This is the story of how I knew I needed to start an anti-anxiety and anti-depressant medication. I was recently diagnosed with ADHD, Anxiety and Depression. I also had to switch Primary Care Physicians because my old PCP was not listening to me and seemed very resistant to prescribing any medication for mental health. Greif, postpartum and the chaos of raising two toddlers sent me spiraling with both anxiety and depression. I got to a point where managing my mental health felt like a full time job, I did not have the time or the energy to manage my depression. My anxiety was making my ADHD worse, I couldn't get anything done and I knew in my gut that it was time to talk with my doctor about getting on something for my depression. I was breastfeeding at the time, and I had read that anti-anxiety medication can help with the symptoms of ADHD. I also knew that often-times one medication can reduce symptoms of anxiety and depression. After talking with my doctor, she recommended Zoloft or Sertraline. I had very few side effects, and after about a week or 10 days, I noticed a difference. After two or three weeks I felt like my old self again. I was laughing and could enjoy my children and my life as it was, instead of drowning everything in toxic positivity and hoping for the best. This week on the Codcast, John McDonough of the Harvard T.H. Chan School of Public Health and Paul Hattis of the Lown Institute talk to Michael Barnett, who is both a primary care physician at Brigham and Women's Hospital and a professor of health policy at the T.H. Chan School, about the ongoing effort to unionize PCPs across the Mass General Brigham system.
“Primary care physicians are your first line of defense against cancer,” said Matthew Farrell, M.D., an Ohio State Wexner Medical Center primary care physician and a clinical associate professor of family and community medicine. Prevention, screenings and early detection are crucial and begins with a patient's family history with cancer and other medical issues. “We are thorough and ask about their first-degree relatives, their parents, siblings and children, and their secondary relatives, such as grandparents and aunts and uncles,” Farrell said. “If I see a pattern, a certain number of relatives with the same cancer, especially at an early age, there could be a genetic factor.” A patient with an inherited genetic mutation will then be screened at an earlier age and more often. Dr. Farrell also talked about the importance of children receiving the Human Papillomavirus (HPV) vaccine that prevents certain types of cancer. He examines patients to detect skin cancer and melanoma in the early stages. “Skin cancer is the most common form of cancer and it's personal for me; my mother was diagnosed with melanoma three times … and she's doing fine.” Breast cancer and prostate cancer are two of the most common forms of cancer. Dr. Farrell discussed when women should begin getting yearly mammograms, and why and when men should begin to undergo regular Prostate-Specific Antigen (PSA) tests. Men and women without a family history of colorectal cancer should get their first colonoscopy at 45. “My brother-in-law was 52 when he had his first colonoscopy,” Dr. Farrell said. “They found a large tumor in his colon; it was removed, and he's been cancer free for 20 years now.” Lung cancer screenings are relatively new and recommended for long-time smokers. They are vital because “if you wait until there are symptoms it's very hard to treat and cure,” Dr. Farrell said, adding “I'm fortunate to be part of the Ohio State system and the James where we have experts and specialists for every type of cancer and medical issue my patients have.”

Madison Cotton went to her Primary Care Physician because of troubling cysts in her breast in 2017 and the fact that her mother had already been through breast cancer. Madison was basically dismissed by her doctor and eventually in 2022 was diagnosed with Breast cancer. Since that time she has struggled both physically and emotionally with a variety of difficult issues that she has gone through as an AYA cancer patient, and has only begun to feel better and have a more meaningful existence during this summer season.
