Urinary tract infections (UTIs) are among the most common infections in older adults, but in people living with dementia, they can trigger a sudden and dramatic change in behavior and cognition. A UTI that might cause mild discomfort in a younger, healthier person can instead set off delirium — an acute, fluctuating state of confusion — in someone with Alzheimer's or a related dementia.Dr. Shouri Lahiri is director of the Neurosciences Critical Care Unit and Neurocritical Care Research at Cedars-Sinai and leads the Lahiri Lab. His research focuses on the inflammatory mechanisms that link acute illnesses and infections to delirium and cognitive decline, including the role of immune signaling in brain changes during infection.In this conversation with Being Patient founder Deborah Kan, Lahiri explained why up to one-third of older adults with a UTI can develop delirium, and why that risk is even higher for people with dementia, who have less cognitive reserve to draw on. He discussed why early antibiotic treatment is critical and what caregivers can do — from monitoring hydration and sleep-wake cycles to recognizing subtle signs of urinary discomfort — to help prevent and manage delirium at home. We hear from Metra Mehran, a self-described person exiled from Afghanistan during the Taliban takeover in 2021, and who now focuses on advocacy work related to gender apartheid with Amnesty International, about how this gender apartheid has unraveled. In Afghanistan, there is a systematic level of discrimination that impacts every single facet of life for women and girls. Women cannot work, go to school beyond primary school, or have freedom of movement. The enforcement and institutionalization of this gender discrimination funnels into the crime of the grand-scale gender apartheid being perpetrated in Afghanistan. Child marriage has peaked in the past five years, interrupting a girl's inability to finish their education. Women and girls face higher rates of domestic violence, reproductive violence, and suicide. And while underground schooling does exist, girls seek education at extreme risk to themselves and their families.Advocates are pushing to use the International Criminal Court to use the Convention on the Elimination of All Forms of Violence (CEDAW) and take Afghanistan to international Criminal Court. You may wonder what you said wrong, whether you caused a trauma response, or whether it is still possible to have an honest conversation without making things worse.In this episode, Tina explains what the word “triggered” can mean, what may be happening in your adult child's nervous system, and how to respond without becoming defensive, dismissive, or afraid to speak.The word “triggered” originally came from trauma and PTSD research, where it described an involuntary nervous system response to something associated with past danger. Today, the word is also used more broadly to describe emotional distress, discomfort, overwhelm, or the desire to stop a difficult conversation.This episode is also partly based on research from the Gottman Institute, particularly its work on emotional flooding during conflict. Emotional flooding happens when the body becomes so activated that heart rate rises, stress hormones increase, and the ability to think clearly, show empathy, and solve problems begins to decrease.The Gottman Institute's research was conducted primarily with couples, not parents and adult children. However, the underlying nervous system response to relational stress can still offer a helpful framework for understanding why difficult conversations sometimes become unproductive so quickly.You do not have to decide in the moment whether your adult child is experiencing a trauma trigger, emotional flooding, or simply feeling overwhelmed. Your job is not to diagnose the reaction.
