Ep. 113 Infant Feeding: How Breastfeeding Has Been a Challenge and a Controversy Throughout Time

History Fix

This episode is all about the history of feeding babies which has been necessary and yet surprisingly difficult since the beginning of mankind. In fact, it's so necessary that to forgo it, up until very recently, the last hundred years or so, was a death sentence for the infant. We don't often think about feeding babies. It's something mothers take care of behind the scenes, part of the invisible load. We certainly don't pause to think about the history of it, the immense challenges faced throughout the ages. But we should. As necessary as infant feeding is, as necessary as it has always been, society still does not make it easy for mothers to pull off. And that should concern you, even if you aren't a mother, even if you aren't a baby. Because you were once, and so was I, and so was literally everyone. About Brittany (in her words) My story is a complex one, with more life experience than I ever dreamed of having. A difficult childhood left me ill equipped to process the difficult things that had happened to me. For years, I turned to addiction and an eating disorder as I had underdeveloped coping skills and a lack of sound guidance. Years of work on myself found me able to cope in healthier ways, but still chasing fleeting fulfillment. My parenting journey led me through miscarriage, parenting children with a rare genetic disorder and the diagnosis of my neurodivergence which left me having to re-parent myself in order to parent my children effectively. Ultimately, becoming a foster parent, taught me the importance of learning about ourselves through our past experiences and helped me to develop tools to help myself cope. In this discovery, I have been able to find consistent fulfillment within my own self-connection and am now teaching others how to do the same. I can be found helping parents in my community through my private practice, Your Feeding Friend LLC, as a Registered Nurse and Inclusive Infant Feeding Educator (Board Certified in Lactation). Text Dr. Black your questions or comments. This episode of Eat Your Greens with Dr. Black delves into the complexities of food allergies, intolerances, and sensitivities in children, emphasizing the differences between them. A personal story from a mother, Carli, highlights the severe reaction (FPIES) her son had to oatmeal, underscoring the challenges parents face when dealing with uncommon food reactions. Dr. Black provides a comprehensive overview of food allergies, detailing how they're an immune response, typically immediate and potentially life-threatening, unlike food intolerances which result from digestive issues and are less severe. The episode also explores the concept of food sensitivity and its vague, non-specific symptoms. Misinformation and the exploitation of parents' concerns by the wellness industry are critiqued, particularly the misuse of IgG tests for diagnosing food allergies. Practical advice is given on diagnosing and managing food allergies and intolerances, with an emphasis on the importance of consulting healthcare professionals for accurate diagnosis and avoiding unnecessary dietary restrictions. Additionally, the discussion touches on the genetic and environmental factors contributing to food allergies, and the hopeful message that some food allergies can be outgrown. Finally, the talk highlights how adjustments in diet, based on solid medical advice, can improve symptoms and quality of life.Links for resources and information discussed in the episode:Food Allergy.orgNon-IgE-mediated gastrointestinal food allergyWhat is causing the rise in food allergy? How we feed babies and young kids greatly impacts their health, growth, and future. This affects not only the children but also women and society. While baby formula has its uses, and it literally be a life saver in certain circumstances, it also comes with significant health, economic, and environmental costs. On the other hand, breastfeeding has proven health benefits for both mothers and babies in high-income and low-income settings alike. Despite that, according to the World Health Organization, less than half of babies and young children are breastfed as recommended. How did this happen? According to a three-paper series published in The Lancet in 2023, the lack of breastfeeding is due to multifaceted and highly effective strategies used by commercial formula manufacturers of infant formula. The strategies are designed to target and influence parents, health-care professionals, and policy-makers. This episode explores the roots of this problem: the infant formula industry in the early 20th century captured doctors and medical associations in order to sell their product. And when they reached the limit of infant formula market in the United States, they simply aggressively sold their powders to mothers in poor countries, with disastrous and deadly consequences. On this week's episode of the pod, we continue our conversation with Dr. Dina DiMaggio (Pediatrician) and Dr. Anthony Porto (Pediatric Gastroenterologist) on all things related to infant feeding. We kick things off with a discussion of our guests' recent study on infant feeding trends and the use of unregulated feeding practices. Before getting into the thick of it, we set the stage with a discussion of the Infant Formula Act and associated regulations and policies. We then shift gears to discuss European infant formulas (with a distinction between illegal importation from 3rd party vendors and those imported legally through Operation Fly). Next, we discuss unregulated donor breast milk and the risks associated with informal milk sharing such as bacterial and viral contamination and improper storage. We discuss safer alternatives to this practice such as seeking breastmilk from regulated milk banks. We also discuss homemade infant formulas and reports of infant hospitalization and death as a result of this practice. Throughout the episode, we acknowledge and discuss the implications of the food formula shortage which understandably led many parents to seek out alternatives. You do not want to miss this! On this week's episode of the pod, we were joined by two peds (specifically, a pediatrician and a pediatric gastroenterologist) to talk all about infant feeding! We kicked things off with some statistics on infant feeding in the US and a discussion of current AAP and WHO recommendations. We spent a lot of time digging into breastfeeding-- the short- and long-term benefits of breastfeeding for infants and mothers, rare contraindications, tongue ties and frenotomies, and ways to boost breast milk supply. Next, we dug into studies that compare outcomes for babies who are breastfed versus formula-fed, focusing on allergies, immune function, and IQ. Then we switched gears and talked all about formula and its composition (including macronutrients, carbohydrates, fats, micronutrients, vitamin D, and more). We also discussed some new ingredients such as MGFM, lactoferrin, and HMOs. We close with a conversation about "organic" formulas and some common misconceptions. You don't want to miss this!
