About the Speakers:Dr. Thureiyya K. Rodriguez, DHA, MSN, CWOCN, has over two decades of experience in healthcare and specializes in wound, ostomy, and continence nursing. Dr. Rodriguez's experience spans beyond inpatient and outpatient nursing. She has an extensive range of knowledge and expertise in staffing methodology and leadership nursing. Dr. Rodriguez's experience in nursing practice and healthcare administration, combined with her tenure in both federal and private healthcare sectors, equips her to offer comprehensive and tailored solutions. Dr. Rodriguez has peer-reviewed publications in Nursing Made Incredibly Easy and is an active peer reviewer for Wolters-Kluwer: Advances in Skin and Wound Care. She is actively involved in the Northeast Region WOCN (NER WOCN) and National Society of Leadership and Success (NSLS). She is the owner of Thureiyya Rodriguez RN, PLLC and North-Browne Consulting Services.Katie Cesario, MA, RN, CWOCN, became a nurse in 2005 after obtaining her BSN from Pace University. She started her career at Visiting Nurse Service of New York as a public health nurse intern. In 2009, she received her Master's Degree in Nursing Education from Pace University and soon after took a position as Orientation Nurse Instructor at VNSNY. In 2013, she studied at the Cleveland Clinic to become a Wound, Ostomy, Continence Nurse. In 2015, she took a WOC nurse position at New York Presbyterian, Columbia University Medical Center. In September 2018, she joined the WOC nurse team at NYP, Brooklyn Methodist Hospital. She held the Secretary position for the Metro NY Affiliate of the WOCN Society from January 2021-December 2022. Since 2019, she has served on the skin and wound care advisory board for Coloplast. Presently, she is embarking on a new path in her career by starting her own private nursing business for wound and ostomy patients and their caregivers, Katherine Cesario RN, PLLC. Gracie was lying in bed. The surgeon had just been in to remove the stitches — but clearly, it was too early. A few minutes later, I came into the room. Gracie asked for help sitting up. She reached for my arm and started to pull herself upright. Then, the unthinkable happened. The wound suddenly split wide open — 10-12 inches long, four inches wide. It's called dehiscence. I was instantly reminded why I studied music and not medicine. But there was no time to be queasy — Gracie needed me. I quickly summoned the nurses, who rushed in. I told them to urgently call the surgical team. Then I grabbed Gracie's hand, helped her recline back on the bed, locked eyes with her and said: “Don't look down.” As her breathing slowed, we began to sing: In my life, Lord, be glorified… Gracie softly changed the words: In my leg, Lord, be glorified today. The surgical team worked around us while she sang. They were stunned. So was I. She wasn't thinking about tomorrow. Just: Be glorified in this. Thou wilt keep him in perfect peace, whose mind is stayed on thee: because he trusteth in thee. Isaiah 26:3 Caregiving comes at you fast. Send us a textWe take a fascinating dive into aquatic medicine as Drs. Anthony Cerreta and Karisa Tang share groundbreaking findings on fish wound healing. Their study reveals that topical naltrexone in iLEX ointment significantly accelerates cutaneous wound healing in black belt cichlids—a discovery with profound implications for aquatic veterinary care.Anthony and Karisa explain how fish wounds present unique challenges due to the constant exposure to water-borne pathogens, making effective treatments critically important yet notoriously difficult to develop. What makes their findings particularly remarkable is the simplicity of the approach: applying naltrexone ointment just once every 3-4 days produced visible healing by day 19, much faster than untreated wounds. Even more surprising was how effective the treatment proved despite minimal contact time with the wounds—challenging conventional wisdom about topical treatments in aquatic animals.This work builds on previous clinical observations at major aquariums where naltrexone showed promise treating head and lateral line erosion (HLLE). Anthony and Karisa meticulously designed their study to provide the aquatic veterinary community with solid evidence of naltrexone's efficacy. They're now expanding their research to chronic wounds across various species, potentially revolutionizing treatment protocols for fish in both professional and home aquarium settings. Bill Padula, PhD, Assistant Professor of Pharmaceutical and Health Economics, and Dr. David Armstrong, professor of surgery and Neurological surgery, are both at the University of Southern California and are concerned about wound care and the impact of potential changes in Medicare reimbursement of skin substitutes. They discuss the importance of skin substitutes in chronic wound care and their effectiveness in reducing amputations and hospital readmissions. With current advancements in wound care,  there is an emphasis on the need for education and advocacy to ensure that Medicare understands the value of skin substitutes and their potential for improving patient outcomes. David explains, "The skin substitutes we are talking about have been really helpful over the past, maybe at least the last decade, maybe even the last two decades, and especially over the last few years. We've seen now in some studies from our group and others that we can not only reduce amputation, but we can also reduce hospital admission and emergency department admission for these patients. So it's a really exciting time. And so that's on the positive end. On the not so positive end is that there have been some changes, and I'm sure Dr. Padula can talk about this. That may limit our access to it as clinicians, which is concerning certainly for the patients that I'm having right across the hallway." Bill elaborates, "Up until recently, Medicare hasn't put a lot of governance on reimbursement for skin substitutes, and physicians have been using them autonomously with the patient to treat non-healing chronic wounds. We see from the data that these skin substitutes work best when applied on a patient every one to seven days in a wound clinic, like what David Armstrong runs here at the University of Southern California. Medicare wants to change the reimbursement pattern and reduce reimbursement so that it would be impossible to get reimbursed for more than ten applications of a skin substitute in twelve weeks. MATTHEW REGULSKI Dr. Regulski is a Graduate of Temple University School of Medicine, where he graduated Summa Cum Laude. He performed his residency at the Main Line Health System in Philadelphia, PA, and is currently the Medical Director of the Wound Care Institute of Ocean County, LLC. Dr. Regulski is also Co-Director of The Center for Wound Healing & Hyperbaric Medicine at Community Medical Center and is involved at the Wound Care Center at the Kimball Institute. He has been the principal investigator for multiple clinical trials for wound healing, diabetic foot, and venous leg ulcers, and has authored several peer-reviewed articles for the treatment of chronic wound healing and limb-salvage surgery. Dr. Regulski is currently on the Board of Directors of the Federation of International Podiatry and is the Scientific Chairman for the Federation of International Podiatry. He is also the Communications Director for the Foot Working Group of the American Diabetes Association. Dr. Regulski is a wound care certified physician and fellow of both the Academy of Physicians in Wound Healing and the American Professional Wound Care Association. He is also a National Delegate for the Association for the Advancement of Wound Care and is a member of the Wound Healing Society. An International and National lecturer of wound healing and limb-salvage surgery, Dr. Regulski has multiple certifications in wound healing, diabetic wounds, and limb-salvage surgery. Armstrong MD from the Keck School of Medicine of USC, are talking with Todd Eury on TWIRx about a recent proposal from Medicare is calling to  eliminate coverage of some of skin substitute therapies, and the potential serious health risks that many Americans will face by limiting them. Skin substitutes, made from donated amniotic tissue, greatly improve wound care outcomes and help patients avoid limb amputations—a procedure that can double the risk of death for diabetic patients. This Medicare proposal under consideration will deny coverage of over 200 skin substitutes and limit how doctors can properly treat patients will leave tens of thousands of Medicare beneficiaries without treatment.

In this episode of Dean's Chat, Dr.'s Jensen and Richey interview Dr. Alton Johnson from Ann Arbor, MI.  Dr. Johnson is a Clinical Assistant Professor of Orthopaedic Surgery specializing in disorders of the Foot and Ankle with an emphasis on Podiatric Surgery at the University of Michigan Medical School. Dr. Johnson completed his medical education at Kent State University College of Podiatric Medicine in Cleveland, OH.   Dr. Johnson went on to complete post-graduate training in Podiatric Medicine and Surgery at HCA Florida Aventura Hospital in Miami, FL. He completed a fellowship in Podiatric Clinical Research and Wound Care at Penn Medicine- University of Pennsylvania Health System in Philadelphia, PA. Dr. Johnson earned a Post-Doctoral Certificate in Clinical Research from the University of Pennsylvania Perelman School of Medicine during his fellowship training.   Dr. Johnson brings a wealth of knowledge and expertise in podiatric surgery with an emphasis on wound healing and prevention. He is board certified in Podiatric Medicine by the American Board of Podiatric Medicine. In addition, Dr. Johnson is a Board-Certified Wound Care Specialist Physician by the American Board of Wound Management. Dr. Johnson serves as an Executive Board Member of Kent State University College of Podiatric Medicine and Chairman of the Board of Directors for the American Society of Podiatric Surgeons. He was recent elected as Vice President of the Save a Leg Save a Life Foundation (SALSAL).
