Today, we talk to Becky White, who is a Research Fellow at the Epidemiology of Cancer Healthcare and Outcomes Research Group at UCL. Paper: Underlying cancer risk among patients with fatigue and other vague symptoms in primary care: a population-based cohort studyAvailable at: https://doi.org/10.3399/BJGP.2022.0371When patients present to GPs with new-onset fatigue and no alarm symptoms for cancer, optimal management is often unclear, as it is not known which of these patients may be at risk of having present but currently undetected cancer. We found that among fatigue presenters without alarm symptoms, the chance of underlying cancer exceeded risk referral thresholds of 3% in older men with fatigue combined with any of another 19 vague symptoms for cancer, and in older women with fatigue-weight loss, fatigue-abdominal pain, or fatigue-abdominal bloating. These findings can support diagnostic management and referral decisions for patients presenting with fatigue in the absence of alarm symptoms for cancer.

In this episode, we talk to Dr Dan Jones, who is a GP and an Academic Clinical Lecturer at the University of Leeds. Paper: Factors influencing symptom appraisal and help-seeking of older adults with possible cancer: a mixed methods systematic review https://doi.org/10.3399/BJGP.2021.0655 (https://doi.org/10.3399/BJGP.2021.0655) The burden of cancer falls predominantly on older (≥65 years) adults, and prompt presentation to primary care with cancer symptoms results in better patient outcomes. The current review, which included 80 studies, found that older adults with cancer symptoms may have prolonged symptom appraisal and shorter help-seeking intervals prior to presenting to general practice. Factors such as knowledge of cancer symptoms, the influence of family and carers, fear, embarrassment, comorbidities, and patient self-management all affected the appraisal or help-seeking interval. Clinicians should be aware of patient difficulty in distinguishing potentially worrying cancer symptoms from symptoms of ageing as a result of frailty or comorbidities.

I have so much good stuff in the pipeline! I am incredibly thankful that YOU are carving out a chunk of your time to spend with me today. Now, I need to warn you this episode is really intense and also one you will not be able to stop listening to. So, be prepared. Today we dive into my friend Michael Hackett's journey with cancer. If you live in Baton Rouge, you have seen Michael's smiling handsome face on billboards all over town in the past few years with the tagline, “We don't treat Cancer, We treat Michael.” Michael is incredible vulnerable about his cancer journey, about what he wished he had done before hand, about the process, and about what he takes away from this difficult journey. For my listeners who have cancer or who have fought it, or have someone close to them who has,  my heart is with you. Michael's candor will draw you into his incredible story and I promise you'll walk away inspired, challenged, and likely need a few tissues as we dive deep into some of life's biggest questions and challenges. When Kristen noticed her right nipple felt different, her first thought was "what the F is this?" Her second thought was that her life was about to change. So she did what everyone does in this situation... she jumped out of the shower, went to Google, and diagnosed herself with a rare form of breast cancer called Paget's Disease. But Google's never right about these things, so a few days later when the radiologist suggested a biopsy for Paget's, shit got real. Overnight, life became a tightly-woven schedule of appointments wedged between work: a biopsy, an MRI, a colonoscopy, and a million different labs and scans leading up to the first chemo treatment. As a single woman with limited resources, Kristen had no choice but to advocate for herself, quickly figuring out the magic words to use with her healthcare providers to move the process along faster and start treatment sooner. Paper: Targeted encouragement of GP consultations for possible cancer symptoms: a randomised controlled trial https://doi.org/10.3399/bjgp20x713489 (https://doi.org/10.3399/bjgp20x713489) Later stage of cancer diagnosis is associated with poorer survival, and may arise from low symptom awareness and delays in presenting to primary care. Population-wide campaigns to increase awareness and encourage help seeking have shown mixed results in terms of stage at diagnosis and numbers of primary care consultations. This randomised controlled trial was targeted at a population whose circumstances suggested that, should they develop cancer, they would be at increased risk of being diagnosed with later-stage disease. This study demonstrates that targeted interventions of this nature, promoting both awareness of cancer symptoms and earlier health seeking, can change consultation behaviour of those who are likely to benefit most from earlier symptomatic presentation. However, in his study the intervention increased the frequency of consultation but not the number of persons consulting.

A new campaign hopes to help younger Kiwi women learn what breast cancer symptoms they need to look out for.The Breast Cancer Foundation's launched its new Change and Check campaign today, which will see thousands of educational stickers put up in clothing stores and gyms.Ambassador and cancer survivor Sarah Gandy told Tim Dower 88 percent of Kiwi women don't know the nine signs and symptoms of breast cancer.She says under 45s don't get free mammograms yet, so self-checking is the last line of defence.She says their study found 88 percent of women don't know there are nine signs of breast cancer."They can include things like armpit lumps, skin colour changes on the breasts, and inverted nipples."LISTEN ABOVE

This video is all about thyroid cancer symptoms and early warning signs you may have cancer. This is a topic worth discussing because many people who believe they have thyroid cancer probably don't. The good news is that even though thyroid cancer is fairly common, it has a great survival rate with a 5-year survival rate of around 98%. So, even if you do have thyroid cancer, it's one of the better types of cancers to have. An interesting fact about thyroid cancer is that it typically does NOT cause any symptoms. In fact, most people who are diagnosed with thyroid cancer present to their doctor with a normal exam and without any symptoms. In some cases, however, if your cancer is sufficiently large then it may cause problems to nearby structures in your neck. Your neck is a tight place which means that if anything grows in that area it may push on or bump into other structures which may result in symptoms. With that in mind, if your cancer is large enough it may present with the following symptoms: - Compression of the esophagus or trachea - Difficulty swallowing - - Swelling or enlargement of the neck (bulge in the neck) - Changes to speech including hoarseness - Enlargement of lymph nodes in the neck - Symptoms of thyroid goiter - Pain in the neck, especially around the thyroid gland - Constant non-productive cough It's important to note that in the majority of cases thyroid cancer will not cause problems with the production of thyroid hormone (but you should still have it tested to be sure). Some other conditions may mimic thyroid cancer and you want to watch out for these as well. They include: - Thyroid Nodules - Thyroid Cysts - Goiter - Hashimoto's thyroiditis These can be differentiated from thyroid cancer with an ultrasound test and certain blood markers. If you suspect that you have thyroid cancer then your next steps should be: - Complete history and physical exam - Thyroid function lab tests (if you have symptoms such as fatigue, etc.) - Thyroid ultrasound - Biopsy (if necessary) - Iodine testing - Other blood tests such as inflammatory markers These steps will help differentiate between thyroid cancer and the other conditions listed above. Symptoms of testicular cancer include a painless lump or swelling in the testicle and acute pain in the scrotum. Other symptoms of testicular cancer can be an ache in the lower back as well as in the groin and a collection of fluid in the scrotum as well. Early diagnosis of testicular cancer is critical, which is why it's important for men to be performing a testicular self exam every month to screen for testicular cancer.
