We detail disruptions in motility seen in dysautonomia, intestinal methanogen overgrowth (IMO), and small intestinal bacterial overgrowth (SIBO). And, we finish with a brief discussion on the impacts of low stomach acid. Topics: 1. Intestinal Methanogen Overgrowth (IMO) - Characterized by an overgrowth of methane-producing archaea in the intestines. - Excess methane production affects smooth muscle activity and can slow intestinal transit. - Symptoms such as bloating, abdominal discomfort, constipation. - Breath testing. 6. Small Intestinal Bacterial Overgrowth (SIBO) - Excessive bacterial colonization in the small intestine, disrupting normal gut function. - Protective mechanisms including stomach acid and intestinal motility. - Low stomach acid (hypochlorhydria) reduces the acidic environment needed to inhibit bacterial colonization. - Bloating, diarrhea/constipation, and more. 7. Overlap Between IMO and SIBO - Excess hydrogen produced in SIBO can serve as a substrate for methane production in IMO. - Can exacerbate each other, creating a loop that worsens symptoms. 8. In this episode, we explore the intricate role of the vagus nerve as a central regulator within the microbiota-gut-brain (MGB) axis, examining its neuroanatomical structure, signaling mechanisms, and interactions with microbial metabolites and immune pathways. We discuss how vagal afferent fibers relay sensory input from the gut to the brain, including signals mediated by short-chain fatty acids (SCFAs) and gut-derived hormones, and how efferent fibers modulate gut motility, intestinal barrier integrity, and inflammation through the cholinergic anti-inflammatory pathway. Finally, we explore vagal dysfunction as well as associated conditions and symptoms, and we touch on just a few potential root causes. Topics: 1. Introduction Focus on the vagus nerve's role in the microbiota-gut-brain (MGB) axis. Bidirectional communication between the brain and microbiota. Overview of communication pathways: neural (e.g., vagus nerve), endocrine (e.g., HPA axis), immune (e.g., cytokines), and metabolic (e.g., SCFAs). 2. Overview of the Nervous System The CNS includes the brain and spinal cord - control centers for the body. Interact indirectly with gut luminal contents via specialized gut cells, including EECs and immune cells. 6. Interaction with Intestinal Cells Enteroendocrine cells (EECs) release gut hormones in response to microbial metabolites. SCFAs, such as butyrate, activate free fatty acid receptors on EECs, stimulating vagal afferents. Immune cells within the gut wall modulate vagal signals during inflammatory responses. 7. Review of Functions Sensory input (afferent fibers): Detects gut-derived signals like microbial metabolites and mechanical stretch. Motor output (efferent fibers): Regulates gut motility, secretion, immune responses, and more. 8. Impact of a Diverse Microbiome on Vagal Activity Enhanced SCFA production boosts vagal activity. Diagnosing Post-Acute Infection Syndromes: Special Series from The Cohen Center for Recovery from Complex Chronic Illnesses at Mount Sinai“Tick-borne illnesses are incredibly stealthy and complicated and if I wasn't living and breathing it every day and seeing the intensity of these symptoms in patients, I would never believe it,” says Shannon Delaney, a psychiatrist in private practice in New York City. The complexity of such illnesses and the symptoms they cause is a key reason it can often take years to reach a diagnosis, which is the focus of this episode of Raise the Line, part of a special series on post-acute infection syndromes that Osmosis from Elsevier has created in partnership with the Cohen Center for Recovery from Complex Chronic Illnesses (CoRE) at the Icahn School of Medicine at Mount Sinai Hospital. In addition to Dr. Delaney, host Dr. Raven Baxter of CoRE welcomes Dr. Amy Kontorovich, an associate professor in Cardiology and Genomic Medicine at the Icahn School and Dr. David Putrino, director of Rehabilitation Innovation at Mount Sinai, to explore the limitations of current diagnostic tools and protocols and what changes are needed to improve patient care. I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Jilian: Hi Dr. Cabral, I have a strange question regarding my leg. I have a dent in my upper right quad that runs horizontally. It seems as if my quad muscle somehow split, however I have no clue why or how. I saw a doctor who said it's not fatty tissue but appears to be my muscle. The dent is pretty noticeable but not painful. I noticed it sometime after having my first child but it either went away or was not as significant. But since having my second child it's come back and more prominent. For reference I'm currently 10 months postpartum with my second and weight 125lbs (nearly pre pregnancy weight). My hormones according to a regular doctor test said everything looks fine, although I know in the past I've been told I have PCOS. Not sure what's causing this dent. Leila: Hi Dr. Cabral! I work as a nurse in the hospital and our hospital is implementing new badges that track us throughout the hospital. Our badges clip to our shirt and hang over our chest and it makes me wonder if we should be doing something to protect ourselves. Thanks! Dacia: My aluminum number is 43 nickel 5.9 and cadmium is high too …I've been tested by a doctor and have been doing chelations Thru an IV for a year ….I'm completely broke because I have spent over $10K on these treatments…I need a protocol I can do at home …I have a red light therapy machine and a doctor grade ionic foot bath at home …very interested in what i can do with products to take internally …my quality of life is about a 3 out of a ten …very fatigued and flared up from Rheumatoid arthritis (or the heavy metals ) please help !!!! My blood sugar was all over the place even with a Perfect diet , your detoxes , protocols , big 5 , working with a coach ect .. I did have improvements but still did not have much energy, stamina and alway dizzy as well as fight or flight. My functional Med Dr. Suggested Ozempic & then Mounjaro. I can't believe how Amazing I feel ! I'm working out again , popping up in the morning with tons of energy , my blood sugar is perfect , my A1c is 4.6 now . I just feel like I have my life back . I don't have one bad side effect. I have 1 to 3 bowl movements a day , I'm still hungry and eat. Why do you think this Med has worked so well for my dysauto. Yvonne: Interested in trying ALA for blood glucose and neuralgia but have a few questions: (1) one site says do not take if you are taking thyroid medication, which I am - I take desiccated thyroid (2) warns that taking may cause harm due to mercury not being removed from the body and (3) if ok to take, what is daily dosage that works without harm and does it matter what type of ALA (type S or R)? Worked with a coach , did several protocols ect . I. Have improved some but the sensory overload/ SEVERE Anxiety is still there . It's even stopped me from attending church and social events. I feel as though I'll have a seizure due to the over stimulation. I've exhausted all avenues besides SSRIs and my spinal issues from an ATV accident where I landed on head & neck . I have lots of issues with it but terrified to have my neck adjusted due to strokes that can occur . Do you think the dysautonomia could be from my cervical spine ? I'm at a loss of what to do besides possible surgery or SSRIs and I really don't want to do either but I've been suffering long Annalize: I'm a healthy 47-year-old woman 6.0 ft tall 165 lb. I've struggled with recurrent vaginal yeast infections since high school, trying various treatments. I worked with a Clinical Nutritionist, adopting a paleo diet, my Candida tests were negative, and the infections persisted. I completed the Functional Medicine Detox and CBO protocol. My daily routine includes Fruit and Vegetable blend and Daily Nutritional Support, along with a healthy diet and lifestyle choices. We've been married 23 years, no STDs, 3 children. Alicja: Hi Dr. Cabral, I hope you are doing well and having a great day. Two years ago my blood work showed I had Hashimoto and hypothyroidism. I listen to your podcasts, I took your course about Thyroid, I implemented now foods to my diet, did heavy metal detox, made some changes and have been doing all the right things. My TSH numbers went lower over time, then fell into a healthy range, but my last labs showed my TSH being very low, it went down to 0.3. Does that mean that I went from hypothyroidism to hyperthyroidism? Also my anty-TPO are ok (very low) but I have still elevated anty-TG. What does that indicate now? Do I still have Hashimoto or is it Graves now? Thank you in advanced for clarifying that for me. You always are a big help!
