How do you navigate rapidly changing and competing guidance from industry, national and various state organizations? Laws regarding outbreak reporting also varies greatly from state-to-state and how do you develop a consistent protocol when your health organization spans multiple states? Catherine Kroll joins Justin Poulin and Dr. Larry Muscarella to share her experiences and insights related to leadership, organizational communication and staffing during the pandemic. Catherine also shares her story of how she combined several of her childhood interests in becoming either a detective, FBI agent or a doctor into a career as an epidemiologist. It wasn't until Catherine's niece contracted C. diff that she became aware of the hospital environment and infection prevention. Even though the pandemic has raised awareness and interest in infection prevention as a career path, one key to Catherine's success as a leader is continuous recruiting and championing the profession. Can carpet be just as safe and possibly more effectively cleaned than vinyl floors in the healthcare setting? In the acute care setting, healthcare professionals are very familiar with vinyl floors and assume that they are easier and more effectively cleaned. However, in long term care facilities there are additional considerations such making the environment have a warmer feel and home-like. On this week's episode of Transmission Control, Catherine Hilliker joins Dr. Larry Muscarella and Justin Poulin to research findings on carpet cleaning and explore considerations such as the cost and cleaning time with carpets versus other flooring options. There is no doubt that Long Term Care facilities were hit hard during the pandemic. In many cases, these facilities do not have a dedicated infection control practitioner on staff which represented a significant gap in preparedness planning and exacerbated the struggles of reacting quickly and effectively to the viral threat. On this week's episode of Transmission Control, Buffy Lloyd-Krejci joins Dr. Lawrence Muscarella and Justin Poulin to discuss long standing limitations and concerns surrounding infection control practices in long-term care and the significant opportunities for training, not only for clinical staff but non-clinical staff and the general public at large. You may be familiar with biofilm but what do you know about small colony variants -- and the impact that they have on infection control and public health? All living organisms have a universal prime directive, which is to survive. What does that mean for those of us whose prime directive is to protect our patients against infectious agents? On this week's episode of Transmission Control, we talk to Wava Truscott, PhD. MBA about how bacteria adapt to their environment and become stronger. While Dr. Truscott “loves bugs” and has made them her life's work, she also has a unique ability to explain complex concepts in microbiology by making them simple and providing real world applications for information that positively impacts healthcare organizations. On this interview, Dr. Truscott joins hosts Dr. Lawrence Muscarella and Justin Poulin to explore the topics of antibiotic resistance, surgical instrument cleaning implications for Sterile Processing, and the future threats that need to be on your clinical radar. Within the world of GI endoscope reprocessing there exists a debate on whether or not sterile water should be used during procedures versus tap water. Running parallel to that debate is whether the industry should move to the sterilization of flexible scopes, continue with high level disinfection (HLD), or consider conversion to single use disposable endoscopes that have recently entered the market. Following up on our recent Transmission Control interview with Dr. Raman Muthusamy regarding the Multi-Society Guidelines for Reprocessing Flexible GI Scopes, we take a deeper dive with Jim Collins, BS, RN, CNOR of the Cleveland Clinic to walk through the addition of population-based guidelines which are at the heart of all of these debates. On this episode we explore the advantages of population-based practices against the potential complexities of introducing two or more standards of care. Tune in to hear Jim's perspectives on facility risk assessments and take a look back at how the pandemic altered the world of GI endoscope reprocessing. #TransmissionControl #Podcast #Education #PublicHealth #InfectionControl
Endoscope drying times and the use of an alcohol rinse has become an area for debate and emerging research is increasing with each passing day. Advanced inspection technology such as borescopes and the development of ATP testing offer additional measures to evaluate the effectiveness of cleaning the small diameter working channels of flexible scopes. On this episode of Transmission Control, Nancy Robinson, Ph.D, and Tamara Behm, MSN, RN, CIC, FAPIC, CER, CQIA, CCSVP, from STERIS Infection Prevention Technologies join hosts Justin Poulin and Dr. Larry Muscarella to discuss recent research that was published in the American Journal of Infection Control. We explore current standards and put them up against real world application, discovering gaps and outline some potential changes to increase compliance and patient safety. Our guests also explain the impact that channel design and air pressure have on the drying process and the pros and cons of using and alcohol flush. Given the rising concerns of endoscope-related infections around the country, organizations, agencies, and providers believed it was imperative to evaluate the current literature and standards for endoscope reprocessing. Joining us for this week's episode of Transmission Control, Dr. Raman Muthusamy joins Dr. Lawrence Muscarella and Justin Poulin to discuss the critical updates made to the Multisociety Guidelines for Reprocessing Flexible GI Scopes in the early 2021. These updates have helped to reduce confusion about how to navigate conflicting industry guidelines and recommendations from various organizations, but ongoing updates are necessary as the industry continues to explore the many factors that have resulted in “superbug” outbreaks. Dr. Raman Muthusamy has been front and center in this conversation, is co-author of the multisociety guideline document. In this interview he shares his knowledge of the recent updates to the guidelines (such as facility based risk assessments), the discussion around sterilization versus high level disinfection, and standards of care that are population based rather than universal.
