In this episode, Prof Hill describes a systematic approach to evaluating lumps in the clinic and subsequent managementProfessor Hill is Professor and Chair of Surgery in RCSI, Beaumont and Head of the School of Medicine, RCSI University of Medicine and Health Sciences. He graduated from UCD and did his basic surgical training in Dublin, and his middle grade surgical training in London. He did a basic two year fellowship with Dr John Daly at The Hospital of the University of Pennsylvania and The New York Hospital/Cornell Medical Center in the United States. He returned to Ireland to do his Senior Registrar training on the National Training Program in Ireland. He also did a Clinical Fellowship in Surgical Oncology at Memorial Sloan Kettering Cancer Centre in New York prior to taking up his consultant appointment. His clinical interests are in the area of breast cancer and melanoma. His laboratory research interest are in the transcriptional control of breast cancer in particular the role of the coregulatory proteins.

https://thegenerehlist.ca/ccfp-exam-105-topics-podcast/ 1 Given a well woman with concerns about breast disease, during a clinical encounter (annual or not): a) Identify high-risk patients by assessing modifiable and non- modifiable risk factors b) Advise regarding screening (mammography, breast self- examination) and its limitations. c) Advise concerning the woman's role in preventing or detecting breast disease (breast self-examination, lifestyle changes). 2 Given a woman presenting with a breast lump: a) Use the history, features of the lump, and the patient's age to determine if aggressive work-up or watchful waiting is indicated. b) Ensure adequate support throughout investigation of the breast lump by availability of a contact resource. c) Use diagnostic tools (e.g., needle aspiration, imaging, core biopsy , referral) in an appropriate manner (i.e., avoid over- or under-investigation, misuse) for managing the breast lump. 3 In a woman who presents with a malignant breast lump and knows the diagnosis: a) Recognize and manage immediate and long-term complications of breast cancer. b) Consider and diagnose metastatic disease in the follow-up care of a breast cancer patient by appropriate history and investigation.c) Appropriately direct (provide a link to) the patient to community resources able to provide adequate support (psychosocial support).

In it, I'm sharing a story about my experience finding a lump in my breast and then having a somewhat terrifying radiologist visit where I got an abnormal ultrasound that required a biopsy. I had a lot of tough decisions to make and ultimately learned new things about my body and my health, was able to connect the dots between this issue and other health issues I'd had, got conflicting opinions from different doctors and got caught up in the insurance vs. out of pocket care dilemma. It was an intense thing to go through, but it taught me so much about our healthcare system and my own health, and I wanted to share that with you.
