAN is a professor in the School of Nursing, Assistant Dean of Relational Excellence at the Adams School of Dentistry, and formerly the Associate Vice-Chancellor for Diversity and Inclusion/Chief Diversity Officer of the University of North Carolina at Chapel Hill. At the end of 2019, she completed her presidency of the National League for Nursing, the national voice for nursing education which has over 40,000 nurse educators and 1200 schools of nursing. In February of 2021, she became the American Nurses Association's Scholar-In-Residence and advises the National Commission to Address Racism in Nursing. Dr. Alexander is the Senior Vice President for Clinical and Professional Services at the Tennessee Hospital Association and their first vice president of color for two decades. Workplace violence in healthcare requires a comprehensive approach, including improved reporting, supportive policies, technology, and legal protections, to create a safer and more respectful environment for healthcare professionals. For this episode of the Straight Out of Health IT podcast, Dr. Rhoda Collins, Chief Nursing Officer at Kontakt.io, has a conversation with host Christopher Kunney about the rising issue of workplace violence against healthcare professionals. Dr. Collins explains that most workplace violence involves verbal abuse from patients, although physical assaults and lateral violence (among colleagues) also occur. She emphasizes the need to remove the stigma around reporting incidents and create a culture where workers feel safe seeking help. Dr. Collins identifies stress as a key driver of coworker-to-coworker violence and notes that improvements in diversity, inclusion, and communication have reduced such incidents over the years. Dave's book, Brand New Nurse: Surviving Your First Day on the Job, should be mandatory reading for all new nurses as it covers topics like transitioning to night shift, professionalism, communication, organization, and time management. These are aspects that nurses don't learn in school. However, it's also a valuable read for seasoned nurses because whether you've been a nurse for two years or twenty-two years, you can probably recall feeling unsure, inept, disorganized, and exhausted during your first weeks and months on the job. It's really important in today's world to get that advanced degree because you can keep it in your back pocket, and then it will allow you to have doors opened for different opportunities in your facility or in education or even as an entrepreneur. There are so many nurse entrepreneurs out now. So I attained my MSN in 2020, and our graduation was outside because of COVID, so we ended up having the graduation outside in August of 2020. And then, during that COVID spring, I realized, "What will I do with the MSN? Will it get me where I want to go?" So on that note, I evaluated, "What would I do with it? What doors will it open?" You always have to consistently evaluate, and I even tell my children, "What doors will your degree open? Where will jobs proliferate? Where can you get employment? What do you want to do with that degree, and what is the propensity of getting a job?" So then, you have to evaluate what employment opportunities does that degree align with.''   08:44 Paying it forward: sharing your knowledge Dr Pagnotta explained why sharing skills and knowledge can help others.  ‘'I think that if you have a skill, knowledge, skill, abilities, whatever you have, and I don't want to keep them for myself. I am on this earth for this many years and if I can share my knowledge and share my path with you, then you can take it forward and go take it somewhere. I realized in the past few months that it's not about me. It's about what tools I can give to you and what tools you could run with. And I don't even have to hear from you ever again as a teacher-student, but just to know that I impart with you with the skills to learn and to grow and to get excited about learning, that's enough for me. And also, with recent times and my recent role in the Monmouth County School Nurses Association, I realized that I'm giving my leadership skills to others, and I'm teaching them school nursing leadership and nursing leadership, and they're rolling with it. It makes me so happy to see others flourish with the skills that I'm giving them and the excitement and fostering that relationship and creating an excitement in them in the role in nursing leadership.   14:06 Offering inspiration and guidance through mentoring Giuseppina said it's important for leaders to examine their motivation behind mentoring.  ‘'I think it's dual fold. It takes a lot of work, but yet it's so rewarding. For myself, I started out as an American Nurses Association mentor. I think it was two years ago, and I was matched with a mentee across the U.S., and we would meet, I think, once every two months for 15 minutes and talk and then give each other... I would give strategies as to what her aspirations were or their aspirations, and we would continue the conversation there. It was interesting because I tend to want to inspire you. I want to inspire you in your path, and I want to look up possibilities for you. If you want to go back to college, I will look up colleges for you and look up different programs that you may like to attend in your area or that have the specific degree or certification program where you would like to go….And also, it's important for you to examine why you would like to be a mentor and what purpose does it serve for you. For me, it just serves the purpose of guiding. I love to guide colleagues into their path, colleagues, students, friends. I really get excited and happy about doing that. It gives me joy, lots of joy to do that, to help others and discover what their potential is because sometimes you don't know your true potential. And I have had several colleagues that have shown me my true potential.‘'   18:49 Self-regulation and building connection  Dr Pagnotta highlighted two key elements to support academic success. ‘'…. as a student and as a nurse already advancing in your degree, even if you are in an inpatient unit with a BSN, and you would like to be on a committee, shared governance committee, there are a lot of opportunities. In academia, I think initially, the administration and the chairs and your professors guide you, but then at a certain point, you kind of take over, and it's all you because you yourself, especially in a DNP program, you have to be self-regulated and to regulate your time and to really understand, "I have this much time today. What can I get through?" So it's important as a doctoral student for me to write down what I need to attain and also the deadlines. When is everything due and when am I going to revise? So definitely in that path. In addition, I think that it's really important for academia to set up paths to have the students connect, especially in an online platform. MSN programs are online. DNP programs are online. They have to have a coffee place online, its own place for students to connect to each other. So the nerd that I am, and friends, colleagues can say this that I've had in class with, I would email all of my classmates in the beginning of the semester, say, "Hi, my name is Gius. Nice to meet you all. If you want to connect and chat about the course, here's my number." And so I've met amazing people throughout my time as an MSN and DNP and actually some classmates that I can call friends today.''   22:03 Leadership tip: Giving the gift of time  Giuseppina said empathy, guidance and giving time through mentoring is highly rewarding for leaders.  ‘'I think the connection with family and friends and colleagues is very important. And also, to have that empathy and caring aspect about yourself, I think that is a big part. I tend to be very empathetic and very caring, even with students and colleagues to guide them in the right direction even if something is... You lost a lot of points on a paper, or something happened, and I will tell them how to get most of the points next time or how to prepare so that it doesn't happen again. I'm that type of person. And also, with mentorship, with my role, with inspiring others, you give yourself, and you give others your gift of time because your time is so important. And you give a gift to others, and you give them a gift of your time because your time is really precious. I think by me giving the gift of time to others, it's rewarding. And you had said nurses burn out…. It's difficult, and you're always giving, giving. But when you are a mentor, you're also getting that feedback from somebody else that's saying, "Thank you. Thank you for your time. I appreciate talking to you. You inspire me." April 20th marked the 25th anniversary of the Columbine High School shooting in Colorado, which at the time was deemed the worst in America's history. In this episode, Maggie hosts Monisha Henley, senior vice president of government affairs for Everytown for Gun Safety. Everytown was formed in 2014 following Columbine, the Sandy Hook Elementary school shooting in Newtown, Connecticut and others. In this conversation, we learn more about the motivators for Monisha's work, Everytown's policy priorities and bipartisan policy successes in the past 10 years, and what's next for Everytown in its second decade.   Monisha Henley is the Senior Vice President of Government Affairs at Everytown. Before joining Everytown, she honed her expertise at the American Nurses Association and PricewaterhouseCoopers (PwC). She began her career in the United States Senate. Monisha is originally from Wilmington, DE and now resides in New York City.

Cannabis nursing is a nursing specialty recently recognized by the American Nurses Association.Through Ashley, I learned about where cannabis nurses can work, who their patients are, and how they can advocate for cannabis use within their communities. Ashley is a pioneer in this field and established cannabis patient care programs in Maryland. She also lobbies for its use.Ashley is also the author of the book, Stigmatized. In this Bell Work Talk, Dr. Clements and Ms. Varto will discuss that it is important to continually educate forensic nurses with evolving forensic nursing science, as forensic nursing now approaches being a formally established specialty for 30 years (American Nurses Association recognized Forensic Nursing as a specialty in 1995), it is additionally incumbent for our members to educate others, both interprofessionally and intraprofessionally. As the unique base of forensic nursing science continues to expand, it is critically important for our professional members to share that information to enhance nursing care to both victims and offenders across all settings – not just those settings that may be specifically or directly considered to be “forensic” in nature. This podcast provides several case examples of how forensic nursing knowledge was shared with other healthcare professionals, both intra and inter professionally, in order to heighten awareness toward enhanced and targeted assessment and intervention for patients in healthcare scenarios that may not have necessarily been considered to be acutely forensic, yet, certainly could have medico-legal implications. Paul Thomas Clements is a forensic psychiatric clinical specialist, a Certified Gang Specialist, and Certified in Danger Assessment. Practicing in the forensic nursing arena for over 30 years, Clements has provided consultation for hospital systems, EMTs, Child Protective Agency personnel, trauma/emergency nurses, psychiatric providers, academic and corporate settings – each regarding vulnerability risk assessment, target-hardening, and decreasing the number of violent incidents in the workplace, as well as bullying and the subsequent sequelae. Clements has provided consultation to public school systems and other child-related agencies related to child abuse assessment, and also related to the aftermath of violence and/or violent death (including homicide of a child and gang-related deaths) for teachers and other classmates. Clements has three upcoming edited textbooks: Gender Violence Across the Spectrum: A Trauma-Informed Approach, Mental Health Issues in Child Maltreatment: A New Perspective, and Violence Against Women: Contemporary Examination of Domestic Violence. Additionally, he has numerous peer-review publications, and a significant number of conference presentations – nationally and internationally – that address assessment and intervention related to the neurobiology of trauma, interpersonal violence and aggression, coping after a violent death, safety assessment, and exposure to interpersonal violence and crime. Nurse Practitioner Hannah Varto works in and led the development of Canada's only outpatient, rapid access specialty clinic providing medical-forensic care to survivors of recent violence. She is an instructor for the local post-secondary forensic health sciences department and a co-investigator in a number of research studies specific to brain injury from head impacts and strangulation in survivors of recent violence.

In the course of their conversation, Keith and Ms Smilios discuss how numerous Black nurses fled the Jim Crow south in search of opportunity in the non-segregated north, and how many ended up working at Sea View Hospital on Staten Island, a facility focused on the treatment — and to a large extent the warehousing — of thousands of TB patients. Listeners may be shocked to learn that the American Nurses Association barred Black nurses from being members until they gave in to pressure in 1949, despite Black nurses' impressive contributions to the development of the profession. A native of New York City, Ms. Smilios holds a Masters of Arts from Boston University in Religion & Literature where she was a Henry Luce Scholar and a Presidential Scholar. She also taught Essay and Research writing in the university's writing program.  In 2007, she left Boston and moved back to New York City to teach at an all-girls high school. There she created and ran an intensive summer writing program for teens.  Maria formerly worked as a development editor in the Biomedical Sciences editing books in lung diseases, pediatric and breast cancer, neurology, and ocular diseases. It was during this time when she read a line in a book that led her to discover the story of the Black Angels.  Through writing the book, she has become in involved in advocating for affordable and accessible TB drugs in TB-heavy countries, working with and supporting organizations such as EndTB and Partners in Health. Part 2 Former Vanderbilt nurse, RaDonda Vaught, joins Nurses Uncorked, for part-two of a revealing podcast. In 2017 RaDonda Vaught, a Registered Nurse at Vanderbilt Medical Center, mistakenly administered a paralytic medication resulting in a patient death. This lead to revocation of her nursing license (2021). In 2022, RaDonda became one of the first nurses to be criminally prosecuted for an unintentional nursing error. Ultimately, she was convicted of criminal negligent homicide and abuse of an impaired adult. Fortunately, she was able to narrowly avoid serving time in prison and was sentenced to three years of probation.  This landmark case quickly garnered international attention and shined a spotlight on Vanderbilt's culpability in this incident. The American Nurses Association, American Bar Association, Institute for Safe Medication Practices (ISMP) and many more all issued public statements in support of Vaught and denouncing the criminal prosecution of nurses. Around the world nurses expressed outrage at the blatant disregard of just culture and the dangerous precedent this set. Overnight the landscape of the nursing profession irrevocably changed for the worse and became significantly less safe for patients and nurses alike. Nurses have left the profession in the wake of the trial. Suddenly we faced not just civil litigation or Board of Nursing action, we faced criminal prosecution and loss of our freedom for committing unintentional human error.  Nurse Erica began covering the RaDonda Vaught case with the Tennessee Board of Nursing hearing, then attended the criminal trial and sentencing in person. Homelessness across the U.S. is on the rise. The National Alliance to End Homelessness reports the number of unsheltered and chronically homeless individuals and families has climbed ~6% annually since 2017, and an estimated 582,462 people experienced homelessness across America in 2022. Advocacy groups and researchers say a big driving force is the decline of affordable housing, a problem decades in the making but one that has grown significantly worse in the past few years. A key component of sound mental health that may help protect against depression and anxiety is resilience, and key to building it is knowing that you're connected to a greater purpose – to a story larger than your own. For Native and Indigenous peoples, the stories of origin, history, and identity are central in building resilience and experiencing optimal health. In this episode, we meet Indigenous nurse researcher John Lowe RN, PhD, FAAN, and discover how he is addressing the long-standing structural impediments that have kept American Indian, Alaska Native and Indigenous youth from connecting to their cultural heritage and identities, and to a history that he describes as a source of great strength. On this special episode of The Healthcare Plus Podcast, host Dan Collard sits down with Quint Studer, co-founder of Healthcare Plus Solutions Group, and Katie Boston-Leary, Director of Nursing Programs, American Nurses Association to analyze key findings of the latest Models of Care Insight Study and what they mean for the healthcare industry.The Models of Care Insight Study Part II, conducted by Joslin Insight, collected 1,450+ responses from direct care nurses, nursing leadership, and healthcare executives in the acute care and ambulatory settings. The goal of the second study was to gain a better grasp of how nurses can operate at the top of their licensure while cultivating leadership qualities to enhance their contributions to their organizations. Key findings include:Retired nurses are willing to help fill the knowledge and complexity gap but most don't want full time work and aren't interested in direct patient care.Nurses rate their weakest leadership traits as change management, positivity, and delegation.63% of respondents say their organization's CNA workforce is understaffed or very understaffed. 47% of respondents say CNAs are not treated with the same respect as their RN peers. Quint and Katie stress that there is not one solution to all of these challenges but that there are steps leaders can take to narrow the scope, reduce caregiver workloads, and improve care delivery.  About Katie Boston-LearyKatie Boston-Leary is the Director of Nursing Programs at the American Nurses Association overseeing the Nursing Practice and Work Environment Division and Healthy Nurse Healthy Nation. She is also the Co-Lead for Project Firstline, a multi-million-dollar grant collaborative with the CDC for training on Infection Prevention and Control. Katie is also the co-chair for the DEI committee for the Healthcare Leaders Network Delaware Valley, an ACHE affiliate. She helped redesign the Nurse Leadership Institute at the University of Maryland with an embedded coaching program. Katie is an Adjunct Professor at the University of Maryland School of Nursing and the School of Nursing at Case Western Reserve University. She is a board member on St. Johns University Health Programs and an editorial advisory board member with Web/MD Medscape and ACHE. Katie serves as staff on the National Commission to Address Racism in Nursing and is also part of the National Academy of Science and Medicine's National Plan to Address Clinician Well-Being supported by the U.S. Surgeon General, Dr. Vivek Murthy. She is the Associate Director of Health Equity Measures at the Yale Center for Outcomes Research and Evaluation (CORE). Tina works with talented clinical investigators, statisticians, analysts, computer scientists, economists, epidemiologists, management experts, and administrative staff, actively collaborating with national and international experts in health equity measures and outcomes research.    A first-generation college graduate, she is a graduate of Cornell University, UCONN, and Quinnipiac University. A perpetual learner passionate about innovation, health equity, and advocacy, she participates in the American Nurses Association's Innovation Accelerator Program and will be a fellow of the American Nurses Advocacy Institute in the fall.   Tina is a Hartford Business Journal Healthcare Hero Award and the Ct Nursing Association Excellence in Team Award winner. “One of the things about futility is many people will say, ‘Oh this is futile care,' when what they really mean is, ‘Who in their right mind would want this?' or ‘I would never ever want this,' and that's different. And so, there was this attempt to try and identify and define when it was that the care we were providing, the treatments we were providing, could no longer work. And so, some people tried ‘qualitative futilities,' some people tried ‘quantitative futility.' People have been working on it for a long time, but the shortest definition is a treatment intervention that will not have its intended effect.” TS 1:52  “And first of all, it says futility is a definition that should be used sparingly. There are lots of times when a treatment may be considered what we call ‘potentially inappropriate.' And when thinking about what's the difference between futility and potentially inappropriate? Futility is, it's clearly not going to work. Potentially inappropriate is, well, it might work, but there are lots of competing reasons why maybe we ought not to do it. And some of those reasons might be significant burden. Some of them may be the patient won't be able to achieve a neurologic state where they be able to actually perceive the benefit of ongoing biological existence. That statement, it has some very clear recommendations about: be very careful about how you use the words.” TS 7:15  “In my work as a clinical ethicist, far and away the more frequent reason we get called is families want to keep going. It's not the other way around. And in fact, when a family or a patient is ready to stop, those become incredibly difficult for the healthcare team, particularly when there's a physician who feels like, ‘But I know this will work. Don't not do this. You have a 50%, 60%, 70% chance of surviving. don't you want to try?' So to know that you have the ability to give them a chance is one thing.” TS 13:33  “And here's the tragedy in this, and I hear oncologists say this, ‘Well, it's not time yet.' That's my favorite response, it's, ‘Not yet. Not yet.' So, when you ask most people, ‘If you knew that you were going to die in the next three months, are there things that you would want to do before you die?' most people are like, ‘Well yeah'. To fail to invite this conversation robs them of this choice.” TS 16:04  “Step one: Don't keep it to yourself. A lot of it is making sure that you talk with other folks, and if you work in an inpatient setting and your hospital is Joint Commission certified, then there is some mechanism in place in your institution for dealing with an ethics challenge. But the idea is what we do is hard. And one of the biggest challenges for people who are experiencing ethics distress or moral distress is very rarely do ethical challenges happen when people are having a good time. There's a tragedy somewhere, and part of the big challenge is to separate the tragedy, like the cosmic unfairness, injustice, from ‘Are we as a healthcare team contributing to the injustice?'” TS 40:51

How do you go from being anti-system to being a system changemaker for racial justice? For this Women of Color Rise episode, Analiza talks with Leah Gordon, Associate Dean for Inclusive Excellence, Diversity, and Belonging as well as an Associate Professor of the Practice at Boston College. Leah shares how she was consistently told she was not enough. She was a mom at 19, on welfare, and told no many times for jobs because she didn't have the right degree. Leah points to not just her perseverance and mentors who spotted her talent that got her where she is today. She also had the vision to be a systems changemaker, to rise to leadership so that marginalized communities would not face injustice. Leah shares the keys to being a systems changemaker: Don't listen to the many voices that will tell you that you can't do it. You can. Source that belief from yourself and seek out mentors who can uplift you. Be your full authentic self and also practice thoughtful restraint. Before speaking your truth and burning up the system, consider whether restraint might help you now. Later, you can share stories of the injustice you faced to urge others to change. Get the support of the “mothership” organizations with authority to back why DEI is important. In Leah's case, she had health organizations such as the National League of Nursing and American Nurses Association that influence nursing education. Take care of yourself. It's broadly accepted that only 20% of health outcomes are determined by medical care. The other 80% are determined by the conditions in which people are born, grow, live, learn, work and age; circumstances shaped by the distribution of money, power and resources at global, national and local levels -- what is better understood as the political determinants of health -- voting, government, and policy. And the more people who vote in your community, the healthier your community is.   As nurses and midwives witness health access and outcomes increasingly being determined by ballot measures or wrapped in policy decisions, they are turning their next steps of advocacy and activism into running for elected public office.
