In a collaborative podcast with the American Psychosocial Oncology Society hosted by Daniel C. McFarland, DO, guests Louis P. Voigt, MD, and Yesne Alici, MD, discussed the ethical and clinical complexities of assessing decision-making capacity (DMC) in oncology, emphasizing its role as the ethical core of person-centered care. The discussion began by dissecting modern medicine's need for formal DMC assessment. Voigt framed DMC as a fundamental issue of patient rights and respect, asserting that every human being has absolute rights and that the integrity of a person requires honoring their self-determination. He advocated for clinicians to act as professionals, earn trust, customize their approach based on individual needs through precision medicine, and demonstrate humility by re-explaining information when a patient cannot summarize their understanding. Alici provided a detailed clinical framework for DMC, defining it as a person's ability to decide on the specific issue at hand while appreciating the foreseeable consequences. She outlined the 4 key elements, or pillars, of the assessment: the patient's ability to understand the information, to appreciate the risks and benefits, to provide a rationalization for the decision to ensure no delusional thinking interferes, and to communicate a consistent choice. She clarified that DMC is decision-specific and time-dependent, emphasizing that conditions like dementia, schizophrenia, major depression, or even a central nervous system malignancy do not automatically mean a patient lacks capacity; it must be assessed for each situation and may be restored if cases like delirium are reversible. She cautioned that clinicians must be mindful of potential mislabeling when patients with aphasia interact, as simple gestures may not fully represent informed consent. Finally, the experts addressed the crucial implications that follow an assessment of incapacity. Voigt highlighted that a lack of DMC is not a "stop sign"; decisions must still be made, requiring the health care team to look to a designated proxy or the patient's next-of-kin. He stressed the need for hospitals to establish system-wide processes and workflows to prepare all stakeholders for these contingencies and help patients become familiar with advanced directives, thereby reducing the immense emotional burden placed on surrogates who must perform substituted judgment. McFarland concluded that being aware of DMC elevates the quality of care, reinforces an ethical climate, and enhances the entire organization. McFarland is the director of the Psycho-Oncology Program at Wilmot Cancer Center and a medical oncologist who specializes in head, neck, and lung cancer, in addition to being a psycho-oncology editorial advisory board member for the journal ONCOLOGY. Voigt is an intensivist and chair of the Ethics Committee at Memorial Sloan Kettering Cancer Center (MSKCC). Alici is vice chair of Clinical Operations in the Department of Psychiatry and Behavioral Sciences, clinical director, associate attending psychiatrist, and medical director of the Biobehavioral Health Clinic at MSKCC.

BUFFALO, NY – November 17, 2025 – A new #research perspective was #published in Oncotarget (Volume 16) on November 14, 2025, titled “A personal perspective of patient-centred clinical trials.” In this perspective, led by corresponding author Jia Liu of The Kinghorn Cancer Centre, St Vincent's Hospital, the University of New South Wales, and the Garvan Institute of Medical Research, three early-phase clinical trial participants — Trevor Tyne, Elizabeth Ivimey, and Leanne Duggan — reflect on their personal experiences with experimental cancer treatments. Their stories offer a unique perspective on the patient journey through early-phase trials and emphasize the need to design clinical research that prioritizes patients' needs, dignity, and lived realities. The authors highlight both the life-changing opportunities that trials can provide and the systemic barriers that still prevent many patients from participating. This perspective captures a turning point in how early-phase trials are viewed. Once considered a last resort, these trials are now increasingly offered earlier in treatment, especially with the rise of biomarker-guided therapies. In this context, the patient experience has become critical. The authors outline key benefits of participation, including access to novel therapies, ongoing medical monitoring, emotional support, and a strong sense of purpose in contributing to future medical advancements. However, they also point out significant challenges, such as restrictive eligibility criteria, high financial and logistical burdens, and communication gaps between patients and trial staff. “While no trial guarantees success, the level of support, access to innovative therapies, and sense of contribution to medical progress can be profoundly meaningful.” Each narrative provides insight into the clinical trial experience. One patient explains how living with a visual impairment required tailored accessibility support throughout the trial process Another shares how genomic testing led to targeted treatment after standard options failed. Despite their different diagnoses and circumstances, all three stories reflect resilience, hope, and a shared call for trial designs that better reflect the realities of diverse patients. To address these issues, the authors propose a number of improvements to trial design and delivery. These include expanding eligibility criteria, offering financial and logistical assistance, improving communication training for research staff, and introducing dedicated trial navigators to help guide patients through complex processes. They also focus on the importance of involving patient advocates in trial design and ensuring smoother transitions for patients moving between treatment centers. While early-phase trials focus on safety and effectiveness, the authors argue they must also be guided by core values like ethics and patient empowerment. This perspective serves as a reminder that the future of cancer research depends not only on scientific innovation, but on an honest partnership between patients and the research community. By sharing these real-world experiences, the article encourages a broader conversation about how to make clinical trials more responsive to the people they are designed to serve. Simpson is the President and Chief Executive Officer and member of the Board of Directors at REGENXBIO. Mr. Simpson previously served as the Company's Chief Operating Officer. In that role, he led key business functions including Research & Clinical Development, Corporate Strategy, Manufacturing & Quality, Regulatory, and Commercial Operations. Mr. Simpson joined REGENXBIO in 2015 with extensive leadership experience across biopharmaceutical operations and served as the Company's Chief Technology and Operations Officer before becoming COO. Prior to joining REGENXBIO, he was the Regional Supply Chain Head for North America and Interim Chief Operating Officer at GlaxoSmithKline (GSK). Mr. Simpson earlier served as interim CEO of Human Genome Sciences (HGS), where he led the integration of HGS into GSK, and as Senior Vice President of Operations and Vice President of Manufacturing Operations at HGS. Prior to HGS, Mr. Simpson was Director of Manufacturing Sciences at Biogen. Earlier in his career, Mr. Simpson served in an overseas assignment at Novo-Nordisk Biochem in Denmark and in various senior development and engineer roles at Genentech, working on Herceptin and Avastin, among other roles.
