In today's multi-generational and borderless world, study abroad programs are critical for introducing students to the world outside their university and even their country. They make up an integral part of the curriculum and the student undergraduate experience, and generally, students look back with very fond memories of their time abroad on their colleagues with whom they experienced these trips. Unfortunately, despite all the “glamour shots” and publicity surrounding these trips, there is a dark side. You typically don't hear about the deaths that occur because universities have not properly vetted the vendors for these programs. Or the risks that students can be exposed to because a low-cost vendor or the tour guide takes students on a dangerous mountain road because their previously scheduled train didn't run on time. But these happen, and when they do, they expose both the students and the University to unnecessary risk. To learn more about study abroad programs, Dr. Drumm McNaughton spoke with Robyn Symon, producer of Emmy Award-winning documentaries and biopics on such notable celebrities as Robin Williams and Michael Jackon, Robyn has spent over 20 years documenting events in the lives of people, and her latest project, Trip of a Lifetime, focuses on the study abroad industry and how we can keep our students safe during these important programs. Podcast Highlights Inquire as to how your programs have been vetted for safety. Risk management is important, even with the study abroad programs. Do not assume that other countries have the same safety regulations that we do in the US. Don't shortchange culture because there will always be resistance that could jeopardize it. Do not throw the baby out with the bathwater. These programs are very important for students' education, growth, and maturity, but they need to be vetted properly from A-Z to ensure the safety of all participants and the liability to the University. Guest Bio Robyn Symon is a two-time Emmy award-winning Writer/Producer/Director/Editor. Robyn joined PBS in 1991 after four years as a TV News Reporter in Texas. At PBS, Robyn produced documentaries, public affairs programs, and TV series, including “Florida!” which also ran for six seasons on The Travel Channel, the PBS Pledge Special, “Boleros: Unforgettable Love Song” featuring top Latin musical stars and the series “Voices of Vision,” hosted by NPR's Scott Simon focusing on the work of non-profit organizations throughout the world. She has produced national TV specials on Robin Williams and Michael Jackson and an investigative program on the opioid epidemic. Robyn debuted in the indie film arena in 2007, producing, directing, and editing the feature documentary “Transformation: The Life & Legacy of Werner Erhard,” about the controversial pioneer of the self-help industry streaming on Netflix. Welcome to Difficult Conversations with Dr. Anthony Orsini. We've had several episodes over the past eighteen months on the topics of physician burnout, the toxic culture of medicine, and the high rates of substance abuse in healthcare professionals. These are topics that I care about deeply and speak about during my presentations and workshops. I recently spoke at a Global Summit on Ending Physician Burnout. As part of the summit, attendees were able to get a free viewing of a groundbreaking and game-changing documentary , Do No Harm. This documentary is an honest, deep dive into the increasing rate of physician suicide, its origins, and why this is a serious healthcare crisis, not only for physicians, but patients as well. Today, we are extremely fortunate to have the producer of this documentary, Robyn Symon, who is a two-time Emmy award-winning filmmaker, A former PBS producer, Robyn has directed six featured documentary films, as well as produced and directed television programs for various networks. We are so lucky and honored to have her as our guest today! We learn more about Robyn and why she started to investigate the causes behind physician burnout. Dr. Orsini tells us some of the jaw dropping statistics. Robyn reveals the truth about physician and medical student suicide and doctors reporting substance abuse. She talks about the shortage of doctors and how we need to make sure that physicians and nurses are cared for and that they are not sleep deprived. Robyn fills us in about the documentary and talks about Hawkins, who is a very courageous med student willing to talk about his story with depression and suicide attempt. Last time she was on the podcast, we talked about Robyn’s documentary film Do No Harm which investigates the high rate of burnout and suicide among physicians and how it links to medical errors and patient safety. And with September being National Suicide Awareness Month, we thought it was the right time to bring her back for an extended conversation. You would think that the medical field would hold itself to a higher standard when it comes to the health and well-being of its own. But in fact, the opposite is true. In general, it seems like people in medicine treat themselves and their colleagues with less regard for their well-being. We’re focused so much on the “demand” and an assembly line model of care. But no one is focusing on the “supply” of physicians needed to take care of the demand. If we don’t address the supply (or doctor) side of the issue, patient care will suffer.Doctors also face tremendous risk when bringing these issues to light. Administrators often see them as “complaining” or “rocking the boat.” As a result, many physicians don’t want to speak out for fear of losing their jobs. We need to educate both physicians and administrators about how to effectively address these issues.The relationship between patients and physicians is eroding. Physicians want to spend more time with patients, but they’re being beaten down by this assembly line model of care. When patients see that they’re stuck in the same “assembly line” model, it really helps build back that trusting doctor-patient relationship.What steps does Robyn suggest we do to address the systemic issues causing suicide in the medical community?Step 1 - Realize that physicians are human beings, not superheroes.Step 2 - Work with colleagues, patients, and administrators to continue the conversation on these important topics.Step 3 - Don’t be afraid to rock the boat! Robyn Symon Interview: Physician Suicide & DO NO HARMRobyn Symon, a two-time Emmy award-winning filmmaker, joined me for the Licensed to Lead podcast on April 30, 2020.Robyn has written, directed, and produced films and television series—and she specializes in documentaries. We spoke about her acclaimed and riveting film that takes an unblinking look at the dark underbelly of medical training. Robyn is passionate about doing what she can to highlight and address the inhumanity of medical training—inhumanity which unfortunately can also be found in healthcare organizations. She’s currently on an international film tour with her documentary about physicians and medical training—the film is:DO NO HARM: Exposing the Hippocratic HoaxDO NO HARM is about a sick culture in medical school and residency that drives depression and burnout and suicide among medical students and physicians. This is an issue that is important to physicians and so critical to the future of medicine. It is phenomenal and hopeful that a filmmaker of Robyn Symon’s caliber chose to create a documentary to help stop the abuse of medical students and residents.Robyn was particularly interested in joining me on the Licensed to Lead Podcast to highlight a series of screenings of the film in May—during Mental Health Awareness Month. When I asked Robyn what messages she wanted to communicate with the film she said she wants people to understand:- Why physician suicides are happening and what the impact is on patients- That physicians and patients must unite in their desire for an excellent healthcare system- That when physicians are burned out or suicidal “The problem is NOT YOU”—the problem is the system and the system must change- The importance of discussing suicide and reducing the stigma about mental illness One of the most urgent messages for medical students and physicians is the importance of understanding the threat posed to their mental health and the critical need to seek mental health assistance. Robyn Symon and Dr. Pamela Wible are on a mission to reduce barriers to mental health treatment including:- Affordability- Accessibility to high-quality support and treatment- Eliminating questions about prior mental health treatment on licensing and credentialing applicationsIn the interview, Robyn talks about resistance some medical schools have to host the film and audience discussions. Robyn and other experts accompany the film to medical schools and healthcare organizations in order to lead the post-screening conversations. Considering the propensity for resistance she also marvels at the fact that over 180 organizations have hosted screenings with follow up audience discussions. We also talked about the science of sleep deprivation and the disingenuous arguments that are used to justify 28-hour shifts for trainees. There is a big question mark about the Congressional decision to leave regulation of work hours and work content to the ACGME.Other topics included:Physician disillusionment with the commoditization of medicine and the dysfunctional EHRRevelations, particularly to the public, as a result of the COVID-19 crisis.Physician leadership and a discussion about whether unionization of a profession like medicine is a good idea.During the month of May, which is Mental Health Awareness Month, Robyn is conducting on-line screenings of the film every Sunday night.
