February 19, 2021 — Redwood Coast Medical Services in Gualala has administered about 3000 doses of the covid-19 vaccine on the Mendonoma coast, about ⅔ of them to residents of Mendocino county and ⅓ to Sonoma county residents in its coverage area. At a quarterly town hall meeting last night, RCMS CEO Ara Chakrabarti said the clinic had 500 doses of the Moderna vaccine from Sonoma county public health and the rest from Mendocino County Public health. About a thousand of the doses have gone to people seventy five and older, and the rest have gone to people who qualify because they are in eligible professions. About 1200 people have received their second doses. The numbers are rounded to the nearest hundred. Chakrabarti reported on the various places where vaccines are being administered on the south coast through RCMS, the financial situation, big changes coming up in the vaccine rollout and why he thinks RCMS should come up with a long term strategic plan.

Pain Relief Chiropractic
Non-Surgical Care for Rotator Cuff Tears

Pain Relief Chiropractic

While the anatomy of the shoulder allows for a wide range of motion and movement, it comes at the cost of a less stable joint, especially for those who routinely perform activities that require lifting the arms. This is likely why shoulder pain is one of the leading reasons patients seek chiropractic care, trailing behind low back and neck pain. The most common cause of shoulder pain is from tearing of the rotator cuff muscles (RCMs), particularly muscles that rotate the shoulder outward. The “typical” rotator cuff tear patient is typically over 50 years of age with shoulder pain that has slowly worsened over time. A 2018 study found that as many as 96% of people over age 50 have RCM abnormalities, of which MANY are asymptomatic or non-painful. The study also reported that 24% of a random sample of 46 young people with an average age of 23 years old with no symptoms and no history of past injury, had degenerative changes in the RCMs. This finding supports the notion that rotator cuff injuries may occur early in adulthood and progress slowly until the symptoms drive a patient to seek care. In a study involving 167 patients with rotator cuff tears, researchers observed no difference in outcomes one year after participants received either conservative care or surgery. This led the authors to recommend that non-surgical care, such as chiropractic care, should be considered as the PRIMARY method of treatment for rotator cuff tears of non-traumatic origin. One study looked at impingement syndrome in a case series of four patients who received multimodal chiropractic care that included shoulder manipulation, shoulder girdle exercises, and ultrasound. In all four cases, the patients reported complete resolution of their shoulder pain and disability with five treatments. When researchers followed up with the patients four to eight weeks later, the participant's symptoms had not returned. A systematic review of data from 200 articles found evidence for the following non-surgical treatment options—which are commonly provided in chiropractic clinics—for shoulder pain: exercise training (specific favored over general), manual therapy, laser, extracorporeal shockwave, pulsed electromagnetic field (PEMF), transcutaneous electrical nerve stimulation (TENS), myofascial trigger point therapy, acupuncture, and microwave and light therapy. For a patient with a rotator cuff tear, conservative chiropractic care is an excellent option for reducing pain and improving function in the affected shoulder!

In the last decades regional climate models (RCMs) have proven their ability to provide valuable information about potential future changes in the earth’s climate system. Research projects like GLOWA-Danube (Global Change of the Water Cycle) are given the possibility to utilize RCM simulations as meteorological drivers for land surface model components. To adequately describe all sorts of water fluxes in the research area of the Upper Danube watershed the different components of the interdisciplinary DANUBIA model require data in high spatial and temporal resolution. While the latter can be satisfactorily provided by most RCMs, the spatial resolution at which atmospheric processes can be resolved is computationally limited to at best 10 x 10 km at present. A clear need has been identified to develop appropriate methods to bridge the gap between RCMs and high resolution land surface models. The application of such downscaling techniques is in particularly necessary in highly complex terrain, where the limited spatial resolution of RCM simulations does not fully capture the natural climatic variability. In the present work a model interface has been developed that provides adequate scaling techniques to overcome the mismatch between the model scales permitting the investigation of climate change impacts at regional to local scales. Besides the downscaling of meteorological simulations, the coupler scales up fluxes calculated at the land surface and provides the aggregated fluxes as inputs for the RCMs. As the latter allows to consider the nonlinearity and complexity of the interactions between the atmosphere and the land surface as well as the mutual dependency of the respective processes at the investigated scale the approach can be expected to contribute to a better understanding of the complex land-atmosphere-system. A comprehensive description of the implemented algorithms is given. Further first results of one-way coupled model runs using the regional climate model REMO to simulate the atmosphere and the hydrological model PROMET to describe all hydrological relevant processes at the land surface are presented. By comparing the results achieved for a potential future climate to those achieved for past climate conditions the climate change impact on the water resources is analyzed. The model interface SCALMET has been developed in the framework of the GLOWA-Danube Project at the Ludwig-Maximilians-University in Munich. The financial funding of GLOWA-Danube by the German Ministry of Education and Research (BMB+F) is gratefully acknowledged.
