Welcome to the latest summer version of the Midlife Minute.  In this episode, we're focusing on the gut. Even though I've written a book about the gut microbiome changes that occur in perimenopause and menopause, I'm often asked whether the gut is truly central to midlife. So today, I provide direct answers to the many questions I received on this topic.   IN THIS EPISODE, YOU WILL LEARN: How the gut microbiome and estrogen work together, and the importance of supporting both  How the estrobolome helps recycle estrogen, and why its function could decline during menopause How poor sleep, blood sugar instability, histamine issues, and dysbiosis tend to be interconnected The value of practitioner-guided microbiome testing when investigating histamine intolerance, dysbiosis, and weight loss resistance Various factors that could contribute to weight loss resistance How my approach to stool testing has evolved as newer microbiome testing technology has become available Essential lifestyle habits for supporting gut health Connect with Cynthia Thurlow   Follow on X, Instagram & LinkedIn Check out Cynthia's website. Have you done everything right and still watched the scale refuse to move? I sat down with fitness industry veteran Chalene Johnson, who built her career telling women exactly how to lose weight, and then walked straight into that same wall herself in perimenopause. Her hormone panel came back "dialed in." She was still doing every workout, every macro track, every habit that used to work. The weight stayed anyway, until she found the piece nobody had pointed her toward, the link between cortisol, insulin resistance, and a metabolism that stops cooperating no matter how hard you push. We get into the stress inventory that changed her day almost overnight, why she traded daily cardio for heavy strength training, her honest concerns about GLP-1s, and the Harvard longevity data that convinced her friendship matters more for a long healthy life than anything you'll find in a gym. Timestamps: 0:00 — Doing everything right and still gaining weight in perimenopause 4:36 — How the fitness industry sets women up to fail on purpose 12:00 — Hormones "dialed in," weight still not moving 13:14 — The cortisol and insulin resistance connection nobody explained to her 13:57 — The stress inventory exercise that changed her days 23:39 — Why she traded cardio for heavy strength training 25:48 — The identical twin study on muscle, memory, and cognition 27:55 — Her honest concerns about GLP-1s and rapid weight loss 33:49 — Why community predicts longevity more than diet or exercise 36:03 — Rapid fire: supplements, sunlight, and energy vampires 42:08 — Camp Be More and where to find Chalene Memorable Quotes: "I have my hormones dialed in, but when I was working with an integrative medical doctor who would look at my panel, she's like, 'Your cortisol is through the roof.'" — Chalene Johnson "The machine needs for you to not succeed so that you believe, okay, something else is going to work for me." — Chalene Johnson "We can't heal a body we hate." — Dr. Anna Cabeca Key Takeaways: A "perfect" hormone panel doesn't rule out cortisol and insulin resistance quietly driving weight gain underneath it Writing out every stressor and circling only what you control turns "reduce your stress" into something actually doable Lower body strength training, not cardio, was the single habit Chalene says moved the needle most for her body and brain The identical twin study she references found more lower body muscle correlated with better memory and cognition at 50 and beyond Rapid weight loss without addressing muscle and detoxification support can cost you the two things that matter most for longevity Community, not diet or exercise, was the strongest predictor of quality of life and lifespan in the longest running Harvard longevity study Vitamin D from sunlight and magnesium are the two non negotiables in Chalene's own daily routine FAQ Why do I keep gaining weight in perimenopause even though my hormones are balanced?  For many women, hormone balance is only part of the picture. Cortisol and insulin resistance can quietly drive weight gain even when a hormone panel looks well managed. Addressing stress load and metabolic health alongside hormones is often the missing piece. Does stress actually cause weight gain, or is that oversimplified? Chronic stress raises cortisol, and elevated cortisol is linked to increased insulin resistance and a slower metabolic rate. This isn't about willpower. It's a physiological chain that can work against weight loss efforts regardless of diet and exercise habits. Is strength training better than cardio for women in midlife? Strength training, particularly lower body work with adequate weight, supports muscle retention, bone density, and metabolic health in ways cardio alone doesn't. Many experts recommend prioritizing strength training first, then layering in cardio. What is Camp Be More? Camp Be More is a live event created by Shelene Johnson for women, focused on connection, aging on your own terms, and building community with other women navigating midlife. How much strength training do I need to see results? Shelene recommends starting with two days a week focused on the lower body if daily training feels overwhelming. Get your copy: https://www.theunbreakablelifestyle.com/unbreakable-book   There's a heat you can't feel until it's already aging you.   Let's talk about something quietly ticking away inside all of us. In this solo episode I'm getting into inflammaging, the second great time bomb of aging, and what happens when your body's emergency crew never clocks out. Think alarm bells ringing around the clock, sirens that never quiet down.   And here's where it gets personal: perimenopause. That's the wake-up call, the moment declining estrogen strips away one of your most powerful anti-inflammatory shields, and suddenly the fire's raging and no one tells you why.   So I'm not leaving you there. I'll walk you through the real blueprint to put those flames out: how you move, how you eat, your hormones, and how you meet your stress. Because hear me on this one. Longevity doesn't just happen to you. You build it. On purpose. By design. What we cover: - Chronic inflammation vs. acute inflammation: what your body actually needs. - Estrogen decline in perimenopause: losing your anti-inflammatory shield. - Inflammaging effects: joints, heart, brain, and metabolism. - Physical activity and myokines: your systemic fire extinguisher. - Gut microbiome diversity: fueling or fighting inflammation. - Processed sugars and inflammation: what to eat instead. - Hormone optimization: working with a clinician to calm inflammation. - Mindfulness, breathwork, and sleep: lowering inflammatory cytokines.

Spend enough time on TikTok or Instagram and almost every symptom from fatigue and weight gain to anxiety, irregular periods, brain fog, and poor sleep, can suddenly become evidence that your hormones are "out of balance" - but a viral video is not a diagnosis. In this episode of Confessions of a Male Gynecologist, Dr. Shawn Tassone breaks down some of the most common hormone claims circulating on social media and explains why women's hormonal health is rarely as simple as one symptom, one lab value, one supplement, or one trending diagnosis. Dr. Tassone takes a closer look at hormone imbalance, cortisol, adrenal fatigue, PCOS, estrogen dominance, perimenopause, hormone testing, and supplements, and why context matters before deciding what is actually happening in your body. In this episode, Dr. Tassone discusses: What people actually mean when they say their hormones are imbalanced Why cortisol has become one of the internet's favorite hormones to blame The problem with diagnosing yourself with "adrenal fatigue" from fatigue, stress, or burnout Why symptoms alone aren't enough to diagnose PCOS What should actually be considered during a proper PCOS evaluation Why the term "estrogen dominance" is often oversimplified online How perimenopause symptoms can overlap with thyroid problems, stress, metabolic issues, sleep disorders, and other conditions Why your medical history can matter just as much as a hormone panel The truth about supplements marketed to "balance your hormones" Why a supplement can be useful without every claim made about it being true The difference between educating women about their bodies and convincing women that every symptom means something is hormonally wrong How social media algorithms can reward certainty and sensational claims when good medicine often requires nuance Dr. Tassone isn't arguing that women should stop asking questions about their hormones or that conventional medicine always gets women's health right. Quite the opposite. Women deserve better information, better conversations, and clinicians willing to investigate symptoms instead of dismissing them. But replacing medical dismissal with social-media self-diagnosis isn't the answer either. Your symptoms are real. The diagnosis still matters. If you've ever wondered whether you really have a hormone imbalance, whether cortisol is behind your symptoms, whether you should be taking another "hormone balancing" supplement, or whether TikTok has convinced you that you have three different endocrine disorders, this episode is for you. Perimenopause changes how your body responds to training, nutrition, caffeine, sleep, recovery, and many supplements, making it feel like the things you always did to support your health and fitness are no longer working. And those shifts can begin as early as your 30s.In this episode, Louise Valentine, a leading women's integrative health and fitness expert for perimenopausal active women and female runners, shares real-life conversations with women who were doing many things “right” but still dealing with injuries, poor sleep, anxiety, cravings, body composition changes, and frustrating fitness setbacks. She explains why creatine quality, mode of delivery, and timing matter, why the right type of strength training becomes increasingly important for protecting muscle and reducing injury risk, and why protein quality deserves closer attention as women age.Louise also breaks down how late-day caffeine, poor fueling, and sugar before bed can affect cortisol, sleep, digestion, and next-day energy. Finally, she explains why extreme programs like 75 Hard may deliver short-term results but are nearly impossible to sustain due to the impacts on hormones, metabolic health, and working against time tested principles like progressive overload of exercise and nutrition strategies. The main message is simple: learn how your body is changing before problems pile up. As you will learn, working with your changing female physiology should not be complicated or confusing. Have you ever found yourself staring at a brightly colored gummy bottle in the pharmacy aisle, hoping a quick supplement fix will finally give you a full night of uninterrupted sleep? The promise of an easy solution is incredibly tempting, but what if these trendy sleep hacks are actually masking deeper behavioral shifts your child really needs? In this episode, I connect with pediatrician and sleep medicine specialist Dr. Craig Canapari to separate social media fiction from clinical fact when it comes to pediatric sleep aids. Dr. Canapari breaks down the true neuroscience behind melatonin as a hormone, shines a light on the dramatic rise in accidental supplement overdoses, and uncovers what the data actually says about the viral magnesium foot rub trend. The Rise of the "Purple Aisle": Why exhausted parents and heavy marketing are driving an unprecedented surge in pediatric sleep supplements. Melatonin is a Hormone, Not Candy: The vital distinction between a natural biological trigger and utilizing exogenous melatonin as a hypnotic knockout pill. The Nuance of Dosing: How melatonin can genuinely support neurodiverse children (like those with autism or ADHD) and why timing it right is everything. The Poison Control Warning: The alarming 500% spike in melatonin overdoses and the critical danger of loose food supplement regulation. Diving into the Magnesium Hype: Sorting through the TikTok claims of magnesium sprays and lotions to find out if there is any real pediatric evidence behind them. The True Root of Sleep Issues: Why natural fads often act as a barrier to established, highly effective behavioral sleep hygiene adjustments. The 20-Second Sleep Pitch: Dr. Canapari's foundational pillars—from consistent wake times to the ultimate goal of independent sleep—to safely reclaim your family's rest. In this thought-provoking episode, we sit down with Dr. Sarah Hill to explore the far-reaching effects of hormonal birth control, not just on the body, but on the brain, relationships, and a woman's lived experience of the world. Dr. Hill shares how her research was sparked by a deeply personal moment: after discontinuing hormonal birth control in her early thirties, she was struck by a profound shift in how she experienced life. Colors felt brighter, music more moving, and everyday moments more meaningful. This transformation led her to question what might be lost when the natural hormonal rhythm is suppressed. Together, we unpack the idea that hormonal birth control creates a kind of “black and white” internal landscape, flattening the natural peaks and valleys of the menstrual cycle. While this can offer predictability and convenience, it may also dampen the richness and intensity that hormones naturally bring to a woman's emotional and sensory world. This conversation places that trade-off within a broader cultural context, where modern life often prioritizes ease and instant gratification, sometimes at the cost of depth and meaning. A major focus of the episode is the adolescent brain. Dr. Hill highlights how little we truly understand about the impact of hormonal birth control during the critical window of development between ages 12 and 19. This is a time when the brain is undergoing significant remodeling, guided in part by natural hormonal signals. Emerging research raises important questions about how altering those signals might influence long-term mental health and development. While animal studies suggest measurable changes to brain structure, human data points to a potential association between adolescent use and increased risk of depression later in life. Still, many questions remain unanswered, making this an area in urgent need of further study. We also explore the fascinating connection between hormones and attraction. Natural hormonal shifts across the menstrual cycle subtly shape preferences, especially around ovulation, when estrogen peaks. During this time, women may be more drawn to cues associated with genetic compatibility. Hormonal birth control suppresses these fluctuations, potentially influencing patterns of attraction in ways that are often invisible but meaningful. For some women, this can translate into changes in relationship dynamics over time, though experiences vary widely. At the heart of this conversation is a call to awareness. Rather than framing hormonal birth control as simply good or bad, Dr. Hill encourages women to understand it as a powerful intervention with real trade-offs. Hormones are not arbitrary they are part of an intricate system that shapes how women think, feel, and connect. Interested in being a Podcast Sponsor with THE HORMONE GENIUS? If you've wondered why moms feel more anxious, overwhelmed, and reactive after 40, you're not alone. Hormonal changes can affect your brain, stress response, and parenting in surprising ways. Dr. Roseann Capanna-Hodge shares how Regulation First Parenting™ helps families navigate emotional dysregulation with confidence and calm.Parenting is already demanding, but when perimenopause enters the picture, many moms suddenly find themselves feeling exhausted, forgetful, short-tempered, and emotionally overwhelmed. In this episode with hormone expert Dr. Mariza, we unpack the science behind these changes and share practical ways to support your brain, hormones, and nervous system. It's not bad parenting—it's a dysregulated brain navigating a major biological transition. Why do moms feel more anxious, overwhelmed, and reactive after 40?Perimenopause is more than changing hormones—it's a major neuroendocrine transition that directly impacts brain function, stress resilience, memory, and emotional regulation. As estrogen fluctuates, the brain has to work harder, making everyday parenting feel much more difficult.A mom who once handled school drop-offs, work meetings, and bedtime with ease may suddenly find herself snapping over spilled milk or forgetting simple words. That doesn't mean she's failing—it means her biology is changing.Key takeaways:Hormones affect the brain—not just reproduction.Irritability is one of the earliest signs of perimenopause. You didn't lose your ability to cope—your brain needs more support.

They discuss how estrogen, progesterone, testosterone, cortisol, neurotransmitters, inflammation, gut health, liver function, and detoxification pathways may all interact and affect emotional and psychological well-being.Emily also shares her personal journey with endometriosis, chronic pain, inflammation, psychiatric medication withdrawal, and the process of understanding how hormonal health can influence both physical and mental symptoms.Topics discussed in this episode include:• How hormone imbalance may affect mental health• The relationship between hormones and neurotransmitters• Estrogen, progesterone, testosterone, and mood• Hormonal changes and anxiety, depression, mania, and psychosis• Endometriosis, chronic inflammation, and mental well-being• Gut health, liver function, and hormone metabolism• The connection between stress, trauma, and hormonal regulation• Endocrine disruptors and everyday environmental exposures• Diet, inflammation, and holistic approaches to health• Looking for underlying causes instead of focusing only on symptomsDr. Lee also shares clinical observations that led her to look more closely at hormonal health in patients experiencing significant changes in psychiatric symptoms. This conversation highlights an important idea: mental health does not exist separately from the rest of the body. Send us Fan MailIn this episode, I get real about where I'm at right now: 42, in the middle of summer chaos with four boys (including twins), and finally admitting that it doesn't just take a village to raise kids—it takes a village to take care of *me*.After our recent vacation, I hit a low point. My back was flaring up, my mood was all over the place, and I realized I was surrounded by males all day and missing my girlfriends' support. So I started rebuilding my “village”:- Seeing a naturopath for hormones and supplements  - Going to physio and doing core work for my lower back  - Looking into acupuncture  - Seriously considering hormone replacement therapy (HRT), even though I used to think it was “unnatural”I've always been very “let the body do its thing” and avoided meds unless absolutely necessary. But after months of feeling like the worst version of myself when I'm tired—snapping, raging over tiny things, feeling almost “crazy”—I decided my quality of life matters more than being perfectly natural. I'm still cautious and doing my research, but I'm open to HRT if it helps.A few big takeaways I land on in this episode:- Hormone decline can start up to 10 years before “menopause age,” and a lot of symptoms (mood swings, rage, anxiety, depression-like feelings) get misdiagnosed or ignored.  - My husband loves me, but he doesn't know what to do with my symptoms. It's on me to lead my own care and then invite him in.  - Taking care of myself takes effort: booking appointments, trying supplements, doing the exercises, saying no to the extra bread, asking for help.  - If what I'm doing isn't fixing it, I need to go back for help—even if I “know what to do,” because consistency is the hard part.I end with a gentle nudge to anyone listening: if you've been debating booking that appointment, making that call, or trying that thing you keep putting off, this is your sign. Get that one more person into your village who helps *you* feel better, so you can show up better for everyone else. Support the show

It's a major neurological and metabolic transition. Estradiol and progesterone influence your brain, nervous system, neurotransmitters, sleep, mood, memory, focus, and even how you respond to stress. Hormones matter. But they don't operate alone. In this episode, Janell explores one of the biggest misconceptions in women's health: that every symptom begins and ends with hormones. You'll learn why hormones are actually messengers responding to what's happening throughout the body, and why addressing sleep, stress, blood sugar, digestion, nutrition, and nervous system health is often just as important as supporting hormones themselves.Janell explains the difference between the body's inputs and outputs, shares real clinical examples from her practice, and offers a whole-body perspective that can help you better understand what your symptoms may actually be communicating. If you've been told your hormones are the problem—but you still don't feel like yourself—this episode will help you ask better questions and see your health through a much wider lens.Learn more:

What if the hormones meant to help you are the very thing making you feel worse? In this solo episode of Let’s Talk Wellness Now, Dr. Deb opens with a patient who started bioidentical hormones and, within three weeks, had panic attacks for the first time in her life. From there, she walks through the five systems that determine whether hormone therapy helps you or harms you and exactly what to address before you ever fill that prescription. If you have been told hormones are a simple fix, this episode reveals why readiness matters just as much as the prescription itself and how getting it right can change everything. In this episode, you will learn: Why estrogen has to be metabolized, and what happens when your liver cannot clear it fast enough How gut bacteria and beta-glucuronidase can recirculate the estrogen your body was trying to eliminate Why normal liver enzymes can still mean you are a poor estrogen metabolizer How estrogen can quietly make you functionally hypothyroid, and why TSH alone misses it The histamine connection behind hives, palpitations, anxiety, and panic on hormones The five-point checklist Dr. Deb runs before anyone in her practice starts HRT What Dr. Deb says:“The hormones aren’t the problem. The problem is whether your body is actually ready to receive them.”“Imagine you’re filling up a bathtub, but the drain is completely clogged. The water just keeps rising. That’s what’s happening when you add estrogen to a body that can’t clear it.” Timestamps:[00:01] A patient in tears: panic attacks three weeks into bioidentical hormones[02:30] Why estrogen is not a magic bullet, and the clogged bathtub analogy[04:53] Protective vs. problematic estrogen metabolites, and why testing matters[06:00] The gut connection: beta-glucuronidase and estrogen recirculation[09:42] The GI MAP, and why the gut has to be ready first[11:00] Liver phase one and phase two detox, and where the bottleneck forms[14:27] Thyroid: how estrogen can make you functionally hypothyroid[16:53] Histamine: the overlooked driver of hives, palpitations, and panic[20:00] The five-point checklist before touching hormones[24:50] The patient’s turnaround, and why HRT is a living protocol, not one and done Links:https://mdcustomrx.com/Fullscript: http://bit.ly/4fqqsPahttps://elliemd.com/DrDebMuth Connect with us:Website: https://www.letstalkwellness.comInstagram: https://www.instagram.com/letstalkwellnessFacebook: https://www.facebook.com/letstalkwellnessLinkedIn: https://www.linkedin.com/company/lets-talk-wellness-now/ Subscribe to Let’s Talk Wellness Now so you never miss an episode, and if this one opened your eyes, share it with someone who needs to hear it.The post Episode 277: Perimenopause Hormone Therapy Can Backfire. GLP-1s, Skinny Culture, and the Question Every Woman Should Ask Herself with Natalie Jill

The Other Side of Weight Loss

Play Episode Listen Later Aug 8, 2026 83:45

Something happened after my conversation with Natalie Jill on her podcast, Midlife Conversations. Women flooded my inbox and my community, not with questions about dosing or side effects, but with something closer to relief. They finally felt heard. That reaction told me this conversation needed a second chapter, so I'm bringing you the rest of it here, along with some of the raw, honest messages from women in my community who are done being quietly shamed for a choice that has changed their health and their lives. In this episode, Natalie and I go back to where it all started. We talk about the judgment that follows women who use GLP-1s, the double standard that shows up the second "my body, my choice" gets applied to weight loss medication, and the uncomfortable question underneath all of it: who actually gets to decide what a healthy body should look like. Natalie shares her own story, one you probably haven't heard, about postpartum depression, an autoimmune diagnosis she didn't know she had, and the moment she decided to stop living from problem and start living from possibility. We also get into the part of this conversation that matters most right now. Yes, there is a subset of women glamorizing extreme thinness, and yes, it deserves a real conversation. But we push back on the idea that this means GLP-1s themselves are the problem. What if the real issue isn't the medication at all, but what we've always done as a culture when a tool works too well. This one gets personal fast. We talk about breast implants, boundaries, midlife reinvention, and why some of the most confident women you know still get triggered by a stranger's opinion online. If you've ever felt defensive explaining a health choice you didn't owe anyone an explanation for, this episode is for you. In This Episode, We Cover: The real feedback from women in the Midlife Solutions community after the first GLP-1 conversation aired Why judgment shows up hardest around weight loss medication, and where that double standard comes from Natalie Jill's personal story of postpartum depression, an undiagnosed autoimmune condition, and her early forties rock bottom The mindset shift Natalie calls living in "possibility" versus living in "circumstance" Whether GLP-1s are fueling extreme thinness or simply exposing disordered patterns that already existed Natalie's honest take on who GLP-1s are right for, and the one type of user she has real concerns about The surprising parallel between GLP-1 judgment and Natalie's own experience with breast implant illness How to tell the difference between being triggered and being genuinely misinformed Why midlife might be the first time in a woman's life she stops apologizing for her choices Who This Episode Is For This episode is for the woman using a GLP-1 who is tired of defending herself to people who have never walked in her shoes. It's for the woman who hasn't tried one yet but keeps getting pulled into the noise online and wants a clearer, calmer perspective. And it's for anyone who has ever felt the sting of being judged for a health decision, whether that decision involved a medication, a surgery, or simply choosing what works for her own body.     Sponsors Get 15% off Masszymes Breakthrough By BiOptimzers products at bioptimizers.com/hormone with coupon code HORMONE .
