So what glorious ideas are both Republicans and Democrats agreeing upon to fix this problem? Dismantle their authority? Of course not! We're going to elect them! People don't know who their county commissioner or park district representative is, and yet they will make an educated decision regarding their Met Council rep? Both parties in our legislature are making excuses as to why they can't strip the Met Council of its power over our counties and cities. There is little political will to make significant reforms over just changing the window dressings. The legislature is rushing this through, wanting some elections as early as 2024. Who will be ready to run? Only the people they are already grooming. We also discuss a recent meeting one of us attended in a room full of lefties regarding John Marty's bill for single payer healthcare in MN. It's interesting how they tried to control the crowd and guide their thought process to accept a single-payer solution. There's just a little bit of pressure. Tune in to see how the other half lives! The Legislature has spent much of this week passing elements of a new two-year state budget.  They're working off a base of the current $52 billion budget and looking to add the $17.5 billion surplus to it. The House and Senate plans do not line up perfectly, and conference committees will have to work out the differences.  MPR News guest host Brian Bakst talks about the state budget under construction at the Capitol with two leading DFL lawmakers who have their eyes on the big picture and a Republican who says the pace of spending is unsustainable.   Later in the hour, a recap of the governor's State of the State speech and look at the busy week ahead.   Guests:   Sen. Today we're tackling Medicare Advantage, which is the option Medicare enrollees have to use a private insurance company to administer their Medicare benefits instead of the traditional public Medicare program. Almost HALF of all Medicare beneficiaries are now enrolled in Advantage plans, which represents a historic level of privatization of the almost 60-year program. Just this weekend, the New York Times published a blockbuster front-page report on everything that is wrong about Advantage plans. We'll get into all of that with our guest, Dr. Susan Rogers. Dr. Rogers spent most of her career at Stroger Hospital of Cook County (fka Cook County Hospital, the basis for blockbuster TV drama "ER") where she was a Primary Care Physician in a neighborhood clinic before becoming a hospitalist and Director of Medical Student Programs for the Department of Medicine. She is a past co-president of Health Care for All Illinois. She retired in 2014, and is now president of Physicians for a National Health Program (PNHP), a national organization of over 25,000 physicians and health professionals whose mission is to advocate for Single Payer Healthcare/Medicare for All.  https://youtu.be/oi1BUAhbx3U Show Notes Dr. Rogers tells us her advocacy for Medicare for All grew from her experience training and working at a large public safety net hospital where providers and patients made decisions about care based on need, not ability to pay. It was the best way to learn to provide care, and the best way for patients to receive care. What's the difference between Medicare and Medicare Advantage? We dig into Medicare Advantage (aka Medicare Part C) plans, and how they differ from the traditional public Medicare program. Traditional Medicare is funded by payroll taxes. Hospital coverage (Part A) is free for eligible people. There are no networks. It's a fee-for-service plan, so providers are paid for each service they provide that's medically necessary. The narrative began in the 1980's that fee-for-service was responsible for "overuse" of healthcare services. (To paraphrase Minnesota single payer hero Senator John Marty: as if people go get an extra colonoscopy just because it's paid for.) The solution was to put private insurance between the doctor and patient to prevent overuse. Medicare Advantage evolved from the introduction of private insurance into the Medicare system, resulting in every insurance company in America skimming massive profits off the top of a taxpayer funded federal program, while providing no actual care. Medicare Advantage plans are required to cover all medically necessary care, but the definition of medically necessary is defined by the insurance company based on cost, not by the physician based on medical expertise. Medicare Advantage replaces the doctor/patient relationship with someone in an insurance company office - potentially with no healthcare training - deciding what's medically necessary. Gillian shares some stories from Healthcare-NOW members who have been enrolled in Medicare Advantage plans. Common themes were delays in care, denial of coverage, limited networks, and limited pharmaceutical formularies. These features (not bugs) of Medicare Advantage can lead to serious, even deadly deterioration of a patient's health. We also heard stories of patients in need of specialty care for conditions like cancer, but few of the large academic centers or cancer institutes accept Medicare Advantage plans. The overhead cost to run traditional Medicare is about 2%. That means approximately 98% of the money in the traditional Medicare pot goes to providing care to enrollees. By law, Medicare Advantage plans only have to spend 85% of their pot on patient care, and they can keep the other 15% (this is how they afford huge executive salaries, among other "overhead.") They make that 15% slice of the pie more profitable by delaying and denying care as well as by fraudulently overc...

In a series of brief and personal essays, Why We Revolt describes what is wrong with industrial healthcare, how it has corrupted its mission, and how it has stopped caring. Montori rescues the language of patient care to propose a revolution of compassion and solidarity, of unhurried conversations, and of careful and kind care. Our conversation will be moderated by book industry professional and historian David Unowsky. The panel will be joined by Sen. John Marty, Nurses Association director Rose Roach and SEIU Healthcare Executive Vice President Jigme Ugen. John Marty has been a state senator since 1987. He is a strong advocate for government ethics, environmental protection, and universal health care. John is the author of the proposed Minnesota Health Plan, which would replace the health insurance system with health care for all, to keep people healthy and enable them to get the care they need when they need it. John is a graduate of St. Olaf College with a B.A. in Ethics. John and his wife Connie live in Roseville, MN. Rose Roach is the Executive Director of the Minnesota Nurses Association. She has thirty plus years of experience in the labor movement in Minnesota and California. A Minnesota native, Roach spent eleven years, from 2003-2014, working for the California School Employees Association and was an active leader within the CA health care policy reform movement. Roach has been recognized as one of Minnesota's 100 influential leaders in health care. She is a board member for the healthcare justice organization Health Care for All-Minnesota. Jigme Ugen was born in Kalimpong, India and graduated from Delhi University with diplomas in Political Science, English and Economics. He is the first Tibetan refugee to be elected as a labor union leader. He worked in several non-profit organizations across the world before immigrating to the US in 2001. In 2007, he was elected Executive Vice President of SEIU Healthcare Minnesota. Jigme is deeply engaged with the Tibetan community and Tibet's independence movement. Victor Montori works at Mayo Clinic in Minnesota (U.S.) as a diabetes doctor. He graduated medical school in his hometown of Lima, Perú, and completed postgraduate training at Mayo Clinic in the U.S. and at McMaster University in Canada. Considered “a patient's doctor,”, Montori received the Karis Award, a patient-nominated recognition for his compassionate care. A researcher in the science of patient-centered care, Montori and his colleagues have authored over 650 research articles. A full professor of medicine by age 39, Victor is today one of the most cited clinical researchers in the world. In 2016, Victor founded The Patient Revolution. John is a Minnesota state senator and a strong advocate for government ethics, social and economic justice, and environmental stewardship of the planet. John works for public policies that serve the entire community, especially the homeless, those without healthcare and other vulnerable people. His focus is on prevention, addressing challenges up front, to build a better future and prevent problems before they start. Senator Marty co-chaired a Legislative Commission on Ending Poverty. He is the author of Healing Health Care, a book spelling out a path to replace our dysfunctional health insurance system with health care for all, saving money while improving health. John and his wife Connie have a long-term connection to Holden. He is a graduate of St. Olaf College with a B.A. in Ethics, and they live in Roseville.
