Join CardioNerds EP Council Chair Dr. Naima Maqsood and Episode Lead Dr. Jeanne De Lavallaz as they discuss the results of the ARREST-AF Trial with expert faculty Dr. Prashanthan Sanders and Dr. Mehak Dhande. The ARREST-AF trial enrolled 122 patients with a BMI of 27 kg/m2 or greater and at least one cardiovascular risk factor with either paroxysmal or persistent AF and were scheduled to undergo de novo AF ablation. They were randomized to an intensive risk factor management (RFM) program versus usual care. The RFM program addressed obesity, sleep apnea, HTN, HLD, tobacco, and alcohol abuse, whereas the usual care arm had a discussion of risk factors but without an extensive risk factor modification or follow-up program. The study population had a mean age of 60 years, a mean BMI of 33 kg/m2, and 56-60% of patients with persistent AF. A third of the study population was female. The trial showed a significant improvement in the primary endpoint of the percentage of patients free from atrial fibrillation after ablation in those receiving the intensive lifestyle RFM program. At the end of the 12.3-month follow-up period, 66% percent of patients in the RFM group were free from AF compared to 42% in the usual care group (HR 0.53, p = 0.03). The RFM group also showed significant improvement in AF symptom severity, decline in body weight, systolic blood pressure, glycemic control, and exercise capacity. On average, patients in the RFM arm lost 9 kg of weight compared to 1 kg in the control group. Similarly, systolic blood pressure decreased by 13.1 mmHg in the RFM group but increased by four mmHg in the control group. Definition.The American Urological Association (AUA) published an erectile dysfunction guideline in May 2018, which is available online at no cost. Based on that guideline, erectile dysfunction can be defined as “the consistent or recurrent inability to attain and/or maintain penile erection sufficient for sexual satisfaction, including satisfactory sexual performance.” Comment: This guideline provides 25 principles for diagnosing and treating ED. Diagnosis.Getting a good history is important when diagnosing erectile dysfunction. The patient should be asked about the onset of symptoms, severity, how much it hinders his sexual performance, whether the patient can get and maintain an erection, psychological factors, social factors, and presence of morning erections. One can use different questionnaires: the five-question International Index of Erectile Function (IIEF-5) or a single-question self-assessment. Single-question self-assessment:Impotence means not being able to get and keep an erection that is rigid enough for satisfactory sexual activity. How would you describe yourself?Not impotent: always able to get and keep an erection good enough for sexual intercourse.Minimally impotent: usually able to get and keep an erection good enough for sexual intercourse.Moderately impotent: sometimes able to get and keep an erection good enough for sexual intercourse.Completely impotent: never able to get and keep an erection good enough for sexual intercourse.Comment: Basically, the single-question self-assessment is a self-diagnosis of erectile dysfunction; the patient is giving you the severity of his condition. This questionnaire seems to be very subjective. International Index of Erectile Function (IIEF-5):IIEF-5 asks five questions, and the patient answers on a scale of 1 to 5 (1 is the worst, 5 is the best)How do you rate your confidence that you could get and keep an erection?When you had erections with sexual stimulation, how often were your erections hard enough for penetration?During sexual intercourse, how often were you able to maintain your erection after you had penetrated your partner?During sexual intercourse, how difficult was it to maintain your erection to completion of intercourse?When you attempted sexual intercourse, how often was it satisfactory for you?Diagnosis can be made based on the total score. 1 to 7: severe ED, 8 to 11: moderate ED, 12 to 16: mild-moderate ED, 17 to 21: mild ED, and 22 to 25: no ED.This is a self-reported questionnaire, and the score should be interpreted in a clinical context. Answers will likely be biased if, for example, the questionnaire is asked by a female medical assistant. Causes of ED:It is important to assess for medical conditions, psychological conditions, and medications because ED can be caused by vascular, neurological, psychological, and hormonal problems. Cardiovascular: Some common conditions related to ED are cardiovascular disease (PAD, CAD) and HTN.Endocrine: DM, HLD, obesity, testosterone deficiency (hypogonadism), hyperprolactinemia, thyroid disorder, metabolic syndrome.Neurologic: Neurologic conditions (multiple sclerosis, stroke, spine injury), trauma, and venous leakage.Lifestyle causes: sedentary lifestyle, tobacco use.Psychological: Performance anxiety, relationship issues, anxiety, depression, and stress are common psychological causes.Medications and substances: Alcohol, illicit drugs, and nicotine are important causes of ED, but some medications also cause or worsen ED: opiates, diuretics (spironolactone), antifungals (azoles), anticonvulsants, antidepressants (SSRIs), antihistamines, H2 blocker (cimetidine) antihypertensives, nasal decongestants, and antipsychotics. Remember to ask about over-the-counter medications and supplements.Physical exam: Measure blood pressure, BMI, and a complete exam, especially a genital exam. A comprehensive genitourinary exam should include the inspection of the testicles (atrophy, varicocele, signs of hypogonadism). The penis should be inspected and palpated (look for scar tissue and Peyronie's plaques) and assessment of penile stretch/flaccid length (it is done by stretching the penis. An elastic penis is a healthy penis). Dr. Winter's expert opinion: consider a prostate exam in older patients presenting with ED.Labs: Following physical examination, some lab tests can be ordered to further evaluate possible causes of ED. -A1C and glucose levels can be ordered to look for diabetes. -Lipid panel for hyperlipidemia.-TSH should be checked for thyroid function and to rule out hypothyroidism. -Testosterone deficiency can be assessed by measuring morning serum total testosterone level, which is defined as total testosterone < 300 with signs and symptoms. -Prolactin (perform pituitary MRI in any degree of hyperprolactinemia. In patients taking medications that cause hyperprolactinemia, get MRI if prolactin is above 100) Why is it important to diagnose ED?ED can be linked to organic causes.- Glucose: ED is linked to increased fasting serum glucose levels (diabetes). People with PMH of DM are 3 times more likely to develop ED. The longer the patient had diabetes, the stronger association with ED. Fasting glucose levels are associated with the highest risk of ED. The probability of having undiagnosed DM is 1/50 in the age group 40 to 59 without ED but increases to 1/10 for those with ED.- Testosterone and obesity: Low serum testosterone levels can contribute to the link between metabolic syndrome and ED. In men with obesity, the adipose tissue enzyme aromatase is more prevalent and can convert testosterone into estradiol to cause hypogonadism. Furthermore, adipocytes can cause inflammation and recruit inflammatory cytokines, leading to impaired endothelial function and ED. - Cardiovascular disease: ED and CVD have some common risk factors: older age, HTN, dyslipidemia, smoking, obesity, and DM. ED is related to an increased risk of CVD, CAD, and stroke. Usually, it is thought that ED arises two to five years prior to CAD. If a patient develops signs and symptoms of ED before CAD, the patient can be counseled and educated to make lifestyle modifications to prevent CAD.Furthermore, men with ED are more likely to experience angina, MI, stroke, TIA, CHF, and cardiac arrhythmias when compared to their counterparts without ED. A study from 2003 suggested that patients with ED have a 75% increased risk of developing peripheral vascular disease. Studies suggest ED can predict silent CAD, and one study concluded that the incidence of CAD in men below 40 years of age with ED was seven times higher than that of the control population without ED. It is important to diagnose ED because it can be used as a marker for assessing cardiovascular risk.ED can be linked to many causes, and we as clinicians should be able to identify those causes to prescribe a more specific treatment. Not all ED will respond to “the blue pill”. We will talk about treatment in another episode. Conclusion: Now we conclude episode number 115, “Erectile Dysfunction Diagnosis.” Male sexual health sometimes can be taboo, and patients may not fully disclose personal issues like erectile dysfunction. Andrew and Adrianna explained that an open discussion about erectile dysfunction can help you diagnose underlying conditions, including cardiovascular disease. Dr. Arreaza reminded us that the diagnosis of erectile dysfunction should prompt a deeper investigation in most cases before you attribute it to psychological factors. At the tertiary care center the patient is seen by an Infectious Disease Specialist and a number of tests are ordered by the Infectious Disease Consultant but they are canceled by Cardiologist who writes in their note “no concern for an infectious process”. We feel that heart health month should be something everyone should think about all year long. Below are 20 plus different ways to get started to a healthier heart. Stop smoking - no ifs, ands, or butts and stay away from secondhand smoke. There are many steps you can take to help protect your health and blood vessels. Avoiding tobacco is one of the best. Control your cholesterol and blood pressure. Slim down - If you're carrying extra fat around your middle, it's time to slim down. Then stay at a healthy weight Have sex - having sex can be good for your heart! lower frequency of sexual activity is associated with higher rates of cardiovascular disease. Engage in hobbies - that are relaxing too! Put your hands to work to help your mind unwind. Read a book, make something ~ cooking, baking, woodworking, knitting, sewing, ect… Eat fiber - a diet rich in soluble fiber can help lower your level of low-density lipoprotein, or “bad cholesterol. salsa offers a delicious and antioxidant-rich snack. Consider mixing in a can of black beans for an added boost of heart-healthy fiber. Listen to music - dancing makes for a great heart-healthy workout. Like other forms of aerobic exercise, it raises your heart rate and gets your lungs pumping. It also burns up to 200 calories or more per hour Eat fish - Many fish, such as salmon, tuna, sardines, and herring, are rich sources of omega-3 fatty acids. Try to eat fish at least twice a week, suggests the AHA Trusted Source. If you're concerned about mercury or other contaminants in fish, you may be happy to learn that its heart-healthy benefits tend to outweigh the risks for most people. Laugh - laughing can lower stress hormones, decrease inflammation in your arteries, and raise your levels of high-density lipoprotein (HLD), also known as “good cholesterol.” Stretch - Yoga can help you improve your balance, flexibility, and strength. It can help you relax and relieve stress. As if that's not enough, yoga also has potential to improve heart health. Drink alcohol in moderation - Moderate consumption of alcohol can help raise your levels of HDL, or good cholesterol. It can also help prevent blood clot formation and artery damage. According to the Mayo Clinic, red wine in particular may offer benefits for your heart. That doesn't mean you should guzzle it at every meal. The key is to only drink alcohol in moderation. Avoid salt - Processed and restaurant-prepared foods tend to be especially high in salt. So think twice before filling up on your favorite fast-food fix. Within the world of GI endoscope reprocessing there exists a debate on whether or not sterile water should be used during procedures versus tap water. Running parallel to that debate is whether the industry should move to the sterilization of flexible scopes, continue with high level disinfection (HLD), or consider conversion to single use disposable endoscopes that have recently entered the market. La « pépite » française de la vision nocturne, Photonis, a enfin trouvé un repreneur tricolore. Bercy s’était opposé à son rachat par un groupe américain, au nom de la souveraineté économique, la nouvelle doctrine en cours au gouvernement. Un engagement qui a trouvé un nouveau terreau propice avec la pandémie de Covid-19. La pénurie de masques a choqué l’opinion et relancé le débat sur le besoin de maîtriser la production de certains produits vitaux, et plus généralement de reprendre la main sur le sort de l'économie française dans un monde globalisé. En décembre, le gouvernement a donc retoqué l’offre faite par l’américain Teledyne sur Photonis alors que l'acheteur avait accepté toutes les conditions posées par l’État. Il y a quinze jours c’est Carrefour qui a dû refuser le rapprochement amical proposé par le québécois Couche-tard à cause du veto de Bercy. De gauche à droite, toute la classe politique applaudit, et pour cause, le souverainisme économique est devenu un thème de campagne. Sa résurgence est quasi automatique en situation de crise. Est-ce qu’il y a des bonnes raisons économiques de protéger les entreprises françaises? C’est parfois dans l’intérêt des salariés, souvent maltraités dans les rachats, comme l’a démontré la reprise de la branche énergie d’Alstom par l’américain General Electric. Un rachat qui avait alors été approuvé par Emmanuel Macron quand il était à Bercy. C’est aussi justifié par la crainte de voir un savoir-faire disparaître ou copié, ce qui dégraderait un peu plus le tissu industriel. C'est le cas avec Photonis dont les applications sont très recherchées par l'industrie militaire. En 2020, 270 sociétés se sont senties menacées par des prédateurs extérieurs. La plupart sont des petites entreprises. Cela fait quinze ans que la France s’est dotée d’un arsenal pour bloquer les investissements étrangers susceptibles de mettre en péril ces entreprises de la défense, et de la sécurité. Puis de l’énergie, des télécom, de l’eau et de la sécurité alimentaire. Beaucoup d’États ont des dispositifs défensifs similaires. C’est le cas du Royaume-Uni, des États-Unis bien sûr qui ont une palette d’armes juridiques bien rôdées. Comment la protection, ou le veto de l'État, est-il perçu par les entreprises visées par ces rachats étrangers? C’est plutôt rassurant quand l'entreprise va mal, encore faut-il pouvoir trouver une alternative française. Les Chantiers de l’Atlantique devaient être repris par l’italien Fincantieri, l’accord longtemps soutenu par le président Hollande puis par Macron a finalement échoué, au grand soulagement des salariés qui redoutaient la casse sociale et les ambitions de l’investisseurs chinois présent au capital de Fincantieri. Ils ont trouvé un sauveur français il y a quelques jours, le fondateur de Bricorama Jean-Claude Bourrelier est candidat avec le soutien d'acteurs publics pour cette entreprise à 80% aux mains de l'État. En revanche, Carrefour qui espérait financer sa rénovation grâce à l'offre de Couche-tard doit maintenant trouver d’autres investisseurs Et cela dans un périmètre qui se réduit désormais à l'Hexagone. Le manque de fonds propres disponibles pour l'investissement est une faiblesse du capitalisme français. Et explique en partie la présence parfois appréciée des investisseurs étrangers. Cela fait bientôt une année que Photonis, une entreprise de taille intermédiaire qui a besoin de capitaux stables pour se développer cherche un repreneur puisque son propriétaire le fonds français Ardian est vendeur. C'est finalement la société d'investissement HLD qui va la racheter, à un prix bien moindre que celui proposé par Teledyne. EN BREF Les consommateurs européens portent plainte contre le réseau TikTok Ils accusent l'application chinoise d'exploiter les droits et les données des utilisateurs. Les associations de consommateurs à l'origine de la plainte déposée auprès de la commission estiment que les jeunes, les principaux clients de cette plateforme de vidéo, ne sont pas assez protégés, leurs publications peuvent être modifiées ou reproduites à leur insu.

Show notes Why should CardioNerds pay attention to diabetes mellitus (DM)? As a cardiovascular risk equivalent, DM is a key CVD risk factor, associated with a 2-4 fold increased risk. 70% of ACS patients have DM. Cardiologists will see more patient with DM given the rising prevalence of obesity, subsequent diabetes and ensuing CVD. Only 6% of patients with DM and cardiovascular disease (CVD) get appropriate care for DM and CVD. Historically, hypoglycemic agents improved microvascular outcomes (retinopathy, nephropathy, neuropathy), but not macrovascular outcomes (MI, CVA, PAD). However, this has changed with the advent of mandatory cardiovascular safety trials with positive data for GLP1 agonists and SGLT2 inhibitors! There aren't enough endocrinologists! They only see ~5% of DM patients. In 2012 the US generated 280 endocrinologists versus 100 million patient with DM or pre-DM. Primary care physicians are key allies in the care of these patients. So as CardioNerds, let's get over this therapeutic inertia and take ownership of our patients' DM as we already do for their HTN and HLD; in collaboration with a multidisciplinary team including the PCP, dietician, pharmacist, DM educators, +/- behavioral therapist, +/- endocrinologist, +/- metabolic surgeon. What is your global approach to the patient with DM? Optimize the non-DM CVD risk factors with lifestyle intervention and medical management: CVD risk factors are very common in patients with DM (sedentary lifestyle, unhealthy weight, HTN, HLD). The Steno-2 Study (Gaede et al., NEJM 2008) showed that in patients with T2DM & microalbuminuria, intensive intervention with multiple drug combinations and behavioral modification was better with regards to: vascular complications, death from any cause, and death from CV causes. Emphasize a healthy lifestyle - use a patient-centered approach with motivational interviewing and shared decision making, provide education, set realistic goals, identify barriers (socioeconomic, etc), engage family and a multidisciplinary team (nutritionist, exercise physiologist), utilize behavioral interventions. Pharmacologic intervention - medical weight loss for BMI > 27 and DM (enjoy upcoming Ndumele episode), anti-HTN (enjoy upcoming Laffin episode), and anti-HLD (enjoy the Navar-Shah episode). NOTE that statins have been shown to have a small effect on increasing incident or worsening DM, but the effect size is small and overcome by the benefit in whom statins are indicated. Treat the Hyperglycemia itself! Let's discuss this deeper… What is your approach to non-insulin DM management? First-line agents: US guidelines: in addition to lifestyle intervention, start with metformin as the first line agent. European guidelines: now give preference to GLP1 agonists and SGLT2 inhibitors in patients with or at risk for cardiovascular disease. Sulfonylureas: increase pancreatic insulin secretion. Dr. Bruemmer feels they obsolete for the preventive cardiologist from the standpoints of safety, efficacy, and cardiovascular disease. There is no efficacy data past 4 years and no cardiovasc...

High-density lipoprotein or HDL is one of the most important of the lipoproteins in most species and there is evidence that points towards a role of HDL in normal immune function. A paper appearing in the May 2019 issue of Clinical Chemistry tested the hypothesis that concentrations of HLD cholesterol are associated with risk of autoimmune disease. That study from the Copenhagen General Population Study and the Copenhagen City Heart Study included over 100,000 individuals.
