    In this episode, we discuss the evidence, safety, and place in therapy of Journavx® (suzetrigine), a newly approved analgesic with a unique non-opioid mechanism of action and additional considerations for its use. Key Concepts Suzetrigine is a first in its class NaV1.8 sodium channel blocker approved for short-term (14 days or less) pain relief in adults with moderate-to-severe pain. Unlike opioids, suzetrigine is non-sedating and non-dependence forming. Suzetrigine is taken as a whole pill without cutting, crushing, or chewing following a particular dosing schedule where the first dose is taken on an empty-stomach.  The most common side effects of suzetrigine include pruritus, muscle spasms, increased CPK, rash, and transient (reversible) eGFR decrease. Suzetrigine goes through CYP3A metabolism and therefore has significant interactions with CYP3A inducers and inhibitors. Use with strong inhibitors and moderate to strong inducers is not recommended. Dose reduction of suzetrigine is required if used with moderate inhibitors of CYP3A.  Although not formally adopted in a guideline recommendation, suzetrigine's current place in therapy can be moderate-to-severe acute pain relief in adult patients after NSAIDs/APAP options are exhausted, but before or in place of opioid therapy.  References Bertoch T, D'Aunno D, McCoun J, et al.     What happens when we lose touch with play, friendship, and authentic human connection? This episode dives into the surprising links between playfulness, friendliness, and showmanship – revealing how these seemingly "purposeless" activities might be the very foundation of what makes us human.I explore Belyaev's groundbreaking fox experiment, which demonstrated something remarkable: when researchers bred foxes solely for friendliness, playfulness emerged spontaneously without being selected for. This suggests these traits may be facets of the same evolutionary adaptation – our innate drive to connect meaningfully with others.The conversation takes a sobering turn as we examine the concept of "play deficit" – the documented decline in children's free play since the 1960s that correlates disturbingly with skyrocketing rates of anxiety, depression, and even suicide among young people. Could our efficiency-obsessed culture be robbing children of something essential to their development?This leads us to consider a parallel concept: "show deficiency." Using the COVID pandemic as a natural experiment, we look at what happened when humans couldn't gather for performances, cultural events, and community celebrations. The resulting 25% increase in mental health struggles suggests these seemingly frivolous activities might be as necessary as food and shelter.Throughout the episode, I challenge the capitalist notion that values only productivity and efficiency. I realized my show might be the only place my “remixes” might ever see the light of day or have ears other than mine; I couldn't afford the permissions and licenses for most of the music I wanted to remix— nor did I have the energy or the funds to secure the means to come across them. And so, it might have been a good idea to start working; I emptied my bank accounts with intention, with a kind of understanding that it didn't matter at all anyway. Kind of nothing mattered, because there was no real money involved— and I had, in fact stumbled upon the opportunity in a suicidal spiral of desperation, being somewhat hopelessly lost at random in what I thought was Williamsburg; it wasn't, I had apparently walked around Brooklyn in an extremely large loop for about an hour before I realized I might be going in the wrong direction because I couldn't see Manhattan anymore, I didn't care. It was probably 77 or something degrees but with the New York humidity it felt like 90, and I was wearing a head to toe full body sauna suit trying to recover from the end of the month's rations of beans, rice, and literally whatever the fuck I really wanted, because it was really also whatever the fuck I could afford without running out of food for the month before my card reloaded. Thinking I should just die, and in the same very moment stumbling across an opportunity that wasn't nessarily a job, but could easily lead to one— and so, after paying my internet bill, I plunged and poured nearly every last cent I had left over Into what? Idk it just ends there.     Pumpkin Wall Farm Animal Sanctuary specializes in Pet Assisted Therapy through their rescue work, including working hand in hand with therapists and their clients alongside the animals to achieve their therapy goals, foster and adopted children, home schooling programs, and children 13 to 18 who have been court ordered to do community service.  They also assist individuals of all ages who struggle with autism, ADHD, ODD, depression, PTSD, anxiety, bereavement, and more.  The resident animals, who consist mostly of rescue animals and a handful of selected animals for the program, aid in assisting humans with improving self-esteem, social skills, coping skills, and mental health.Today I speak with Brendena Fleming owner and operator. She tells us about the great work they are doing at Pumpkin Wall Farm Animal Sanctuary to give back to those in the community.
