The Truth About Abnormal Embryos (Should You Discard Them?) with guest Meaghan Doyle

I'm so delighted to have Meaghan Doyle back on the show today to talk about the truth about abnormal embryos. Meaghan is a genetic counselor, and founder of DNAide, where she specializes in infertility, preimplantation genetic testing, and preconception genetic counseling. In this discussion, Meaghan and I are talking about the different kinds of embryo testing, what genetic test results should be used for, and why all fertility patients need to ask for reports on "abnormal" embryos. The truth is that sometimes, patients are told that their embryos are abnormal but never learn much more about the embryo itself. In some situations, those abnormal embryos can become healthy pregnancies and healthy, live babies, so it's important to learn as much as you can about all of the results of the genetic testing done on your embryos. To set the stage for this conversation, I asked Meaghan what it means to genetically test an embryo, and here's what she explained: "It means that we've taken a sample from the outer layer of the embryo, usually a few cells, and sent that sample to a genetic testing lab. And there's actually a few different types of genetic testing that can be done on that sample. Generally, when we say an embryo has had genetic testing, we're referring to a test called PGT-A. And so when we say an embryo has been genetically tested, usually that's what we're referring to." So what happens when an embryo comes back as "abnormal," and what can you do if it happens to you? Tune in to hear Meaghan explain. Thank you for joining me, Meaghan! You can find Meaghan's site for DNAide Genetic Counseling here: https://www.dnaide.com/ Do you have questions about IVF?Click here to join Dr. Aimee for The IVF Class. I'm trying something new: differentials. Each episode, I'll pick one ocular sign and one ocular symptom, and walk you through everything that could cause them. This week: the mysterious “abnormal red reflex” and the all-too-common “my eyes are burning.” From cataracts and retinoblastoma to blepharitis and bad makeup glue, I'm breaking down the real possibilities, one red, irritated eyeball at a time. It's part med school, part comedy, and yes, you might just become an honorary ophthalmologist by the end of it. Takeaways: Why an “abnormal red reflex” might mean anything from cataract to retinoblastoma. The simplest test pediatricians use to spot dangerous eye conditions early. How blepharitis and clogged oil glands cause that all-too-familiar burning sensation. The surprising eye problems linked to makeup, contact lenses, and preservatives. Why ophthalmologists love differentials and how this new series might just teach you to think like one. ‘Alcohol use disorder is very, very prevalent and the harm occurs long before people have physical dependence,' Dr Steve Brinksman says in this episode of the MIMS Learning Clinical Update podcast. He adds that if patients are concerned that they are drinking too much, they may hesitate to raise the issue with their GP because they might fear being told to stop drinking completely, and GPs may feel ‘uncomfortable' about asking people how much they drink because alcohol misuse is ‘a common problem' within the profession. Dr Brinksman, who worked as a GP for 30 years, says that GPs should encourage colleagues to seek help if they feel they have problems, and can encourage many patients to cut down rather than cut out alcohol completely. ‘If you are drinking 50 or 60 units a week, then you reduce your harm by cutting down even to 40 units a week,' he explains. The episode also reviews how to assess alcohol misuse, how to advise patients to cut down, and how to support patients with severe dependence.  You can access the website version of this podcast, along with a list of key learning points, on MIMS Learning — and make notes for your appraisal. Motor neurone disease is a devasting condition that's recently come to prominence in the media with the diagnosis of former English rugby captain Lewis Moody. Another famous person who suffered from the condition was Stephen Hawkins. What is motor neurone disease? It's a progressive nerve disease. It damages and kills nerve cells in the spinal cord and brain that control muscle movement in the body. The damaged nerve cells mean the brain can no longer signal muscles in the body. Muscles slowly lose the ability to function. Why does it occur? The exact cause is not fully understood. It's thought that up to 10% of cases are inherited and related to a patient's genes, while 90% are random with no clear genetic cause. Several things may be important: Abnormal accumulation protein in the nerve cells, destroying them. A possible inflammatory or immune system malfunction. Possible environmental factors like exposure to heavy metals or pesticides, however there's no clear evidence. What are the symptoms? It's difficult to diagnose initially with subtle changes: clumsiness in hands, slurred speech, tiredness. As time goes on, the main issue is weakness in the arms, legs, and hands – the person gradually losing the ability to use them. Speech and swallowing become difficult. Breathing problems as the muscles around lungs weaken. There can be mood swings. Can it be treated? Unfortunately, it is incurable. On average, patients live 2 to 5 years. Stephen Hawkins was an example of someone who actually lived with it for much longer – he was diagnosed at age 22 and lived to 76, which is very unusual. There are some experimental drugs that may slow progression that are being looked at. However, most treatment is supportive – trying to improve the person's quality of life. His book, "The Gift of Injury: The Strength Athlete's Guide To Recovering From Back Injury To Winning Again" co-written with Dr. Stuart McGill is highly praised by athletes and rehab professionals alike.Since entering his first powerlifting meet in 1999, Brian Carroll has since risen to the absolute pinnacle of the sport. Brian totaled 2730 at 275 and 2651 at 242 with more than ten times his body weight in three different classes (220, 242, 275), and both bench pressed and deadlifted over 800 pounds in two other weight classes. He's totaled 2600 over 20 times in 2 different weight classes in his career. He has squatted over 1000lbs more than 60 times and was the first man to squat of 1300lbs.After ten years of high-level powerlifting competition and an all-time World Record squat at 220 with 1030, in 2009, Brian was competing for a Police academy scholarship. On a hot and humid July morning, Brian, hurdling over a barricade at 275lbs, landed on, fell, and hurt his back. The resulting back pain has been a huge part of his professional journey, leading him to seek out Stu McGill in 2013 who he latered co-authored The Gift of Injury with in 2017. It's worth nothing that his 1306lb squat came AFTER all of the back back and working with Dr. McGill. He owns his own coaching and consulting business Power Rack Strength where he works with a wide range of athletes and frequently delivers professional education for rehab professionals.Abnormal magnetic-resonance scans of the lumbar spine in asymptomatic subjects. This week on Fat Science, Dr. Emily Cooper, Andrea Taylor, and Mark Wright explore the dangers of under-fueling and challenge dieting myths with guest Gina Davito. This episode dives into Gina's journey from extreme restriction and over-exercising to finally achieving metabolic balance and freedom from food obsession. Dr. Cooper sheds light on the science behind metabolic adaptation, appetite regulation, and why fueling your body—rather than restricting—is essential for long-term health. Listeners will come away with a new perspective on weight management, the power of metabolic healing, and a celebration of food as a source of nourishment and joy.Key Takeaways:Chronic dieting and excessive exercise can trigger severe metabolic adaptation and worsen weight gain, creating a cycle that is hard to break without proper medical support.Abnormal hunger and weight gain are symptoms of underlying metabolic dysfunction, not lack of willpower—often exacerbated by early and repeated dieting.True progress involves nourishing the body and trusting medical guidance, sometimes including tailored medications to reset metabolic balance.Mental freedom from food-related anxiety arrives with metabolic healing—allowing for joy in eating and exercise for health rather than punishment.Early interventions for kids showing unusual hunger or weight patterns should focus on metabolic evaluation, not restriction.Personal Stories & Practical Advice:Gina describes her transformation from compulsive restriction to vibrant health and athleticism—no longer trapped by thoughts of food or weight.Andrea and Mark share their relief at experiencing food freedom and highlight the importance of enjoying both food and movement.Resources from the episode:Fat Science is a podcast on a mission to explain where our fat really comes from and why it won't go and stay away. Planning to buy a condo in 2025? Don't just compare PSF and assume it's a good deal. In this episode of NOTG, Melvin Lim from PropertyLimBrothers defines 3 condo-buying frameworks that help condo-hunters make sense of why some new launch and resale prices.   Understand why some resale units are now asking north of $2,200 PSF—the same range as new launches—and what that really means when lease balance, renovation costs, and exit potential is factored in. With more OCR and RCR projects hitting the market, including first-mover launches like Springleaf Residence, it's becoming trickier to tell what's actually worth buying.   Sitting on the fence between resale and new launch? The difference lies in how each holds value over time—not what it costs today. Before making an offer, running it through these frameworks could reveal risks or upside that aren't immediately obvious—and potentially prevent costly mistakes.   00:00 Introduction 02:40 5 year cycle 05:30 TOP Effect 07:40 No COV 08:44 3rd Exit Audience 09:31 Popular with tenants 09:51 New Design & Low renovation cost 10:21 Progressive payment 11:11 SSD / Black out period 14:00 Brand new lease 16:00 1 year delayed installment 16:18 Defect Warranty 16:43 1st Mover Advantage 18:49 Caveat Emptor - What you see is what you get 19:10 Summary 19:36 2 Types of Disparity Effect Strategy 25:40 2025 | Abnormal inflation to future price expectations by resale sellers 27:00 2028 | Real estate market prediction 27:25 Closing words 28:00 Outtakes

Body of episode: FATHER'S DAY  MEN'S HEALTH  So, well before President Biden's prostate cancer diagnosis, I had planned on discussing men's health for this year's Father's Day episode. In particular, I wanted to discuss causes of male childlessness, such as Azoospermia.  I've decided to put links in the show notes in regards to Azoospermia. Please check out the links for further information.  The basic definition of Azoospermia per The Cleveland Clinic website is:   ‘Azoospermia means there's no sperm in your ejaculate. Its causes include a blockage along the reproductive tract, hormonal problems, ejaculation problems or issues with testicular structure or function. Many causes are treatable. For other causes, it may be possible to retrieve live sperm to be used in assisted reproductive techniques like IVF.‘    As I think back through my episodes, I don't believe I've ever discussed men's health, or medical reasons men may be childless.  We've discussed fibroids, PCOS, endometriosis, adenomyosis, miscarriage, running out of time, aka the biological clock, MRKH, etc.  But I've never gone into much detail regarding issues surrounding the causes of childlessness in men.    I have had the great opportunity to interview several men about their experiences with childlessness, including how they have dealt with it. I will put links in the show notes on interviews I have conducted over the years with these wonderful men. Be sure to take a listen!    I also believe male childlessness is not as openly discussed as female childlessness because men and their spouses may find the issue embarrassing, shocking to their ego in a different way than a woman may feel about her childlessness, or maybe in much the same way.  In either case, there can be emotional and mental pain.    Some of the things that can cause infertility in men include:   AI Overview Male infertility can be caused by a variety of factors, including problems with sperm production or quality, hormonal imbalances, genetic disorders, infections, and environmental factors. Additionally, lifestyle choices like smoking, excessive alcohol consumption, and certain drug use can also contribute to infertility.  Here's a more detailed look at some of the key causes: 1. Sperm Problems: Low sperm count (oligospermia): A low number of sperm in the ejaculate. Absent sperm (azoospermia): No sperm in the ejaculate. Poor sperm motility: Sperm are unable to swim properly. Abnormal sperm morphology: Sperm are abnormally shaped, making it difficult for them to fertilize an egg.  2. Hormonal Imbalances:  Reduced testosterone: Low levels of testosterone can affect sperm production.  Hyperprolactinemia: Elevated levels of prolactin can interfere with sperm production.  Hormonal disorders affecting the pituitary gland or hypothalamus: These glands regulate hormone production.  3. Genetic Disorders: Klinefelter syndrome: A genetic condition affecting males. Cystic fibrosis: A genetic disorder that can cause blocked reproductive ducts. Myotonic dystrophy: A genetic disorder that can affect sperm production.  4. Infections and Inflammation: Epididymitis: Inflammation of the epididymis, the tube that carries sperm from the testicles.  Orchitis: Inflammation of the testicles.  STIs: Sexually transmitted infections like gonorrhea or chlamydia can cause inflammation and damage to the reproductive organs.  5. Environmental and Lifestyle Factors: Smoking: Smoking can reduce sperm count and quality.  Alcohol consumption: Excessive alcohol use can lower testosterone levels and affect sperm production.  Certain drug use: Anabolic steroids and other drugs can negatively impact sperm production.  Exposure to toxins or chemicals: Exposure to certain toxins, like pesticides, can affect fertility.  Obesity: Being overweight or obese can impact sperm quality.  6. Medical Conditions: Diabetes: Diabetes can affect sperm production and quality. Kidney failure: Kidney failure can also impact sperm production. Cystic fibrosis: A genetic disorder that can cause blocked reproductive ducts. Autoimmune diseases: Some autoimmune diseases can attack sperm.  7. Other Factors: Varicocele: Enlarged veins in the scrotum, which can raise testicular temperature and affect sperm production.  Testicular trauma: Injury to the testicles can damage sperm production.  Previous cancer treatment: Chemotherapy and radiation therapy can affect sperm production.  Undescended testicles: Testicles that don't descend into the scrotum.  Blockages or absences of tubes: Blockages in the reproductive tract can prevent sperm from being released.  This is for informational purposes only. For medical advice or diagnosis, consult a professional. AI responses may include mistakes. Learn more Whether you have children or not, your health is important. Be sure to go to all of your check-ups, especially if there is a family history of health issues. Get checked, don't wait.   Be OK with advocating for yourself. The healthcare industry may say no, you are too young for a particular test. But if you want that test, be insistent. In today's episode of Tech Talks Daily, I reconnect with Abnormal AI's CIO, Mike Britton, to explore one of the most pressing topics in the tech world—AI regulation and cybersecurity.  Speaking from his base near Dallas, Mike brings a pragmatic perspective shaped by decades of experience at the intersection of enterprise technology and security. As the debate around artificial intelligence evolves, we examine the growing divide between the United States and Europe on regulatory approaches and what business leaders can learn from each side. While Europe takes a more cohesive, application-based approach, the US remains fragmented, relying on state-by-state policies and sector-specific laws. Mike unpacks why this patchwork complicates global alignment and what an effective risk-based, standardized framework might look like. He argues that regulation must focus on how AI is applied, not just its scale, especially as the technology becomes embedded in everything from healthcare to email automation. We also touch on the unintended consequences of overregulation, including the risk of pushing innovation into regions with fewer safeguards. As Abnormal Security works with some of the world's largest brands, Mike offers a frontline view into how threat actors are already leveraging AI to outpace traditional defenses. His insights reinforce the need for transparency, human oversight, and "kill switches" to ensure AI remains a tool for good, not a liability. From real-world examples to strategic recommendations, Mike outlines what CIOs and CISOs need to know now. His advice is clear, grounded, and actionable, whether embracing regulatory sandboxes, staying alert to geopolitical nuances in AI models, or maintaining continuous learning in a fast-moving space. So, how do we keep innovation ethical and secure in a world where AI is moving faster than ever? And what steps should technology leaders take to avoid falling behind or losing control of the tools meant to drive progress?

I'm delighted to have Dr. Ramy Ghayda join me today to talk about sperm, and the importance of sperm DNA Fragmentation testing and how to improve sperm health. When trying to conceive it takes both an egg and sperm cell. And, as you'll hear Dr. Ghayda explain, we now have access to more tests for male factor infertility, and they are more affordable and accessible than ever. He and I both believe it's very important for people, and fertility patients (everyone, really!) to know their options and to be educated about what they can do to improve sperm quality, to test sperm quality. This can help make the whole journey the best it can be, and be one that hopefully leads to good outcomes.  Dr. Ghayda is a reproductive urologist. He is currently an assistant professor at University Hospitals in Cleveland, Ohio and the Chief Medical Officer at Legacy, a company that is revolutionizing how men and couples test and freeze sperm, from home. Thank you for joining me, Dr. Ramy!  Read the full transcript and show notes at Dr. Aimee's website. Join Howard and Janeen Arbuckle for this discussion of pediatric and adolescent gynecology essentials. Pediatric and adolescent gynecology is a newer discipline bringing specialized care to young women with unique gynecologic needs, with a focus on counseling, education, and age-appropriate interventions. • Abnormal uterine bleeding in adolescents is rarely caused by structural problems (unlike in adults) and typically relates to immaturity of the hypothalamic-pituitary-ovarian axis• Hematologic workup should be considered for adolescents with heavy menstrual bleeding as this may be the first time their clotting system is challenged• Hormonal therapies are safe to use once menarche has occurred, with no impact on bone growth• Long-acting reversible contraceptives offer superior pregnancy prevention (1 in 10,000 for implants vs 8 in 100 for typical pill use) but require thoughtful counseling• Private interviews with adolescent patients create trust while preparing them for independent healthcare navigation• Tranexamic acid is effective for heavy menstrual bleeding in adolescents but pill size and frequency can limit compliance• Most ovarian cysts in adolescents represent normal physiologic function and rarely require intervention• Preservation of reproductive organs should be prioritized in adolescent surgery, including leaving ovaries after torsion when possible• Vaginal bleeding in pre-pubertal girls requires assessment for secondary sexual characteristics to distinguish precocious puberty from other causes00:00:00 Introduction to Pediatric Gynecology00:07:20 Abnormal Uterine Bleeding in Adolescents00:19:36 Contraception Choices for Young Patients00:29:40 Managing Difficult Patient-Parent Conversations00:38:04 Pelvic Pain and Endometriosis00:46:58 Adnexal Pathology and Ovarian Issues00:50:51 Congenital Anomalies and Vaginal BleedingFollow us on Instagram @thinkingaboutobgyn.
