Manual removal of the placenta is a procedure sometimes required when the placenta does not deliver on its own after the baby is born. Typically, the placenta separates and is delivered within 30 minutes of birth, but in some cases, it remains attached to the uterine wall — a condition known as retained placenta. When gentle traction on the umbilical cord and uterine massage fail to deliver the placenta, the next step is manual removal. This involves the provider inserting a hand into the uterus to separate and extract the placenta, often requiring pain management or anesthesia depending on the situation. Prompt management of a retained placenta is essential to prevent complications like heavy bleeding (postpartum hemorrhage) or infection. After removal, the uterus is typically massaged firmly to stimulate contraction and minimize blood loss. In some cases, medications like Pitocin or antibiotics may be used. Although it can be an intense experience for both patient and provider, manual removal of the placenta is a life-saving skill — a critical part of birth work that ensures safe recovery after delivery. In today's episode we're talking with Danielle Turk-Bly, two time mom with a third on the way. Danielle is a self described "crunchy mom" (#represent) and we have an amazing conversation on this episode. We talk about all kinds of things from the birth planning to cloth diapering and homeschooling, running while pregnant, facing your fear demons around birth, placenta previa and how that has been impacting her current pregnancy, having a community of like-minded people to support you, cultivating your voice for what you want, and intimacy with your partner during pregnancy. Danielle's first birth was in a birthing center. When it came time to plan for the second pregnancy, she and her husband Ryan made the shift to home birth. Despite Ryan being a little bit more conservative than Danielle, he was fully on board and in support of the decision. Danielle doesn't revel in “going against the grain” or defying anyone else's way of going about things. She just simply does what resonates most with her. I really appreciate that perspective, because often times you think that home birth is the choice as a result of a defiance or rebellion against something. Danielle's example is a powerful one in just being proactive and true to yourself. And heads up - Danielle has thought about doing a podcast of her own. Emma goes briefly into her first two births when she was younger and feeling good through her pregnancies. With baby number 3, she experienced a lot of pain and discomfort and chose to induce her labour. It was a short labour, and resulted in a water birth. Her placenta needed help to be removed. Emma is very pragmatic about women's ability to birth, and she reminds us all how powerful women are, and we can give birth, even when we think we can't. Enjoy Emma's stories. Send us a textWelcome to the February Q&A! Today, Cynthia & Trisha kick off the episode by discussing reasons why our community has lost friendships or relationships over their parenting choices. In our regular episode, we answer the following questions:1. Is there such a thing as a lazy newborn, and if so, what can you do about it?2. Can my placenta detach in pregnancy? My OB told me I can't have sex because my placenta is low-lying and it could separate. 3. I have a diagnosis of gestational hypertension. If my blood pressure goes down, do I still need to be induced at week 37 as the OB is recommending? In our extended episode, we discuss when or not medications may be needed for postpartum depression, what increases the risk of cervical tears and hematomas, and what to do about respiratory distress in newborns born at home. Lastly, in quickies, we talk about doulas at birth, suggest the one question you should ask your midwife or doula, and answer whether you need to track kick counts. We discuss breastfeeding and laser hair removal, phototherapy and jaundice, and whether or not we start our day with a dry or wet toothbrush, among other morning routines. The location of the placenta plays a significant role in pregnancy and birth, but its impact can vary depending on specific circumstances. Placenta positioning is important for several reasons, particularly when it affects the path of delivery, but in many cases, it doesn't interfere with the general progress of pregnancy or fetal movement. One of the key factors in the placenta's location is whether it is positioned low in the uterus, a condition known as a low-lying placenta or placenta previa. This occurs when the placenta attaches near or over the cervix, the lower part of the uterus. When this happens, it can lead to complications, especially if the placenta partially or completely blocks the cervix (a condition known as placenta previa). In such cases, a vaginal delivery might be dangerous due to the risk of bleeding, and a cesarean section is usually recommended. Therefore, the location of the placenta in relation to the cervix is crucial for determining the safest mode of delivery. However, while the placement of the placenta can influence the type of delivery, it often doesn't affect other aspects of the pregnancy, such as fetal movement. The baby's ability to move inside the uterus is primarily determined by factors like amniotic fluid levels, the baby's size, and the integrity of the uterine wall, rather than the exact position of the placenta. In fact, many pregnancies with low-lying placentas do not lead to significant movement restrictions, and fetal kicks and rolls are still felt in the usual manner. It's also important to note that the placenta's position can change over the course of the pregnancy. As the uterus grows, the placenta may shift, especially if it was initially low-lying. In many cases, what appears to be a low-lying placenta early in pregnancy will resolve by the time of delivery, with the placenta moving upward and away from the cervix, allowing for a vaginal birth. In summary, while the location of the placenta is crucial for determining the safest method of delivery, it does not generally interfere with other aspects of pregnancy like fetal movement. Conditions such as placenta previa or low-lying placentas are primarily concerning in terms of the risk they pose to the delivery process, but they are often not a barrier to a healthy pregnancy overall. Today's podcast, The Role of Legal Nurses in Placenta Accreta Cases, discusses a serious condition called placenta accreta. When this occurs, the placenta attaches to the uterus, causing major difficulties in the delivery of the infant. Nurse specialist Katie Roberts goes into depth about this medical condition. While the condition is rare, it's on the rise. Katie explains that cesarian sections and placenta previa are some of the risks for placenta accreta. Another risk factor is a pregnancy in which there have been no ultrasounds and sometimes no medical supervision at all. This prevents the obstetrician from preparing safely for the delivery. A carefully planned delivery, often by cesarian section, involves a complete team of medical practitioners. A primary goal is to prevent excessive blood loss, and when deemed appropriate, to schedule the delivery in advance of the anticipated due date. An LNC experienced in obstetrics can make a major contribution to the evaluation of a case involving placenta accreta. He or she can assess the medical personnel's consideration of the risk factors and preparedness to prevent crises, as well as determining whether the optimal procedures were used. Katie points out that, due in part to women becoming pregnant at increasingly older ages, the risk of placenta accreta correspondingly increases. That alone makes this a valuable podcast for you.
