In this episode of Risk! Engineers Talk Governance, due diligence engineers Richard Robinson and Gaye Francis revisit the topic of Safety Culture. They review the work of Professor Patrick Hudson, who identified five levels of safety culture, from pathological (who cares as long as we're not caught) to generative (safety is how we do business around here).Richard and Gaye observe that many organisations tend to be more reactive, focusing on implementing controls after incidents occur, rather than striving for a generative safety culture. They note that the transition from a bureaucratic, rule-based approach to a proactive, thinking-based approach is challenging, and caution the use of AI, which can lead to a lack of critical thinking. They end with how organisations should aspire to a generative safety culture, even though it may be an aspirational goal, and highlight the need for clear commitment to safety at all levels of the organisation, rather than just from Board level. For further information on Richard and Gaye's consulting work with R2A, head to https://www.r2a.com.au, where you'll also find their booklets (store) and a sign-up for their quarterly newsletter to keep informed of their latest news and events.Gaye is also founder of Australian women's safety workwear company Apto PPE https://www.aptoppe.com.au.

In this episode of the Safety by Design, Nick sits down with Pam Tompkins, CUSP to discuss her recent article, "Safety by Design, Safety Management System Planning." This is the third installment of a six-part series where Pam delves into the critical role of planning in creating an effective safety management system (SMS). Pam explains why a proactive, systemic approach is essential for identifying and mitigating workplace hazards, moving beyond a reactive safety model. She emphasizes the importance of conducting a gap analysis to understand the current state of safety, using a systems-thinking approach to risk management, and setting clear, measurable objectives. The discussion also covers the use of leading indicators and key performance indicators (KPIs) to track progress and ensure continuous improvement, all within the framework of the ANSI/ASSP Z10-2019 standard. Key Takeaways: A proactive "safety by design" approach is crucial to move beyond a reactive safety culture that waits for accidents to happen. Conducting a gap analysis is an essential first step to identify weaknesses in your current safety practices and align improvements with fieldwork. Leading indicators, such as safety observations and near-miss reporting, provide a more accurate picture of safety performance than lagging indicators like incident rates. A systems-thinking approach helps to understand how people, processes, and equipment interact, leading to more effective risk management. Setting SMART (Specific, Measurable, Achievable, Relevant, and Time-bound) objectives is key to making safety goals actionable and accountable. The ANSI/ASSP Z10-2019 standard provides a comprehensive framework for building and maintaining a robust safety management system. Engaging employees at all levels in the safety planning process is vital for building a strong safety culture and ensuring that procedures are practical and effective. Q&A: Q: What is the main problem with a reactive approach to safety? A: A reactive approach to safety is insufficient because it waits for accidents to happen before taking action. In high-risk industries like electric power, the consequences of an incident can be catastrophic, making it too late to react after a fatality or serious injury has occurred. Q: Can you provide an example of an effective leading indicator? A: One effective leading indicator is near-miss reporting. A high level of participation in near-miss reporting is a positive sign that employees are engaged in the safety process and feel comfortable speaking up about potential hazards. This allows organizations to address issues before they lead to an incident. Q: What is "systems thinking" in the context of safety? A: Systems thinking is about looking at the bigger picture and recognizing that safety is influenced by the interaction of people, processes, and equipment. Instead of blaming an individual for a mistake, a systems-thinking approach looks for flaws in the system that may have contributed to the error, such as impractical procedures or a lack of resources. #SafetyByDesign #UtilitySafety #SafetyManagementSystem #ProactiveSafety #LeadingIndicators #WorkplaceSafety #SafetyCulture OSHA Power Electric Standards Course - https://ip-institute.com/osha-electric-power-standards/ Subscribe to Incident Prevention Magazine - https://incident-prevention.com/subscribe-now/ Register for the iP Utility Safety Conference & Expo - https://utilitysafetyconference.com/ ________________________________ This podcast is sponsored by T&D Powerskills. The document "ISO 45003:2021 Gestión de la seguridad y salud en el trabajo Seguridad y salud psicológicas en el trabajo Directrices para la gestión de los riesgos psicosociales" is identical to the International Standard ISO 45003:2021. It was developed by the technical committee CTN 81 Seguridad y salud en el trabajo. This standard provides guidelines for managing psychosocial risks and promoting well-being at work. It is intended to be used in conjunction with ISO 45001, which outlines requirements and guidance for planning, implementing, reviewing, evaluating, and improving an Occupational Health and Safety (OHS) management system. The primary goal of this document is to enable organizations to prevent work-related injuries and health deterioration in their workers and other interested parties, and to promote well-being in the workplace. It is applicable to organizations of all sizes and across all sectors for the development, implementation, maintenance, and continuous improvement of safe and healthy workplaces.Key Concepts:Psychosocial Risk is defined as the combination of the likelihood of exposure to psychosocial hazards related to work and the severity of injury and health deterioration that these hazards can cause.Psychosocial Hazards are related to how work is organized, social factors at work, and aspects of the work environment, equipment, and hazardous tasks. Examples are detailed in the sources.Well-being at Work is the fulfillment of a worker's physical, mental, social, and cognitive needs and expectations related to their work. It can also contribute to the quality of life outside of work and relates to all aspects of working life. The terms "psychological health" and "mental health" are considered interchangeable for the purposes of this document.Impact and Benefits of Managing Psychosocial Risks:Negative impacts for workers can include poor health conditions (e.g., cardiovascular disease, musculoskeletal disorders, diabetes, anxiety, depression, sleep disorders), unhealthy behaviors (e.g., substance misuse), and reduced job satisfaction, engagement, and productivity.Negative impacts for organizations can include increased costs due to absenteeism, staff turnover, reduced product/service quality, recruitment and training expenses, workplace investigations and litigation, and damage to the organization's reputation.Effective management of psychosocial risk can lead to positive outcomes such, as improved worker engagement, increased productivity, increased innovation, and organizational sustainability.Core Elements of Psychosocial Risk Management within an OHS Management System:Context of the Organization (Chapter 4): Organizations should understand external factors (e.g., supply chain pressure, economic conditions, technological changes, workforce mobility, public health issues) and internal factors (e.g., organizational structure, culture, management style, commitment to psychological health, workforce characteristics) that can affect their OHS management system and psychosocial risk management. They also need to understand the needs and expectations of workers and other interested parties.Leadership and Worker Participation (Chapter 5):Planning (Chapter 6):Support (Chapter 7):Operation (Chapter 8):Performance Evaluation (Chapter 9):Improvement (Chapter 10):

When it comes to managing safety at large-scale events, most people think of stewards, barriers, and emergency plans. But behind the scenes, there's a growing role for technology - and one Irish company is leading the charge. Safe Events Global, led by Strategic Director Mark Breen, has quietly built a reputation as an international leader in crowd and event safety - and much of that success is down to the tech they've built in-house. Mark Breen said, "we couldn't find a tech or software platform that really supported what we do as a specialist crowd and event safety company, so we built our own." What began as a bespoke system to streamline internal operations has since evolved into a robust suite of digital tools that now supports clients around the world. With a dedicated six-person Digital Operations Department, Safe Events Global isn't just bolting on tech - they're building it into their DNA. The company's proprietary systems manage everything from live crowd monitoring to risk assessments and communication protocols. "It started as something just for us," says Breen, "but we've iterated and improved it over time. Now we provide similar systems to international clients who face the same complex challenges we do." But Safe Events Global's ambitions go beyond technology for tech's sake. The company's vision is rooted in the belief that the best safety outcomes come from the right people, using the right tools. "Our core pillars are people + process," says Breen. "We integrate the best digital tools and technology to enable skilled safety professionals to be as effective as possible." This people-first approach also extends into training and education. Recognising a gap in formal qualifications for those working in crowd safety, Safe Events Global developed a Professional Diploma in Risk and Safety Management for Crowds and Events, accredited by a UK university. Now, they're working with an Irish university to launch a range of micro-credentials in key safety areas, with a view to creating a 'stackable' qualification path - a major leap forward for an industry where hands-on experience traditionally trumps formal education. "We want to professionalise this space," Breen says. "Ultimately, that means creating a third-level pathway for people who are responsible for the safety of hundreds of thousands of people. We see that as part of our long-term impact." On the ground, Safe Events Global is deeply involved in both local and international events - from music festivals to sporting fixtures - using tech to complement what Breen calls the "art and science" of safety. "There's a danger in adopting too much tech, too fast," he warns. "You still need experienced people who can read a crowd, react quickly, and make calls that an algorithm can't. Tech should support, not replace, that." As events become larger and more complex, and safety becomes ever more scrutinised, Safe Events Global is positioning itself not just as a supplier, but as a thought leader in the space - combining Irish innovation, hands-on expertise, and a future-facing approach to technology. For Mark Breen and his team, the mission is clear: safer events through smarter systems, better-trained people, and thoughtful use of technology - always with humanity at the core. Find out more about Safe Events Global at www.safeevents.ie An Irish Tech News podcast with Mark Breen will be published tomorrow. See more stories here.
Safety experts are sceptical about a new Government directive for WorkSafe. The health and safety regulator has been instructed to shift focus from enforcement, to offering advice. Workplace Relations Minister Brooke van Velden says it is currently feared for its punitive actions - and not appreciated for guidelines. But the Institute of Safety Management's Jeff Sissons says that characterisation from the Minister is not what he hears about the agency. "Most employers haven't seen or heard much of WorkSafe. I take the Minister at her word, she's heard from some employers - but that's certainly not something I've come across." LISTEN ABOVESee omnystudio.com/listener for privacy information.

Welcome to this week's episode of the Only Human with Dr. Tony Kern podcast! After the tragic collision of an American Airlines plane and Army helicopter this week—the first commercial aviation crash since 2009—Tony and special guest D. Smith discuss repeating safety cycles, the aviation industry, dealing with depression, personal faith, and more. The Executive Director of Safety Management for the Transportation Safety Institute with 40 years of experience in the field of aviation safety, D. has plenty of insights to share on the likely systemic issues that led to this accident, as well as how the industry has responded to big incidents like this in the past. In this discussion, we dissect various models of safety culture, scrutinizing how organizations perceive, measure, and manage these concepts. From artifacts like management systems to individual attitudes and behaviors, we delve into the inconsistencies and challenges of these models. We also revisit historical perspectives, such as Dov Zohar's work, to understand their influence on contemporary safety paradigms. Our conversation critically examines the missteps of industries like nuclear and aviation, which have mandated the management of ambiguous concepts without solid scientific grounding. We advocate for a shift from vague cultural mandates to actionable strategies, offering insights into enhancing clarity and effectiveness in both regulatory practices and organizational improvements. This episode aims to inspire a reevaluation of safety culture, pushing for a more scientifically grounded and practical approach to safety science. Some highlights from the paper:Safety culture as a concept is examined from scientific and pragmatic perspectives. The case is made for the removal of safety culture from the safety science lexicon.Much safety culture research is vulnerable to a fallacy of logic – we should not take a selection of parts to equal the whole.Robust research of the various individual ‘elements' of safety culture, in methodologically appropriate ways, will enhance the field of safety science and better support improvements in practice. ESG compliance has fast become a focus for many organisations looking to address their wider sustainability profile. However, its broad framework has left many scratching their heads on exactly where to start with evaluating and addressing various elements of Environmental, Social, and Governance compliance. For those looking for some direction, you may already have a solid foundation in place if you're certified to one or many ISO Standards. Today Steph Churchman will explain what ESG is, how it can be scored and what role ISO Standards can play in ESG compliance. You'll learn · What is ESG? · What scoring systems are available for ESG? · How can ISO Standards support ESG compliance? · What ISO Standards can support each pillar of ESG? Resources · Isologyhub In this episode, we talk about: [00:30] Join the isologyhub – To get access to a suite of ISO related tools, training and templates. [02:05] Episode summary: Steph will be breaking down what ESG compliance means, how ISO Standards can support ESG compliance and give some examples of what ISO Standards can support each pillar of ESG. [02:50] What is ESG? – ESG stands for Environmental, Social, and Governance. Analysis and evaluation against these three elements help organisations to consider different areas within their overall sustainability profile. The Environmental section looks at issues surrounding climate change and actions to address an organisation's environmental responsibility. This includes monitoring and management of your energy consumption, waste management and pollution. It also seeks to tackle how organisations can address, reduce and mitigate their overall environmental impact. The Social aspect is based around the relationships an organisation has with its stakeholders. This is focused on employees and looks at a broad range of topics including employee wellbeing, fair and competitive pay, benefits and human resource related policies. Considerations can also include wider business relationships such as supplier relations, local community and government work. Governance criteria focuses on creating a business environment that is fair, transparent, and accountable. Considerations in this area include board composition, fairness in pay structures and executive compensation, business ethics and risk management. [04:15] An evolution of CSR – CSR (Corporate Social Responsibility) is very similar to ESG, but is less sustainability focused. It also lacked substance in the form of effective and accountable scoring systems that held businesses to account. This is where ESG differs, with many scoring systems, certifications and even mandatory requirements driving businesses to address their compliance. [04:45] ESG scoring – There are many schemes, scoring systems and certifications available for ESG, some of which are specific to industry sectors and company sizes. What one you pick will be up to you (note that some many be mandatory in select countries), however, here are a few examples: The S&P Global ESG Score – This assesses a company's performance and management of ESG risks and opportunities using a combination of company disclosures, media analysis, and industry-specific questionnaires. A score of 0-100 is given based on their findings and are relative within a company's industry sector. Fitch Ratings ESG Relevance Scores - Fitch Ratings assigns ESG Relevance Scores alongside their traditional credit ratings. These scores assess how ESG factors could impact a company's creditworthiness. Their scores range from 1-5, with 5 indicating the highest ESG relevance to credit risk. MSCI – They offer ESG ratings for a broad range of companies, it's not really limited by sector or size. They use a letter grade system, going from AAA-CCC, to assess a company's relative ESG risks and opportunities compared to its peers. The scoring for this one assigns companies as either an ESG leader, average or laggard within their industry. [06:10] How can ISO Standards support ESG Compliance – It's important to clarify that there's no single ISO standard that guarantees ESG compliance because ESG is a broad framework. However, ISO standards provide a strong foundation for implementing many aspects of an ESG strategy. [06:35] Supporting ESG – Structure and Framework: ISO standards offer a structured approach to managing environmental, social, and governance practices. This helps companies identify key areas for improvement and develop a systematic plan to address them. [07:10] Supporting ESG – Improved Performance: By following ISO standards, companies can demonstrably improve their environmental performance, social responsibility, and governance structures by putting in frameworks that align with best practice standards [07:30] Supporting ESG – Transparency and Credibility: Achieving certification to a relevant ISO standard involves a third-party audit, which verifies that a company's systems and processes meet the standard's requirements. This certification acts as a credible signal to stakeholders such as your investors, customers, regulators, that you're committed to ESG principles. [07:55] Supporting ESG – Risk Management: Proactive management of ESG risks is a key component of any ESG strategy. Many ISO standards focus on risk identification and mitigation. For example, ISO 37001 (Anti-Bribery Management Systems) helps identify and address bribery risks, which can have significant financial and reputational consequences. Or ISO 45001 health and safety management, which requires risk assessments to be carried out to ensure the safety and well being of your employees on site locations, which would fall under the social aspect of ESG. [08:30] Supporting ESG – Competitive Advantage: Strong ESG performance is increasingly sought after by investors and stakeholders. Implementing ISO standards can help companies demonstrate their ESG commitment and gain a competitive advantage in the marketplace. You'll also feel the benefit of gaining multiple badges, through ISO certification and possibly an ESG score if you choose to go through one of the official scoring schemes. [08:55] Think of ISO standards as building blocks. They provide the foundation and structure for a strong ESG strategy. By implementing relevant standards and achieving certification, you can demonstrate a dedicated commitment to ESG principles. [09:50] Join the isologyhub and get access to limitless ISO resources – From as little as £99 a month, you can have unlimited access to hundreds of online training courses and achieve certification for completion of courses along the way, which will take you from learner to practitioner to leader in no time. [11:55] What ISO Standards can support the Environmental aspect of ESG Compliance?: · ISO 14001: Environmental Management - This provides a framework for managing environmental impacts, reducing waste, and improving your resource efficiency. · ISO 50001: Energy Management – this helps companies monitor and optimize their energy use with the aim to help reduce greenhouse gas emissions. · ISO 20400: Sustainable Procurement – This will help you to adopt sustainable procurement principles and practices within your organisation, by looking at how you can reduce waste, choose more sustainable options for required resources, how you can extend the life of resources available through remanufacturing and recovery of waste, and encourages the use of more innovative products and services. · ISO 20121: Sustainable Event Management – This Standard is mostly applicable to the events sector, and aims to help reduce the amount of waste produced during events, either through potential energy savings and the production and recycling of resources used during an event. It's recently had an update, so check out our latest episode to find out what the changes are. · ISO 14064: Greenhouse Gas Verification – This provides a framework for measuring and managing greenhouse gas emissions. This is a crucial step if you're working towards Net Zero, as you need to know what your baseline is before you can work on reducing and offsetting remaining emissions. · ISO 14068: A framework for helping businesses achieve Net Zero, this standard will replace PAS 2060 in November 2025, so anyone looking into PAS 2060 now may be better off going with ISO 14068 as it includes more guidance on purchasing credible carbon credits. [14:15] What ISO Standards can support the Social aspect of ESG Compliance?:– · ISO 26000: Social Responsibility – which offers guidance on integrating social responsibility practices throughout your organization. · ISO 45001: Occupational Health and Safety Management - which helps companies create a safe and healthy work environment. It provides a robust set of requirements designed for improving workplace safety in organisations and supply chains, with the aim of reducing workplace injury and illness. · ISO 45003: Psychosocial Health & Safety Management aka Mental health in the workplace. For the last 4 years or so, work related stress, depression and anxiety has been the leading cause for work related ill-health cases and lost working days. That's according to the annual HSE reports, which clearly highlights a big issue that many more need to consider and address. [14:15] What ISO Standards can support the Governance aspect of ESG Compliance?:– · ISO 9001: Quality Management – this is the leading global ‘quality mark' for businesses and designed as a vital business improvement tool. It's quite simply A blueprint for running your business successfully. · ISO 22301: Business Continuity Management - Which provides a basis for planning to ensure your long-term survivability following a disruptive event. This is a Standard that many align with, but don't always certify to, and for good reason as it provides some invaluable guidance for establishing robust Business Continuity Plans. · ISO 27001: Information Security – This is a Standard that is common place for most sectors now, given how reliant we all are on tech. ISO 27001 will help you to implement an Information Security Management System (ISMS), which is a systematic approach to managing sensitive company information, ensuring it remains secure and available. It encompasses people, processes and IT systems. · ISO 37001: Anti-Bribery Management Systems - It's the International Standard that allows organizations of all types to prevent, detect and address bribery by adopting an anti-bribery policy, appointing a person to oversee anti-bribery compliance, training and carry out risk assessments. · ISO 44001: Collaborative Business Management – This was originally a British Standard that had been created to provide a framework for creating and managing collaborative business relationships between organisations. The standard promotes the best way for businesses to work together, thus effectively developing and managing their interactions with each other for maximum benefit to all. In this informative episode from Pre-Accident Investigation Safety Moment, Todd Conklin presents an invigorating discussion on the contrasting concepts of recovery and restoration in the realm of safety management. While at first glance, these terms may seem interchangeable, Conklin explains that they imbibe very different meanings. Recovery, as Conklin describes, is about bringing a system back to its original state post disruption. On the other hand, restoration brings in an evolved perspective. It goes beyond mere recovery by probing deeper into three significant queries - identifying what or who has been harmed, determining what needs to be done for improvement, and assigning accountability for ensuring necessary actions. According to Conklin, the true efficacy of restoration surfaces during challenging times such as the last four years of continuous change and uncertainty. It has been observed that organizations employing a restorative approach have bounced forward rather than simply bouncing back, arriving at a configuration that is distinct, better suited to the changing environment, and in many ways, superior. Encouraging listeners to mull over these ideas, Conklin concludes his insightful session with a safety moment. In line with his belief in continuous learning and fostering goodness and safety, this episode provides an enlightening perspective on the multifaceted nature of safety management.

You can find the show notes to each episode, links to the information mentioned on the podcast, the social media platforms we're on, and anything else related to the podcast at WarehouseSafetyTips.com. If you're a seasoned Podcast Listener, this podcast will be different from most you listen to. It's based on exactly what the name implies - Warehouse Safety Tips. And since the people in that industry are busy - we know time is money so each episode will be as short and to the point as possible. And now that all that is out of the way - let's get to the Podcast! Navigating Safety Management Approaches Grasping the subtleties of Risk-Based, Compliance-Based, and Severity-Based safety management methods can profoundly influence the success of your safety measures. Let's delve into four actionable insights to aid you in selecting the optimal strategy for your workplace. Adopt a Risk-Based Method for Forward-Thinking Safety: The Risk-Based method is centered on the early identification and reduction of potential dangers. This forward-thinking strategy includes frequent risk evaluations and fosters a workplace culture where safety is a priority. This method is beneficial, particularly in factories and warehouses, where accident risks are elevated. It prepares you to deal with risks associated with machinery, hazardous substances, and operational processes, thereby enhancing both safety and efficiency. Implement Compliance-Based Tactics to Uphold Legal and Ethical Norms: Focusing on Compliance-Based safety management involves conforming to legal standards and industry norms. This tactic is essential to prevent legal issues and uphold ethical practices. In settings like factories and warehouses, adhering to standards such as OSHA regulations is imperative. Through consistent training, audits, and keeping abreast of safety regulations, your facility can comply with and surpass industry norms. Utilize Severity-Based Strategies for Managing Critical Risks: The Severity-Based approach prioritizes risks based on their potential impact. In environments with high stakes, such as factories and warehouses, this strategy aids in focusing resources and attention on the most critical issues. Managing heavy machinery or hazardous chemicals, for example, requires strict safety measures due to the grave consequences of mishandling. Combine Approaches for an All-Encompassing Safety Plan: While each method has its advantages, combining Risk-Based, Compliance-Based, and Severity-Based strategies leads to a more robust safety management system. This comprehensive approach ensures proactive risk management, legal compliance, and focus on severe risks. Regular safety checks, staff training, and an ongoing improvement culture are essential to effectively integrating these strategies. Safety management in factories and warehouses is not a one-solution-fits-all scenario. Tailoring a mix of Risk-Based, Compliance-Based, and Severity-Based strategies to your facility's unique requirements and hazards is typically the most effective approach. Remember, maintaining a safe workplace is not just a legal requirement but also a fundamental aspect of operational effectiveness and employee welfare. Note: Always ensure these tips align with your specific facility's guidelines. Merging these strategies can further boost the prominence and efficacy of safety protocols in industrial environments. By championing a safety culture across various marketing platforms, you can guarantee that these crucial messages reach a broader audience, ultimately cultivating a safer, more productive workplace.
