(2020). Racial and ethnic disparities in the delayed diagnosis of appendicitis among children. Academic Emergency Medicine, 28(9), 949-956. https://doi.org/10.1111/acem.14142.Presenter: Yelise Foon.Summary: Delay in the diagnosis of appendicitis is associated with perforation and significant subsequent morbidity.  Currently the pre-operative perforation rate for appendicitis is 30%.  Some known risk factors for a delayed diagnosis for appendicitis include a young age, female gender, African-American race, non-English speaking backgrounds and government insurance factors (predominantly in the US).  This was a multi-centre retrospective cohort study conducted over 3 years utilizing the PECARN registry, which aimed to compare the rate of appendiceal perforation, delayed diagnosis of appendicitis and diagnostic imaging use among different ethnicities. Of the 7298 children diagnosed with appendicitis where race/ethnicity data was collected, 34.9% of non-Hispanic white children had a perforated appendix, compared with 36.5% of non-Hispanic black children.  Non-Hispanic black children also had higher rates of delayed diagnosis (defined as having a relevant emergency visit in the week prior to diagnosis): at 4.7% compared with 2.0% for non-Hispanic white children.  Further non-Hispanic black children with a delayed diagnosis of appendicitis were less likely to undergo definite imaging (including MRI, ultrasound, or CT) compared with their non-Hispanic white counterparts (with 28.2% versus 46.2% undergoing imaging).  However, given that this study came out of the US, the generalisability of this study to Australian emergency departments is unclear.  Health literacy is likely to be a large contributor to delayed presentation of appendicitis and something to be carefully considered when treating any child (and their family).  There are tools such as the Paediatric Appendicitis Score (PAS) and the Alvarado Score which may assist in objectively quantifying the possibility of appendicitis (particularly for junior staff).   Take-Home Points:It is important for us to consider how we can minimise racial and ethnic disparities and make sure we are conscious of them. Credits:This episode is produced with help of HETI's Emergency Medicine Training Network 5.Music/Sound Effects Feel Me by LiQWYD | https://www.liqwydmusic.com, Music promoted by https://www.free-stock-music.com, Creative Commons Attribution 3.0 Unported License, https://creativecommons.org/licenses/by/3.0/deed.en_US. Robb Leigh Davis is a writer, producer, and director in New York City. As a playwright, his work has been featured in the Dixon Place HOT! Festival and the New York International Fringe Festival. He was commissioned by Brooklyn College for two performance pieces, In The Eyes of God and Intolerable, and has performed original works at conferences across the country, as well as being a selected playwright for the Tectonic Theater Project's LGBT Theater Artists of Color Training Lab. He is the creator and host of the performance series, MEDITATION ON A THEME (www.meditationonatheme.com), and the Co-host of the podcast Takin U Back, with comedienne Jenn Wehrung. Each episode revisits a calendar year, and all the pop culture insanity that ensued. As a director, he has overseen the one-woman shows, Seeing the Glass, with Jenn Wehrung, White America Hero, with Erica Bradshaw & One Who Succeeds, with Marina Franklin. He has worked extensively in the NYC public school system, creating and advising the development of arts-focused after school programs across the five boroughs, and previously served as the Director of Arts & Culture for the LGBT Center of NYC. It is available at Blockbuster, Amazon, and now Pay-Per-View. “In the Eyes of a Killer” is Directed by Louis Mandylor and starring Louis Mandylor (Big Fat Greek Wedding), Costas Mandylor (SAW FRANCHISE), Gwendlyn Edwards (The Last Confederate,) James Marshall (A FEW GOOD MEN),  and Petri Hawkins-Byrd (JUDGE JUDY). Her new movie The “Magic Of Christmas II” is has been released and is selling at Walmart, BestBuy, Barnes & Nobles, etc. In addition, Mamie Jean just recently produced and directed “Marvin’s Business Trip”, which is in pre-production and she is currently filming “Magical Mrs. Claus” here in Elk Grove. “In The Eyes of A Killer” won 12 awards, including Best Picture, Best Actor, Best Feature, Best Thriller. Mamie Jean also won Writer of the Year for the script “Sunshine” and Best Screenplay for “Murder By Design” script. Mamie Jean was a stand-up comedian for many years and won a National comedy contest that New Line conducted  to promote the cassette release of Billy Crystal’s Mr. Saturday Night. She now has out a CD called “Kentucky Fried Laffs”. Some of her work can be found on Amazon, Channel K and The Son of Channel K and Bachelor Pad. Mamie is also one of the executive producer of “Bad Faith”. Mamie has been in many movies such as “Thelma and Louise”, “Point Blank”, “Barton Fink”, “Dead Again” and the 1991 Television version of “What Ever Happened to Baby Jane”, etc. Most recently she also appeared in “In the Eyes of a Killer”, “The Magic of Christmas II”, and “Bad Faith”.
