Physio Podcast: Neuigkeiten zur Blankoverordnung in der Physiotherapie Willkommen zu einer neuen Episode vom Physio Podcast! Heute sprechen wir über die kurz bevorstehende Einführung der Blankoverordnung in der Physiotherapie, die am 1. November 2024 in Kraft tritt. Am 02. August 2024 haben sich die vier großen Physiotherapieverbände (IFK, Physio Deutschland, VDB und VPT) mit dem GKV-Spitzenverband auf alle wesentlichen Punkte geeinigt. Die Blankoverordnung ermöglicht Physiotherapeuten mehr Verantwortung bei der Behandlung von Patienten im Bereich der Schultertherapie. Sie können nun Heilmittel auswählen, die Behandlungsfrequenz festlegen und verschiedene Therapien kombinieren. Die Blankoverordnung bringt auch neue vergütete Leistungen mit sich: Physiotherapeutische Diagnostik: 34,34 Euro vor Therapie. Bedarfsdiagnostik: 25,76 Euro während der Therapie. Mehraufwandspauschale: 55 Euro für zusätzlichen organisatorischen Aufwand. Es wird kein neues Verordnungsformular benötigt. Der Vermerk „BLANKOVERORDNUNG“ wird einfach auf dem bestehenden Muster 13 eingetragen. Die Regelungen bleiben den Heilmittel-Richtlinien und dem Vertrag nach § 125 SGB V unterworfen. “Een van mijn levensmotto's is ‘Ken u zelve'. De weg naar binnen gaan, zoals in het labyrint, en met zelfkennis weer naar buiten gaan, de ander ontmoeten en bewust in de wereld staan. Ik ben zielsgelukkig dat ik 10 jaar geleden OMPT (opstellingen op basis van meergenerationele psychotraumatologie) leerde kennen. Het is rijk en dikwijls ontroerend om wat voor mijzelf waardevol is, te mogen delen met anderen. Interakt is de plek waar ik mijn bezieling vorm kan geven.” Margriet is gespecialiseerd in het werken met Interactieve Zelf-Resonantieprocessen. Met behulp van deze methode begeleidt ze mensen bij het bereiken van hun doelen en verlangens, bij het verwerken van traumatische ervaringen en bij het ontwikkelen van zichzelf. ‘Met compassie naar de kern' is haar motto, waarbij aandacht, verfijning, empathie en vakkundigheid hand in hand gaan met respect voor de autonomie van de cliënt. Aan haar huidige werkwijze gaat meer dan een decennium van intensieve scholing, supervisie en ervaring vooraf. Vanaf 2005 specialiseerde ze zich in de traumaverwerkingsmethode van dr. Franz Ruppert (IoPT), wiens werk ze introduceerde in het Nederlands taalgebied. Samen met haar partner Wim Wassink vertaalde ze een aantal van Rupperts boeken naar het Nederlands. Om de methode toegankelijk te maken voor een breder publiek, begon ze met de publicatie van de serie Breek de stilte, waarvan het eerste deel Je verlangen dwaallicht of kompas in 2014 verscheen en het tweede deel, Het mamatrauma in 2018 verschijnt. Vanuit haar centrum voor meergenerationele psychotraumatologie, Interakt in Tiel, biedt ze verschillende activiteiten aan die gebaseerd zijn op de traumatheorie van Ruppert. In Nederland, België, Duitsland en Frankrijk geeft ze workshops waarin mensen met behulp van interactieve zelf-resonantie aan eigen levensvragen kunnen werken. Daarnaast geeft ze trainingen en opleidingen aan professionals die de methode willen gebruiken in hun werkveld (GGZ, jeugdzorg, onderwijs, politie, pastoraat, etc.) Ook geeft ze op verzoek lezingen en workshops in het onderwijs, de geboortezorg, de jeugdzorg en op themadagen binnen de GGZ (over vroegkinderlijk trauma en over het thema van haar nieuwste boek, het mamatrauma).

Abstract: Prototypic antidepressants, such as tricyclic/tetracyclic antidepressants (TCAs), have multiple pharmacological properties and have been considered to be more effective than newer antidepressants, such as selective serotonin reuptake inhibitors (SSRIs), in treating severe depression. However, the molecular mechanisms underlying the high efficacy of TCAs have not been completely understood. Herein, we found that lysophosphatidic acid receptor 1 (LPAR1), a G protein-coupled receptor, mediates the antidepressant effects of amitriptyline, a typical TCA. Amitriptyline directly bound to LPAR1 and activated downstream G protein signaling without affecting {beta}-arrestin signaling, which implied that amitriptyline could act as a G protein-biased agonist of LPAR1. This biased agonism is unique to TCAs and has not been observed in other antidepressants, such as SSRIs. Long-term infusion of mouse hippocampus with 1-oleoyl-2-O-methyl-glycerophosphothionate (OMPT), a potent G protein-biased LPAR1 agonist, induced behavior similar to that induced by antidepressants. In contrast, LPA, a non-biased agonist of LPAR1, induced anxious behavior, indicating that LPAR1 may regulate conflicting emotional behaviors because of the downstream signaling bias. Furthermore, RNA-seq analysis revealed that LPA and OMPT have opposite patterns of gene expression changes in hippocampus. Ingenuity pathway analysis indicated that chronic intrahippocampal administration of OMPT could activate LPAR1 downstream signaling (Rho and MAPK), whereas LPA suppressed LPAR1 signaling. The results reveal the unique antidepressant effects of TCAs and indicate the potential of G protein-biased agonists of LPAR1 as targets for novel antidepressants. In 2019, her second child was born three weeks after graduating from an OMPT fellowship at Berlin College in collaboration with Evidence in Motion. She shares what factors contributed to her being able to successfully complete her residency and fellowship while being pregnant during both. She also highlights physical therapist's natural affinity for giving to others before we give to ourselves and it's effects on our mental health. He currently practices at Active Life and Sports PT in Baltimore, Maryland and had most recently practiced in the outpatient Orthopedic Department at Johns Hopkins Hospital where he served as clinical faculty for the Johns Hopkins/George Washington University Orthopedic Residency Program. He currently teaches in the South College DPT program and has developed a blended-model PTA program for Rasmussen College. Dr. Shepherd's clinical and research interests include chronic and persistent pain disorders, spinal and extremity manual therapies and clinical reasoning in physical therapy. He has presented at both regional and national physical therapy conferences speaking about professional development as a new professional and post-professional residency and fellowship training.

Dr. Whitman is the Director of Evidence In Motion's Orthopedic Manual Physical Therapy Fellowship Program and the Evidence in Motion Post-Professional DPT in Musculoskeletal Management. She is board certified in Orthopaedic Physical Therapy and is a Fellow of the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT).  Dr Whitman is widely published, including educational CD-ROMs, e-books, and texts on Orthopaedic and Manual Physical Therapy. She has 40+ peer-reviewed manuscripts, and over 30 published abstracts in the areas of orthopaedics and manual physical therapy.  She has been either the primary author or co-author on research grants from the Foundation for Physical Therapy, the Orthopaedic Section of the APTA, and the AAOMPT, and served on the Steering Committee for Physical Therapy Journal.  She has presented research both nationally and internationally at professional conferences, and awards include: AAOMPT's Kaltenborn “Teach I Must” Award, Section on Geriatrics Excellence in Research Award, JOSPT George J. Davies - James A. Gould Excellence in Clinical Inquiry Award, AAOMPT Outstanding Research Award, Discovery Health Clinical Excellence Award in Back Care at the International Federation of Orthopaedic Manipulative Therapists Conference, Outstanding Clinical Educator in the State of Texas, the Jack Walker Award, and Steven J. Rose Excellence in Research Award.  Dr. Whitman continues to maintain an active research agenda in the areas of spinal and extremity manipulation, low back disorders, and the development of clinical prediction rules.  She also currently lives in Roseville, California, and enjoys spending her life with her wonderful husband Bernie and two little girls, Krista and Anna.
