In this episode I'm going to help you understand your Pap smear and colposcopy results, particularly if you have an abnormality. Abnormal cells on the cervix are referred to as "dysplasia," and that means high-risk HPV (human papilloma) virus is active and now is the time to prevent it from getting worse. I want to help you understand what your results mean and what your options are. I've seen so many different scenarios involving women of all ages and backgrounds from around the world. I've put my attention into helping women prevent cervical cancer, vaginal cancer, and other forms of HPV-related cancer. I'm so delighted to have Meaghan Doyle back on the show today to talk about the truth about abnormal embryos. Meaghan is a genetic counselor, and founder of DNAide, where she specializes in infertility, preimplantation genetic testing, and preconception genetic counseling. In this discussion, Meaghan and I are talking about the different kinds of embryo testing, what genetic test results should be used for, and why all fertility patients need to ask for reports on "abnormal" embryos. The truth is that sometimes, patients are told that their embryos are abnormal but never learn much more about the embryo itself. In some situations, those abnormal embryos can become healthy pregnancies and healthy, live babies, so it's important to learn as much as you can about all of the results of the genetic testing done on your embryos. To set the stage for this conversation, I asked Meaghan what it means to genetically test an embryo, and here's what she explained: "It means that we've taken a sample from the outer layer of the embryo, usually a few cells, and sent that sample to a genetic testing lab. And there's actually a few different types of genetic testing that can be done on that sample. Generally, when we say an embryo has had genetic testing, we're referring to a test called PGT-A. And so when we say an embryo has been genetically tested, usually that's what we're referring to." So what happens when an embryo comes back as "abnormal," and what can you do if it happens to you? Motor neurone disease is a devasting condition that's recently come to prominence in the media with the diagnosis of former English rugby captain Lewis Moody. Another famous person who suffered from the condition was Stephen Hawkins. What is motor neurone disease? It's a progressive nerve disease. It damages and kills nerve cells in the spinal cord and brain that control muscle movement in the body. The damaged nerve cells mean the brain can no longer signal muscles in the body. Muscles slowly lose the ability to function. Why does it occur? The exact cause is not fully understood. It's thought that up to 10% of cases are inherited and related to a patient's genes, while 90% are random with no clear genetic cause. Several things may be important: Abnormal accumulation protein in the nerve cells, destroying them. A possible inflammatory or immune system malfunction. Possible environmental factors like exposure to heavy metals or pesticides, however there's no clear evidence. What are the symptoms? It's difficult to diagnose initially with subtle changes: clumsiness in hands, slurred speech, tiredness. As time goes on, the main issue is weakness in the arms, legs, and hands – the person gradually losing the ability to use them. Speech and swallowing become difficult. Breathing problems as the muscles around lungs weaken. There can be mood swings. Can it be treated? Unfortunately, it is incurable. On average, patients live 2 to 5 years. Stephen Hawkins was an example of someone who actually lived with it for much longer – he was diagnosed at age 22 and lived to 76, which is very unusual. There are some experimental drugs that may slow progression that are being looked at. However, most treatment is supportive – trying to improve the person's quality of life. Body of episode: FATHER'S DAY MEN'S HEALTH So, well before President Biden's prostate cancer diagnosis, I had planned on discussing men's health for this year's Father's Day episode. In particular, I wanted to discuss causes of male childlessness, such as Azoospermia. I've decided to put links in the show notes in regards to Azoospermia. Please check out the links for further information. The basic definition of Azoospermia per The Cleveland Clinic website is: ‘Azoospermia means there's no sperm in your ejaculate. Its causes include a blockage along the reproductive tract, hormonal problems, ejaculation problems or issues with testicular structure or function. Many causes are treatable. For other causes, it may be possible to retrieve live sperm to be used in assisted reproductive techniques like IVF.‘ As I think back through my episodes, I don't believe I've ever discussed men's health, or medical reasons men may be childless. We've discussed fibroids, PCOS, endometriosis, adenomyosis, miscarriage, running out of time, aka the biological clock, MRKH, etc. But I've never gone into much detail regarding issues surrounding the causes of childlessness in men. I have had the great opportunity to interview several men about their experiences with childlessness, including how they have dealt with it. I will put links in the show notes on interviews I have conducted over the years with these wonderful men. Be sure to take a listen! I also believe male childlessness is not as openly discussed as female childlessness because men and their spouses may find the issue embarrassing, shocking to their ego in a different way than a woman may feel about her childlessness, or maybe in much the same way. In either case, there can be emotional and mental pain. Some of the things that can cause infertility in men include: AI Overview Male infertility can be caused by a variety of factors, including problems with sperm production or quality, hormonal imbalances, genetic disorders, infections, and environmental factors. Additionally, lifestyle choices like smoking, excessive alcohol consumption, and certain drug use can also contribute to infertility. Here's a more detailed look at some of the key causes: 1. Sperm Problems: Low sperm count (oligospermia): A low number of sperm in the ejaculate. Absent sperm (azoospermia): No sperm in the ejaculate. Poor sperm motility: Sperm are unable to swim properly. Abnormal sperm morphology: Sperm are abnormally shaped, making it difficult for them to fertilize an egg. 2. Hormonal Imbalances: Reduced testosterone: Low levels of testosterone can affect sperm production. Hyperprolactinemia: Elevated levels of prolactin can interfere with sperm production. Hormonal disorders affecting the pituitary gland or hypothalamus: These glands regulate hormone production. 3. Genetic Disorders: Klinefelter syndrome: A genetic condition affecting males. Cystic fibrosis: A genetic disorder that can cause blocked reproductive ducts. Myotonic dystrophy: A genetic disorder that can affect sperm production. 4. Infections and Inflammation: Epididymitis: Inflammation of the epididymis, the tube that carries sperm from the testicles. Orchitis: Inflammation of the testicles. STIs: Sexually transmitted infections like gonorrhea or chlamydia can cause inflammation and damage to the reproductive organs. 5. Environmental and Lifestyle Factors: Smoking: Smoking can reduce sperm count and quality. Alcohol consumption: Excessive alcohol use can lower testosterone levels and affect sperm production. Certain drug use: Anabolic steroids and other drugs can negatively impact sperm production. Exposure to toxins or chemicals: Exposure to certain toxins, like pesticides, can affect fertility. Obesity: Being overweight or obese can impact sperm quality. 6. Medical Conditions: Diabetes: Diabetes can affect sperm production and quality. Kidney failure: Kidney failure can also impact sperm production. Cystic fibrosis: A genetic disorder that can cause blocked reproductive ducts. Autoimmune diseases: Some autoimmune diseases can attack sperm. 7. Other Factors: Varicocele: Enlarged veins in the scrotum, which can raise testicular temperature and affect sperm production. Testicular trauma: Injury to the testicles can damage sperm production. Previous cancer treatment: Chemotherapy and radiation therapy can affect sperm production. Undescended testicles: Testicles that don't descend into the scrotum. Blockages or absences of tubes: Blockages in the reproductive tract can prevent sperm from being released. I'm delighted to have Dr. Ramy Ghayda join me today to talk about sperm, and the importance of sperm DNA Fragmentation testing and how to improve sperm health. When trying to conceive it takes both an egg and sperm cell. And, as you'll hear Dr. Ghayda explain, we now have access to more tests for male factor infertility, and they are more affordable and accessible than ever. He and I both believe it's very important for people, and fertility patients (everyone, really!) to know their options and to be educated about what they can do to improve sperm quality, to test sperm quality. This can help make the whole journey the best it can be, and be one that hopefully leads to good outcomes. Dr. Ghayda is a reproductive urologist.
