Dr. Marla and I discussed numerous aspects of the business of PT, primarily how larger clinic employers set standards and key performance metrics for clinicians, such as length of stay, script frequency, and number of patients per day. We discussed how these metrics may actually be damaging to the shift towards primary care PT and talked about some solutions which would help employers move away from this. In this episode, I sat down with Dr. Chris McCallum to discuss how primary care PTs are changing the landscape of “health coverage” by contracting directly with employers. The “Direct to Employer” model of care puts PTs on-site where the workers are and significantly reduces the need for self-insured employers to redirect care claims to health insurance companies. It's advantageous for the worker, the employer and the PT! And in my opinion, it's the direction that we MUST pursue in order to preserve our profession. In this episode, I had the chance to sit down with Dr. Adam Matichak who is an oncology PT embedded in an acute care hem-onc team. Dr Matichak explains how his passion for improving access and outcomes for this patient population led him to seek more involvement in the hem-onc specialty team. This is an excellent reminder that team-based care is not limited to the primary care space. We can provide better care when we work together in specialty teams as well!

In celebration of Cancer Survivors Week (June 2-8, 2024), we had the privilege to chat with cancer survivor, Scott Capozza, who is a board certified oncologic physical therapist at the Yale Smilow Cancer Center. Scott talks about his experience being diagnosed with testicular cancer at the age of 22 as a young, healthy distance runner. He goes on to discuss his journey as a survivor, not only dealing with the long term effects of treatments, but also how various healthcare professionals have managed his care. It's not uncommon for healthcare professionals to write out 20 different health issues, then say "patient is otherwise healthy" at the end of their note. What is the gravity of this? In this episode, I had the chance to sit down with Dr. Brian Schaudt who practices in the most rural of rural locations in the frontier of Oregon. Having the opportunity to work in this setting and location gives healthcare providers a rapid baptism by fire in literally anything that could come in the door. Pete Contos tells a story about a patient that was referred to him when he was deployed in Korea. The consult said to "neck pain - eval and treat with neck stretches". In this episode, I sat down with my colleague, Dr. Micah Wong from the Samaritan Health system in Lebanon, Oregon. In this episode, we had a special patient guest share her postpartum story. She developed postpartum preeclampsia and acute heart failure. We discuss the benefits of a pre-discahrge PT evaluation, including testing vitals with movement, lifting and stair climbing, which may have detected key diagnostic features earlier. In this clinical spotlight, I share my own personal experience with POTS/dysautonomia along with Dr. Rebekah Griffith and Dr. Jessica Klain. I share my experience of feeling brushed off and gaslit by several healthcare providers who made me believe my symptoms were caused by anxiety before comprehensively putting the pieces together. To quote Dr. Griffith "if it truly is anxiety, I have tools to be able to handle this... but don't jump to that without thoroughly ruling out other things". Individuals with dysautonomia often go years before receiving a medical diagnosis due to lack of awareness by many healthcare professionals. It our responsibility to be informed... and never to gaslight a patient.
