BUFFALO, NY- May 8, 2024 – A new research paper was published in Oncotarget's Volume 15 on May 7, 2024, entitled, “Deep learning-based whole-body PSMA PET/CT attenuation correction utilizing Pix-2-Pix GAN.” The sequential PET/CT studies oncology patients can undergo during their treatment follow-up course is limited by radiation dosage. In this new study, researchers Kevin C. Ma, Esther Mena, Liza Lindenberg, Nathan S. Lay, Phillip Eclarinal, Deborah E. Citrin, Peter A. Pinto, Bradford J. Wood, William L. Dahut, James L. Gulley, Ravi A. Madan, Peter L. Choyke, Ismail Baris Turkbey, and Stephanie A. Harmon from the National Institutes of Health's National Cancer Institute proposed an artificial intelligence (AI) tool to produce attenuation-corrected PET (AC-PET) images from non-attenuation-corrected PET (NAC-PET) images to reduce need for low-dose CT scans. “AI-generated PET images has clinical potential for reducing the need for CT scans for attenuation correction while preserving quantitative markers and image quality in prostate cancer patients.” Methods: A deep learning algorithm based on 2D Pix-2-Pix generative adversarial network (GAN) architecture was developed from paired AC-PET and NAC-PET images. 18F-DCFPyL PSMA (prostate-specific membrane antigen) PET-CT studies from 302 prostate cancer patients, split into training, validation, and testing cohorts (n = 183, 60, 59, respectively). Models were trained with two normalization strategies: Standard Uptake Value (SUV)-based and SUV-Nyul-based. Scan-level performance was evaluated by normalized mean square error (NMSE), mean absolute error (MAE), structural similarity index (SSIM), and peak signal-to-noise ratio (PSNR). Lesion-level analysis was performed in regions-of-interest prospectively from nuclear medicine physicians. SUV metrics were evaluated using intraclass correlation coefficient (ICC), repeatability coefficient (RC), and linear mixed-effects modeling. Results: Median NMSE, MAE, SSIM, and PSNR were 13.26%, 3.59%, 0.891, and 26.82, respectively, in the independent test cohort. ICC for SUVmax and SUVmean were 0.88 and 0.89, which indicated a high correlation between original and AI-generated quantitative imaging markers. Lesion location, density (Hounsfield units), and lesion uptake were all shown to impact relative error in generated SUV metrics (all p < 0.05). “The Pix-2-Pix GAN model for generating AC-PET demonstrates SUV metrics that highly correlate with original images. Nick Hounsfield began his career as an osteopath, following in the footsteps of his father, and set up his own practice alongside his wife. A passionate surfer since he was knee high to a grasshopper, Nick has since founded The Wave, a pioneering artificial surfing park which opened its first site near Bristol in 2019. But less than a year later, at the age of 46, Nick suffered a series of strokes, and his infant business was forced to close because of the pandeminc. If you were asked to name a world-class place to surf, a field near Bristol isn't the first location that would spring to mind. But this slice of the English countryside is home to The Wave, an artificial surfing lake that is one of just a handful in the world to use cutting edge technology and was the first of its kind. The Wave is the fruit of the ambitions of Nick Hounsfield, a pioneering British entrepreneur who wanted to build a unique business that had a positive social impact, with improving health and wellbeing for surfers and non-surfers alike baked in. For this special bonus interview episode of the This is Money podcast, Simon Lambert visited The Wave to meet Nick, be shown around and hear the story of his more than decade-long journey to get waves breaking and people riding them. It's a fascinating tale, not least because Nick didn't come from a background in property or business, but was an osteopath, who started with £500 in his bank account and managed to raise £27million to build his dream.  He tells Simon about the challenge of doing that when potential investors thought it was a great idea but were reluctant to take the risk on it, with a theme of ‘we'll back the second one, but not the first one' coming through. Eventually Nick and his business partners got traction in raising the funds to get the Wave off the ground, but he says it was important to find the right people to back it: those who bought into the social impact element as well as making money. ‘We talk about profits with purpose', says Nick. ‘But generally, I think across the finance industry, it seems that people are understanding how important it is to be future facing - from a profit perspective but also looking after people and the planet at the same time and how important that is in building a brand and building a business.' He adds: ‘Right at the beginning we very much set out our stall that we were going to be environmentally conscious and socially conscious, but also be profit making.' But Nick's rollercoaster ride hasn't just been about getting a hugely ambitious business off the ground, he also faced a double whammy of unexpected events as it finally opened its doors. The Wave started welcoming surfers in late 2019 but shortly afterwards Covid and lockdowns struck throwing plans into disarray. Yet Nick was already facing his own personal challenge, as he had suffered a stroke in February 2020, which left him in hospital for weeks and then needing six to nine months of rehabilitation at home through the disconcerting times of the first Covid lockdown. Nick tells the story of how he found himself working alone in the water at The Wave, while it was shut during lockdown, and benefitting himself from the impact of ‘blue health': the idea that spending time in or near water is good for people, which is a cornerstone of his business dream. The Wave has flourished since it was allowed to open again during the Covid lockdowns and there are now plans for more facilities in the UK, including one in north London's Lee Valley, close to the Olympic water sports facilities. Nick shares more details on those plans, explains more about how The Wave works and what visiting surfers can expect and need to know – and at the end of the podcast Simon – a self-described painfully average on-and-off surfer – explains what it was like to ride the waves.

Wavemaker' Nick Hounsfield was introduced to surfing by his father in the 1970s and soon developed a passion for it. He started to see surfing as a tool for having conversations around wellbeing, which led to him founding The Wave – an inland surfing destination for people of all abilities and backgrounds. Nick's idea was to create a space where people could connect with each other, themselves, and with nature. He champions ‘blue health' – the physical and mental health benefits of being in and around water.The Wave opened in Bristol in 2019 using ground-breaking technology, and Nick is planning on expanding the concept across the UK.Nick has been involved with supporting and growing the para-surfing community, and The Wave hosted the English Adaptive Surf Championships in 2020 and 2021. Nick experienced first-hand the role surfing can play in overcoming health issues when he suffered a series of strokes in February 2020. Luckily he has made a good recovery but recognises that things could have been so much worse. And I got right back into it with a visit to Bristol, where I was shown around the new Wave site by Nick Hounsfield, the man behind the entire project. Like most British surfers I’ve been following the story of the Wave for a good few years now, and I’ve long been intrigued by Nick’s role in the whole escapade. Sure, it’s about building the facility itself. But on another level, it’s a classic ‘if you build it, they will come’ story of one man and his unlikely vision. Rich territory for a podcast conversation indeed, so with the project close to completion and the first waves about to flow, I headed down to Bristol to meet Nick and dig into his whole story. The result is a conversation that covers a lot of diverse, emotional ground. Because the Wave is much more than a story about - well - a wave. There’s a deeper purpose at play, and a wider community vision for which surfing is the vehicle. The effect of bone mineral density (BMD) on outcomes from total ankle arthroplasty (TAA) has not been studied. BMD can be estimated by measuring Hounsfield units (HU) on standard computed tomography(CT), which is frequently performed prior to TAA. We aimed to identify whether tibial and talar HU measured from preoperative CT scans were associated with periprosthetic fracture or revision risk in patients undergoing TAA In conclusion, lower tibial HU on preoperative CT was strongly associated with periprosthetic fracture risk with TAA. In patients with tibial HU less than 200, surgeons may consider prophylactic internal fixation of the medial malleolus. To view the article click here.

Background: Metal implants, surgical clips and other foreign bodies may cause `streaking' or `star' artifacts in computed tomography (CT) reconstructions, for example in the vicinity of dental restorations or hip implants. The deteriorated image quality complicates contouring and has an adverse effect on quantitative planning in external beam therapy. Methods: The potential to reduce artifacts by acquisition of tilted CT reconstructions from different angles of the same object was investigated. While each of those reconstructions still contained artifacts, they were not necessarily in the same place in each CT. By combining such CTs with complementary information, a reconstructed volume with less or even without artifacts was obtained. The most straightforward way to combine the co-registered volumes was to calculate the mean or median per voxel. The method was tested with a calibration phantom featuring a titanium insert, and with a human skull featuring multiple dental restorations made from gold and steel. The performance of the method was compared to established metal artifact reduction (MAR) algorithms. Dose reduction was tested. Results: In a visual comparison, streaking artifacts were strongly reduced and details in the vicinity of metal foreign bodies became much more visible. In case of the calibration phantom, average bias in Hounsfield units was reduced by 94% and per-voxel-errors and noise were reduced by 83%. In case of the human skull, bias was reduced by 95% and noise was reduced by 94%. The performance of the method was visually superior and quantitatively compareable to established MAR algorithms. Dose reduction was viable. Conclusions: A simple post-processing method for MAR was described which required one or more complementary scans but did not rely on any a priori information. The method was computationally inexpensive. Performance of the method was quantitatively comparable to established algorithms and visually superior in a direct comparison. Dose reduction was demonstrated, artifacts could be reduced without compromising total dose to the patient.

Assessing Pulmonary Perfusion in Emphysema Automated Quantification of Perfused Blood Volume in Dual-Energy CTPA

Objectives: The objective of this study was to determine whether automated quantification of lung perfused blood volume (PBV) in dual-energy computed tomographic pulmonary angiography (DE-CTPA) can be used to assess the severity and regional distribution of pulmonary hypoperfusion in emphysema. Materials and Methods: We retrospectively analyzed 40 consecutive patients (mean age, 67 13] years) with pulmonary emphysema, who have no cardiopulmonary comorbidities, and a DE-CTPA negative for pulmonary embolism. Automated quantification of global and regional pulmonary PBV was performed using the syngo Dual Energy application (Siemens Healthcare). Similarly, the global and regional degrees of parenchymal hypodensity were assessed automatically as the percentage of voxels with a computed tomographic density less than -900 Hounsfield unit. Emphysema severity was rated visually, and pulmonary function tests were obtained by chart review, if available. Results: Global PBV generated by automated quantification of pulmonary PBV in the DE-CTPA data sets showed a moderately strong but highly significant negative correlation with residual volume in percentage of the predicted residual volume (r = -0.62; P = 0.002; n = 23) and a positive correlation with forced expiratory volume in 1 second in percentage of the predicted forced expiratory volume in 1 second (r = 0.67; P < 0.001; n = 23). Global PBV values strongly correlated with diffusing lung capacity for carbon monoxide (r = 0.80; P < 0.001; n = 15). Pulmonary PBV values decreased with visual emphysema severity (r = -0.46, P = 0.003, n = 40). Moderate negative correlations were found between global PBV values and parenchymal hypodensity both in a per-patient (r = -0.63; P G 0.001; n = 40) and per-region analyses (r = -0.62; P < 0.001; n = 40). Conclusions: Dual-energy computed tomographic pulmonary angiography allows simultaneous assessment of lung morphology, parenchymal density, and pulmonary PBV. In patients with pulmonary emphysema, automated quantification of pulmonary PBV in DE-CTPA can be used for a quick, reader-independent estimation of global and regional pulmonary perfusion, which correlates with several lung function parameters.
