Physician and psychologist Heidi Feldman is a pioneer in the field of developmental behavioral pediatrics who says that the world's understanding of childhood disability is changing and so too are the ways we approach it. Where once institutionalization was common, today we find integrative, family-centered approaches, charting a more humane, hopeful path forward. For example, for children born prematurely with increased likelihood of disability, increasing skin-to-skin contact – what is called  “kangaroo care” – can literally reshape that child's brain development, she tells host Russ Altman on this episode of Stanford Engineering's The Future of Everything podcast.Have a question for Russ? Dr. Kathryn Thompson shares her findings on how the care that pregnant Black Medicaid enrollees receive compares to their more affluent white peers, and the policy and practice opportunities for addressing the social determinants of health that are involved.  Kathryn Thompson is an Assistant Professor at Boston University's School of Public Health in the departments of Community Health Sciences and Health Law, Policy, and Management. She is also an IRP 2024–2025 Visiting Poverty Scholar.

One challenge in developing treatments for rare diseases is finding enough people with the condition to mount valid clinical trials.  Databases that contain diagnostic codes for specific conditions can help, but those miss people who may have the condition but have yet to be conclusively diagnosed.  That's where Verana Health comes in, a digital health company that uses AI to mine its data network of more than 20,000 healthcare providers and the clinic notes they make about patient encounters. “If you wanted to find somebody with a specific genetic defect, or a specific condition for which a diagnostic code doesn't even exist, clinic notes represent really the only place you could discover that information,” explains Dr. Michael Mbagwu, an ophthalmologist and Verana's Senior Medical Director. Verana partners with the American Academy of Ophthalmology, the American Academy of Neurology and the American Urologic Association to manage their clinical registries which were built to improve quality of care, answer research questions and help physicians report quality metrics. He started his career in acute care Medical-Surgical nursing and spent the last decade in home care and hospice settings. Dr. Fontenot is currently an Assistant Professor of Nursing in the LHC Group • Myers School of Nursing at the University of Louisiana at Lafayette. Related Report: Failure to Follow a Consult Process Resulting in Undocumented Patient Care at the Chillicothe VA Medical Center in OhioThe VA OIG conducted a healthcare inspection for 10 allegations related to the quality and management of patient care and the availability of resources within the Urgent Care Center at the Chillicothe VA Medical Center in Ohio.One allegation involved an urgent care provider sending a patient with a T12 vertebrae compression fracture to have chiropractic care at the Complementary and Alternative Medicine (CAM) clinic. The patient returned a week later with a T12 burst fracture and rib fractures.The OIG found that an urgent care provider verbally referred a patient for pain management and not for chiropractic care. However, the OIG found that the urgent care provider did not enter a CAM consult until eight days after seeing the patient. Due to this delay, the chiropractor and clinical massage therapist failed to review the consult prior to seeing the patient. Additionally, the chiropractor and massage therapist could not link documentation to the consult and had no other process to complete the documentation resulting in the failure to document care provided within the medical record.The patient returned to the Urgent Care Center eight days later where a computerized tomography scan showed an acute burst fracture and acute rib fractures. Because of the lack of documentation and provider recall, the OIG could not conclusively determine the relationship between the actions taken by the chiropractor and clinical massage therapist and the patient's bone fractures.The OIG found the nine additional allegations to be unsupported and lacked merit.The OIG made two recommendations to the Facility Director related to education of providers, chiropractors, and clinical massage therapists on the use of consults and timely documentation, and conducting an internal review of the CAM program processes related to patient care, reviewing consults, scheduling appointments, checking-in patients, and documentation.
