In this solo episode, Sarah takes the “doctor vs brand” framework that went viral on Instagram and runs it as a real-time case study on a real company. The target is Cadense, an adaptive shoe that claims to help with foot drop, toe catch, and neurologic walking difficulties using “variable friction” tech, basically a glide-to-grip outsole design meant to reduce toe snagging while still giving traction during stance and push-off. Sarah breaks down what foot drop is, who this type of device might help, who it might put at risk, and why any rehab-adjacent product should be judged on more than vibe, testimonials, or white-coat authority.Then she gets into incentives, the part everyone wants to ignore until it's their wallet. She walks through Cadense's ambassador, coach, and affiliate pathways, and uses the full checklist to evaluate where Cadense lands on the clinician-led spectrum, including what they disclose well, what they oversimplify, and what they should tighten up if they want to be truly “do no harm” about a product that can literally change someone's fall risk. Beginning with intuitive living as an everyday spiritual practice, Lisa explores how learning to recognize and trust intuitive “hits” naturally extends into intuitive eating as a form of soul-level guidance, not just hunger management. At the heart of this practice is the balance of tender and fierce self-compassion. By distinguishing healthy guilt from paralyzing shame, Lisa shows how we can accept ourselves unconditionally without excusing behaviors that no longer serve us. The result is a grounded, integrated approach of equal parts “do no harm” and “take no shit” designed to foster deep healing, embodied self-trust, and lasting transformation.Topics Include:Mind-Body-Soul ConnectionFierce Self-CompassionOvercoming GuiltLasting Behavior Changes[1:07] Lisa begins by setting an intention to slow down and provide more detailed, nuanced content, recognizing the importance of the topics. Lisa identifies her primary mode as "claircognizance", a clear knowing. She shares personal examples that highlight the importance of honoring intuition even when it conflicts with personal desires.[18:09] Lisa introduces a model of the self consisting of the conscious adult, the inner child, and the Higher Self. When the Higher Self provides guidance that the conscious self resists, it creates an opportunity to practice self-compassion, similar to a parent lovingly managing a child's resistance.[23:55] Lisa recaps the practice of tender self-compassion which involves three steps: being mindful of one's pain, remembering shared humanity, and practicing kindness towards oneself. Lisa explains that tender self-compassion is about being present with the discomfort without judgment, which alleviates the second wound and allows for healing. It is a prerequisite for changing behavior.[43:17] Lisa explains the difference between tender and fierce self-compassion and that authentic living requires embracing both energies. Lisa discusses the difference between guilt (about a behavior) and shame (about oneself) when reflecting on actions. Lisa examines the need to hold yourself accountable and feel shame then looks at the proper response of meeting that painful emotion with tender self-compassion. Psychiatrist Sabooh S. Mubbashar discusses his article, "Why 'do no harm' might be harming modern medicine." He reveals that the principle "First, do no harm," or Primum non nocere, is not part of the original Hippocratic Oath but a later, misattributed addition that has become a dangerously oversimplified slogan. Sabooh argues that a literal interpretation makes medicine impossible, as every intervention, from surgery to medication, technically inflicts harm. This creates a dissonance for clinicians and can paradoxically lead to overly aggressive care, especially at the end of life, where the pressure to "do everything" causes more suffering than benefit. The conversation explores how the interplay of this flawed mantra with patient autonomy and fear of liability can lead to choices that are legally defensible but ethically and clinically harmful. As a solution, he suggests returning to the oath's original, more honest language, which calls on physicians to use their "ability and judgment" to weigh risk and benefit, acknowledging that real ethics live not in mottos but in the nuanced, gray areas of clinical practice.
