Eyes on the Future: Hunter Cherwek's Mission to End Preventable Blindness

Every day, in many communities without enough resources, millions suffer from blindness that could be prevented. A lack of proper eye care, old training methods, and few modern tools leave many in the dark. In places where a simple cataract surgery could bring back both sight and hope, big challenges and a shortage of new ideas block the way. Inspired by these problems, an amazing journey spans continents and changes lives. Born in a small town in Virginia, Dr. Hunter Cherwek—Vice President, Clinical Services and Technologies at Orbis International—took a path from the halls of Duke and Emory Universities to remote parts of the world. What began as a simple interest in medicine grew into a strong promise to end preventable blindness. A life-changing experience in Kenya set him on a course that reshaped his mission in eye care and pushed him to find better solutions for communities in need. Dr. Cherwek is a well-known eye doctor and he is also a visionary who mixes the latest technology with a deep passion for teaching. His work at Orbis International, especially on the famous Flying Eye Hospital, has helped perform surgeries that restore sight in over 20 countries. He also created a prize-winning online learning and remote care program that supports health workers around the world. His six years as Medical Director of Strategic Markets in Beijing show his steady drive to change old ways and bring better eye care all over the world. In this episode, Dr. Cherwek explains how a lucky scholarship and early global experiences shaped his promise to end preventable blindness, the challenges of providing modern eye care in low-resource areas, and how using tools like remote care and training simulations can change how we teach and treat patients. Here is our conversation.

Orbis International has been a pioneer in preventing and treating avoidable blindness for forty years. While much of that work takes plane on its Flying Eye Hospital, the organization also uses advanced technology to bring quality care to individuals worldwide.Cybersight is the global nonprofit's telemedicine platform. Through it, the organization offers 24/7 live peer-to-peer consults for specialists worldwide. Over 24,000 eye care practitioners have sought expert opinions through the platform, gaining access to mentorship and advice they wouldn't have otherwise. Users can also access live surgical demonstrations, courses, and other training: its library is free for all professionals.Orbis also leverages artificial intelligence to help improve access to eye care. Cybersight consult supports AI-grading of color fundus images attached to consult cases. In seconds, a doctor in a remote region can get an interpretation of an image to aid in diagnosis of diabetic retinopathy, hemorrhages, glaucoma, and other conditions. These are just two of the many ways Orbis International continues its fight against avoidable blindness. As Hunter Cherwek, MD, vice president of clinical services for Orbis, makes clear: 75% of visual impairment is preventable and 90% of those preventable conditions occur in low- and middle-income countries.Hosted by Rob Rothman, MD, this episode covers these and other topics:How Cybersight democratizes technology-driven eye care.Cybersight in practice. Full-stack machine learning engineer Gabriella Lanouette describes how it's used in the real world and how it lessens the burden for clinicians. How Orbis International is developing more globally representative datasets to eliminate geographical and other biases in research.Managing a non-governmental organization. Venture partner, consultant, and Orbis UK board of trustees member Sophia Pathai, MD, Ph.D., MPH, discusses how the board balances fiscal and fiduciary responsibilities with the organization's mission. How Orbis fundraises at the corporate level, worldwide. How you can get involved. Episode 80 Oshkosh 2021 was an adventure! More than 600,000 people and 10,000 planes were onsite. Greg and John took it all in and share the highlights. The Orbis Flying Eye Hospital was a special exhibit. John had worked on the plane when it was in service for an airline. The Flying Eye Hospital is a state-of-the-art teaching facility complete with operating room, classroom and recovery room. Part of the innovation is a modular interior. Several unique restored planes were on display. Greg talks about the plane from the 1950s Sky King television show. He also shares the fascinating history of a restored Aero Commander 500 that served as Air Force One to transport President Eisenhower to his Pennsylvania farm. The event was well attended by Flight Safety Detectives listeners. Greg and John enjoyed meeting folks. Comments from those conversations and emails will be used to shape future shows. Expect to hear more about the nuts and bolts of accident investigation, dissections of lesser-known investigations and more! John and Greg also examine 10 flight instruction related accidents that happened in a recent two-week period. They are looking for trends. Observations include lack of operational discipline, rusty skills following 2020 shutdowns, and over-reliance on technology.

Did you know that 80% of the world’s blindness can be cured, and 90% of those cases occur in low to middle income countries? The Orbis Flying Eye Hospital works to decrease preventable and curable incidents of blindness with its mobile operating room and teaching facility. The Orbis Flying Eye Hospital started in 1982 with a DC-8 plane that flew to locations around the globe that are in desperate need of basic eye treatment for preventable and curable conditions. Now, Orbis boasts an MD-10 outfitted with an operation room that can be set up and sterilized in 6-7 hours, and that can serve as a teaching hospital for doctors to learn procedures that they can implement in their own communities. This week we interview Dr. Hunter Cherwek, who joined Orbis as soon as he completed medical school and hasn’t looked back since.
