Interviewees: Tom Pisano, MD PhD and Laura Ashley Stein, MD, MS, Ed. Interviewer: Lisa Meeks, PhD, MA Description: In Episode 111, Dr. Lisa Meeks talks with Dr. Tom Pisano (former Penn neurology resident; now neuro-hospital medicine and neuro-immunology fellow) and Dr. Laura Stein (Adult Neurology Program Director at Penn) about building an accessible neurology residency for a physician who uses a wheelchair—and how trust, planning, and culture made it work. Together, they trace Tom's match-day disclosure strategy and “find-your-people” approach, an accessibility walk-through with tape measures and ADA checks, and the practical creativity of equivalent rotations when sites differ in accessibility. They also unpack a real barrier—a security policy that blocked ED entry during stroke alerts—and how reframing access as patient safety moved the system. Listeners will hear candid advice for residents (hold onto your “why,” communicate early, invite teaching adaptations) and for program directors (start with goals, not habits; assume success; apply the same creativity you offer patients to your trainees). This episode accompanies a written case study and a Mini Grand Rounds conversation in Learn at ACGME. Part of the ACGME/DWDI Disability Resource Hub, supported by the Josiah Macy Jr. Foundation Catalyst Award, it's a practical guide for PDs, residents, and anyone committed to equitable clinical training. I teach residents on multiple inpatient services as well as in outpatient continuity clinic and stroke clinics. I also lead didactic sessions and workshops for resident onboarding and in our resident lecture series. I am particularly interested in expanding resident exposure to structured experiences in medical education and quality improvement and currently mentor multiple residents per year in medical education and quality improvement projects. In 2020, I was honored to receive the University of Pennsylvania Neurology Residents teaching award. I received my masters in medical education from the University of Pennsylvania in 2018. I have been a member of the American Academy of Neurology (AAN) Residency-In-Training Examination Committee since 2019. I have also been a member of the American Heart Association (AHA) Stroke Professional Education Committee since 2016 and currently serve as the Vice Chair of this committee. Clinically, I function predominantly as a neurohospitalist and attend on the stroke inpatient and consult services, the neurology ward service, and the PPMC consult service. I am dedicated to ensuring delivery of high-quality neurologic care across our system and currently am the physician co-lead for our neurovascular disease team, which spans our six-hospital network, as well as our HUP inpatient neurology unit-based quality improvement team. Tom Pisano I enjoy weekend bike rides with my wife, followed by trying out a new restaurant. When traveling, if possible, I always try to get in some monoskiing or scuba diving. I would also consider myself a (very) mildly competitive board gamer. I will be pursuing a combined neurohospitalist/neuroimmunology fellowship to develop expertise in myelopathies. Interviewees: Dr. Uyen Troung, and Dr. Nalinda Charnsangavej Interviewer: Dr. Lisa Meeks Description: What does it take to create true accessibility in residency training? In this episode, Dr. Lisa Meeks is joined by Dr. Uyen Truong, Chief Resident at the UT-Austin Dell Medical School Pediatric Residency Program, and Dr. Nalinda Charnsangavej, the program's Director, for a candid conversation about disability inclusion in graduate medical education. Together, they share the story behind Dr. Truong's residency journey as a wheelchair user—from early planning and proactive accommodations to the team-wide commitment that made access possible. Listeners will hear how trust, open communication, and creative problem-solving helped dismantle barriers, and how the presence of physicians with disabilities strengthens patient care, especially for disabled children and their families. The discussion also dives into the practical side of accommodations in GME: what an access assistant does (and doesn't do), how to navigate procedural requirements, and the role of accrediting bodies like the ACGME and the American Board of Pediatrics in supporting inclusive training. This episode is part of the Disability Resource Hub series, made possible by the Josiah Macy Jr. Foundation Catalyst Award for Transformation in Graduate Medical Education, and offers valuable insights for program directors, residents, and anyone committed to building equitable clinical learning environments. "Therefore, if anyone is in Christ, he is a new creation; old things have passed away; behold all things have become new. Now all things are of God, who has reconciled us to Himself through Jesus Christ, and has given us the ministry of reconciliation, that is, that God was in Christ reconciling the world to Himself, not imputing their trespasses to them, and has committed to us the word of reconciliation" (II Corinthians 5:17-20). The goal of the Christian's ministry is to be reconciled to God and to help others find peace with God also. There are three ways of approaching one in need: 1. Condemnation: judging a person (Romans 8:31-37) 2. Accommodation: making light of sin (Romans 6:1-4) 3. Reconciliation: helping a person in their walk with God (Titus 3:1-7)

Tiffany Falzon has worn her fair share of hats in her almost 37 years. With a professional background in accounting, Tiff was in Sydney when she first donned her hosting crown, opening a glamping tent on a block of land owned by her Dad in her home town of Marion Bay on South Australia's Yorke Peninsula and opting to manage it from afar. After the incredible initial success of the tent, Tiff eventually decided to move home and manage the tent herself and - once again with her industrious and resourceful Dad in tow - build from scratch property number 2, a tiny home. In the years following, Tiff went on to meet her now partner Luke and welcome a son, Tasman, with Luke stepping in to help with the business through the challenging initial years of Covid and beyond. Most recently, Tiff, Luke and their extended team have opened property number 3, a sprawling retreat of warm timbers, enticing outdoor nooks and calming indoor zones they very appropriately named 'The Sanctuary'. Now known as Bayside Glamping & Co, Tiff has over the past 8 years created something very special that draws on the richness of her local community with a great deal of pride. Please enjoy this terrific conversation about her journey today. Thanks for finding our podcast! We are a family of 5 who does most of our travel using credit card points and miles and we share how we leverage credit card offers to earn a ton of points/miles so we can afford travel as a larger family.Follow us on Instagram @TravelPartyof5The towering sequoias of Mariposa Grove, the refreshing mist from Bridal Veil Falls, and the unexpected joy of watching our typically reserved 11-year-old exclaim "This is amazing!" – our family adventure to Yosemite National Park delivered all this and more. Our road trip strategy began with renting a hybrid minivan for the 2,000-mile journey, earning American Airlines miles while saving our personal vehicle from mountain roads and inevitable snack debris. After an overnight at the Holiday Inn Express in Oakhurst (booked with an IHG free night certificate), we kicked off our Yosemite exploration with the Valley Floor Tour – a perfect introduction that provided fascinating insights into the park's history and ecology, including how indigenous fire management practices were disrupted by European settlers. For families considering Yosemite with young children, we discovered some perfect trails: Lower Yosemite Falls offers an easy walk with spectacular views, while Bridal Veil Falls rewards visitors with cooling mist after a short hike. The more ambitious Mariposa Grove trail showcased magnificent giant sequoias but required multiple breaks to manage the heat and terrain with our little ones. One unexpected highlight? Encountering a rattlesnake – though we had hoped for bears and deer during our visit!Accommodation was a crucial part of our experience. Rush Creek Lodge, located just minutes from the west entrance (though still a 45-minute drive to the valley floor), proved worth every penny of its $538 nightly rate. The resort offered heated pools, zip lines, game rooms, nightly s'mores, and endless activities that kept our kids entertained from sunrise to sunset. Even without televisions in the rooms, we connected as a family through board games and outdoor adventures. Our biggest tips? Map your route carefully to minimize backtracking on winding mountain roads, prepare for motion sickness with Dramamine (administered a full hour before departure), and check multiple weather sources before packing – we brought cold-weather gear for forecasted 60-degree days but encountered 90-degree heat instead!Looking to create your own Yosemite memories? Follow us on Instagram @TravelParty5 for photos, videos, and more insights from our adventure. And tune in next week when we continue our journey with our Lake Tahoe experiences!

Send us a textOne of the biggest challenges for people pleasers is the fear of disappointing, inconveniencing, or hurting others. As women, we have often been programmed to avoid these situations in order to maintain happy and peaceful relationships since childhood. But how can we truly find happiness and peace when we constantly betray our own wants and needs to avoid conflict and keep others satisfied? In today's episode, I speak with Celeste Davis, who writes about deconstructing patriarchy and learning how to take up more space in the world as a woman on her Substack, Matriarchal Blessing. We explore the themes of her essay, "The Cost of Chronic Accommodation," which addresses the sacrifices women make to ensure the happiness of others—and how to break that cycle. Here's what we cover:The invisible cost of never prioritizing your own desiresHow a woman's self-sacrifice is expected and even romanticized by our societyWhy doormat or b*tch are not your only options for how you show up–there is a middle ground where you can assert yourselfHow social structures that depend on doing invisible work for free keep women from valuing our time and energy Two unique definitions of boundaries to help you protect what is sacred to youHow to deal with the avalanche of emotions–fear, anxiety, shame, guilt–that you have been programmed to feel when you disappoint someoneI can't wait for you to listen.Terri Cole on the We Can Do Hard Things Podcast: https://podcasts.apple.com/us/podcast/are-you-a-high-functioning-codependent-find-out/id1564530722?i=1000703548035 Find Celeste here:https://celestemdavis.substack.com/http://celestemdavis.substack.com/p/hidden-costs-of-accommodation https://celestemdavis.substack.com/aboutFind Sara here:https://sarafisk.coachhttps://pages.sarafisk.coach/difficultconversationshttps://www.instagram.com/sarafiskcoach/https://www.facebook.com/SaraFiskCoaching/https://www.tiktok.com/@sarafiskcoachhttps://www.youtube.com/@sarafiskcoaching1333What happens inside the free Stop People Pleasing Facebook Community? The episode gets personal and in-depth to make a case that collective resistance that envisions positive change rather than mere survival can transform these "worst of times" into the "best of times," and to indicate some features it might involve. The three reactions to Trumpism the episode considers are first passive accommodation like some students and faculty obeying their Trustees, like some lawyers obeying their bosses, like some non-profits self censoring their web sites and budgets, and like some householders bemoaning but not fighting Trumpism; second, active collaboration like some university trustees bowing to Trump, some law firm partners kissing Trump's ring, some elected officials marching with Trump in violent array, and I guess the Proud Boys too; and then, third, there is resistance, growing and diversifying.Accommodation often reflects fear and exhaustion, but it also and perhaps mainly rests on beliefs that fighting corporate and poilitical power is futile and in any event even if it won some change the gains would lead back to similar problems. Accommodation, afflicting tens of milions, can and mst be respectfully overcome. In contrast, collaborators actively enable Trump's agenda despite and even due to knowing its effects. They lack empathy for those harmed. Collaboration must be overrun. Finally, to be really effective, resistance must not just oppose Trump but offer positive alternatives that can inspire sustained involvement. Resistance wins by raising costs to elites until they abandon their agenda. Movements can accomplish that when they connect specific struggles to broader solidarity. Before signing off the episode indicates varous program-like steps that resistance can continue unfolding, refine, and augment but then the episode raises a troubling concern. Where are the young people in this movement--not a relative few but a great many many? Hopefully they are meeting, talking, practicing, and preparing to explode onto the scene soon.Support the show
In this Huberman Lab Essentials episode, I discuss the science of vision and share simple, effective tools to enhance eyesight and preserve eye health. I explain how the eyes and brain work together to process light, color and motion using specialized structures such as the retina and photoreceptors, and why conditions like nearsightedness, visual hallucinations and lazy eye occur. I also cover specific visual protocols to increase alertness and focus during work, improve sleep, and support visual health. Additionally, I highlight key vitamins essential for vision and discuss supplements such as lutein and astaxanthin for maintaining long-term eye health. Huberman Lab Essentials are short episodes—approximately 30 minutes—focused on essential science and protocol takeaways from past Huberman Lab episodes. Essentials will be released every Thursday, and full-length episodes will continue to be released every Monday. Read the episode show notes at hubermanlab.com.
