Part 2 Dr. Flemming described the functional results obtained from their intervention. They specifically chose the activity measure proposed for post-acute care, abbreviated as AM-PAC. It showed improvement over time and it allowed looking at specific subsections within rehabilitation, such as basic mobility, daily activity, and cognitive scores. An AM-PAC score is a recognized tool by Medicare used in multiple post-acute care settings. Dr. Cuccurullo indicated that according to an article published in 2011 in the journal Stroke, the all-cause mortality for stroke patients in the U.S. is 31%. In one year, the all-cause mortality in their program was 1.47%. Their results were published in the American Journal of Physical Medicine and Rehabilitation. The paper received the Excellence in Research Writing Award for the most impactful article of the year. Significantly, the manuscript attracted the attention of Medicare officials who asked them to present their findings. CMS wanted to know if it can be proved that the stroke recovery program saves money, can they validate their previous improvement in mortality with more patients, and can they replicate the results at other institutions nationally. Drs. Fleming and Cuccurullo described what they did in response to these questions. A Question & Answer period followed.

Part 1 Dr. Cuccurullo began by discussing their stroke recovery program, specifically investigating and analyzing the effects of a comprehensive initiative on all-cause mortality, function, and readmissions. She listed the learning objectives for today's grand round presentation. Strokes are the number one admission in their inpatient rehab facility. Twenty-two percent of their patients comply with going to the facility. Their patients have a finite resource for Medicare once they leave inpatient or the acute care setting. Payment caps compromise the ability to have outpatient therapies that prevent them from having a full recovery. Dr. Flemming pointed out that there is an overlap with patients who have neurologic disease after stroke as well as patients who have cardiac disease. So, they designed their program to combine both elements of neurorehabilitation and a modified cardiac rehabilitation program, which starts with an outpatient visit with a stroke physiatrist. Common challenges that need to be addressed are patients with: weakness on their one side versus the other, cognitive or attention deficits, poor safety awareness, and post-stroke fatigue. They decided that it would be important to collect medical and functional outcome data to see if the program could affect hospital readmissions, the recurrence of stroke, and overall mortality.

Pete Johnson, Pharm.D. is Professor of Pharmacy Practice and President's Associate Presidential Professor at the University of Oklahoma College of Pharmacy and Adjunct Professor at the University of Oklahoma College of Medicine in Oklahoma City, Oklahoma. Over the last 17 years, Dr. Johnson has practiced in both the Pediatric Intensive Care Unit (PICU) and Cardiac Intensive Care Unit (CICU) at Oklahoma Children's Hospital at OU Health. Tria Kinnard does not just survive; she thrives. While surviving is merely getting by, thriving is living and thinking abundantly. Tria is an Advanced Practice Provider practicing in the Neonatal Intensive Care Unit at the University of Kentucky. She received her Bachelor of Arts in Biology at Berea College, a Master of Arts in Behavioral Ecology and a Master of Science in Physician Assistant Studies at the University of Kentucky. She completed a one-year post-graduate residency in neonatology at the University of Kentucky. She has been practicing as PA, formerly termed Physician Assistant now Physician Associate (PA) for over 15 years. She provides and manages the complex care of infants and participates in training and education of resident physicians, fellow physicians, physician assistants, and nurse practitioners in a 90-bed Level IV Neonatal Intensive Care Unit. She has authored peer-reviewed publications, serves on a multitude of UK Enterprise and NICU professional practice committees, and received the Program Directors' Award For Excellence in Curriculum Development in 2016 in acknowledgement of enhancements to pediatric residency education. During her undergraduate program, she received the Hilda W. Wood Award which was awarded to a student who greatly contributed to the life and work of the college. Her contributions throughout her professional career continue to advance our professional practice as APPs. She is a Lead Inpatient APP, Adjunct Faculty in the College of Health Sciences for PA Studies, and has served as a Residency Program Director for over 10 years. Within the last year, Tria began serving on two impactful Academic Committees at the University of Kentucky: the Associate Dean for Wellness and Well-being Search and the Physician and APP Experience Advisors Committees. Be on the lookout for her continued impact as she continues to direct the Neonatalology Physician Assistant Residency Program where she began her career at the University of Kentucky. Con's of having midyear virtually Limitations regarding one-on-one interactions due to the system limitations The inconvenience of programs using different platforms (TIP – consider familiarizing yourself with different platforms ahead of time to be sure you're comfortable using them) Advice for virtual midyear from an RPD Be yourself! Let the program get to know you. Have meaningful questions Be prepared Be professional and polished Have appropriate attire (even on zoom) Advice for preparation of virtual midyear The earlier you prepare, the better.
