It's so easy to get caught up in the "physical" when you are stricken with chronic pain out of nowhere. This is exactly what happened to Adam. Otherwise healthy for his whole life, Adam began having severe pelvic and back pain, and spent years in the frustrating cycle of specialists and failed treatments. In retrospect, Adam can see so many things in his life that he was unconsciously avoiding, key among them facing his social anxiety and "addiction" to the gym for safety. You all know I've been fascinated with the world of chronic pain science for a long time now. In fact, I did an episode on lowering pain signals through calming the brain and nervous system back in 2019 but I've been on a continuously evolving research journey since. According to today's guest - yoga therapist Rosemary Holbrook - addiction is often more of a symptom of past trauma. Her story of how she went from addiction (and sexual trauma which led to chronically painful intercourse the medical model had nothing to offer for) to being pain-free, and experiencing pleasure and a healthy relationship is illuminating. Honestly, I think it's important to speak to these topics here because they are so common in our communities, and often hidden and NOT talked about. THE BALANCED MOMTALITY- Pelvic Floor/Core Rehab For The Pregnant and Postpartum Mom
Hey Friend, For those that haven't been following along, I have been struggling with symptoms of Pelvic Venous Disorder or PeVD or better known as, Pelvic Congestion Syndrome, for the last year. It all started with an ovarian cyst like pain, but I later learned that my “menstrual cramps” I was feeling on and off throughout the month, not just during my menses, was also due to these enlarged veins. The heaviness and cramping was starting to get so uncomfortable that standing or sitting for too long was painful, and even exercise started to become painful. Thankfully sex was never painful, but painful intercourse or dyspareunia, especially pain or cramping with and after arousal or orgasm, is a very common symptom of PeVD. I was not a typical presentation for this diagnosis, so it surprised me when I found out from an ultrasound. So I share my story so that others with pelvic pain, constipation, heaviness, sharp abdominal pain, low back pain or pain with sitting, standing, or exercising, can start to become aware of this diagnosis and advocate for yourself. As many gynecologists do not believe this condition is a cause of pain…. And I can tell you… it most definitely IS! After two procedures using coil embolization and sclerotherapy to close off the dysfunctional varicose veins in my pelvis, it was identified that a compression of my Iliac vein was pretty severe and needed to be treated with a stent. I am two weeks postop and recovery has been a journey. Many things I was not expecting, so again, I wanted to share with the World so that maybe you can feel more prepared and aware of the treatment options out there for you! Lizzie discusses regular menstrual cycles, typically 24-35 days long, changes in cervical mucus becoming clearer and more stretchy around ovulation, increase in basal body temperature by 0.3°C or 0.5°F after ovulation, increased libido and sexual desire around ovulation, and mild cramps and breast tenderness around ovulation. Lizzie provides several actionable tips to help you optimise your ovulation and fertility. She recommends maintaining a balanced, anti-inflammatory diet, engaging in regular, moderate physical activity, tracking the menstrual cycle and symptoms, and getting the thyroid function checked, as these can all have a significant impact on ovulation. In today's episode Sonja takes us through her two pregnancies and births but also talks at length about her pelvic pain. She's been diagnosed with pelvic congestion syndrome which was exacerbated after birth and especially when her menstrual cycle returned. Her symptoms were very similar to a prolapse - a heaviness and dragging sensation in her rectum - but she also experiences sharp, debilitating pain that can last for up to an hour and significantly affects her day to day life. She is planning for a laparoscopy to investigate endometriosis and talks about the importance of specialist care and the comfort she finds in the stories of other women. Research Nerd Alert! This is an overview of our current understanding of what central sensitisation (aka nociplastic pain) is and how it impacts pelvic pain syndromes and of course...what we are goibg to do about it! Some great research around this topic has emerged over the past couple of years, with a growing understanding of why just focusing on the issues in the tissues is not enough and how we can incorporate lifestyle strategies such as sleep, nutrition and physical activity into our anti-inflammatory treatment toolbox So although this review is very evidence based for the first part, I do explore why this means when we are working with real life people - why and how to screen for CS and what steps we can take to get better outcomes - ultimately to help women with persistent pelvic pain live well!
