In June of this year, the UK Parliamentary and Health Service Ombudsman released a report summarizing their findings of their review of 22 patient safety incidents that were identified as avoidable deaths, with the aim of highlighting failings in systems and, support learning from the analysis. The report identified repeated broad themes of clinical failings leading to these deaths and also several factors that contributed to compounded harm experienced by the families of these patients following the deaths of their loved ones. The Ockenden Review of the Shewsbury and Telford Hospital NHS Trust, which was released at the end of March, exposed shocking failures at every level throughout a healthcare service and those who were appointed to regulate them. The Review found that the service had failed to investigate, failed to learn, failed to improve and failed to safeguard patients who were at their most vulnerable, with the regulator providing false reassurance of the quality of the care being provided. Although arising from a maternity service, this HCI webinar facilitated by Oonagh Gilvarry, CTO at HCI, presents the key findings of the Shewsbury and Telford Hospital NHS Trust Review and considers the recommendations that have arisen and their application throughout all healthcare services.

In January 2021 HIQA published the Fire Safety Handbook to assist Registered Providers to develop, implement and sustain an effective fire safety programme. This guidance has a significant impact on the current structures within residential homes, with the requirement for fire safety to become a central focus throughout the service provision.
