EHA25 highlights: EHA President John Gribben talks AML, myeloma, polycythemia vera, and COVID-19 What were the late-breaking and practice-changing presentations at EHA25 Virtual? John Gribben, MD, DSc, president of the European Hematology Association, highlighted some of them in this podcast. Dr. Gribben and host David H. Henry, MD, discussed presentations on acute myeloid leukemia (AML), multiple myeloma, polycythemia vera (PV), and COVID-19. Randomized, double-blind, placebo-controlled study of venetoclax with azacitidine vs. azacitidine in treatment-naïve patients with acute myeloid leukemia ineligible for intensive therapy—VIALE-A: Adding venetoclax to azacitidine improved survival, response, and transfusion independence in older patients with treatment-naïve AML. Older AML patients have seen “very little progress” in outcomes for decades, but advances such as these are “really moving the field,” Dr. Gribben said. Results of the pre-planned interim analysis: Ropeginterferon was safe and more effective than phlebotomy for keeping hematocrit on target in patients with low-risk PV. These findings suggest ropeginterferon is a viable option for PV patients, but “old-fashioned phlebotomy can also be quite efficient,” Dr. Gribben said. Physicians will have to weigh the risks and benefits, including cost-effectiveness, of each treatment, he added. Overall survival data are not mature. The study was stopped early because the primary endpoint was met, as isatuximab “clearly demonstrated superiority,” Dr. Gribben noted. It isn’t clear how isatuximab stacks up against daratumumab, but these results suggest “people now have another CD38 antibody to consider as part of their armamentarium,” Dr. Gribben said. Endothelial cell and platelet markers were elevated in COVID patients who required intensive care, and soluble thrombomodulin was linked to survival. These findings prompted the decision to give all COVID patients aspirin. Satellite data has shown an 85% increase in the number of fires across Brazil this year. There are more than 2,500 fires active across the Amazon region. This represents the most active number of fires since 2010. The increase in fires has been attributed to deliberate deforestation and clearing for agriculture or mining. The new president of Brazil, Jair Bolsanaro, supports the commercialisation of the Amazon forest and this is said to have encouraged the wide scale burning. Professor of Earth System Science at the University of California Irvine, Jim Randerson and Luiz Aragão of Brazil’s National Institute of Space Research are just two scientists concerned about the destruction and carbon emissions from the extensive burning. Thursday 20 July 2013. Shepparton Regional Lecture. Tim Whitford and John Gribben Tim Whitford and John Gribben take their presentation on the road to visit the community of Shepparton. During the Second World War thousands of Prisoners of War, ‘enemy aliens’ and Australians of German, Japanese and Italian descent found themselves imprisoned inside a huge POW/Internment Camp complex in farmland in Victoria’s Goulburn Valley. In the years that followed there were murders, escapes, riots and intrigue. There was also much humour, compassion and demonstrations of the better parts of human nature. John and Tim talk about this little-known aspect of Australian History and shine a searchlight on some of the incredible characters in the drama.
Professor John Gribben from Barts and The London Trust Cancer Centre, UK, is joined by Professor Peter Hillman from St James’s University Hospital, Leeds, UK, and Professor Kanti Rai from Long Island Jewish Medical Center, New York, USA, at ASH 2012 to talk to ecancer.TV about the exciting times being witnessed in CLL and the new data presented at the meeting. Professor Rai points to the “astounding” advances that have been made in CLL over the past 4-5 years based on improved understanding of the molecular biology of the disease, eg, the B-cell receptor and signalling pathways, that have led to the development of the PTK inhibitors and the PI3 kinase inhibitors. These new drug groups have given previously refractory patients new hope although Professor Rai comments on the caution still required concerning durability of response with ibrutinib and GS1101 as single versus combination agents. He also comments on the chemo-free approach to treatment and when this might be an appropriate therapeutic option. Professor Hillman notes the impressive and sustained response rates seen with some of these newer agents, and comments on the hope of cure of CLL in the future. Professor Hillman also notes the promise for the combined use of monoclonal antibodies with newer agents. The experts discuss the importance of educating patients on the expected response to newer agents, eg, on speed of response and lymphocytosis, and note from new data presented at ASH 2012 that many questions remain on the mechanism of action of newer agents. They also comment on promising new data being presented with Bcl-2 inhibitors and the challenge of getting all the new agents into logical clinical trial development programmes to speed effective agents to market as safely and rapidly as possible. Finally, the experts discuss the importance of the variety of patients being treated in clinical trials, eg, elderly, younger fitter patients and previously refractory patients.

They also comment on the clinical, safety, ethical and economic consequences of long-term treatment in lymphoma, on the value of drug-free holidays, on compliance issues with new oral agents and on finding the balance between efficacy, toxicity and quality of life in lymphoma management. They discuss data presented for the newer treatments in different sub-types of lymphoma, eg, mantle cell lymphoma. Also on advances with targeted therapies, with summaries of data on new antibodies to direct therapies to the tumour cells. They discuss the important balance between aggressive vs. less aggressive therapy in different patients, and the future possibility of cure vs. long-term remission. Finally, the experts emphasise the palpable excitement in lymphoma from ASH 2012 and comment on where this might lead to.
