Episode 181 Assisted Vaginal Birth, a woman's perspective

Play Episode Listen Later May 17, 2025 50:58 Transcription Available

Trigger warning: This episode discusses birth trauma. When a birth doesn't go as planned and requires intervention, how does it feel from the woman's perspective? Jacqueline Edwards, mother of five, shares her powerful firsthand experiences of both traumatic and positive assisted vaginal births, challenging healthcare professionals to see beyond the clinical procedure.Through our conversation, Jacqueline offers a rare glimpse into the psychological impact of instrumental deliveries. She describes her experiences with both forceps and ventouse deliveries, revealing how communication, respect, and pain relief dramatically affected her perception of each birth. Surprisingly, her forceps delivery – often considered the more invasive intervention – proved less traumatic than her ventouse births due to better communication and adequate pain management. The emotional weight of assisted birth emerges through Jacqueline's moving marathon analogy: "You've prepared for it, trained for it, you can see the finishing line... but all of a sudden, for some reason, you fall down and someone runs out of the crowd and picks you up and carries you." This powerful comparison highlights the sense of incompleteness many women feel when intervention becomes necessary, despite having done most of the work themselves. What shines through is how small, seemingly insignificant actions from healthcare providers can transform a potentially traumatic experience. When an obstetrician told Jacqueline "I can't get this baby out on my own. This is something we're doing together," it fundamentally shifted her perception from passive recipient to active participant. These simple words acknowledged her essential role in bringing her baby into the world.Jacqueline also shares practical suggestions for improving care during instrumental deliveries – from better lighting arrangements to privacy screens – alongside powerful insights into how birth trauma can manifest in unexpected everyday situations, like trips to the supermarket. Her testimony stands as both a call for change and a roadmap for more compassionate, woman-centered care during assisted births.Whether you're a healthcare professional seeking to improve practice, an expectant parent preparing for birth possibilities, or someone processing their own birth experience, this episode offers invaluable perspective on centering women's dignity and agency during one of life's moThank you all for listening, My name is Florence Wilcock I am an NHS doctor working as an obstetrician, specialising in the care of both mother and baby during pregnancy and birth. Have you ever considered the impact of a simple medical term on a person's experience? Midwife Jenny Cunningham and Catriona Forbes of a research collective join us to unravel the complex issue of weight stigma in pregnancy.  Jenny shares her path from hands-on midwifery to academic research, while Catriona offers a personal narrative on the stigma she faced as an expectant mother, lending depth to our understanding of these early, often negative categorizations.Language has power, especially in healthcare, and this episode sheds light on the need for respectful terminology when discussing body weight during maternity care. We've replaced the word "obesity" with "higher weight" to foster a stigma-free environment. Catriona provides invaluable insights into the response from individuals with higher body weights, and together, we discuss the evolution of our research methodology to ensure it's inclusive, community-based, and sensitive to the needs of those it aims to serve. We also tackle the practicalities of prenatal care, such as folic acid dosages and the importance of creating a healthcare environment free from anticipatory anxiety.Lastly, we navigate the emotionally charged waters of maternity scans and medical conversations about body size. From the choice between induction and cesarean delivery to the importance of transparent and patient-centered communication, we underscore the crucial need for honesty and personalized care in addressing weight-related health risks. I am excited to bring you this episode on hyperemesis gravidarum. Joined by renowned experts Professor Catherine Nelson-Piercy, Dr. Melanie Nana, and Professor Catherine Williamson, we dissect the impact of this condition and the ground breaking RCOG guidelines that are transforming care for pregnant women. These leading figures pull back the curtain on shocking statistics like the 5% termination rate for wanted pregnancies due to severe nausea, emphasizing the urgency for healthcare providers to truly listen and validate the experiences of their patients.Navigating through a maze of myths and half-truths can be daunting for anyone dealing with hyperemesis gravidarum. This episode dismantles the misplaced faith in ginger remedies and sheds a revealing light on the ineffective use of ketones as a dehydration marker. We also engage in a crucial discourse on the controversial use of ondansetron, weighing the systematic review findings against the necessity for evidence-based treatment. Our guests don't just talk facts; they advocate for a compassionate approach to care, urging clinicians to tune in to their patients' needs and foster a supportive healing environment.Wrapping up this crucial conversation, we turn to the poignant, often hidden psychological repercussions of hyperemesis gravidarum. The episode shares heartening stories of how pre-pregnancy counselling and proactive treatments can dramatically improve women's pregnancy experiences. Furthermore, we delve into the potential that genetic studies on HG hold for future treatment avenues, as our guests offer a glimpse into the exciting possibilities on the horizon. Have you ever experienced a moment of cultural awakening, realizing that there's so much more to learn about the traditions of those around us? That's exactly what happened to me, Florence, after reading an enlightening tweet from the outstanding obstetrician Nisha Knot, which set me on a path to discover more about Ramadan and its effects on pregnancy. Join me on the OBSpod as I navigate my own journey of understanding, driven by the realization that many of the Muslim women we support in healthcare are observing this pillar of their faith. This episode is a heartfelt quest for knowledge, as I tackle the complexities of fasting during pregnancy without the guiding voice of an expert guest, but with the earnest desire to learn more and share that knowledge with you.As we ponder the impact of fasting on expecting mothers and how this practice varies across the globe due to differences in daylight hours, I turn to a pivotal article that serves as an evidence-based guideline for obstetricians. This episode isn't just about sharing what I've learned; it's an invitation to engage in a more inclusive and informed maternity care conversation. I hope that by openly discussing my own educational gaps and curiosities, we can all feel empowered to ask questions and seek answers together. What if the treatments we believe to be beneficial in Obstetrics aren't as helpful as we thought? Or worse, might they bring along hidden risks for mothers and babies? As your host, Florence, I invite you to journey with me as we explore the complex world of antenatal steroids, a commonly used intervention for preterm labor and planned cesarean births. This episode will not only shed light on the perceived positives, like improved baby outcomes, but also dissect the potential downsides, such as its impact on diabetic control for mothers. We'll also delve into emerging evidence that raises questions about the true scale of these benefits.Let's take a closer look at the long-term implications of administering antenatal steroids. Is it possible that even a single course of these steroids could lead to a higher chance of mental or behavioral disorders later in life for the baby? Or increase the risk of serious infections in their first year? Informed discussions between doctors and parents are crucial when it comes to this controversial topic. Drawing to a close, we'll talk about how you, our dedicated listeners, can support this podcast and ensure its accessibility to all, and how to propose topics for our future episodes.
