Join us for a new edition of our global surgery series! On this episode, Dr. Jon Williams is joined by Dr. Sudha Jayaraman and Dr. Justina Seyi-Olajide to discuss how we define global surgery today and how health infrastructure interacts with global surgical care.  Dr. Jayaraman is a trauma and acute care surgeon at University of Utah, and the director of the Center for Global Surgery. After attending UC Davis for medical school, Dr. Jayaraman completed general surgery residency at UCSF, during which time she obtained a masters in public health in developing countries from the London School of Hygiene and Tropical Medicine. During this time, her efforts were dedicated to researching and implementing trauma systems development in Uganda. After residency she then completed a trauma and critical care fellowship at Brigham and Women's, during which she received the Harvard Medical School Health Disparities Fellowship to continue her trauma systems work in Rwanda. Her ongoing work investigating injury burden and trauma systems in low and middle income countries has been well funded by the NIH, DOD, and others and published in numerous forums, as she is a well-renowned expert in this field.  Dr. Justina Seyi-Olajide is a pediatric surgeon at the Lagos University Teaching Hospital in Lagos, Nigeria. She completed her medical school training at the Ahmadu Bello University in Zaria, Nigeria and subsequently her general surgical and pediatric surgical training at the Lagos University Teaching Hospital, earning the Fellowship of West African College of Surgeons in Pediatric Surgery and the Alinta Nwako prize for best graduating pediatric surgical trainee. Dr. Seyi-Olajide's vision is to provide equitable pediatric surgical care in resource-limited settings, and has been highly influential for developing initiatives such as the National Surgical, Obstetric, Anesthesia and Nursing Plan for Nigeria. Often, these realities are bleaker than most people expect. One thing physicians, and the general population, need to be more aware of is the very real racial disparity in surgical care. Are the outcomes for Black and Hispanic patients truly different than white patients when it comes to surgery? The answer is an astounding yes. Are the differences related to other factors such as socioeconomic class, insurance status, gender, income, extent of disease at presentation or other comorbidities? One of the goals of the show is to highlight African global health leaders who are leading public health initiatives on the continent and we have just one such leader to open our season. We are so excited to have host Prof. Salome Maswime, joining us from Cape Town South Africa. Salome Maswime is Professor and the Head of Global Surgery at the University of Cape Town in South Africa. She trained as an Obstetrician and Gynaecologist and is the President of the South African Clinician Scientists Society. She was the Discovery MGH research fellow in 2018 at the Massachusetts General Hospital and Harvard Medical School, and a former lecturer at the Wits University.  Salome is from Limpopo province, South Africa. After qualifying as a medical doctor at the University of Kwa-Zulu Natal, Maswime completed a Fellowship and a Masters in Medicine degree in Obstetrics and Gynaecology, and a PhD in Obstetrics and Gynaecology. She has worked in various hospitals in South Africa, including the Chris Hani Baragwanath Academic Hospital in Soweto, Johannesburg for approximately 10 years.She is a global surgery expert with significant research contributions on access to quality obstetrics care in Ghana. She is an advocate for women's health rights, and equity in surgical and maternal care. She is an advisor and consultant to several institutions, including the World Health Organization and the Global Surgery Foundation. Providing over 1,000 certifications to beauty industry entrepreneurs.    She has also worked with clients of Mia Esthetics and Goals plastic surgery, some of the prominent facilities in Miami and Atlanta. With her experience working and creating an honorable reputation in the surgical community, Crystal has made it a goal to be transparent and authentic with her services, revealing the truth about post-operative surgical care.  With the glorification of plastic surgery, most people aren't aware of the toll it can take on their bodies until after surgery.    The pre-and postoperative period is a critical time for patients to be prepared. As Crystal points out, “post-operative surgical care is more important than the actual surgery because if you don't take care of your body after surgery, you will get infections.” Being properly informed and guided can help reduce the risk of complications and support the surgical recovery process.   I had such a fun time chatting with Crystal. Carpal tunnel syndrome (CTS) is a condition that occurs when the median nerve is compressed as it passes through the wrist. One treatment option available to patients is carpal tunnel release surgery, which severs the carpal tunnel ligament to reduce pressure on the affected nerve to resolve the numbness, pain, tingling, and weakness symptoms associated with CTS. When is surgical treatment for CTS necessary and when should a non-surgical option be pursued? The short answer is that surgery should only be considered as a first option in an emergency situation, such as a serious wrist fracture that pinches the median nerve. Beyond that, treatment guidelines generally advise patients to exhaust non-surgical, conservative approaches before consulting with a surgeon. Aside from potentially higher healthcare costs and a prolonged recovery, surgery also carries the risk for serious complications. Another thing to consider is that the current research suggests that jumping straight to surgery may not necessarily produce better long-term outcomes than non-surgical treatment options. In one randomized clinical trial, researchers recruited 120 female CTS patients to receive either surgery or a conservative treatment approach that involved manual therapies. The research team evaluated each patient after one month, three months, six months, and one year. In the short term—one month and three months—the results favored the conservative approach. However, both groups reported similar outcomes after six months and one year. The same research team repeated the study with another group of female CTS patients and reported similar results. In the short term, conservative care achieved greater results while both approaches had similar outcomes over the long term. A systemic review that looked at results from ten studies involving patients with confirmed CTS in one or both hands came to a similar conclusion. The review found that non-surgical care provided more satisfying results in the short term with both approaches achieving similar results over time. While these studies show that conservative treatment to reduce pressure in the carpal tunnel is an effective option for the CTS patient, doctors of chiropractic will also examine the full course of the median nerve to identify other places the it may be compressed, such as the neck, shoulder, and elbow. Median nerve compression in these areas can often co-occur with CTS and will need to be addressed to achieve a satisfactory result. www.PainReliefChiroOnline.com

Dr. Kurt Newman came to Children's National Hospital in 1984 to do a surgical fellowship, and now 36 years later, is the President and CEO of the hospital. Dr. Newman is also a professor at the George Washington University School of Medicine, and is also the author of the book, Healing Children, whose proceeds go to the Pediatric Health Opportunity Fund. Before acting as President and CEO, Dr. Newman served as the Surgeon-in-Chief and Senior Vice President for the Joseph E. Robert, Jr., Center for Surgical Care at the hospital. Dr. Newman is also a strong advocate for expanding mental health access for kids, and as President and CEO has focused on patient and family centered care. Dr. Newman quotes the great Nelson Mandela by echoing, “The true character of a society is revealed in how it treats its children.”
